When driving, between the speeds of 40-50mph, the wheels would lock up without warning. This happened 4 times during the month of January, sometimes in 4wd. This is a known issue on previous years of this truck with the same transmission but has not been expanded to cover this vehicle. Thankfully, I was able to maintain contorol and was not in traffic when this occurred. It has experienced persistent engine misfiring even after a mechanic performed repairs on cylinder 1. In addition, the vehicle exhibits rough engine shaking during driving and idling, engine stalling, and rough or delayed gear changes continuously since 2024. Despite attempted repairs, the problems continue to occur to present. The stalling, misfiring, and unstable engine operation create a potential safety risk, particularly while driving at speed or in traffic. The rough shifting and loss of smooth power delivery further impact safe vehicle operation. Given the relatively low mileage, proper maintenance, and the recurrence of these symptoms, this does not appear to be normal wear and tear. I am aware that similar engine-related complaints have been reported by other owners of 2016 Honda Pilot vehicles and that this model year is included in an ongoing federal investigation into potential engine defects. I am requesting that this complaint be reviewed as part of that investigation and considered for further action, including possible recall or service remedy, to address the safety risks posed by these engine and emissions system failures. The vehicle has been inspected by my out-of-pocket expense extended warranty company Endurance Warranty Services sub-contracted repair claims adjuster. No warning lamps, vehicle messages, or other symptoms of the problem prior to the failure were noticeable; the vehicle had multiple diagnostic scans prior to original sale and after sale to present no issues or trouble scans. the first issue appeared or was noticeable 12/08/2025. These symptoms match the known failure pattern of the molded lead frame in the 6R80 transmission. My truck exhibited intermittent behavior, harsh shifting, and unsafe drivability. I had to pay $1,250 out of pocket to replace the lead frame to restore normal operation. I later learned that NHTSA has opened an Engineering Analysis into 2015–2017 F‑150 trucks for this exact issue (OSS signal loss due to internal electrical degradation of the lead frame). My vehicle’s symptoms, diagnostic codes, and required repair are consistent with the defect under investigation. This failure creates a safety hazard because the truck can suddenly lose the ability to accelerate or shift correctly, especially in traffic or at highway speeds. The Integrated Charging Control Unit (ICCU) has intermittent failures. During the last failure, my wife was on a busy freeway (Interstate 5 near Hwy 120 northbound) in heavy but fast moving traffic. She was driving in the #2 lane (of 4 lanes) when she experienced sudden loss of propulsion. The vehicle slowed to 5 MPH in traffic that was moving 70 to 80 MPH. She had to traverse through the #3 a
... nd #4 lanes to the narrow emergency shoulder. Cars were honking and flipping her off. In the emergency lane, the vehicle completely died after 30 to 60 seconds. She was at risk of being rear ended during the dangerous 5 MPH lane changes and in the time parked in the emergency lane. She could not start the car for 15 minutes, after which the system came back to life. She was then able to start it and continue home. The first time the ICCU failed was in a parking lot. This is a known defect with Hyundai which they have attempted to rectify with Recall Campaign 272. However, they refuse to replace the ICCU. After the first event, they replaced the battery after 22 months/32429 miles. Now they want to replace the battery again after another 20 months/34k miles. What they don't get, is that the batteries have not prematurely failed. Instead the ICCU has intermittently and temporarily drawn the batteries down causing the vehicle to lose propulsion and die. They refuse to replace the ICCU because an error code they were looking for was not in the memory. They have performed three software updates on three separate dealer visits, but the issue has not been resolved. During the last visit they kept my vehicle for six weeks while I appealed through Hyundai Consumer Affairs and the Service manager appealed through his Dealer rep. But the factory engineers held firm and would not approve the ICCU warranty replacement, and instead wanted me to pay for 12V battery replacement. We don't know if there were any warning lights since my wife was busy navigating the danger.Read more

