After starting to accelerate from a stop light ,the Dashboard had a warning message that the accelerator and brake pedals were pressed simultaneously. The acceleration stopped and the car slowed automatically. After a few seconds the warning indicator went away and the car could accelerate. The cars behind my auto had to stop suddenly. At the time. I though that I might have hit both petals at the
... same time. When it happened again I opened an incident report with Toyota. Incident opened on 07-19-25 Case # 250719000440. The Toyota rep told me it should be take to a Toyota dealer to diagmose any error codes. The next time the same problem occured I took it to my Toyota dealer and was told there were no error codes to indicate a problem. I have also taken the car to a local repair shop after the problem happened and was shown there were no stored errror codes. The dealer wants to have my auto for an extende time to reproduce the problem. If they need error codes to solve the issue but no error codes are recorded when the problem occurs what can they determine? I am hoping that there are other Toyota Avalon owners which might have had the same issue.Read more

While driving on U.S Highway 521 in South Carolina, the vehicle experienced SUDDEN, UNEXPECTED BRAKING AND VIOLENT SHAKING even though there were no cars or obstacles directly in front of me. The sensation was so strong it felt as if the car had been rear-ended, but there was no impact. The issue has happened MULTIPLE TIMES since I purchased the vehicle new in 2023. Each time, traffic was flowing
... normally, and no immediate hazard was present. The closest car in front of me was several car lenghts away, yet the PILOT suddenly braked on its own. The problem appears to be linked to the COLLISION MITIGATION BRAKING SYSTEM or related driver-assist sensors, which seem to be misreading conditions and triggering false emergency braking. This defect creates a serious SAFETY HAZARD. The abrupt, forceful braking puts me and my family at risk of being rear-ended by following vehicles. It has also caused panic and near-miss incidents in traffic. I am filling this complaint to ensure it is formally documented and investigated as SAFETY DEFECT. This is a serious risk to DRIVER AND PASSENGERS and could lead to accidents. I am requesting NHTSA to investigate whether this defect affects other HONDA PILOT vehicles.Read more

My son was driving the vehicle down the interstate and attempted to exit. He hit the brakes and the car would not slow down. The vehicle went off the exit ramp, through a grassy hill area, across the entrance ramp, and finally stopped when the tire broke off. Even with the tire broke off and the vehicle not moving

The entire front end of the vehicle has been replaced, including the frame, and yet no airbags were deployed. I did receive and ABS failure alert on my app but was told that was because of the tire breaking off and not what caused the accident. I have requested the vehicle be inspected since the accident occurred. Insurance stated it would need to be repaired before an inspection could occur. I have filed a case with Stellantis who now states they will not have anyone look at it because it has to be done before repairs are made and since insurance has paid for the repairs, they have to request an inspection. We had multiple issues with the TPM system not working properly and had it to a dealership for repairs, that was the only warnings we received. The vehicle is still sitting at the dealership/collision repair shop as they feel it warrants further investigation as well.Read more

I own a 2018 Chevrolet Equinox with a 1.5L engine. While driving with my children in the vehicle, the car experienced a sudden and frightening failure. The engine began malfunctioning, and after inspection, my mechanic and the repair shop confirmed damage to the vacuum pump, broken engine pieces, and issues with the timing chain and camshaft. These issues are identical to those described in an ex
... isting recall for this vehicle, but my VIN is not included. The shop even questioned why my mechanic had to replace the vacuum pump, which shows this failure is directly related to the known recall problem. This situation created a very unsafe condition while my kids were in the car. It is unfair and dangerous that my VIN is excluded when the exact same recall defects are present. I am requesting that my vehicle be included in the recall and that NHTSA investigate this defect further, as it poses a serious safety risk for me, my family, and potentially other drivers.Read more

While driving, the brakes completely failed without any warning. There were no signs, alerts, or symptoms prior to the failure. The braking ability was suddenly and entirely lost, creating an extremely dangerous situation. Upon inspection, it was determined that the brake booster had failed. This component failure directly caused the complete loss of braking power. Because this occurred without w

I was within the town limits so I was driving slowly. When I approached the first stop light in town I saw that it was red. There were of three or four vehicles stopped at that light. At that point I was probably only going 5mph. I took my foot off of the gas pedal and put my foot on the brake to make a slow stop. Instead, the car accelerated rather quickly. I continued to brake but the car
... seemed to speed up. I was able to avoid hitting the truck in front of me by going around it. No vehicles were coming towards me but there were some stopped at the other side of the light. I drove up on the sidewalk to avoid hitting anyone. At that point the front passenger side of the car hit & snapped a pole. The car did not stop immediately upon impact. It stopped acclerating and came to a stop about 10 feet past the pole. I am not sure how fast the car was moving when it hit the pole but I was only going about 5 mph when I put my foot on the break. I would estimate the distance my car traveled from the moment I put my foot on the break until the car stopped was approximately 100-120 feet. It felt like the car went into the cruise resume mode and was trying to accelerate to a predetermined speed. I had not been using the cruise control that day but that is exactly what it felt like. There was one other time this year that the car lunged forward without using the gas pedal. It stopped quickly for me and I just assumed that I had hit the cruise resume button, although I also hadn't been using the cruise that day. The insurance company took the car to an impound lot two hours away. I am sure there has been no inspection and my insurance has not informed me of the status of having my car totaled or repaired. Because of the age of the car, and damage, they think it will be totaled. Yes, my safety and the safety of anyone near me was at risk. The car malfunctioned and the fact no one was hurt was miraculous. There were no warning messages before this accident.Read more

