After the automatic start/stop system engages, the engine stalls and completely shuts off shortly after the vehicle begins moving again. This occurs after the vehicle restarts and travels only a few feet. When the stall happens, the vehicle loses power and the engine stops functioning while the car is in traffic. This has occurred two separate times: once on a busy highway and once while entering
... a roundabout. Both incidents created a dangerous situation where the vehicle could not maintain power in active traffic and could have resulted in a crash. A yellow emissions warning light also appeared on the dashboard. The dealer indicated that an intermittent oil pressure sensor code was present but no parts were replaced and no corrective action was taken. The dealer reset the system and stated they could not reproduce the problem. The vehicle is now being returned to me without repair despite the safety risk. An engine stall immediately after the start/stop system restarts the vehicle creates a serious hazard because the vehicle can lose power in the middle of traffic. I believe this issue may be related to the automatic start/stop system or an oil pressure sensor fault affecting engine operation.Read more

Vehicle: 2020 Toyota C-HR Mileage: 77,980 miles About a year ago, when the vehicle had fewer than 60,000 miles, I began noticing unusual noises and that the vehicle was not always accelerating smoothly. The symptoms were initially intermittent but gradually became more noticeable over time. Meanwhile, there were no warning lights or signs displayed and I was so insured that Toyota produces very
... safe cars that I wasn't so aware it could be unsafe to drive it. Recently the vehicle developed a severe rattling noise while driving. And even now - no warning light appeared on my car screen. Toyota dealership service inspection indicated an internal transmission failure requiring full transmission replacement, with an estimated repair cost of approximately $15,000. The vehicle currently has only 77,980 miles. A major transmission failure at this mileage appears premature. I have also learned that other Toyota C-HR owners have reported similar CVT transmission issues. Because the transmission affects acceleration and drivability, I am concerned that this condition could potentially create a safety risk in traffic, particularly during merging or acceleration situations.Read more

Vehicle: Lincoln MKZ 2013 Dealer: Auto Quest Inc. Description of the Safety Issue: I am submitting this report regarding a vehicle safety concern involving a 2013 Lincoln MKZ that I purchased from Auto Quest Inc. in Renton, Washington. Shortly after purchasing the vehicle, within the first few days of ownership, the vehicle began experiencing significant mechanical problems including transmissi
... on issues and persistent brake noise that required multiple repair attempts. These problems appeared almost immediately after the purchase, which raises concerns that the vehicle may have had pre-existing mechanical or safety-related defects that were not disclosed at the time of sale. Since purchasing the vehicle I have had to spend thousands of dollars on repairs in order to keep the vehicle operational. Despite these repairs, the brake system has continued to produce abnormal noise and the vehicle has experienced ongoing mechanical issues. These types of issues may represent potential safety risks to drivers and passengers if the braking system or transmission were to fail while the vehicle is in operation. I am reporting this issue so that the National Highway Traffic Safety Administration can evaluate whether this may represent a broader safety defect affecting this vehicle model or vehicles sold under similar circumstances.Read more

Incident date: Dec 20, 2024
The vehicle exhibited charging failures on multiple occasions over the course of several days after being connected to a Level 2 home charging unit overnight. On repeated mornings, the high-voltage battery was found to have not charged. The vehicle also exhibited intermittent slow charging behavior during this same period. Approximately six months prior to the charging failures, the 12-volt auxili
... ary battery was found to be completely discharged. During this same period, the vehicle was started on one occasion to find that every warning and alarm indicator on the dashboard had triggered simultaneously. The vehicle was completely inoperable at that time. After several minutes, all warning indicators cleared and normal operation resumed without any intervention. The vehicle was brought to an authorized Hyundai dealership on March 6, 2026, where the technician diagnosed the root cause as a failed Integrated Charging Control Unit (ICCU). The ICCU is responsible for managing both high-voltage battery charging and maintaining the 12-volt auxiliary battery; a failure of this component can result in loss of charging capability, auxiliary battery drain, and potentially a complete and sudden loss of vehicle operability. As of the date of this report, the vehicle is awaiting parts with an estimated repair time of approximately one week. The repair is expected to be covered under the manufacturer's warranty. This failure occurred with fewer than 10,000 miles on a 2025 model year vehicle. Based on publicly available owner reports and online forums, ICCU failures appear to be a recurring issue across multiple Hyundai IONIQ model variants.Read more

