On Saturday, April 18, 2026, I was driving home from a neurology appointment with my two daughters, including my child with special needs. I intentionally chose not to take the expressway and instead drove a lower-speed route home for safety reasons. While traveling approximately 35 mph, the vehicle suddenly displayed an “Engine Power is Reduced” warning. As I attempted to safely maneuver thr
... ough traffic and pull over, the brake pedal became extremely stiff and difficult to depress, requiring significant physical force to slow and stop the vehicle. The sudden reduction in braking assist created a serious safety hazard for my family. Had this occurred at highway speeds on the expressway, the outcome could have been catastrophic. A post-incident inspection identified failure of the vacuum pump drive assembly and related timing/variable valve timing (VVT) components. Photographs taken during disassembly document the condition of the timing chain, camshaft, VVT actuator/phaser, vacuum pump assembly, and associated components at the time of repair. Diagnostic records also documented DTC Code P1101 following the incident. The vehicle is currently undergoing further inspection and diagnostic evaluation at a certified Chevrolet dealership to validate CPO warranty coverage and determine the full extent and cause of the failure. Based on the sudden loss of engine performance and reduction in brake assist, I believe this condition represents a significant safety risk to vehicle occupants and the public.Read more

I have had this vehicle since about August 2025, the brakes and wheel bearings have failed twice, the strut failed, the power steering system has messed up repeatedly, the coolant came FLYING out of the car (while parked) FOR ABSOLUTELY NO REASON! There have been multiple warning lights on since I got the vehicle and I have been told they are just sensors from multiple mechanics and that they don'
... t mean anything. As it relates to the issues, the brakes, calipers, and wheel bearings have given out twice WHILE DRIVING without any sort of warning up until they seized! The strut on the front passenger side has broken for no known reason and the power steering system has been non stop eating power steering fluid with no leaks. Since I have gotten this vehicle, I have put OVER 7,000 dollars worth of work into it. This went to fixing the brake calipers, brakes, rotors, all the steering mechanisms (tie rods, sway bars, etc.) and wheel bearings twice, trying to figure out why coolant flew out of the vehicle for no known reason, trying to figure out where the power steering fluid is going, all to no avail. I have had multiple mechanics look at and check all the components of the vehicle, and they can't even figure out what is going on other than things just fail without warning non stop. There is something wrong with these vehicles, EVERY SINGLE OWNER OF A 2011 FORD FLEX THAT I HAVE SPOKEN TO HAS SAID THE SAME EXACT THING, THEY ARE DEALING WITH THE SAME EXACT ISSUES AS I AM, AND NO ONE KNOWS WHY! THESE VEHICLES NEED TO BE RECALLED AND A CHECK FOR THE PRICE OF A PERFECT 2011 FORD FLEX GIVEN TO EVERY 2011 FORD FLEX OWNER, THEN THEY NEED TO BE TAKEN OFF THE STREETS AS THEY ARE A DANGER TO THE PEOPLE DRIVING THEM AND ALL OTHER DRIVERS AS WELL AS PASSENGERS IN THE FLEX AND ANY OTHER VEHICLE! This was very unnerving to me, and I don't see how I can drive the car without some corrective measures taken. I was fortunate that these incidents happened where no person or other car was nearby, but that was only luck. 2015 Hyundai Santa Fe Limited, 3.3L Lambda II GDI V6, ~102,000 miles. In December 2022, an authorized Hyundai dealer replaced three specific parts under a Hyundai recall/campaign (records requested). On Sunday [DATE] at approximately [TIME] PM, I was driving on [FREEWAY], [STATE], at 55–70 mph at night. With NO prior warning — no warning lights, no chimes, no loss of power buildup, no knocking
... , no engine protection mode — the engine suddenly and completely died. The failure was instantaneous. There was no limp-home mode and no opportunity to reach the shoulder under engine power. The vehicle lost forward momentum and rolled backward in a live highway travel lane, where it remained stranded until a tow truck recovered it. No injuries. The 12V battery and electrical system remained fully active throughout. The instrument cluster was operational and capable of displaying warnings before the failure. None were displayed. Dashboard warning lights illuminated only seconds AFTER the engine had already died — a post-failure consequence of engine shutdown, not a pre-failure warning. An independent mechanic diagnosed a seized engine due to loss of oil flow. No oil pressure or low-oil light had ever illuminated. The vehicle was then transported to the Hyundai dealer that performed the 2022 service; they promised a diagnostic call and have not provided one or any written diagnostic. This failure mode matches Hyundai TSB 24-EM-003H (15yr/150k extended warranty for 3.3L Lambda II engines in 2013–2019 Santa Fe), NHTSA Recall 21V-727 (acknowledging stall risk at highway speed), and the active class action Burns v. Hyundai Motor Company (C.D. Cal. 2025). Hyundai's KSDS is documented to trigger a blinking MIL, audible chime, and reduced-power Engine Protection Mode upon bearing wear. None of these protective behaviors occurred.Read more