I purchased my 2014 Ford Edge in December 2023. March 2023, I started having issues with the driver side door lock sensor malfunction as shown in the last photo. The door would not automatically lock when I started driving, sensing that the driver’s side door was still open. It was not. I kept pushing the lock button to lock it, but it kept unlocking even while driving. Eventually it would lock.
... The cost to fix that was around $600 which I could not afford, and my limited warranty expired. July/August 2025 my ABS and traction control sensor lights were lit on my dash. I took the car for service and my very reputable mechanic diagnosed with no codes showing up a few times. Performed a reset twice and the lights came back on after only driving a couple of miles. He contacted Ford who advised he couldn't sell the part because there was a "national hold" on the part. On 9/2 my brakes and traction control failed resulting me rear ending another vehicle. The other safety issue is that my airbags did not deploy upon the collision. I contacted Ford Corporation and was advised that Ford will not do anything since my vehicle was older than 10 years. The vehicle is a total loss leaving me with a loan balance of approximately $4,700.00. This is neither fair nor right for a multimillion-dollar company that will not or choose not to address or acknowledge their responsibility regarding these two very serious safety issues. Updates to the case#11679471. Since my initial report, Jeep arranged for an independent technician to retrieve and review the Event Data Recorder (EDR) from our vehicle. While Jeep has not shared the expert’s conclusions with us, we obtained a copy of the raw EDR data file from the same tech (attached). Based on the EDR data: 1. The brake pedal was fully applied approximately 0.4 seconds bef
... ore impact and remained depressed. 2. The vehicle’s deceleration was only ~0.5 g, significantly below the expected ~0.8–1.0 g for dry asphalt conditions. 3. The ABS did not engage, indicating that hydraulic brake pressure was not reaching the level required to approach wheel lock. 4. The Automatic Emergency Braking (AEB) system was “On” but did not engage. 4. The Advanced Brake Assist (ABA) activated only ~0.3 seconds before impact, too late to reduce the stopping distance. 5. No DTC (diagnostic trouble codes) were recorded at the time of the event. These EDR findings indicate that the driver executed proper emergency braking, but the brake-by-wire system did not deliver sufficient hydraulic pressure to achieve normal deceleration. This may suggest a potential electronic or hydraulic brake system defect. The vehicle remains undrivable, and we have been declined by two separate Jeep dealerships to obtain an effective inspection or repair. Given the direct connection to FMVSS 105 (Hydraulic and Electric Brake Systems) requirements for service brake performance, we respectfully request that NHTSA treat this case as an urgent safety concern.Read more

Since early ownership, my Palisade’s ABS activates improperly on bumpy/uneven surfaces like railroad tracks, construction zones, causing brake pedal pulsing, vibrations, and significantly longer stopping distances. Braking feels ineffective, as if the brakes “don’t catch,” unlike my wife’s new SUV, which brakes normally on similar surfaces. This occurs at low to moderate speeds (10-40 mp
... h) on dry, rough roads like potholes or gravel, but not on smooth pavement. I believe the ABS/traction control system misreads wheel speed, reducing braking efficiency and posing a safety risk. The issue occurs 2-3 times weekly crossing railroad tracks near my home and daily on my highway commute through construction zones. I must maintain extra following distance (4-5 car lengths at 40 mph) to compensate, which is challenging in traffic. This defect nearly caused two collisions. In both, braking on rough surfaces at ~30-40 mph (with 2-3 car lengths ahead) extended stopping distances, forcing me to swerve into the breakdown lane to avoid rear-ending vehicles. This endangers my family and contradicts Hyundai’s safety ratings, a key reason for my purchase. I reported this to Branfon Hyundai in East Haven, Connecticut at my 7,000-mile oil change around November 2024 and was verbally told it was fineand again at 14,000 miles and asked them to document it April 2025. Both times, they documented my complaint but claimed it was “normal ABS behavior” and felt typical, offering no repairs or diagnostics. After another recent incident today I research the issue and the [XXX] v. Hyundai lawsuit (filed May 2025, CA) confirm similar ABS issues in 2023-2025 Palisades. Please investigate this ABS defect for a recall to fix braking performance. My family relies on this vehicle’s safety and was the main reason I bought it and most people who have families are sold at this safety award. I can provide service records or videos if needed. Henderson Dealership: Scheduled inspection with Mercedes-Benz of Henderson. Technician inspected my ML350 at my home, confirmed the brake booster issue, and arranged for towing. Improper Repair Demands: After holding my vehicle two weeks, Henderson dealership issued an $8,200 estimate for unrelated repairs (tires, control arms, starter). On Saturday, August 9, 2025, my brakes failed at highway speed. My vehicle's stopping distance was severely increased, forcing me to perform an emergency swerve into the leftmost lane to avoid rear-ending a truck. The maneuver was extremely hazardous, as it occurred in a construction zone with concrete barriers and no shoulder, leaving no safe escape path and endangering myself and others. I conf
... irmed the cause was a catastrophic failure of the four-way brake line union block due to severe corrosion. There were no warning lamps or symptoms prior to the incident; the failure was sudden and total. I found that this defect is difficult to spot during inspections because the part is hidden by a plastic panel. The failed union block and corroded lines were removed and are available for inspection upon request. My research shows this is a widespread issue and the exact failure mode covered by Subaru Recall WQK-47R for 2005-2009 models. This known safety defect persists in the 2010-2014 generation, which is not covered by the recall. Prohibitive dealer repair costs are forcing unsafe workarounds. This defect presents an unreasonable risk to public safety, and I urge the NHTSA to investigate.Read more
