Today, June 11, 2026, my wife and 9 month old baby were on their way home from running errands in town when the vehicle started to violently shake, becoming frightened my wife tried to apply pressure to the brakes. When she pushed down on the brake pedal nothing happened and the pedal hit the floor. Our brand new 2025 GMC Acadia AT4 had just suffered catastrophic brake failure. So she pulled off o
... nto the side of the road in hopes the uneven terrain would slow her down just enough to put it into park. Luckily enough it did. If it would have been 10 seconds later she would have been on a one way bridge where the speed limit is 60mph with no brakes. Four days prior to today on June 07, 2026 I sent a message to the individual that I bought the car from at the GMC dealership and told him that we had warning lights come up on the dash. He had me try and reset the screen but nothing happened. I asked when we could get it into a shop so it could be looked at he said that he would have someone reach out and schedule a time. I proceeded to ask if we were ok to drive the vehicle until then.To which he responded "If the vehicle isn't giving you issues mechanically, you should be good". Fast forward to today June 11, 2026 my wife is driving and has catastrophic brake failure. The vehicle was then towed by roadside assistance to the GMC dealership.Read more

Component or System: The brake system, specifically what appears to be a false ABS activation or IEB (Integrated Electronic Brake) system malfunction. The component is available for inspection upon request. Safety Risk: While braking at speeds between 25-40 mph, typically on dry pavement and more frequently on downhill grades, the brake pedal exhibits a chugging sensation, erratic stopping behavio
... r, and pedal softening consistent with unintended ABS activation or IEB malfunction. This occurs on dry roads where ABS activation is neither warranted nor expected, creating unpredictable braking response and extended stopping distances. The defect has occurred 24 documented times since [XXX], and existed prior to that date. Dealer Confirmation: Yes. During a ride-along at Edmond Hyundai on [XXX], a technician directly witnessed and confirmed the occurrence. Despite this confirmation, it was not documented on the repair order. Two prior repair attempts returned cannot-reproduce results: March 2, 2026 at Bob Howard Hyundai and March 10, 2026 at Edmond Hyundai. Additional Repair/Inspection Attempts: On April 7, 2026, Norman Hyundai opened and closed a repair order without the consumer's knowledge and refused to provide a copy despite an explicit request. On May 23, 2026, a scheduled appointment at Bob Howard Hyundai was missed; a reschedule request was subsequently ignored by the dealership. HMA Case No. [XXX] is active. Warning Lamps or Prior Symptoms: No warning lamps have illuminated. The defect presents exclusively as a physical sensation—pedal/brake chugging, softening, and erratic stopping during normal dry-pavement braking. The issue has been present since delivery on [XXX]. While slowing to a stop, the vehicle suddenly experienced multiple (3–4) violent jerks. The vehicle fought against my firm brake pedal application, overpowered the brakes, and accelerated uncontrollably, resulting in a crash. During the impact: • The driver frontal airbag and pretensioners deployed. • The driver seatbelt unlatched mid-impact and then locked/retracted in a way that prevente

I have only been provided with the limited Airbag Control Module (SDM) data, which shows the brake switch as “Off” and low accelerator pedal readings. This data does not match the events I experienced. I have formally requested full downloads from all other modules (Powertrain/PCM, ABS/EBCM, BCM, etc.) from both the insurance company (ESIS) and GM, but they have only provided the single SDM report. This appears to be a sudden unintended acceleration event combined with a brake override failure and seatbelt restraint system malfunction.Read more

I would like to create a complaint because my 2023 Toyota Sequoia is not included in the NHTSA recall #25V767 for the V35A 3.4L i Force Max twin turbo V6 hybrid. My Sequoia was manufactured on the same production line as engines that have been recalled for engine debris in the Tundra trucks and Sequoia's. Toyota's reasoning for not including my hybrid vehicle is it's not a safety concern as the hy

CATASTROPHIC SAFETY COMPONENT FAILURE AND INCIDENT.The Antilock Brake System (ABS) Hydraulic / Electronic Control Module suffered an immediate, internal electronic failure, triggering active diagnostic trouble code C2200. ... aused a severe, life threatening incident on a high speed highway in TEXAS. While driving with my children inside the vehicle, the brakes failed to respond normally during an essential braking event, resulting in a near-miss collision and nearly causing our JEEP to roll over due to the sudden loss of electronic stability and traction controls August 2024. The lack of basic safety infrastructure makes this vehicle completely unsafe to operate, posing an extreme risk to my family and other drivers on the road. The component failure and active code C2200 were officially inspected, reproduced, and confirmed by a factory authorized Chrysler Dodge Jeep Ram dealership workshop. However, the manufacturer (Chrysler/Mopar) has placed this critical safety component on an indefinite, long term engineering backlog. The dealership parts department formally verified a national backlog of over 436 unfilled consumer orders for this exact part, stating that many safety compromised owners have been left stranded without a repair for 1 to 2 years! Mind you I immediately called JEEP customer care August 2024 and I am still dealing with them till today! The manufacturer is failing to provide safety critical parts within a reasonable timeframe, forcing consumers like my partner and I to choose between operating a vehicle with a severely compromised braking system or losing our primary livelihood transportation. This massive supply chain breakdown constitutes an ongoing, severe public safety hazard that requires immediate federal investigation and a mandatory safety recall!! What are our rights?Read more

