I own 2 2009 Toyota Prius. Both cars less than 175,000 miles and both have failed ABS modules. The ABS module in this vehicle is also the power brake booster. This failure can cause braking issues such as loss of braking or sudden brake application. Only known fix is replacement of the ABS brake module which requires the Toyota Techstream service computer.

I was driving home after dropping my kids off at school and when I was almost home my dash lit up with the traction control light, check engine light, ABS light, red break light, service ESC message, forward collision system unavailable message and break system failure 62 MPH top speed ( was switching between 43 MPH top speed and 62 MPH top speed ), I went to slow down and I had no brakes at all.

I live on a dead end road and thank god I had already crossed the major highway before my road because it would have been an incredibly bad accident. I finally got it to switch into neutral to stop and into park where I sat with my foot on the brake in park until someone could come out to me. I called my local dealership whom had it towed to them. My vehicle is a 2021 Tahoe with 23,014 miles at the exact moment this happened. The dealership said it was the master cylinder/brake module which is not covered under the power train warranty I still have. Actuator needs replacing, currently at 127k miles and displaying traction control, brake and ABS warning lights. The repair will be a few thousand dollars, the cars market value is only around $6k. This truck was parked on a job site, on a flat parking lot over blacktop, doing a regen for the 5th time in just a few days. The truck had 4,000 miles on it. The business property is covering the tool loss, and said it isnt worth going after GM as they make it impossible. I know this isnt the only vehicle this has happened to, and they simply refuse to even answer. I want to file a formal complaint against GM, and want them to cover the loss, the loss of use. Why This Requires Federal Attention: This is not an isolated incident. Hundreds of 2013 Prius owners have reported identical symptoms, with associated diagnostic trouble codes C1391, C1252, C1256, and C1253. Toyota acknowledged this defect by issuing Technical Service Bulletin TSB-0089-20 and a Customer Support Program (ZJB) providing limited extended warranty coverage. However, that program was not well publicized . I was never informed as a vehicle owner . And it has now expired, leaving thousands of owners with a dangerous defect and repair bills costing thousands . Although I was physically unharmed, I was extremely shaken by the incident. Immediately after the collision, the brake warning light, ABS warning light, and another warning indicator illuminated on the dashboard. Prior to the incident, I had not experienced braking problems severe enough to suggest imminent failure. On 4/17/26, I brought the vehicle to Toyota of Cerritos for diagnostic evaluation. I was informed that the brake booster was the issue and that the vehicle was unsafe to drive. The vehicle was towed to a collision repair center affiliated with my insurance company. After repairing the external collision damage, the collision center informed me that the brake booster malfunction that caused the incident could not be repaired there and would require dealership service. External repair alone: $3706, in addition to increased automobile insurance premiums expected over the next 3 years. I also contacted Toyota Motor Corporation regarding Customer Support Program coverage related to brake booster issues and was informed that the program expired in 2025 and no assistance would be provided. The dealer now states they cannot reproduce the concern, but at the same time is recommending another brake/rotor replacement due to uneven wear. This seems inconsistent because the brakes and rotors were recently replaced, and repeated uneven wear may indicate an unresolved issue with the braking system, calipers, rotors, ABS/stability system, suspension, or related components. The vehicle has been inspected by the dealer/manufacturer representative. The dealer also stated the lower control arms are fine, but the root cause of the recurring uneven braking has not been clearly identified. No crash or injury has occurred to date. I checked [XXX] for a HCU/ABS part and it is available for purchase and explained to FCA it is available. The Customer Care, Eddie, stated that the brake service was over due; however, it was just purchased as a certified mini 3 months prior and all services should have been done prior to us purchasing the vehicle. Vehicle was serviced and their 21 point inspection stated everything was fine. At this time, we are out $8,500, $5K cash down payment, $1600 trade, $1900 in car payments. The bulletin however states the cost needs ot be paid by th cusotmer if it is out of warrnaty. I am submitting this complaint to document ongoing brake related concerns on my 2020 Hyundai Elantra that may affect vehicle safety. Since April 2024, I have experienced repeated brake rotor warpage that causes vibration during braking, reduced braking consistency, and a noticeable change in stopping behavior. ... eported the issue to Hyundai Motor America and followed their required warranty process, including an inspection at High Desert Hyundai. The dealership indicated the condition should be covered under warranty, but I was charged twice for what should have been a no cost warranty inspection. HMA later denied warranty coverage based on an undefined claim of “outside influence” without providing any criteria, explanation, or evidence supporting that conclusion. The denial prevented me from understanding the basis of their determination or how it applied to the brake condition. HMA then directed me into the BBB Auto Line program under case number HYU2605570 as the next step for disputing the decision. Throughout this process, Hyundai did not provide any additional information or documentation explaining the cause of the brake condition or addressing the safety concerns I raised. Because braking performance affects stopping distance and vehicle control, I am reporting this issue to NHTSA so the agency is aware of the repeated brake vibration and rotor warpage, the inconsistent braking behavior, and the lack of clarity from the manufacturer regarding the cause. I possess all invoices, communications, photographs, and documentation related to the inspections and interactions with the dealership, HMA, and BBB Auto Line, and I have maintained these records since April 2024. The car essentially performs an uncommanded emergency stop or severe deceleration without warning. The force is strong enough to create a serious risk of being rear-ended by vehicles behind me. This has happened at highway speeds and on local roads, making the vehicle unsafe to operate. If a following driver is unable to react in time, this defect could easily cause a crash, injury, or death. The problem appears related to the vehicle’s automatic emergency braking / Active Brake Assist / forward collision avoidance system and possibly other linked driver-assistance systems. The vehicle has already been serviced on two separate occasions for this exact issue, yet the problem has occurred again. Despite bringing the vehicle in for repair more than once, the defect remains unresolved. Because this dangerous unintended braking continues to happen, I cannot continue to drive the car safely with this issue unresolved. On April 1, 2025, I took the vehicle to Aristocrat Motors in Merriam, Kansas for the car slamming on the brakes and beeping when no one was around. The service center calibrated the augmented reality camera and performed a test drive. The invoice indicates that they were unable to duplicate the braking event during the test drive. Based on the symptoms, I am concerned that the brake booster vacuum pump has suffered an internal failure, a documented issue for this model year where oil sludge or debris causes the pump to seize. In addition to the physical resistance of the pedal, I am concerned about the reliability of the Electronic Brake Control Module (EBCM). The failure experienced on the highway demonstrates that this vehicle is unsafe for public roads, as the force required to engage the brakes in an emergency is far beyond safe operating standards. I request that the NHTSA investigate this brake failure and determine if a mandatory recall is necessary to prevent similar incidents from resulting in catastrophic collisions. Recently, Indiana dealer (Lockhart Cadillac Fishers) got more and more vehicles back so now believes it should have a TSB to fix due to noise caused by semi metallic pad issue. Dealer charging $469.78 to apply anti squeal to pads which they know is only a temporary fix (so not recommending it). Dealer cannot help unless GM acknowledges issue.Read more

This is a major saftey concern and there have already been multiple accidents attributed to this defect. ABS systems not functioning properly, showing engine code U0121 that states lost communication with ABS module. This is a common occurrence with jeeps and is a safety issue due to the abs system not working properly. Also causes the brakes to randomly engage and disengage.