I own a 2025 Toyota Tundra 1794 with the 3.4L twin-turbo engine. The engine failed at approximately 53,000 miles while I was driving on the freeway in Houston, Texas. I was traveling at highway speed when the engine suddenly lost power and the truck became unsafe to continue driving. I had to pull over to the shoulder of the freeway with traffic moving at high speed. My [XXX] daughter was in the
... vehicle with me at the time. The sudden loss of engine power at highway speed created a serious safety hazard because it left the vehicle disabled in active freeway traffic. The vehicle had been properly maintained with regular oil changes and normal driving conditions. After the failure the truck had to be towed to the Toyota dealership, which I had to pay for out of pocket. The dealership performed an engine teardown that took about one week. They informed me that the engine block and most internal components that come into contact with oil will need to be replaced. The dealership told me the failure is not currently related to the existing Toyota Tundra engine recall. The truck is currently at the dealership and they estimate approximately 30 days to complete the repair. The dealer also informed me they have multiple Toyota Tundra trucks waiting for engine repairs and that no truck loaner vehicles were available. My understanding is that a Toyota case number has been opened for this failure and that the diagnostic details are recorded in that report. I am concerned that sudden engine failures at highway speed represent a serious safety risk INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

The transmission began slipping and hesitating within days of purchase. I have been instructed that parts are still not available for them to fix the recall on my truck. It has been (8) months ago that I received an initial recall notice from Ford Motor Co. A few weeks ago I called the Ford Recall Assistance Center at 1-866-436-7332 in hopes of getting a resolution and timeline for a repair work to be completed on my Ford F-150. Approximately one month ago, I noticed that the vehicle would not accelerate the way it normally does. A error code came on. I took the vehicle to Auto Zone for error code check. I was informed that the powertrain control module speed sensor had gone bad. I looked this up and found that Ford had a recall on 2011 Ford F-150s for similar issues. I disconnected the battery, as recommended on an inter

On further investigation I fount that the vehicle would not shift out of third gear (up or down). I again disconnected/reconnected the battery, and again it began operating normally. I contacted the local Ford dealership and they said that I should have received a letter in May 2019 about the PCM recall. I didn't recall receiving the letter, but Ford Motor Co. Customer Service says the PCM was actually reprogrammed on March 29, 2019 to repair the issues related to the recall. The contact owns a 2016 Dodge Dart. The contact received notification of NHTSA Campaign Number: 25V674000 (Power Train); however, the part to do the recall repair was unavailable. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The check engine warning light was illuminated. The vehicle was taken to AutoZone to test the battery, alternator,
... and transmission fluid. A mechanic diagnosed that the thermostat was sticking. The vehicle was then taken to another mechanic, who replaced an AC cord due to liquid leaking from underneath the vehicle. This mechanic suggested replacing the thermostat if the check engine warning light remained illuminated. While at a stop sign, the vehicle jerked before accelerating, and while driving approximately 35-45 MPH. The vehicle was driven to the mother's residence, where the vehicle remained for 3 days before being restarted. The vehicle remained parked for approximately a week before the contact drove back to the residence, and the check engine warning light and the battery light were illuminated. A certified mechanic replaced the thermostat, and another inspected the vehicle but could not identify any other failures. After the repair was completed, the contact noticed that the vehicle continued to jerk while driving at various speeds, and the sensation was more prominent whenever the AC unit was activated. While driving approximately 35 MPH and attempting to accelerate, the vehicle decelerated, the battery light illuminated, and the vehicle lost power. The contact was able to restart the vehicle and coast downhill into their grandmother's driveway nearby. After waiting a minute, the contact started the vehicle and was able to drive to the residence, approximately 5 minutes away, at 20 MPH to avoid additional failure. The vehicle was taken to AutoZone, where no battery or alternator failure was found. The mechanic suggested it might bRead more