A few minutes later, I tried to turn on the car and some displays were off, there was no drive power, the steering wheel did not turn, the door locks were flakey, and I wasn't able to tell if I had shifted into Park or if the parking brake was engaged. When the tow truck arrived, the car was completely unresponsive and there was no way to release the parking brake or shift the car in neutral, so the car was dragged across the pavement and onto the tow truck. The failure was quick and comprehensive and could have easily caused injury. There are many highways near me with fast, busy traffic and either no emergency shoulder or a very narrow one. If my car tells me that I need to stop the vehicle, that can create a very dangerous choice between stopping somewhere less safe quickly or risking the car being completely disabled while in the travel lane while I look for a safer spot. Loss of cabin heat can easily become dangerous very quickly in the winter. Many drivers may not even understand the urgency of the warning until the car starts failing to drive. The car is at a Kia dealership for repair who confirmed an ICCU failure. See related NHTSA recall no 24V-867 and others. ICCU failures are endemic to Kia and Hyundai EVs. ICCU failures are widely discussed on many internet forums and seem much more common than the 1% failure rate claimed by Hyundai/Kia. After reviewing there are two error codes the vehicle relayed that the mechanic said are related. During these 4 instances there were two time (highway on ramp and city street at 30 MPH when the loss of power to the wheels caused the vehicle directly behind the F-150 to have to swerve around to prevent a collision. P0722 (Output Shaft Speed Sensor Circuit No Signal) and P061A (Internal Control Module Torque Performance). The vehicle is available as well as the Scan results from the OBD2 sensor from 4 February 2026. While stopped in traffic with the vehicle’s automatic engine start-stop feature engaged, the vehicle failed to respond when the accelerator was pressed. The engine had shut off normally due to the start-stop system, but when attempting to move again, the vehicle did not accelerate and appeared temporarily disabled. This condition has occurred on multiple occasions over the past year, including recently, and is intermittent and difficult to reproduce on demand. This condition creates a safety risk because the vehicle can become temporarily immobilized in active traffic, including at stoplights and intersections, leaving the driver unable to proceed as expected. The vehicle has been inspected multiple times by authorized Land Rover dealers for related electrical and charging system issues. Components such as the battery have previously been replaced, but the condition has continued to occur. At this time, the issue has not been consistently reproduced by the dealer. Warning messages associated with the incidents include “Gearbox fault detected” and “Charging system fault detected.” These warnings typically appear during or immediately after an automatic engine restart event. ... re was not noticeable otherwise. I believe this is a very dangerous condition, as the fumes were evidently getting into the vehicle somehow and causing symptoms of carbon monoxide poisoning. The estimate to repair both is $5000, but according to the repair shop, there are currently none available. Ford has acknowledged this defect in Customer Satisfaction Program 21B35 and the 2025 service bulletin covering cracked weld failures. At about the same time, the transmission suddenly. The failure is very specific, and is documented in Ford Technical Service Bulletin 22-2411 as a known defect. The vehicle experienced harsh shifting, delayed engagement, and ultimately complete transmission failure requiring full replacement at a cost of approximately $7000. The vehicle experienced a sudden loss of propulsion while driving at highway speed after normal operation. The transmission began slipping and then would no longer engage gears. The vehicle had to be pulled over and towed. This vehicle is subject to GM Recall N242454440 related to a known transmission control valve defect that can cause loss of propulsion. I was not notified of this recall by GM
... and discovered it independently. Once aware, I completed the recall software update. After the failure, the dealer documented pressure control fault codes (P0746 and P0798), burnt transmission fluid, and internal debris, and recommended full transmission replacement. I requested that GM evaluate whether this failure is related to the known defect addressed by the recall and Special Coverage N242454441. GM acknowledged my escalation request but did not schedule an inspection, did not contact the dealer, and closed the case without performing a causation review. The vehicle remains disabled, no technical inspection has occurred, and I am incurring rental costs. I am reporting this as a safety-related failure and recall handling concern.Read more

The vehicle was returned to me after a manufacturer-authorized high-voltage battery replacement that was completed on December 10, 2025. Immediately after pickup, the vehicle began exhibiting safety-related issues. ... immediately after the repair and before the vehicle was driven by me. I returned the vehicle to the dealership multiple times for the same issues. During the final visit, the dealership retained the vehicle for approximately 12 days. After this inspection period, I was informed that the vehicle had underbody damage affecting the coolant system and high-voltage battery and that the damage was likely caused by road debris. The issues began immediately after the battery replacement and involved safety-related systems including steering, coolant, and the high-voltage battery. The vehicle was returned with unresolved warning messages related to these systems. I am concerned that the vehicle was released after a major battery repair in an unsafe condition and that the affected systems may not have been properly inspected or repaired prior to release.Read more

About the safe issue I have the transmission when you wanna have the throttle you wanna pass a car the transmission had don’t go down and make a limp control and I scanned the transmission. It’s no code. It’s not safe when you in the highway you want a best truck or best car and switch down shift to limb control.

Transmission downshifts from 6 to 5 and locks up. I have taken to dealership many times still not correcting the problem. They have replaced celluloid ,spark plugs as well as coils. Although better it is dangerous to rapidly accelerate especially when changing lanes or pulling onto a highway because it loses power.

Transmission will randomly shift gears. At speed sometimes it will shift into a lower gear causing rapid deceleration. From a start it will jump to 5th then back to 1st and stall the rear tires.
