Before, I was driving and occasionally the traction control light would flash when I put my foot down on the brakes but would go away when released. I didn't think much of it but then a few days later, I went to brake to stop the car and then the abs, traction control and brake system lights lit up on the dash and wouldn't go away. I pulled over safely and turned the car off and on again but it wo
... uldn't go away. The brakes were also noticeably more firm compared to before as it was quite squishy but now it was solid and all regen braking went away and it appeared I had no more ABS which could lead to a collision. I immediately took it into the dealer to have it diagnosed. I did some reading and if I kept driving with this problem, eventually the brakes would fail and collapse completely and leave me unable to stop the vehicle. At the service center, they were able to read the code DTC C1246 and clearing the codes did not work. The part that was replaced was the brake booster and the brake master cylinder and the brake pedal stroke sensor. It appears that a warranty program exists for this issue but it is not a recall. I am also not sure if the part that was replaced is a revised part that will no longer have this issue or there is no change in the part design. I believe this is a massive safety issue and should be a recall and not a warranty replacement program as every prius should be recalled for this defect. I was very close to being out of warranty and if I had taken any longer to bring it into the dealer I would have been out of the extended CSP warranty. Even the dealer initially thought I would be out of warranty but I had to argue that I was in warranty and should be eligible for a warranty replacement and that I should not need to pay for a diagnostic fee despite it not being a recall.Read more

What component or system failed or malfunctioned, and is it available for inspection upon request? The defects involve multiple electronic control systems, including steering, braking, and airbag modules, which continue to generate active and stored Diagnostic Trouble Codes (DTCs) even after the May 2, 2023 recall repair. The vehicle remains in our possession and is available for inspection upon r
... equest. 2. How was your safety or the safety of others put at risk? Because the vehicle fails to illuminate dashboard warnings, we were unaware of critical malfunctions occurring in real time while driving. This creates the risk of sudden failure in steering, braking, or airbag deployment, directly endangering both occupants and the public. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. Independent diagnostic scans repeatedly confirmed active and stored DTCs following the recall repair. These findings show the defects are reproducible and ongoing. 4. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others? The vehicle was inspected by Kia’s authorized dealer, Spradley Motors in Pueblo, CO, for the recall repair. Since then, diagnostic evaluations have been performed by independent service providers. Documentation of these inspections is attached. 5. Were there any warning lamps, messages, or other symptoms of the problem prior to the failure, and when did they first appear? No dashboard warning lamps illuminated, even though faults were recorded internally by the vehicle’s control modules. The absence of warnings despite active codes is itself the central safety issue. 2012 Toyota Prius. The brake system malfunctioned and my daughter was almost in a wreck. The ABS, traction control, and brake warning lights on the bashboard all illuminated at the same time. The brake pedal feels soft and is less responsive requiring more effort to stop the vehicle. The brake actuator assembly is making a hissing noise. Ran a ABS test using a scan tool and received a C1391
... error code. The Code points to a leak within the brake actuator system. Upon research I have found that this is a very common and costly repair that Toyota has known about for many years. Toyota issued a customer support program related to this issue that ran for 10 years or 150,000 miles from the vehicle's date of first use. This car, being a 2012, is beyond the support program. I contacted Toyota, hoping they would do the right thing and cover the repair. I was told that they would not cover the repair or supply the parts to cover the repair. I told the Rep. for Toyota that I did not feel like it was fair for Toyota to continue to put millions of drivers at risk for a product that they clearly realize for years now is defective and could hurt or kill someone. The code and the braking issue did not appear until after the customer support program ended and therefore we had no warning or way to anticipate this braking issue or the danger to my daughter and anyone else riding with her in the car or on the roadway near her travel routes. I traveled 300 miles, rented a Uhaul car tow trailer and brought the car back home. My mechanic confirmed the code C1391 and the fact that the brake actuator has failed. The mechanic has ordered the parts (the same defective parts) to replace the broken ones. Toyota is still selling the same part as a replacement part clearly knowing that it is not safe and no warning came with the replacement part explaining the potential for a braking issue. Based on internet research this same part is in many 2005 to 2013 Toyota and LexusRead more

ABS system failing
Incident date: Aug 10, 2024
Upon inspection, our mechanic determined that the master cylinder assembly and the actuator assembly require replacement (error code C1391). It is currently available for inspection, but will need to be repaired soon. Our safety and the safety of others is at great risk, because there is impending brake failure if driven and the end result could be catastrophic. On 09/13/25, the BRAKE, TRACTION
... CONTROL, ABS and ((!)) lights came on about 10 minutes from home. We drove home, parked the car and did not drive it again until we took it to our mechanic on 09/15/25. During this incident, the brakes failed to respond properly, which could have led to a serious accident. We took the car to a local mechanic and repairs are currently pending. The estimated cost for the necessary parts is approx. $3,000 and the labor costs are estimated to be between $1,100 and $2,000. Our local Toyota dealership was also contacted and they confirmed the presence of code C1391. Please reference Toyota Service Bulletin POL18-03, which addresses issues related to the braking system in certain Toyota models, including the Prius V. This bulletin indicates that various brake system-related warning lamps may illuminate due to internal malfunctions within the brake system, and it outlines affected parts that may require replacement. Additionally, please reference Toyota Customer Support Program 20TE07, which provides extended coverage for certain brake system repairs. It is concerning that, despite this well-known, very serious problem, Toyota has not issued a recall. While our vehicle currently has 116,975 miles, we are aware that the warranty period ended 10 years from the first date of use or 150,000 miles. However, this malfunction not only endangers our safety, but also that of other drivers on the road. We urge the NHTSA to investigate this matter thoroughly and consider the implications for a potential recall for this model, because Toyota is well-aware of this issue.Read more