I am reporting a potential safety defect involving unintended automatic emergency braking/forward collision warning/brake-control behavior, reverse automatic braking, parking brake behavior, and driver-assistance/sensor calibration. ... lane was clear; only a car was parked in a driveway on the right. Sudden braking created serious risk of rear-end collision/loss of control. Vehicle used for family transportation; toddler (~30.8 lb) had been in vehicle ~10 minutes before and family includes blind/visually impaired passenger. History of unwanted reverse emergency braking ~every 45 days: Jan 2026 near Crust Pizza Woodforest (backing up) and March 2026 at H-E-B 7988 FM 1488 Magnolia (backing out), jolting child in rear-facing car seat. Prior service at Keating Chevrolet Nov 12, 2025 (including recall/service for Unintended Parking Brake Engagement N252503010 and missing chassis harness plugs update) did not resolve unwanted braking. After June 10, AEB/front pedestrian braking/intersection collision braking were disabled as temporary safety mitigation. Vehicle removed from daily family use, available for inspection, and I request NHTSA review. My 2021 Ford F-150 intermittently displays the warning message “Reverse Brake Assist Not Available – See Manual” when the vehicle is placed in reverse. The dealership confirmed DTC U3000:49 in the
... IPMA module and identified Ford TSB 23-2040 as applicable to the concern. The dealer stated that the repair requires an IPMA software/module update and calibration. The quoted repair cost was approximately $1,100. This appears to be a software/programming-related condition affecting a driver assistance and brake-related safety feature, not normal wear and tear or a failed mechanical component. The warning is intermittent, but it has appeared multiple times. The vehicle is available for inspection upon request. No crash, injury, or fire has occurred. Ford Customer Support was contacted and case number XXX was opened. Ford advised that no active warranty, recall, or customer satisfaction program currently provides repair cost coverage, and that because this involves Reverse Brake Assist, I may choose to file a safety concern report with NHTSA. OnXXX, as we approached what I call the 6 o’clock position of a round-about in [XXX], we had braked almost to a full stop (0-5 mph) to wait on the car ahead of us to enter the round-about when we felt increasing movement. Despite braking, to wait on that car, our car with increasing speed started shoving that car into the circle. Fortunately, he was able to drive away from us, but that car had
... been damaged on the back and right side. With that obstacle out of the way, our car rapidly and exponentially increased speed - all while we were fully braking. Two witnesses said they saw our brake lights as we sped along. We hit the curb as we approached the 12 o’clock exit, and that turned us so that we crossed both lanes of the road and headed towards the far sidewalk and storefronts. When we hit that curb it pushed us diagonally to face the street again. Hard to estimate but we must have been doing 50 mph when we hit a very large brick planter and we finally were wedged to a stop when we slammed in between two vehicles parked parallel to the sidewalk. The motor ran for a few seconds after we stopped, and finally quit on its own. We KNOW the brake was applied the whole time; there were no visual or audible alarms from the car as we entered “UNINTENDED ACCELERATION”. Despite the incredibly strong impact (our car has been deemed totaled) the front air bags did not deploy. Only the driver’s side curtain deployed. The bumper of the parked transport van on our front left was damaged, and the small SUV on our front right was deemed totaled. No people were in those two parked cars; the first car was hit at the initial slower speed so those people were not injured. By some grace, no pedestrians were in our path or they would surely have been killed. The driver of our car (my husband) was miraculously not injured, but CT scans and x-rays showed I had several contusions and a fractured tibia that will require surgery due to brake, propulsion, no air bags. In November 2025, with approximately 6,900 miles on the vehicle, I reported brake issues to the dealership. After inspection, the dealership resurfaced (turned) the brake rotors at no charge. At that time, I was led to believe that the issue had been corrected. However, in June 2026, with only 14,300 miles on the vehicle, I returned to the dealership because the same symptoms had reappeared. I was
... informed that the rotors again require resurfacing and that the brake pads now need replacement as well. why has the same issue returned so quickly? Is there a defect in the rotors, brake pads, calipers, brake system design, or another component that has not been identified and corrected? I do not believe that my driving habits are the cause of this problem. Prior to purchasing this Sportage, I owned two vehicles from other manufacturers. Neither vehicle required brake rotor resurfacing until well over 50,000 miles, despite being driven under similar conditions. The fact that this vehicle has required rotor service twice before reaching 15,000 miles strongly suggests that an underlying issue exists. I believe that that premature rotor wear puts me and others at risk of collision. The Sportage is available for inspection.Read more
