I was on the highway going at approximately 60mph when the car without any warning reduced speed to first around 40mph and then to 20mph all within a matter of a few minutes, causing me to pull over to the side of the highway. Luckily, I wasn't injured. I exited the highway, called my insurance company Geico and had the vehicle towed to the dealership where I leased this brand new vehicle from jus
... t about 7 months ago on a 2year lease. The dealer was kind enough to provide a smaller loaner vehicle but had no idea when my car would be returned, as there were others with similar failures ahead of me that were waiting for the part to arrive. Apparently, this is a known failure issue of the ICCU that Hyundai has done a poor job of addressing. I continue to make my lease payments while waiting for the part to arrive. In the meantime, I wanted to let the government know of my experience and how this could have had a disastrous effect on my family and other families with a similar build.Read more

I am submitting this complaint regarding a sudden and premature transfer case failure on my 2021 Acura TLX Type S equipped with Acura’s Super Handling All-Wheel Drive (SH-AWD) system. The failure occurred during normal driving conditions and without prior warning indicators. Based on my experience and numerous similar reports from other owners of this vehicle, this appears to be an ongoing and s
... ystemic issue that warrants formal investigation and recall consideration. While accelerating under normal road conditions, I heard a loud and distinct “pop” noise from underneath the vehicle. The sound was abrupt and mechanical in nature, as if an internal drivetrain component had failed. Immediately following the noise, the vehicle’s driving characteristics changed noticeably. Although the car continued to move forward and there were no immediate warning lights illuminated on the dashboard, the drivetrain behavior was clearly altered. After the incident, the vehicle began operating as though the majority of drivetrain load was being directed to the front wheels. The vehicle now feels significantly front-heavy during acceleration and general driving. There is a distinct change in weight transfer and torque delivery that was not present prior to the failure. The balance that is characteristic of a rear-biased SH-AWD performance system is no longer present. There were no grinding sounds, fluid leaks, or vibration at the time of failure. There were also no warning messages, check engine lights, AWD system warnings, or drivetrain alerts displayed to notify me of any malfunction. The failure occurred suddenly and without notice.I respectfully request that NHTSA open a formal investigation into premature transfer case failures on 2021–2023 Acura TLX Type S vehicles equipped with SH-AWD. This issue should be evaluated for recall eligibility or mandated corrective action to ensure driver safety. Thank you for reviewing this complaint.Read more

I have had many issues with this vehicle. The engine leaks oil onto the exhaust causing the engine to smoke, the engine uses antifreeze, the transmission will downshift at highway speeds to such a low gear that I need to put the van into neutral so that the engine doesn't blow up. It uses brakes quickly, (New brakes every 10,000 miles or so). Gas milage is less then 12MPG as well.

This technical failure in the BMW B58TU engine (2019–2021) involves the variable-displacement oil pump's internal thermoplastic slide. In low ambient temperatures, the high viscosity of cold oil creates mechanical stress that causes this plastic component to fracture. The Safety Hazard The primary risk is a sudden loss of motive power. When the slide fails, oil pressure becomes unregulate
... d or drops entirely, triggering an immediate "Drivetrain Malfunction" or engine shutdown. Immobilization: Vehicles may stall in active traffic or high-speed environments without sufficient warning. Environmental Risk: Failures peak in extreme cold, leaving occupants stranded in freezing conditions while awaiting recovery. Detection Gap: The electronic oil level sensor often fails to complete a measurement ("Measurement Aborted") before a low-pressure warning appears, depriving drivers of a clear precursor to mechanical failure. Critical Technical Challenges Obfuscated Parts History: BMW has superseded the original plastic-vane pump with a revised metal-vane version. However, the Electronic Parts Catalog (ETK) now lists the metal version as the "original" part for affected VINs, making it impossible for owners to verify which version is physically installed without engine teardown. High Repair Barrier: Correcting this defect requires extensive disassembly, often costing $6,000–$8,000. If the failure occurs at speed, total engine replacement is frequently required.Read more

