Description of Safety Defect: While operating the vehicle at approximately 25-30MPH and make a right turn, the vehicle experienced a sudden and total loss of power/engine stall, accompanied by a "Check Engine" light and a "Reduced Engine Power" notification. This resulted in a near-collision on a public roadway. The vehicle's steering and braking response were significantly compromised during thi
... s event, creating an immediate life-safety hazard for the driver and passengers. Dealer/Manufacturer Response: The vehicle was recently in the possession of McGovern Buick GMC Westborough for service. Despite the vehicle exhibiting an active Check Engine Light (CEL), the dealership staff pressured the consumer to take delivery of the vehicle and drive it on public roads. The service advisor admitted in writing that the CEL was active but insisted the vehicle was "feel free to drive." Furthermore, the dealership attempted to falsify internal corporate safety notes to claim the consumer "insisted" on taking the vehicle, masking a known mechanical defect. This represents a willful suppression of a safety defect by a franchised dealer, which led directly to a hazardous operating condition. Injuries/Crashes: No crash occurred, but a near-miss was documented. Passenger experienced severe emotional distress due to the sudden loss of vehicle control in traffic.Read more

Consistent with NHTSA Recall bulletin 24V444000, my truck suddenly downshifts on its own from 5th or 6th gear to 1st or 2nd gear, even at highway speeds. My vehicle is equipped with the same V35A-FTS 3.4L Twin-Turbo V6 engine and #1 main bearing assembly that is currently under federal recall for catastrophic engine failure (Recalls 24V-381 and 25V-767). Despite the shared mechanical defect of machining debris and bearing tolerances, Toyota has excluded the i-FORCE MAX Hybrid models based on the assumption that the electric motor provides sufficien
... t 'limp home' capability. I believe this is a critical safety oversight, as a sudden internal combustion engine seizure at highway speeds creates an unpredictable loss of motive power that a small hybrid battery cannot safely manage in high-speed traffic. To the National Highway Traffic Safety Administration, I am filing this formal safety complaint regarding a dangerous engine failure incident involving my 2017 Hyundai Sonata Plug-in Hybrid. Description of Incident: While traveling on CA-85 near Mountain View, California, at approximately 65 miles per hour, the engine suddenly and without warning lost all power. The vehicle began to slow down i
... nvoluntarily in active highway traffic, creating an immediate and serious risk of rear-end collision. The driver had no ability to maintain safe highway speed or accelerate out of danger. The vehicle had to be towed from the scene to the dealership. There was no warning light or prior indication that engine failure was imminent. The failure was sudden, complete, and occurred at highway speed in heavy traffic conditions. Safety Risk to the Public: This incident represents a significant and ongoing public safety hazard. A sudden, unpredictable loss of engine power at highway speed — with no prior warning — places the driver, passengers, and surrounding vehicles at serious risk of collision. This is not an isolated mechanical inconvenience; it is a life-threatening failure mode. Known and Documented Defects: The 2017 Hyundai Sonata Plug-in Hybrid has been the subject of multiple recalls and class action litigation related to engine defects. My incident is consistent with the known failure patterns documented in those proceedings. Despite this history, Hyundai Motor America has denied my warranty claim, citing missed services — a characterization I dispute, as the vehicle was serviced exclusively at an authorized Hyundai dealership whenever the onboard system indicated service was due, and no technician ever identified or disclosed any engine damage during those visits. Request to NHTSA: I respectfully request that NHTSA: 1. Open or expand an investigation into engine failure incidents in the 2018 Hyundai Sonata Plug-in Hybrid 2. Investigate whether a recall is warrantedRead more

While driving on the highway at approximately 70 mph, the vehicle unexpectedly downshifted without any prior warning or indication of an issue. This sudden loss of speed created a hazardous situation, as the vehicle travelling behind me was forced to swerve quickly to avoid a rear-end collision. Immediately following this, the check engine light illuminated. From that point forward, the transmissi
... on would not shift below 3rd gear for the remainder of my commute. This significantly impacted the drivability of the vehicle and made continued driving concerning, particularly in traffic conditions. After arriving at my work, I retrieved the diagnostic trouble code, which was identified as P0721 (Output Shaft Speed Sensor). Notably, there were no prior warning signs, drivability issues, or alerts indicating a potentail failure before this incident occurred. Given the sudden and unpredictable nature of this event, especially at highway speeds, I am concerned about the safety implications of this issue. A failure of this kind, without warning, creates a substantial risk not only to the driver but also to surrounding traffic.Read more