Vehicle Information: Year: 2021 Make: Volkswagen Model: Jetta Date of Purchase: March 2022 Approximate Mileage at Time of Incident: 25,000 miles --- NATURE OF COMPLAINT: I am filing this complaint regarding premature and dangerous rear brake pad and rotor wear on my 2021 Volkswagen Jetta, purchased new in March 2022. At approximately 25,000 miles — well below any reasonable expectation of nor
... mal brake wear — I was informed by a service technician that my rear brake pads required immediate replacement due to excessive wear. The front brakes showed no comparable wear, which is inconsistent with normal braking patterns and strongly suggests a manufacturing or design defect. SAFETY CONCERN: Premature rear brake failure at such low mileage presents a serious safety hazard. Degraded braking performance increases stopping distances and reduces driver control, particularly in emergency situations. I was given no prior warning indicators or alerts from the vehicle's onboard systems before being advised the brakes were critically worn. This lack of advance warning compounds the safety risk. SUPPORTING EVIDENCE OF A KNOWN DEFECT: This is not an isolated incident. Volkswagen has already acknowledged premature rear brake pad wear as a systemic issue in 2020–2023 Jetta vehicles by issuing a Rear Brake Pad Limited Warranty Extension, extending coverage from the standard 1 year/12,000 miles to 2 years/24,000 miles. Despite this acknowledgment, Volkswagen has refused to provide adequate and lasting remedy to affected owners, and many consumers — including myself — have been denied reimbursement or further assistance after the extended warranty window expired. Notably, my vehicle reached 25,000 miles just outside the extended warranty coverage limit, yet exhibits the exact defect the warranty extension was designed to address. Multiple complaints filed with NHTSA and reported on consumer safety platforms describe the identical pattern.Read more

I am submitting this complaint regarding a serious brake-related safety issue involving my Audi vehicle after recent brake service performed by an authorized Audi Meadowland dealership in [XXX] . In December 2025, I brought my vehicle to an authorized Audi dealership for replacement of the brake pads. The repair was completed by the dealership, and I trusted that the vehicle was safe to drive aft
... erward. However, only approximately six months later, and after driving only around 1,000 miles since the repair, the vehicle experienced a dangerous brake-related failure involving the brake pad assembly becoming loose/detached while the vehicle was in operation. This incident occurred while my entire family was inside the vehicle, creating a serious safety hazard and placing all occupants at risk. A brake system component should not fail after such a short period of time and limited mileage following dealership service. In addition to the safety concern itself, I am also extremely dissatisfied with the dealership’s handling of the matter. The dealership did not proactively contact me regarding the repair status, and multiple attempts to contact them by phone went unanswered. I believe this issue may involve improper installation, defective parts, insufficient inspection procedures, or other serious safety-related service failures. I respectfully request that NHTSA review and investigate this matter to determine whether similar incidents may affect other vehicles serviced under similar conditions. Please consider opening or expanding an investigation into frame, underbody, brake-line, and suspension-mount corrosion affecting Toyota FJ Cruisers. I own a 2013 Toyota FJ Cruiser 4WD. A professional inspection found multiple fluid leaks, heavy rust, rusted exhaust, leaking rear shocks, and brake line/hose concerns. After reviewing the condition, the mechanic essentially declined to repair it and
... recommended scrapping the vehicle. The attached photos show conditions that appear safety-relevant: rust perforation through underbody structural metal near a bracket/crossmember area; severe corrosion around brake lines and fittings; widespread scaling near suspension and frame components; a leaking rear shock; and badly deteriorated exhaust connections. This is not just appearance rust. Also note the contrast between the observable condition of the vehicle versus the problems hidden beneath. This rust is clearly outpacing normal wear and tear. The safety risk is straightforward. At interstate speeds, a corroded brake line could fail during emergency braking. A weakened shock mount, suspension bracket, or crossmember could fail when the vehicle hits a pothole, uneven bridge plate, or highway debris. In dense, aggressive traffic, especially where vehicles travel 70+ mph, any sudden loss of braking, stability, or rear-end structural integrity could cause a spinout, rollover, or multi-vehicle crash. I understand Toyota has previously addressed severe corrosion on other truck platforms. I urge NHTSA to review whether the FJ Cruiser has a similar pattern of premature or excessive structural corrosion. Owners may not know the vehicle is unsafe until a mechanic refuses repair or a component fails. Please review consumer complaints, inspection photos, and corrosion-related failures before this becomes a fatal incident.Read more

