Hello, I am formally documenting a serious safety incident involving my recently purchased 2026 Dodge Ram 3500 Tradesman 6.7L dually, purchased from Towbin Dodge in [XXX] , on May 9, 2026. ... a pickup when a critical brake failure occurred. While descending the mountain grade, the truck’s braking system failed to respond. A warning light appeared on the dashboard indicating an issue with the electric brake system. We had tow/haul mode and exhaust braking engaged, but neither system functioned properly. When my father pressed the brake pedal, the vehicle did not slow down. With a steep descent ahead and no effective braking response, my father was forced to steer the vehicle off the road into a ditch to avoid a direct impact with the mountain, which could have resulted in a fatal collision. After stopping, we noticed signs of extreme brake overheating, and the system appeared to be close to catching fire. I have video and other documentation of the incident. The vehicle is currently in the dealer’s possession. This is a major safety failure occurring within days of purchase, and I am requesting a full investigation into the brake system and all related components, including any recalls, defects, or electronic braking malfunctions. I do not want the truck anymore! Given the severity and risk to life involved, I am requesting immediate action and a full resolution regarding this vehicle. In the service report, Technician stated that the steering has fuse to the sway bar. I am now very concerned the sway bar is unsafe. Mazda did not comment on this when I ask. It appears as the fix one thing more continue to break. with The issues the lane assist continues to have issues seems to struggle with alignment issues. Car was with Mazda for 7 weeks during the 1st year of owning it. I am also concerned with some of the finding as I look at them with details pertaining to original purchase and miles being reported on our documents. Living in Kentucky the Lemon law does help with providing any sort of real action plans on CPO. Break have been squeaky causing uneven feedback. This issue has worsened over time. ... rienced front brake hose failures related to prior Ford brake hose recalls. One front brake hose failed first, and the opposite front side later failed as well. Ford previously reimbursed me for one of those repairs after the recall became officially active. The current rear brake failure symptoms closely match Ford Recall 25S87 / NHTSA Recall 25V544000 involving rear brake hose rupture and brake fluid leakage in 2015–2018 Ford Edge vehicles. Landmark Ford in Tigard, Oregon and the Ford Customer Relationship Center have both confirmed that this repair is tied to the pending “advanced notice” rear brake hose recall affecting this vehicle platform, although the recall is not yet officially active. This failure created a serious safety hazard because the vehicle experienced sudden loss of braking, with no warning, while the vehicle was in motion creating a serious risk of collision, injury, or death to myself, my passengers, and others on the roadway because the vehicle could not safely stop under normal braking conditions. The electric brake booster pump is heard cycling (straining) every 10–20 seconds even when the vehicle is stationary and the brake pedal is not depressed. Loss of Braking Safety Features: The system has disabled regenerative braking and Anti-lock Braking (ABS), causing significantly increased stopping distances and a "wooden," unresponsive brake pedal feel. Predictability of Failure: This failure matches the exact symptoms described in Toyota’s Customer Support Program 20TE07 (and previously ZJB), which acknowledges an internal defect in the brake actuator. Safety Risk: The sudden transition from power-assisted braking to manual hydraulic braking during a drive creates an immediate safety hazard. As the pump motor continues to cycle excessively to compensate for the internal leak, it is at risk of total thermal failure, which would lead to a near-total loss of braking assist while the vehicle is in motion. Additional Notes: Despite the vehicle having only 70,000 miles (well under the 150,000-mile limit acknowledged in Toyota’s support programs), Toyota has declined to provide assistance because the vehicle has surpassed the 10-year age mark. ... get the trailer back to a local 4 Star dealer, and the diagnostic was a failed hydraulic brake pump, resulting in a complete failure of the trailer brakes. The hydraulic pump was replaced, and the brakes worked normally afterward. The trailer was driven back from [XXX] to [XXX] on April 7th and was not used again until May 7th. Thursday, May 7th to Sunday, May 10th The trailer was driven with (4) horses to a horse show approximately 1 hour and 30 minutes away from our PA barn. The trailer performed normally with no issues. The trailer was parked in trailer parking from Thursday evening until Sunday afternoon. Sunday afternoon, the horse trailer was hooked up and loaded with (4) horses, and we started making our way back to our home barn. At the time of departure, the horse trailer was operating normally. After roughly 20 minutes of driving, we came to a traffic light and were unable to stop the trailer; the trailer brakes were not responding. Again, we were very lucky that there were no other vehicles coming in either direction. We decided to return to the horse show and made our way back, with four lanes, at speeds below 25 mph. Upon arrival, we troubleshot the problem and ultimately had a mobile trailer repair unit respond