I’ve only had this car for 2.5 weeks. Today on my way to work (the same roads I take everyday) I had the automatic breaking system pop on with a “brake” light pop up and the vehicle was braking on its own, with no other vehicles to the front or side of me. Semi sunny day, camera clear of dirt etc. My safety was put at risk, because the car behind me had to slam on their brakes after my car

However I have been looking online and this seems to be a very known issue with Honda. I hope it doesn’t continue to happen. This is very dangerous for me and my family as well as anyone else on the road ways. A extreme safety hazard. Now I’m more worried about my new car purchase instead of being excited.Read more

Our VW Atlas Cross Sport 2023 was involved in an accident on August 3, 2025 where the Automatic Emergency Braking and Forward Collision Warning did not work. Firstly, we reported issues with our brakes several times and took the vehicle to Rick Case VW in Weston/Davie more than once to rectify this problem. However, the problem has never been fixed, and persisted up until the time of the accident.
... Secondly, the vehicle's navigation system stopped working during the week of July 28th, and this was reported to Rick Case VW on July 30th. Attempts to address the fact that the car's non-working features contributed to the accident remain unresolved with Rick Case VW (seller) and VW Corporate (manufacturer), as of September 17. We made several amicable and fair requests, up to and including: 1) giving us another/newer vehicle without any charges or fees whatsoever, or change to payment amount unless it is less, and/or 2) refunding all monies paid to date so we could find a new car on our own. To date, there has been very little communication from either party and no genuine care or effort to assist us with this case. The Rick Case Collision Center has been in possession of our vehicle for approximately 6 weeks to fix an array of damages. At this point, we do not feel the car is road-worthy or safe to drive and we have made this expressly clear in emails and voicemails to VW/Rick Case VW. The management of Rick Case VW has made no offer of a loaner nor have they acknowledged our request to exchange this vehicle. They are well aware that their faulty, non-functioning systems in our VW 2023 Atlas SE contributed to the accident in August. Their professionalism and respect for the customer would have gone a long way in resolving this in a timely and amicable manner. Now we may have no choice but to resort to taking this a step further, legally.Read more

The 2021 GMC Sierra 1500 has an Electronic Parking Brake (EPB) that bolts on to the rear calipers. The EPB does not release, in order for service, you have to long press the parking brake, then the truck goes into service mode & will release the brakes for service, some newer models may not have this feature. The vehicle has 36,000 miles on it & the inside rear brake rotors are worn down from t
... he EPB dragging & not releasing as supposed to, causing excessive premature wear & that could cause a safety hazard fleet wide. The EPB should not be in use while the vehicle is moving, but it is coded to not release, meaning whenever you press the brake it never moves back out away from the rotor, causing drag & or even can cause warping of the rotor. I do not understand the thought process in regards to not letting the EPB release, it acts like the screw type caliper that would have the same issues of every time it moves out it will not go in until taken apart & screwed in, again meaning the brake pad is constantly dragging on the rotor which could in turn also affect the miles per gallon. I see the thought process to use EPB as useful but not a good execution by the manufacturer.Read more

I am writing to report a persistent and dangerous safety defect in my 2014 Honda Accord involving the VSA (Vehicle Stability Assist) / ABS / ABS modulator / ABS ECU system. The ABS, VSA, and other warning lights have been on for years, first noticing around 60-70,000 miles indicating an ongoing fault in the system. The dealer has diagnosed a bad ABS/VSA modulator and/or ECU and stated this is a co
... mmon issue and that often this part is on backorder. Most recently, while turning the car, the system locked or mis applied the brakes, causing the car to lose control and spin out. Luckily I did not crash as the car behind me was at a safe distance, but I ended up on the side of the highway in a dangerous situation. The Center for Auto Safety shows multiple complaints for 2014 Accord, including: ABS, VSA, traction control, stability control failures; owners say they were told a VSA modulator needs replacing Owners report DTC codes like 81‑19 (“yaw rate sensor internal circuit malfunction”) and 81‑20 (ABS CPU internal circuit malfunction) in 2014 Accords. Owners report ABS and VSA not functioning, lights all on, and concerns that braking or stability control will fail, especially in poor conditions. Given the large number of similar reports for this model and year, this appears to be a systemic defect in the ABS/VSA modulator/ECU system in 2013‑to‑2017 Honda Accords, including 2014. Because this is a safety system (brakes, stability control), the defect creates a real risk of crash, injury, or death. I request that NHTSA: Open a formal investigation into the ABS/VSA modulator/ECU failures in 2014 Accords (and related years). Determine whether a recall is needed. Require Honda to provide fixed or replacement units without cost to owners or reimbursment Ensure parts are available (i.e. not back‑ordered) and that the repair is effective and safe.Read more