The diagnosis indicated no spark and that the ignition coils were not receiving signal, although the crank sensor signal was present. The PCM was replaced, and the vehicle started and operated normally after replacement. No additional wiring, fuse, or grounding issues were identified. There were no prior warning lights, dashboard messages, or drivability symptoms before the failure occurred. The failure happened unexpectedly at approximately 52,889 miles. While ascending a steep hill in San Francisco, the engine began operating roughly and the check engine light came on and began flashing. Pulled over as soon as possible, let the engine cool, and confirmed in the operator manual that the flashing light indicated severe misfire and possible loss of engine power. After cooloff and restart, the flashing check engine light went off, but a scanner rev
... ealed a pending P0302 error code, indicating a misfire in cylinder 2. On the first restart of the engine, it started roughly before settling down, and I could hear loud water sloshing. On the short drive home, I could continue to feel the engine running roughly on each startup cycle (periodically on the drive since this is a hybrid), indicating to me a blown head gasket. In my research, this seems to be a common issue with this engine or model year (see NHTSA Preliminary Evaluation PE25016 or Bissell v. American Honda Motor Co., Inc. et al). I'm confident that if I continue to drive it, another misfire like this on a hill in San Francisco could easily cause a loss of power, steering, or braking action. I will be taking it in for inspection at a Honda service center. The service representative at my local service center (Berkeley CA) indicated this is a common issue on this car that they see regularly. No prior mechanical issues or engine roughness with the vehicle, or anything that would've indicated an issue prior to this misfire and leak. Regularly serviced as a leased vehicle from Honda dealership until it was purchased 2 years ago from the dealership with 70,000 miles. During interstate travel in rainy conditions, the vehicle appeared to lose traction and hydroplane. The tires appeared to have adequate remaining tread; however, they did not provide expected traction and the vehicle felt as though it was driving on ice. This was inconsistent with my prior experience operating the vehicle for over a year in various weather conditions without encountering similar i
... ssues. The Electronic Stability Program (ESP), traction control system, and 4MATIC all-wheel drive system did not appear to intervene to stabilize the vehicle. The vehicle spun and was involved in a crash. Despite the impact, no airbags deployed. The vehicle was rendered undrivable and towed from the scene. It is currently located at a collision repair facility and is available for inspection upon request. The suspected systems involved include the Electronic Stability Program (ESP), traction control system (TCS), 4MATIC all-wheel drive system, and Supplemental Restraint System (SRS/airbags). The vehicle has not yet undergone a full manufacturer engineering inspection, but such a review has been requested. The loss of traction at interstate speed in wet conditions created a serious safety risk to the occupants of the vehicle and surrounding motorists. The apparent lack of stability control intervention and the absence of airbag deployment increased the potential risk of injury. The vehicle was inspected by law enforcement at the scene, towed, and has been reviewed by insurance representatives as part of a claim. A manufacturer inspection is pending based on a defective claim that was filed. There were no warning lights, alerts, or messages related to traction control, stability control, or airbags prior to the incident.Read more

Safety Defect Report — Engine Oil Pump Failure (BMW B58TU Engine) Component or System Involved The failure involves the variable-displacement engine oil pump installed in BMW vehicles equipped with the B58 Technical Update (B58TU) engine, commonly found in model years 2019–2021. The oil pump contains an internal thermoplastic adjusting component that regulates engine oil pressure. Field repor
... ts indicate this internal component can fracture, causing unstable or complete loss of engine oil pressure. The failed component may be available for inspection upon request if retained after repair. Description of Failure and Safety Risk When the internal thermoplastic adjusting component fails, engine oil pressure regulation becomes erratic or is lost entirely. This results in insufficient engine lubrication and may cause sudden engine shutdown or severe mechanical damage due to oil starvation. Loss of engine power or complete engine disablement can occur without adequate warning, creating a safety hazard if the vehicle becomes immobilized while driving or shortly after startup. This risk is heightened when the failure occurs in active traffic or in cold weather conditions, where occupants may be exposed to freezing temperatures while awaiting assistance. Operating Conditions and Failure Pattern The failure most frequently occurs during cold-start operation in low ambient temperatures. Failures appear to progress rapidly once symptoms begin. Warning Lamps, Messages, or Prior Symptoms Reported precursor symptoms may include: Inability to electronically measure engine oil level. Abnormal or unstable oil pressure behavior. Sudden drivetrain malfunction warnings. Minimal or no actionable warning prior to engine damage or shutdown. These symptoms may appear shortly before failure or may not provide sufficient time for corrective action. Inspection or Confirmation of the Problem Field failures have been reported across multiple vehicles and production years. In many casesRead more