to the location. After their diagnosis, it was determined that the hydraulic brake pump had failed again, leaving the trailer brakes completely inoperable. Highway 89A is a narrow, winding two-lane canyon road with limited shoulder and significant elevation changes. ... en loss of power steering and brake assist on this road presented a serious risk of loss of vehicle control. Fortunately, she was able to get the car stopped off of the roadway. However, she was then unable to place the car into Park or shut off the ignition. Multiple warning lights were illuminated across the instrument cluster. Her roadside assistance plan towed the car to the nearest repair facility, where on Monday, technicians were unable to replicate the issue. A previously stored DTC P3055 was found in the diagnostic system. This code relates to a known and documented issue with Chevrolet Equinox vehicles, for which Chevrolet has issued multiple Technical Service Bulletins, including TSB 21-NA-139, which identifies the Multifunction Power Supply Converter (GM Part #13510484) as the known culprit for this failure mode. Upon bringing this to the attention of the service center in Fort Myers Florida, the service center manager took a drive test with me, and was able to replicate the issue, but immediately started dismissing it as "normal" and within Tesla specs. This is becoming a regular practice at that service center, where they deem warrantable issues as normal and within (their tolerance). I insisted that this issue is not to be taken likely by Tesla, because it is something that involves a safety hazard. Upon further research, I was able to find that this issue is happening to many Teslas model S and X, where the front axles bend due to the weight and high torque of the vehicles. The issue falls completely under warranty and the half shafts should be replaced, but apparently Tesla has instructed their service centers to dismiss and disregard the symptoms, and just tell the customers that no warranty replacement will occur, because it is not necessary, we just have to drive the cars like this, until the car completely fails, or an accident happens because one of the bent axle shafts breaks on a highway during normal drive. I recording this report as evidence that the issue has been reported to the highest vehicle safety authorities, and I expect a corrective action so Tesla can fix my car under the existing unexpired warranty.Read more

My Pathfinder has 19798 miles and I was informed that the brake pads are 3mm or less need replaced and the rotor resurfaced. The front brakes are at 7mm at the same mileage. I do not tow anything and use for city and highway driving. My complaint is that at this rate I will have to change my rear brakes twice as often and have them inspected more often. ... the rate of front brakes is 2x. I am concerned that there is a design flaw that can lead to rear brakes failing if I do not have them inspected twice as often as the front brake wear. Typically the front brakes handle 60-70% of the breaking force due to weight transfer. If the rear brake pads are wearing out before the front brake pads, and the rotors need resurfaced it seems that heat buildup due to increased brake wear is affecting the rotors. While cruising at 36mph, we decided to initiate the cruise control. Upon reaching a slow-down traffic ahead, the brakes were applied to slow the vehicle, however the cruise control did not disengage and the brakes did not stop the vehicle. We saw an opening in between the stopped vehicles and crossed over from right to left lane into the grassy median, hit a ditch, and did a sharp turn into opposing traffic, almost hitting another vehicle as they moved out of way, and stopped the vehicle on the opposite side of the road. When the vehicle hit the ditch crossing the median, the right front tire popped and helped in stopping the vehicle. Four family members including myself were in the vehicle and there were several cars already stopped in the lanes. This incident caused great safety concerns for my family and the vehicles around us. Had we hit the cars while crossing over, it would have caused a chain reaction of "hits" as the cars were already lined up and stopped. We brought the car to the Jeep dealership and they did confirm that the internal valve body failure and TCM fault contributed to improper communication within the transmission. They did replace the TCM and valve body assembly, reprogrammed the module and road tested the vehicle. The auto stop/start warning light came on after the incident. They performed a battery test for the main and auxiliary, and both failed load test and read at low voltages. Both batteries were replaced and fault codes cleared. Upon receiving the vehicle, I tested the cruise control component and it has been working for the past three months now.Read more

