What failed: The repairs include replacement of the Brake Fluid Reservoir and the Brake Fluid Level Sensor. This is the link to the dealer report [XXX] . No one other than the dealer has inspected these parts to my knowledge. There was no accident of any kind. I personally felt unsafe after these parts failing and reached out to Mazda Customer Care to request investigation of the issue. The case manager just called me back today to say they could not help me. Mazda has denied warranty coverage, claiming the parts are not covered. However, I really do feel this is a critical safety system component that failed after normal emergency use with no collision or impact. Why I feel this is a safety concern: - The brake warning system became unreliable after a single emergency stop. - My car only has 42k miles and is in excellent condition and is fully dealer serviced. - A driver could lose the ability to receive proper brake system warnings if this failure occurs again. - A critical brake sensor should not fail under normal driving conditions, and its failure could lead to delayed driver response in an actual brake fluid loss scenario. It makes me worry for the future. For your consideration: I believe this represents a safety-related defect that should be investigated, as it affects the reliability of the brake system warning indicators in the 2019 Mazda 3. The ABS control module in my car is inoperable. I have lights on for ABS, brakes, and stabiltrac. Upon further investigation the ABS module is no longer available and no aftermarket part available. This leaves my car useless being I no longer feel safe driving it. If I lock up brakes in an emergency, my car could go into an uncontrollable slide (being front wheel drive) especially on wet roads.

I also would not have slammed on the gas thinking it was the brake since there would have been no reason to slam on the brake when I was only going 5-10 miles per hour max around the tight turn. My shoe was not caught on a pedal, both sandals were still on my feet after the accident. I am not a two footed driver. I feel that it was an accelerator malfunction, as the gas pedal had always been a little strange with a delayed response, but may have also involved the transmission after driving up the steep hill and then returning to a short but flat stretch leading up to the house. I was very bruised up, particularly my shins and ankles. Two months later, I still have some visible bruising. I also had injuries from the seat belt on my chest and what appeared to be a powder burn from the airbags. I was quite traumatized by the sudden acceleration and violence of the crash and not understanding how or why it happened. If someone had been standing in front of the house to greet me on my return, they would have been killed. The car was towed away and totaled by the insurance company, so the problem hasn't been reproduced or confirmed. There were no warning lamps or messages that I'm aware of prior to the incident, except that the gas pedal has always felt inconsistent concerning when it engages. I inspected the vacuum hose and brake fluid level — both were normal. After researching, I discovered this is a known issue in 2019 Equinox models with the 1.5L turbo engine, related to vacuum pump failure or loss of vacuum supply to the brake booster. There are service bulletins and limited coverage programs (N192268490 and N182202780), but my VIN does not show an open recall. I believe this is a serious safety defect that could affect many vehicles not yet covered by the recall or service campaigns. GM needs to expand the recall and properly warn drivers before someone gets killed. Please investigate this problem and hold GM accountable for fixing this safety-critical issue. Check engine light came on and another driver alerted me that the underneath of my vehicle was on fire. I pulled over to the side of the highway and my truck became engulfed in flames in. Truck is under factory warranty, unmodified, never been in an accident and has had all recommended services performed at local dealership. There was a significant amount of traffic and luckily I did not collide with anyone or set neighboring dry grass field on fire. Fire department responded and extinguished the fire, but it is a total loss. Insurance company has been notified, and vehicle will be available for inspection by appropriate agencies. While local law enforcement did respond, they did not take a report because I didn't hit anything, and the only property damage was my own. (There is documentation of the event, but just dispatch notes.) Prior to the situation above the truck had been at operating temperature for at least 20 minutes and I had traveled for about 20 miles on the highway. When I brought this to their attention, I was told that while the issue was indeed covered under a past recall, there is nothing they can do now because it has expired. This vehicle is our only mode of transportation, which is critical for my ongoing medical treatments, as well as for my husband who recently suffered a heart attack. As I write this, we have no other option but to continue using the vehicle—despite the serious safety risk—because of the urgent and necessary care both of us require. On August 4, 2025, I contacted Toyota headquarters. They reached out to the dealership and spoke with management, but unfortunately, no resolution was offered except for information about a separate, newer recall. I followed up again with the District Manager and spoke with Ashley, who informed me that after escalating the case, there was still nothing further they could do and she expressed her apologies. This situation has caused further deterioration to my health due to the stress and anxiety of potential vehicle failure, and it has added to the burden of my husband’s fragile health condition. At this time, he has been unable to return to work, and we have no income to cover the cost of a repair that should have been addressed through the original recall. I am submitting this supplemental complaint regarding a repeated and dangerous brake failure in my 2025 Hyundai Palisade. This is a follow-up to my original complaint submitted in July 2025. On multiple occasions while driving at low speeds (around 5 mph), including turning into driveways or approaching intersections, the braking system failed to respond properly. ... t connection — similar to how a bike slips when its brake fails. The vehicle kept moving forward even while I was applying the brake. This nearly caused accidents several times. I believe the automatic emergency braking system (AEB) also failed to engage. One incident caused me to hit a fence, resulting in a small dent. Hyundai is now refusing to repair the safety systems under warranty because of that dent — even though the dent was a result of the brake malfunction. I brought the vehicle to an authorized Hyundai service center, and they refused to cover the issue under warranty or provide a loaner vehicle, leaving me to drive a potentially dangerous car. I also want to note that I have seen multiple recent NHTSA complaints from other Hyundai Palisade owners describing similar brake issues. This suggests a wider pattern of brake failure, which must be urgently investigated before someone is seriously injured or worse. Hyundai has failed to properly investigate the issue or assist me in any meaningful way. This vehicle is still under warranty, and I am unable to safely use it while they delay responsibility.Read more
