2021 Ford F-150 Lariat, 5.0L Coyote V8, purchased new, now 65k miles. Two interrelated safety defects: OIL CONSUMPTION: On 3/21/2026 at 62k miles, I discovered the engine was critically low on oil. Ford TSB 19-2365 acknowledges this exact defect on the 5.0L Coyote for 2018-2020 model years, attributing it to high intake manifold vacuum during deceleration fuel shut-off pulling oil into the combustion chamber. The 2021 engine is mechanically identical. TRANSMISSION: Hard and slipping gear shifts reported to Ford within months of purchase in 2021. Ford issued multiple software updates over the years to mitigate the behavior, most recently late 2025. Fix was temporary; hard/slipping shifts returned. CATASTROPHIC FAILURE: While awaiting Ford HQ authorization on repairs, the vehicle suffered complete drivetrain failure on Friday night. Engine lost power and transmission failed entirely. I was near home and coasted safely off the road. Had this occurred at highway speed during my daily commute, the result could have been catastrophic. Vehicle towed to Koons Ford Woodbridge. SAFETY CONCERN: This is my only vehicle, used for daily highway commuting. The oil consumption defect provides no warning before engine damage. The transmission has degraded despite years of Ford software patches. The vehicle has demonstrated total drivetrain failure capability with no driver warning. The 2018 F150 I purchased started out a good vehicle like most vehicle do. The problems then began to come one after the other. ... ey all burn alittle oil” and topped it back off. I had another “incident” where I was driving on the highway a few weeks later and my engine light came on, and I lost oil pressure. The red “low oil pressure” message came on the dash and I promptly limped my truck to the dealership once again. The tech diagnosed the truck and found a plugged PCV valve. It was changed for a brand new one from the dealer, followed by another oil change well under the recommended interval naturally, due to the motor burning almost all of the oil. The truck still burns oil, and the issue seems to be impossible to remedy. The issue that followed the engine burning oil is the 10R80 Transmission installed from the ford manufacturer. Once again everything seemed fine when I purchased the truck. After driving it for 20-30k miles, the transmission has developed a flare shift between the 3rd-4th gears. The transmission is getting more and more hesitant to upshift and maintain and gain speed. This is considerably noticeable when attempting to get on the highway. This is unacceptable. It puts me in a very vulnerable spot because it doesn’t happen all the time. So I have to be EXTREMELY aware and prepared for the truck to become inoperable while merging into the flow of traffic. On two occasions now, while merging onto the highway with traffic, I have had to pull over to the shoulder of the road and put my hazards on to allow traffic to go around me due to my 2018 F150 with the 5.0/10R80 combination NOT increasing speed, and reving close to 5k RPMs. THESE ISSUES MUST BE ADDRESSED.Read more

It had the error on the dashboard, but we were able to get it safely home, where it was towed to the dealership later. The vehicle was towed to Dick Hannah Honda (the local authorized dealership). The service department scanned the onboard computer (from the parking lot, NO actual physical inspection done) and pulled internal transmission fault codes P0730 (Incorrect Gear Ratio) and P0746 (Pressure Control Solenoid "A" Performance/Stuck Off). Based solely on these electronic fault codes, the dealership stated that the internal mechanics of the ZF 9-speed transmission had completely failed. They stated the transmission is completely unrepairable and quoted a full replacement cost of $11,984.41. Prior to this failure, the vehicle had service documentation and inspection records from authorized Honda franchise technicians, who repeatedly marked the transmission condition and vehicle health check sheets as completely healthy and "GREEN" at every visit for 60,000 miles by American Honda Motor Co. Case Management was contacted (Case #16371194), but they refused any financial goodwill assistance. This sudden operational failure of the ZF 9-speed transmission platform without ANY prior Maintenance Minders given by the computer system. At the same time, my idling was excessively rough and felt dangerous to drive, as was later confirmed by my mechanic. I have not taken this into a Ford dealership, nor do I want to because they have refused to help others in the same situation. I am considering contacting my insurance to figure out if they will do an evaluation that is less biased than Ford. My vehicle is a 2024 Maverick and is NOT included in 24V-267, which Ford limited to 2022-2023 Mavericks built Feb 2021–Oct 2022. I am reporting because my 2024 shows the identical failure mode, including the total absence of warning before power loss, matching the owner reports that prompted Recall Query RQ24014 into the adequacy of the 24V-267 remedy. I request NHTSA consider whether the affected population should include model year 2024 Mavericks. Complaint: I am writing to report a transmission defect in my 2018 Toyota Highlander AWD (UA80 8-speed automatic transmission). Prior to the check engine light illuminating, I noticed a burning smell coming from the vehicle. Subsequently, the following fault codes were retrieved: •P0741 – Torque Converter Clutch Circuit Performance/Stuck Off •P2714 – Pressure Control Solenoid “D” Perfo

However, my VIN was not included in the covered vehicles under this program, despite experiencing identical symptoms. I am also aware that multiple class action lawsuits were filed in December 2025 against Toyota alleging the company knowingly concealed this defect from consumers for years. I am requesting NHTSA to investigate whether Toyota’s CSP ZJC coverage should be expanded to include additional affected VINs, and to consider issuing a mandatory recall for all vehicles equipped with the defective UA80 transmission.Read more

