This shear created a high-velocity, high-heat exhaust leak directed at the brake master cylinder and vacuum assembly. This "heat soak" event compromised the engine-mounted mechanical Brake Vacuum Pump (Recall N192268490 / NHTSA 19V645) and destroyed the internal seals of the Master Cylinder (Special Coverage 15143 / NHTSA 11V599), resulting in total depletion of vacuum assist and hydraulic pressure. Simultaneously, the "shotgun blast" and violent swerve were caused by structural failure consistent with Rear Brake Caliper Bolt Fracture (GM 13247 / NHTSA 13V342) and Rear Axle Shaft/Pin Fracture (GM 14892 / NHTSA 14V819). This mechanical collapse caused the caliper to strike the wheel, resulting in a catastrophic rim blowout and immediate loss of tire pressure. Despite the vehicle having aftermarket long-tube headers installed, the root cause of the heat-related brake failure was the failure of the OEM GM manifold fasteners which snapped off in the cylinder head, as documented in the aforementioned bulletins. Under the Magnuson-Moss Warranty Act, the manufacturer remains liable for these pre-existing safety-critical defects. The selling dealership refused to assist, claiming the defect was not present at the time of sale; Honda America acknowledged the head gasket concern but denied a goodwill claim, citing the high cost of the issue and lack of NHTSA enforcement on this issue. An independent repair shop then provided a second inspection/estimate consistent with the same head-gasket repair scope. The vehicle and repair records are available for inspection. Rear cross traffic breaking also fails and can cause an accident. Eighteen months before failure, steam came from the engine compartment on a humid day, later connected to the water pump electrical connector recall. Following this, the vehicle ran consistently hotter than normal and lost noticeable power. Twelve months before failure, an intermittent oil pressure warning began triggering under load at highway speeds, particularly on inclines above 70mph. I brought the vehicle to Sandia BMW in Albuquerque, NM and reported all symptoms when I was contacted by them for the recall. The service advisor falsely claimed elevated operating temperature was optimal, suspected a faulty sensor, and returned the vehicle without offering further diagnosis or making me aware of the impending issue, all classic signs of the engine faults. Eight weeks later, the oil pressure light activated on a highway, followed immediately by a drivetrain malfunction warning, a violent grinding sound, and complete loss of power. The vehicle was towed and has not restarted since. I contacted BMW of North America's recall line. A case was opened and an advocate stated BMW would accept no responsibility because the class action was closed and I'mpast 70K miles. I raised the dealership's direct culpability. The advocate stated a dealership representative would contact me. No call was ever received. This failure is consistent with BMW Service Bulletin SI B11 03 17. Because Lexus designed this sensor as part of the transmission assembly, the dealer has stated the sensor cannot be replaced separately and that the only repair is to replace the entire transmission. Lexus is refusing to acknowledge this as a manufacturing defect and is requiring me, the consumer, to pay the full cost of a complete transmission replacement, estimated at approximately $9,040.03, solely due to the failure of this internal temperature sensor. This is a critical component used to monitor drive motor/transmission temperature. A malfunctioning sensor of this type can lead to improper system operation, possible overheating, loss of power, or unpredictable behavior of the hybrid drive system while the vehicle is in motion, which presents a safety risk to occupants and other road users. This failure occurred during normal use on public roads and not due to any accident, abuse, or modification. I am requesting that NHTSA investigate whether there is a design or manufacturing defect in the drive motor/transmission temperature sensor and its housing or circuitry on the 2020 Lexus RX 450h (and related Toyota/Lexus hybrid models) that forces owners to replace the entire transmission when this internal sensor fails. When I was pulling out of my driveway, my car wouldn't accelerate and started making a terrible noise. I lost all power and couldn't get it to move leaving me stuck in the middle of the road. On January 6th, I wrote to you expressing my concerns about Ford’s failure to address a known safety recall (Safety Recall Notice #25S82/NHTSA Recall 25V512) for my 2023 Ford F-150. To date, I have received no response to my initial inquiry which prompts me to send this second letter. In fact, I am writing this letter as I sit in my local Ford dealership for
... routine maintenance service. At check-in today, I again requested that they determine if my vehicle was in need of the repair specified in the safety recall. Again, I was told “sorry, but Ford is not sending us the necessary parts, so there is nothing we can do at this point”. They said that I should periodically check back with them. I reminded them that I was put on a waiting list for these parts. I have included a copy of my original letter to Ford, so I will not repeat all my concerns. I believe it is important to fully document the facts and my repeated attempts to resolve this matter so that there will be a clear trail of evidence should this unaddressed and known safety recall ever create subsequent safety issues for me or my family. How a company with the size and reputation of Ford can issue a safety recall and then not provide the means to correct a known safety problem is unfathomable. The safety recall notice clearly states that the vehicle “...may roll in park without the parking brake applied, or, alternatively, lose motive power, both of which can increase the risk of a crash”. It then goes on to state that “parts are now available to repair your vehicle”. Frankly, I do not know if parts are in fact available, but my dealership has made it clear that they are not getting these parts. ... les behind the driver. The truck attempted to place itself into Park before it had fully stopped, and once stopped in the roadway it could not be shifted out of Park. The driver had to move the disabled vehicle off the roadway while traffic was approaching from behind. Safety Concern: The vehicle experienced a sudden loss of transmission fluid and shut down in an active traffic lane without any warning, creating a serious safety hazard for the driver and surrounding vehicles. Photos show the truck stopped in the middle of a busy highway with traffic passing while the owner documented transmission fluid pouring from underneath the vehicle. Additional photos show the dealership repair report stating the transmission cooler line was left unclipped from the fan shroud assembly, allowing it to contact the A/C compressor clutch and wear a hole in the line, causing rapid transmission fluid loss. The service manager also stated that other trucks have arrived from the factory with the transmission cooler line not properly clipped or secured. Videos available as needed. Since this incident and repair, the vehicle has continued to exhibit abnormal drivetrain behavior including shuddering and harsh or unexpected downshifts during acceleration and when using cruise control. On December 6, 2025 I purchased a used 2013 Mercedes-Benz ML63 AMG from Jaguar Land Rover Willow Grove (Piazza Auto Group) in Pennsylvania. The delivery documentation for the vehicle included a written service commitment stating: “Replace air compressor for suspension system.” On January 23, 2026 at approximately 6:30 PM, while operating the vehicle at highway speed on the [XXX] in [XXX] ,
... the vehicle experienced a sudden cooling system malfunction. An “Engine Overheat – Shut Off Engine” warning appeared on the instrument cluster and steam began emitting from the engine compartment. Because the failure occurred while traveling at highway speed, the driver was forced to execute an emergency maneuver across traffic lanes in order to reach the shoulder safely. The vehicle became disabled and required towing. The vehicle was transported to a Mercedes-Benz dealership for inspection. Initial repairs involved replacement of the coolant reservoir and hose assembly. Within several days the vehicle again displayed a low coolant warning and was returned for further inspection. At that time the radiator was found to contain a prior non-OEM epoxy repair and the radiator required replacement due to leakage. During a separate independent inspection, the air suspension compressor was observed to be mounted using plastic zip-tie fasteners rather than mechanical mounting hardware. The compressor is part of the vehicle’s AIRMATIC suspension system and is subject to vibration, pressure cycling, and heat during normal operation. Improper mounting of this component could allow the compressor assembly to move or detach during operation and may damage associated air lines or electrical connections. Because the air suspension system affects ride height and vehicle stability, this condition may present a safety risk during vehicle operation.
