Subject: Urgent Safety Concern – Immediate Attention Required Dear To Whom it may Concern, I am writing to urgently express serious safety concerns with the vehicle I recently [purchased/test drove/had serviced]. Seat Belt Malfunction: The children’s seat belts are not retracting properly, posing a significant safety risk. Braking Issues: Despite being told the brakes are at 90%, they feel
... like they are grinding metal. This is highly concerning. Alignment Problem: The vehicle is veering aggressively to the right during normal driving, which indicates a serious alignment or suspension issue. During the test drive, I was told the alignment would be addressed before delivery. However, the vehicle still exhibits unsafe behavior. Additionally, the braking condition does not align with the claim that the brakes are at 90%. These are critical safety issues that must be addressed immediately. I am extremely concerned about the roadworthiness of this vehicle and request a full inspection and immediate resolution before any further use. Please respond as soon as possible to confirm that these issues will be addressed without delay. Sincerely, [XXX] [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

My 2021 Toyota Highlander Hybrid has a braking issue identified in Toyota Service Bulletin T-SB-0059-23, Hybrid Regenerative Brake Transition Feeling Improvement. When going downhill and applying the brake, the vehicle lurches forward temporarily as the braking system transitions from regenerative to hydraulic braking. The first time my vehicle did this, it was very alarming and I had to take addi
... tional measures to slow my vehicle as I was driving downhill. My vehicle frequently demonstrates this problem, however, I did not report it to my local Toyota dealer because I read in an online chat group that it was a characteristic of this vehicle. Although the TSB recommends a Reprogram of the Brake Actuator Assembly Control Module, the repair is only covered under the Toyota Basic Warranty which is in effect for 36 months or 36k miles, whichever occurs first. My vehicle is past the warranty limit, and because I did not report it to my dealer previously, this repair is not covered under the TSB or the Toyota Basic Warranty. Toyota Highlander Hybrid model years 2020-2022 are affected. The Technical Service Bulletin was issued August 29, 2023, so if a 2020 model Highlander Hybrid has this problem, 2020 models would have already passed the warranty limit by the time the TSB was issued and thus would not be covered. This is a safety risk as drivers can lose control of the vehicle when braking, particularly when going downhill. The Warranty should be extended, or this problem should be elevated to a RECALL so that older affected vehicles, or those with higher mileage can be covered under warranty. Drivers should not have to pay for a recognized problem that affects the safety of this vehicle and has been identified in a Technical Service Bulletin. This problem puts drivers at risk of losing control of the vehicle and the problem can be mediated with a software reprogram. T-SB-0059.23 should be elevated to a RECALL, or extended regardless of age or mileage.Read more

I have had multiple issues when it comes to braking on uneven roads, going over railroad tracks, and going over speed bumps in parking lots. I have now taken my vehicle in to 3 different dealers to have it inspected and have had the issue replicated several times by a 1 of the

The service manager from the dealer i originally bought the vehicle from refused to even inspect the vehicle and told me the only way they will even look at it was if I brought a video showing the issue of the vehicle lunging forward into oncoming traffic. They also stated that this wasn't a warranty issue and that I would be billed for it. The vehicle is brand brand new 2025 Palisade(2 months old) with 2,000 miles. I was advised to open a case with Hyundai and was then told by the case worker that this is how the vehicle is supposed to operate and there are no issues. After further research to see if anyone else has reported this issue, I found 2 class-action lawsuits filed and over 100 complaints filed with the NHSTA as well. Despite applying the emergency parking brake, the vehicle continued to roll on its own, presenting a serious risk of collision or injury. There were no warning lights, messages, or alerts prior to or during the failure. I had no indication that a critical fault was occurring. The vehicle was later towed to a dealership. Upon inspection, a certified JLR technician discovered faulty wiring in the gear shifter assembly, which had caused a short circuit that disabled the gear selector, the Transmission Control Module (TCM), the Engine Control Unit (ECU), and the gearbox inside the transmission.Read more

On three occasions my car has initiated automatic emergency braking while I was driving across the same grated bridge deck (Montlake Bridge in Seattle). I did not receive recall notices until after both of the accidents occurred, which demonstrates a systemic failure by the manufacturer, Ford. This negligence is not only irresponsible—it violates basic safety obligations to consumers and the public. I request immediate action from NHTSA to investigate this matter and enforce compliance. A description of the incidents and associated recalls that have incurred are described on the following page. Additionally, just last week while driving on a rural road, my vehicle’s transmission unexpectedly shifted from standard Drive into Sport mode (“S”) on its own without any input from me. I am filing this complaint regarding a serious and ongoing safety defect in my 2015 Ram 1500 involving the failure of the Anti-lock Brake System (ABS) module (Part No. 68228991AD). This failure completely disables the ABS, significantly reducing the vehicle’s braking performance and increasing the risk of loss of control or collision during emergency braking situations. This is a well-documente
... d and widespread issue among Ram 1500 owners. Chrysler has discontinued the manufacture and supply of the affected ABS module, leaving owners without a viable means to restore this critical safety system. I have been placed on a waiting list for over two years for a replacement part, yet I have been informed repeatedly that there is no assurance one will ever become available. The dealership has even advised me to “buy a new truck” if I wish to have properly functioning anti-lock brakes. This situation is not only unreasonable—it is dangerous. The lack of a functioning ABS system places my family, my passengers, and others on the road at significant risk. The inability to obtain a replacement for a vital safety component effectively renders the vehicle unsafe for operation. I am aware that Ram recently issued a recall affecting approximately 1.5 million trucks for the same ABS defect. However, my model year has been excluded, despite exhibiting the identical failure. This selective recall leaves hundreds of affected consumers without recourse or protection. I respectfully request that Chrysler and the relevant regulatory authorities investigate this matter, extend the existing recall to include the 2015 Ram 1500, and compel the manufacturer to resume production and distribution of the necessary replacement parts to remedy this critical safety defect.Read more

My 2024 Volkswagen Atlas SE began collecting visible moisture inside the rear tail light assembly less than 18 months after purchase. Over time, this led to intermittent lighting failure, sometimes the lights function, sometimes they don’t. This is a safety concern, especially at night or in poor weather. The bulletin calls this “normal condensation,” but this is beyond that there is visible water pooling and electrical malfunction and I know of others who report the same thing. VW is down playing this to avoid the recall. Despite being under extended warranty, this is not covered because VW considers it cosmetic unless there’s proven seal failure. The vehicle is available for inspection and has been evaluated by a certified VW service center. Volkswagen needs to address this as a design or sealing defect, especially when it affects vehicle safety systems like brake and signal lights.Read more

Both times it was sunny, dry weather, and the road conditions were normal—no rain, no slick surfaces, and no obstacles in front of me.