I am concerned that this engine failure may be related to known Hyundai engine issues involving the 2.4L GDI engine. The failure occurred well before the expected service life of the engine and created a potential safety hazard due to the loss of power while driving. I am requesting that NHTSA review this failure and determine whether similar failures are occurring in 2020 Hyundai Tucson vehicles equipped with the 2.4L GDI engine.Read more

The vehicle was purchased brand new from an authorized franchised dealership, sold and contracted as being equipped with a dealer-installed 2.5-inch suspension lift. At approximately 20,000 miles, a severe front-end squeaking symptom developed. The vehicle was brought to a separate authorized Toyota dealership service center for inspection. An authorized Toyota Master Technician formally inspec
... ted the vehicle and documented that the selling dealership did not install an engineered suspension lift. Instead, they installed unauthorized, budget aftermarket spacer pucks and non-certified upper control arms. The Master Technician documented in writing that the vehicle "isn't even lifted properly" and a factory front strut has failed prematurely. SAFETY RISK: The unauthorized spacer modification has severely altered and compromised the factory steering and suspension geometry. Because the front suspension, upper control arms, ball joints, struts, and steering rack are forced to operate continuously at unsafe, extreme, out-of-spec angles, the components are subjected to premature, destructive stress. The Master Technician confirmed that this improper configuration creates an imminent highway life-safety hazard, presenting an extreme risk of the front CV axles binding or snapping at highway speeds, which would result in an immediate, catastrophic loss of steering control. The vehicle is available for inspection upon request. The safety defect has been explicitly reproduced and confirmed in writing by an authorized Toyota Master Technician. The selling dealership management has been formally notified of this safety diagnosis but has actively refused to remediate the unsafe components, leaving a structurally compromised and highly dangerous vehicle operating on public highways.Read more

There was no traffic, allowing us to rely on driving slow speeds and letting the car to come to natural stops. We took it to the dealership immediately who were unable to duplicate. They diagnosed it as a network overload and attempted to release the car back to us. Currently working with a Ford Advocate as a Network Overload will shut our brakes off again. It's just a matter of when. Which is why they can't duplicate it. They can't predict when the Ford Network that communicates to the MachE braking mechanism will overload again. Reporting an additional case in support of ongoing defect petition investigation DP26001. 2022 Acura MDX Type S SH-AWD, currently at 63,700 miles. While driving and making a turn, I heard a grinding noise with no warning lights illuminated. I stopped immediately and shut off the engine but noticed no abnormality. ... scheduled oil change, I discovered that the area between the transmission and PTU was covered in oil. The Acura dealership diagnosed a bad PTU seal and replaced the PTU under powertrain warranty, but completely disregarded my concern regarding investigation DP26001 and the known issue with stripped splines. The leak was repaired, but several weeks later while driving in the rain, the vehicle felt very unstable during acceleration on wet pavement. I raised the vehicle on a lift and engaged drive to inspect AWD function. The rear wheels were not spinning while the front wheels were, and the driveshaft was also stationary. When I brought this to the dealership’s attention, they stated this is normal SH-AWD behavior — however, I have personally observed all four wheels spinning under identical conditions in another Acura of the same model and configuration. I am confident the transmission-to-PTU splines are stripped and am escalating this to Acura corporate for further investigation.Read more

My commute is approximately 20 minutes each way. The vehicle has not yet been involved in a crash, but I believe a crash is likely if this condition continues. He confirmed the codes align with the recall condition and that the downshifting behavior is consistent with a transmission control module fault. Safety Concern The recall notice states that this defect “may result in a loss of vehicle control, increasing the risk of a crash.” Because the issue has already occurred twice at moderate speeds, I am concerned that a recurrence at highway speeds could lead to loss of control. Remedy Delay Ford and my local dealer (Colorado Springs, CO) informed me that the software remedy will not be available until June 30, 2026. The recall notice states that the remedy is “not yet available.” Current Status Given the active defect, warning lights, and repeat occurrences, I believe the vehicle may be unsafe to operate. Even without the recall we can't get it fixed because all available parts are going towards the recall. We deserve to have our vehicle added to this recall so we can get in line to fix it. Despite these efforts, I have not been provided with a fix that addresses the root cause of the problem. Ford has long been aware of widespread TCM issues affecting certain Ford Focus models, making the continued failure of this vehicle especially concerning. My greatest concern is safety. During a recent incident, the vehicle unexpectedly slammed into gear with significant force while my son and I were inside. The sudden engagement nearly caused me to lose control of the vehicle and almost sent us into a ditch. Had road conditions or traffic been different, the outcome could have been much worse. I have dash camera footage documenting the incident and am willing to provide it upon request. A vehicle that has already undergone a transmission replacement should not continue experiencing severe transmission-related failures, particularly failures that create a risk of loss of vehicle control. This ongoing issue places not only my family at risk but also other motorists sharing the road. I respectfully request a thorough investigation into the recurring transmission and TCM failures affecting my vehicle, including a review of the October 2022 transmission replacement and Ford's response to these repeated issues. Because of this, we called corporate in hopes they will pay for it under their Goodwill Assistance program. However, because we never had the Highlander serviced at a Toyota dealership before, they told us they won't cover anything. We've routinely serviced this vehicle. We happened to purchase it at a Nissan dealership, so getting maintenance at Toyota didn't cross our minds. Regardless of where we got the car serviced, it's obvious there's a recurring problem with the Highlander's transmission that Toyota has refused to acknowledge, which is costing consumers around $9,000 apiece.