On February 15, 2026, the vehicle shut off again while driving under similar conditions. The vehicle was taken to a H
... yundai dealership for diagnosis. The battery failed a load test and was replaced. According to the repair order narrative, after the battery replacement, the stall condition recurred during dealership testing. Multiple extended road tests and datalogging were later performed. The dealership has stated they are unable to re-replicate the condition and that no active diagnostic codes are present. The vehicle has experienced multiple loss-of-power events while in motion. An intermittent stall while driving presents a significant safety hazard, particularly in traffic conditions. The root cause has not been definitively identified.Read more

Oil pump failures have been reported across multiple production years, and affected vehicle population boundaries remain unclear. Incident date: Aug 22, 2024
I'm reporting a safety issue with BMW vehicles equipped with the B58TU engine, primarily 2019-2021 models. These vehicles have an oil pump with an internal plastic component that regulates oil pressure, and this part is fracturing in the field, causing sudden loss of oil pressure. The failure typically occurs during cold starts in low temperatures. When it fails, oil pressure becomes erratic or is
... lost entirely, often with little to no warning before engine damage or vehicle disablement. Warning signs, when present, include inability to measure oil level electronically, abnormal oil pressure, or sudden drivetrain malfunction warnings. These can rapidly progress to engine shutdown or severe damage from oil starvation. This creates a dangerous situation because the vehicle can become disabled without warning while driving or after startup. This is especially hazardous when cars become immobilized in traffic or during winter conditions when failures occur most frequently, leaving occupants exposed to freezing temperatures while awaiting assistance. Pump replacement requires extensive disassembly and commonly costs $6,000-$8,000, assuming no engine damage. A critical concern: owners cannot determine if their vehicle has the original plastic pump or the later metal revision. BMW's VIN-based parts lookup now shows updated components even when original plastic parts remain installed, and BMW has removed the original plastic part number from dealer networks entirely. Failures are reported across multiple production years, and the scope of affected vehicles remains unclear. Because this condition causes sudden vehicle disablement without adequate warning, especially in cold weather, I believe this warrants investigation as a potential safety-related defect.Read more

Transmission has known issue by Ford Motor Company concerning defective components leading to wear and improper function causing potential safety issues with acceleration and deceleration. My vehicle, I purchased used with 32k miles in April of 2025. I started to have issues with shifting approximately 6 months ago when it would shift erratically when cold and when vehicle was not warmed up. O
... n several occasions the vehicle, when shifted into reverse to back out of a parking space, the vehicle hesitates and drops harshly into reverse. This has occurred at least 4 times within the last 3 months. The next issue is when the vehicle is initially started up in the morning and placed into gear to leave my driveway I pull out and it appears to be operating fine. When the transmission goes to upshift it will shift on the gauge screen indicating it is in 3rd gear but either I have no gear and the engine will rev without acceleration, or as in the last circumstance, it will shift to 3rd, jump to 8th gear, and have no accelerator input for approximately 5-10 seconds. The shift indicator shows the transmission then shifts from 8th, to 5th, then down to 3rd and drops harshly into gear and proceeds operation as normal. The transmission will not shift as smoothly as it does when it is warm for approximately 3-5 miles. This type of operation has been identified as being contributed to the interior components of the 10R80 transmission. In particular, the CDF clutch is identified in the TSB for this issue 23-2250. I have had my vehicle at the Ford dealer on 2 occasions for this issue and they have not been able to replicate the issue. I was advised the 2nd time that the service technician stated the transmission was low 2 quarts of fluid and they were unable to get the transmission to replicate the harsh shifting condition. This vehicle was serviced less than 2 months prior and no indication of low fluid was noted in the transmission at the time of service.Read more

The part they repaired is the same part that is recalled on other trucks the same age as mine. So my truck should be recalled for this issue. How do I get this recall to include my truck and how do I get reimbursed for this cost. My truck only has 88k miles on it. This needs to be covered and I need your help in making this happen please.Read more

I'll have to come to a complete stop, or I'll have to leave my foot on the pedal, then it's like it kicks on with high RPM. There's no warning lights or messages. This time while I was on the highway during stop and go tra