The center high-mounted stop lamp (CHMSL / third brake light) fails on 2021 and newer Tesla Model S vehicles (including Plaid). Pre-2021 CHMSL units are incompatible and will not fit. Repair requires removal and replacement
... of the entire rear liftgate glass assembly, which is extremely expensive and labor-intensive for owners. Prior model years did not experience this problem. A non-functional CHMSL reduces the vehicle's visibility to following traffic and increases the risk of rear-end collisions, especially at night or in poor weather. The front and rear brake system has repeatedly malfunctioned across at least 7 dealer visits since 2022 on my 2021 Ford Bronco Sport (VIN: [XXX] ). Brake squealing and grinding (particularly in reverse at low speeds) have persisted despite multiple repair attempts. Ford's own SSM 52515 acknowledges engineering was working on a fix and that rotor resurfacing is "not a guaranteed fix." TSB 23-2308
... was performed in March 2024 (caliper bushings/guide pins replaced). In June 2024 a dealer verbally confirmed a brake recall was coming in Oct/Nov 2024 and that rotors would be covered — that recall never materialized. Front brake pads were replaced under Recall 21C31 in September 2022 at ~8,797 miles. Front pads and rotors required full replacement again in April 2025 at ~28,833 miles — just 20,000 miles later and well below the typical 50,000–70,000 mile service life — at my out-of-pocket cost of $747. Rear brakes were documented as "in the red" in January 2026 at ~38,000 miles. The vehicle is currently at a Ford dealership. In August 2024, one dealer attributed the issue to driving habits despite Ford's own TSBs acknowledging the defect. Brake wear across both axles is abnormally premature for a [XXX] vehicle. ... tires unlocked and i was able to drive it again. About 4 days later, while driving 70 mph, the vehicle started to make a grinding noise on the interstate, and I started to slow down, but unfortunately, I was on the left side of the highway on a narrow shoulder, and upon getting to a lower speed while in 2WD, my front axle locked up. I placed the vehicle in park, but due to safety concerns, I put the truck back in drive and tried to switch from 2WD to 4WD to unlock the axle, but unfortunately, the truck would not change gears. I then had to quickly get the vehicle to the other side during a time with fewer vehicles, while the left front tire was locked, and I had to drag it across the highway to ensure my safety. The left front tire remained locked, and after multiple attempts, the brake partially unlocked but continued to make a grinding and clunking noise. After some diagnostics, we determined the left front wheel hub was the issue, overheating and locking up. I reached out to mechanics for quotes on repair of said part, and upon reaching out to the dealer that I have used in the past for recalls and oil changes, I was informed there was a recall on said part/software that I was not aware of; the service rep used the last 8 of my VIN to look up my vehicle for the quote and provided the details about the recall. I use the RAM Guardian App which provides updates about recalls and upon checking it does not show a recall nor does it show a recall when i search my VIN and License Plate on the website. This seems to be a common issue based on customer reviews.Read more

What component or system failed or malfunctioned, and is it available for inspection upon request? Automatic braking system brakes for no reason when I am not near any cars traveling at high speed or even when I am already at a complete stop or moving very slowly but not anywhere near hitting the car in front of me. This has happened multiple times since I leased this car in July 2025 and it seems
... to be getting worse. I had a 45 min drive home today and it happened about 7 times all for no reason. How was your safety or the safety of others put at risk? It slams on the brakes unnecessarily and the cars behind me almost rear end me. This has happened on residential roads and highways. Has the problem been reproduced or confirmed by a dealer or independent service center? I just called to take it in. They seemed to have no idea what I was talking about. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? not yet but I am taking it in asap Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear?