At this point I was approaching a traffic light and stopped at the red light. Decided to return home and attempted to turn right on red at the light. When I hit the gas to turn, the gas engine did not engage, the car limped forward a few feet in electric mode, and the car started beeping with the “Check Hybrid System. Stop Safely” warning. If I had less less space when turning right, or the car would have a moved a few feet farther, I would have been hit in the intersection by an oncoming car. Thankfully I had left a lot of space and my car did not pull that far into the intersection. I turned the car off, when I turned the car on again, the gas engine engaged, and I was able to slowly drive the 1 mile home. Car was then towed to the dealer 2/18 for repair. As of 3/17 dealer still has the vehicle (so do not have a second repair invoice) and waiting on an Customer Care escalation from KIA as dealer recalibrated the Hybrid module but can't recreate the issue after driving 400miles. I contacted the dealership immediately to report the issue, and the vehicle was towed back to JT Wholesale Auto Sales on Friday, January 9, 2026. On Tuesday, January 13, 2026, I was informed by the dealership that the vehicle had undergone a complete inspection and that no mechanical issues were found. Relying on this representation, I retrieved the vehicle and drove it home. However, after retrieving the vehicle, I continued to have concerns regarding its condition. On Thursday, January 15, 2026, I took the vehicle to Stellar Auto Service located at 2209 N. Yellowood Ave., Broken Arrow, OK 74012 for an independent inspection. Stellar Auto Service specializes in Lexus and Toyota vehicles. During their inspection, they identified the following issues: • Engine timing chain cover leak • Worn drive belt Stellar Auto Service was informed of the slip indicator light activation and the engine stalling that I had experienced. However, at the time of their inspection, they were unable to identify or reproduce any issues related to the slip indicator light or the stalling concern that I reported. On Sunday, February 15, 2026, the slip indicator light illuminated again, and the vehicle stalled while in operation. I was forced to safely pull off the roadway. Thank goodness I wasn't traveling on the highway when this happened. Beware to anyone if this was to happen on the highway while in the middle lane while in traffic. And the check engine light never turned on and thus no code to help detect the issue. Took it the day before to have it looked at and was told nothing was wrong with it since there was a code and they could not duplicate the issue I described. WHAT?? They reset some transmission computer setting but it only made it worse by the next day. Now this 6 yr on 2020 GM Yukon is at the dealership service department requiring a new transmission for $7200. Said that the transmission fluid was showing particles of stuff and color was 'burnt', I think. Transmission fluid was changed at it's regular check up time so that didn't cause the issue and nor should it have bits and pieces in it and discolored. Is there any recourse to GM and it's crappy transmissions??Read more

As of today, March 17, 2026 , my 2025 Hyundai Tucson keeps having safety issues. On March 8th , 11th, 14th and 25th the car once again shut down mid-drive. This issue occurred last month as well but dealership claims no DTCs were found. I am noticing that the forward collision warning comes on without any cars or objects in front periodically since the purchase of my car. The light will remain o
... n for a few seconds and disappear. The automatic brake system seems to be causing issues as well which I believe causes the car to shut off completely mid-drive. Once the car shuts off the panel lights turn on to include the engine light, but then after a few seconds I able to change gears back to park in order to turn car back on and continue driving. This has happened now several times since last month. However, I have had issues with my brand new vehicle since I purchased it last year. My car remained in the dealership for 3 months last year due to an engine, ac and power train issues. Prior to the car shutting off, No actual warning lights illuminate other than the forward collision warning light and the notification of headlights are turned off even though it’s daylight and headlights do not need to be turned on . Periodically and intermittently an alert about consider taking a coffee break alert has also popped up in the past. It seems Hyundai has a severe issue with the FCA operating system and over 100 complaints have been made about this vehicle nationwide and no recalls have been issued. I am afraid for my life and the life of others on the road as this car should not be stopping and shutting off while driving for no apparent reason.Read more

My 2018 Ford F-150 had 85,000 miles on it when the transmission began to slip out of gear, delays in shifting, gets stuck in neutral, and or slam back into gear. My wife was barely able to get home from work, and experienced dangerous driving conditions due to the failing transmission. Her attempts to merge onto high speed roadways became dangerous when the vehicle would suddenly lose drive during
... the merge. We fear what might have happened if she was attempting to make a left on green and the transmission suddenly became stuck in neutral. This truck has the infamous 10R80 Ford 10 speed transmission that thousands of people have reported as defective. Ford has refused to compensate me in anyway, and the Ford dealership has quoted me $9,000 dollars to have the transmission replaced. Ford issued a TSB on this faulty transmission, but has refused to issue a recall despite the widespread failure of this part and the safety concerns.Read more

I purchased this car from Carvana on 02/25/26, 2022 Nissan Versa S Manual Transmission. On four occasions over the course of two separate days, while under way, I lost power and the car went into a limp mode. Twice I was able to pull off the road, twice I had to stop on the road. After coming to a complete stop I was able to place the car in first gear and continue on driving. This power loss
... puts me at risk to be rear ended by the following car. I took it to the Nissan Dealer on 03/17/26 expressing my concern. I was told that they could not find any code in the car that indicated the trouble I experienced, nor was there any illumination of the check engine light. Because of that I was told that my car could not be serviced.Read more