On the day of the incident, I was driving my vehicle at a very low speed while approaching a red traffic light. I applied the brakes normally behind another vehicle that was already stopped. It should be noted that it had rained earlier and the road surface was wet. ... cle comes to a complete stop. However, unexpectedly, the vehicle accelerated on its own. I immediately attempted to apply the brakes in order to avoid a collision, but I was unable to stop the vehicle in time, resulting in a minor impact with the vehicle in front of me. I believe there may have been a mechanical or electronic malfunction related to the vehicle’s acceleration, braking, or automatic assistance system. I cannot identify with certainty the exact component that failed; therefore, the specific cause is UNKNOWN at this time. The vehicle remains available for inspection upon request. The situation placed both my safety and the safety of the other driver at risk, as the vehicle moved unexpectedly despite previously being stopped with the brake hold system engaged. At this time, the issue has not been reproduced or confirmed by a dealership or independent repair shop. I am also not aware of any technical inspection performed by the manufacturer. However, the incident was reported to the appropriate insurance company. Before the incident, I did not observe any warning lights, dashboard messages, or obvious signs of a mechanical or electronic problem. I turned the truck off and called the tow. At the dealership they diagnosed and confirmed an electronic brake booster (EBB) failure. This EBB failure mirrors a recent recall from Ford affecting 2025 Broncos. The dealership has refused warranty because I am 6 months outside the warranty period even though I only have 29,960 miles. This is not normal wear and tear. The 2025 recall # is 25V488. This EBB recall may also affect previous years such as mine and should be investigated.Read more

Vacuum pump failed: On May 28th, 2025 the brake pedal on our 2019 Chevrolet Equinox became very hard along with check engine light, a "Reduced Power" notification, and "Brake Assist Failure" notification. Online searches reveal widespread owner reports across multiple model years describing identical failures related to defective brake pressure switches. These failures impact both brake light operation and cruise control functionality. The safety implications are extremely serious. Motorcyclists may unknowingly operate their motorcycles without functioning brake lights, significantly increasing the risk of rear-end collisions. Owners may be completely unaware their brake lights are not functioning. In my case, I only discovered the defect because the cruise control stopped working. Had that not occurred, I may have continued riding without realizing my brake light system had failed. I also found reports indicating that other owners filed complaints with NHTSA and were subsequently contacted regarding dealership repairs facilitated through KTM. This suggests the issue may already be known within the manufacturer and regulatory system. Given the repeated reports across multiple KTM models and years, I respectfully request that NHTSA investigate this defect and issue a safety recall . Brake light failure caused by defective brake pressure switches presents a substantial and potentially life-threatening safety hazard to motorcycle operators and surrounding traffic. ... ported a noise from the left rear; the vehicle was then parked. On May 10, 2026, I removed the left rear wheel and found the left rear brake caliper completely detached from the rear knuckle. Both caliper bracket mounting bolts were missing. The caliper was held only because it had seized on the rotor. Vehicle unsafe to operate. Towed by flatbed to Tesla May 11; available for inspection on request. Properly torqued caliper bracket bolts do not back themselves out in ~800 miles of light use. Realistic causes: (a) bolts installed but not torqued to spec, worked loose in service; (b) bolts missing at state inspection and certified anyway; (c) suspension disassembled during the pre-sale repair, bracket reassembled with bolts omitted. All implicate Tesla's workmanship; (b) and a visibly-loose (a) also implicate the inspection. Tesla previously acknowledged this failure mode in recall SB-21-33-002 ("Loose Brake Caliper Bolts," 2019-2021 Model 3 / 2020-2021 Model Y): caliper bolts "may not be torqued to internal specifications" and "may be loose, allowing the brake caliper to separate." My vehicle is outside that range but the recall shows this is a known Tesla pattern. Risk: caliper detachment can wedge the wheel, lock the brake, damage tire/wheel, boil brake fluid, or break free at speed — any can cause loss of control. Vehicle driven ~800 miles in this condition. After finally picking up the vehicle, I drove and tested it for many miles without noticing any obvious issues. I then parked in front of a shopping center. The vehicle was fully stopped, and I shifted the transmission into PARK. After engaging PARK, I removed my foot from the brake pedal. At that exact moment, the vehicle suddenly and violently lunged forward on its own without any warning or driver input. The vehicle accelerated into a concrete pillar located less than two feet in front of the parking space. The impact was severe enough to deploy two airbags and caused a very loud, explosive sound inside the vehicle. The force of the collision shocked the interior seats and coverings, and I was left disoriented and briefly unconscious. This incident could easily have resulted in a fatality if a pedestrian or another person had been standing in front of the vehicle at the time of the unintended acceleration. I am requesting a full investigation into the cause of this sudden forward movement after the vehicle had already been stopped and shifted into PARK, especially considering the recent repair work involving sensors and collision-related components. When the forward collision warning activates, the braking pedal loses its effectiveness and causes the car to brake much more slowly than it should, which actually increases the chance of a collision occurring. This has happened multiple times to me in the past few weeks. It is a very dangerous problem.