After a heavy rainstorm in [XXX] , my 2005 Land Rover LR3 experienced catastrophic water intrusion through the A-pillar design flaw documented in Land Rover’s own TSB LA501-007. When I opened the passenger door, ~1.5 inches of water poured out. Immediately afterward, the vehicle developed multiple failures: flickering lights, radio inoperative, driver’s window stuck, ABS warning lights illumin
... ated, “Transmission Fault” error displayed, and the air suspension automatically lowered into its fail-safe mode. This is not wear and tear. Land Rover has acknowledged the defect internally (A-pillar locator holes allowing water ingress into the footwell), but never issued a recall or owner notification. Flooding the footwell shorts the electrical/fuse boxes, causing cascading failures in braking, suspension, powertrain, and electrical systems. This presents a serious safety hazard as the vehicle may lose proper braking, suspension height, or drivetrain function simply from exposure to rain. Ford Edge 2012 Limited 2.0L [XXX] The brake pedal went all the way to the floor while my wife was driving a few times. The brakes DID NOT STOP OR SLOW THE CAR AT ALL! She pumped the brake pedal and braking came back. But, after that we could not trust the brake system. We paid AutoNation Ford in Fort Worth, Texas $2,694.45 on 10/11/2023 to replace the ABS system, after their recommendation that
... this will fix the problem. This did fix the problem until recently this past week on 9/21/2025, when the brake pedal went to the floor without any braking, just continued to move forward in our driveway. The next push on the brake pedal did provide braking but now seems a little soft. My wife’s sister also has a Ford Edge with similar or the same symptoms, a 2013 Edge. My daughter has a 2012 Ford Fusion with similar or the same symptoms. This is a very dangerous SAFETY PROBLEM with these Ford cars! I suspect that this problem is more widespread than just my family, more Ford Edge vehicles with this ABS problem. A Ford Edge ABS RECALL might be required to help Ford customers be safe! The car made a sudden and very hard brake in the middle of the street at around 40 - 50 miles per hour, on a empty road.
Incident date: Jul 1, 2025
The contact owned a 2025 Nissan Pathfinder. The contact stated that the ABS warning light had illuminated. The vehicle was taken to the dealer, where it was diagnosed that the ABS system needed to be repaired. The vehicle was repaired; however, the contact believed that the ABS system was not calibrated due to the ABS warning light re-illuminated. On another occasion, the contact stated that while
... driving approximately 70 - 75 MPH, a vehicle in the middle lane switched to the last left lane, and the contact maneuvered the vehicle to the middle lane due to the collision avoidance alarm activating. The tires spun out of control, and the front brakes failed and locked. The vehicle skidded and crashed head-on into a semi truck. The air bags did not deploy. The vehicle was towed and deemed totaled. The contact sustained a traumatic brain injury (TBI), and medical attention was required. A police report was filed. The manufacturer was not made aware of the failure. The failure mileage was approximately 5,000. Read more

1am filing this complaint regarding my 2016 Ford Edge (VIN: [XXX] ), which has experienced a severe and dangerous failure. Ford has acknowledged a defect in the flexplate at the transmission bellhousing area under Customer Satisfaction Program (CSP) 22N12. This defect causes rattling noises and eventually results in flexplate cracks that can lead to sudden loss of power while driving. My vehicle
... is currently undrivable due to this defect. I have documentation from February 2025 (A&M Auto Repair, Invoice #[XXX]) showing that a technician identified noise from the rear of the engine/front of the transmission area and suspected flexplate cracks at that time. Despite this, my vehicle never received the CSP repair. Ford is refusing to cover the repair because my vehicle has exceeded the 120,000-mile cutoff for CSP coverage. However, this is not acceptable because: •The defect was documented earlier and existed within the coverage period. •More importantly, this is a serious safety defect that can cause sudden power loss while driving, creating a high risk of accidents. •Ford has incorrectly limited the remedy under a CSP with a mileage cutoff, instead of issuing a proper safety recall without mileage limits. Requested Action: I am requesting NHTSA to investigate and require Ford to: 1.Treat the flexplate defect as a recall, not a CSP. 2.Provide full repair coverage for affected vehicles regardless of mileage. 3.Ensure consumer safety is not compromised by arbitrary mileage cutoffs. In addition, my vehicle has also experienced repeated brake warning lights, which aligns with Ford’s brake hose recall (NHTSA Recall 25V544). The combination of braking issues and flexplate failure creates an extremely unsafe condition. Thank you for your attention to this urgent safety matter. rear brakes were worn only after 13 mths of driving a brand new range rover and that was driving slow and never pushing the car. Out of the 20+ vehicles i've owned in my lifetime i've never seen a vehicle requiring new brake pads at 10-15k miles. Basically, we need to replace the brake pads every year with the annual service which is unacceptable and clearly faulty.