The Electronic Parking Brake (EPB) on my 2022 Hyundai Ioniq Hybrid has a recurring safety defect. The EPB warning light continues to come on and off intermittently even after EPB components were replaced. Diagnostic Trouble Codes C2416 and C2417 have been detected multiple times. This issue first occurred at around 10,000 miles, while the vehicle was still under factory warranty. At that time, th
... e dealer only performed a cleaning procedure, which did not resolve the problem. Over the years, I returned to the dealer multiple times, but they refused to perform repairs unless the warning light was present during diagnosis. Frequently, the light turned off before inspection, leaving the defect unresolved. At approximately 87,000 miles, the dealer finally replaced EPB components at my expense (out-of-warranty repair). However, even after this replacement, the EPB warning light has continued to reappear, proving that the underlying safety defect was never fixed. This is a serious and ongoing safety concern. The EPB is a critical braking system component, and its failure could prevent the vehicle from safely parking or holding on a slope. Because this problem began during the warranty period, was never properly resolved, and persists even after paid repair and part replacement, I believe this represents a manufacturing defect that requires investigation and potential recall.Read more

I am writing to bring to your attention several safety issues with my vehicle that have not been addressed despite my previous attempts to have them resolved. Despite bringing this issue to the attention of your team, the necessary repairs have not been carried out, and the problem persists. This is extremely concerning as the continued malfunction of the brakes poses a significant threat to the safety of both myself and other road users. I urge you to take immediate action to rectify this issue and ensure that the brakes on my vehicle are functioning correctly. Furthermore, I would also like to highlight another safety issue with the seatbelt system in my vehicle. There have been instances where the seatbelt, the third row seat and passenger seat, fails to latch properly sometimes. I have personally experienced difficulties with the latching mechanism and have had to pay for the third-row seatbelt to be fixed as well. Given the critical role that seatbelts play in ensuring the safety of passengers in a vehicle, it is imperative that this issue is addressed promptly to prevent any potential accidents or injuries. I request that the seatbelt system in my vehicle be thoroughly inspected and repaired to ensure that it is functioning correctly and reliably. I trust that you will take the necessary steps to address these safety concerns promptly and effectively. Failure to do so not only compromises my safety but also places the lives of others at risk. While driving my 2020 GMC Terrain (VIN: [XXX] ) with approximately 110,501 miles, I experienced a sudden and complete loss of brake assist. After about 2 hours of highway driving, I exited and the brake pedal became extremely hard to press, making it very difficult to stop. No warning lights or messages appeared before the failure. The dealer diagnosed the cause as the brake booster vacuum pipe ex
... ploding internally, which immediately eliminated vacuum to the booster. This created a serious safety hazard and nearly caused a crash. I have since had the part replaced, but the fact that this component can fail without warning at highway speed is alarming. GM has issued TSB 21-NA-107 describing this exact failure mode, which indicates it is a known issue. Hello, I own a 2015 Lexus NX200T. On 9/13/2025 I was pulling into a spot to park, going slow at maybe 4-5 mph, my vehicle suddenly violently accelerates on its own. I had to slam multiple times on the brakes to avoid killing two pedestrians right in front of my vehicle. I was in seconds of killing them and pinning them between the vehicle that was in front of me. I did everything in my power to ke
... ep hitting the breaks as hard as I could until it stopped within inches of the two pedestrians. This was extremely dangerous and could have killed a mother and daughter in that split second. This appears to be a case of unintended acceleration, which I understand is a serious safety issue. I've been told by both the dealership and Lexus that because no damage occurred, it's not their problem. But this is clearly a safety defect, not just a maintenance issue. I would like Lexus to take this very seriously. I was shaken and very scared. I have NEVER had this happen before. Lexus suggested that the floor mats may have got caught - NO absolutely not. I have the mats locked in place with clips. I did not hit the accelerator instead of the breaks as that is what is assumed. No external damage occurred, but the unintended acceleration was violent. This incident took control away from me. Even though no accident occurred this time, the potential for injury or death is very high. I contacted Lexus and the dealership, but both declined to inspect the vehicle, stating it was not their issue because no damage occurred. I believe this a very serious safety defect that should be investigated. I am requesting NHTSA to investigate this potential defect in the Lexus NX200T model. If other owners experience the same, this could indicate a widespread safety problem. I was very lucky I did not kill anyone; it would have changed the day's events forever for the lives of two innocent people and myself. I don't feel safe to drive the vehicle after this incident in fear that it could happen again.Read more

This all started when I purchased a 2018 Honda CRV from Motorworld in the late months of 2021 used. When this issue started happening, Motorworld ignored me when I was asking questions. Then, when I brought it in for service, they used tactics like "if we don't find anything, we will have to charge you a service fee" or call me the next day and say "before we hook it up, it may be your easy pass".
... They took the car in after I had complained about going down [XXX] , and with nothing in front of my car, it slammed on the brakes going from 40-0mph. When they got done, I went to pick up the car, and they told me it was the aftermarket windshield, but it was the one I bought with the car. I called Honda, they made Motor World take it in, and then called me and told me that they have the information and would share it with the appropriate parties. It has continued to happen, and I could get killed if it happens to me. I called to give Honda a more detailed recent incident, and the lady was like They closed this case. I have been waiting for a solution, and Honda just closed the case. I have been worried about the family's safety for 4 years. When it happened the brake warning comes on and the car just stopped. Please dont let them hide this issue. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more
