The DEF tank in my truck is defective and its the second time in the last 14 months it has broke. I am submitting this complaint to report a serious safety issue involving my 2021 Chevrolet Suburban equipped with the 3.0L Duramax diesel engine. The vehicle has experienced a recurring problem related to the engine wiring harness. While driving, the vehicle will unexpectedly stall without warning. At the same time, there is an immediate loss of power steering assist, significantly reducing the
... ability to safely control the vehicle. This creates a dangerous situation, especially at highway speeds or in traffic. The failure appears to be linked to a defect or malfunction in the engine wiring harness, which may be causing intermittent electrical interruptions to critical engine systems. There were no prior warning signs sufficient to prevent the incident. This issue poses a substantial safety risk not only to the driver and passengers but also to other motorists. Sudden engine shutdown combined with loss of steering assist increases the likelihood of a crash. I believe this may be a systemic defect affecting other vehicles of the same model and configuration. I urge the National Highway Traffic Safety Administration to investigate this issue and determine whether a recall should be issued. Within the past year there have been a few times when driving in 6th gear (primarily, but also happens in 5th) if I press the gas and it downshifts into 5th, it will trip a gear icon warning light, either not shift, shift into 4th, 3rd, or neutral and sometimes VERY abruptly. The engine in my 2017 Lincoln MKC (2.0L EcoBoost) has a documented manufacturing defect where the cylinder block design allows coolant to leak into the combustion chambers. This has caused persistent, unexplained coolant loss and rough engine performance on cold starts since February 2025, matching the diagnostic criteria in Ford TSB 19-2346. The defective engine assembly is available for inspecti

While Ford provided a service program (21N12) for 1.5L engines, they have failed to issue a safety recall for the 2.0L variant despite identical failure modes. This vehicle is part of the class in the consolidated litigation Miller, et al. v. Ford Motor Company (Case No. 2:20-cv-01796). The transmission AND the torque converter are now doing the same thing it was doing prior to the replacement. The transmission slips in 4th, 7th and 8th gears. The torque converter whines loudly. Vehicle Issue Description – 2020 Ford Explorer Limited (Transmission Failure) The issue involves the transmission system in a 2020 Ford Explorer Limited. The transmission began exhibiting hard and erratic shifting, particularly between lower gears and when decelerating to a stop. A within days experienced complete failure, leaving the vehicle unable to properly move or engage gears. There were
... no warning lights, messages, or dashboard indicators at any point prior to the failure. This lack of warning prevented any opportunity to address the issue before it became severe. This failure created a significant safety risk, as the vehicle became unreliable in traffic conditions. Hard shifting and delayed engagement made it difficult to predict acceleration and deceleration, increasing the risk of being rear-ended or unable to safely merge into traffic. The final failure rendered the vehicle inoperable, which could have been dangerous if it occurred in a high-traffic or unsafe location. The problem appears consistent with known transmission issues reported by other owners of 2020 Ford Explorers, including hard shifting, jerking, hesitation between gears, and sudden loss of drivability. Many drivers report similar symptoms progressing to severe transmission problems or complete failure. At this time: •Confirmation by dealer or service center: 3/18/26 Despite the number of similar complaints and documented issues, there has not been a specific recall issued for the transmission system itself, although other drivetrain-related recalls (such as rear axle or driveline failures) have been issued for this model year. Based on the pattern of symptoms and widespread reports, this appears to be a systemic issue affecting the transmission in this model year, rather than an isolated incident.Read more

My 2022 Chevrolet Tahoe has a recurring pattern of excessive oil consumption, smoke from the exhaust, repeated check engine light events, P0420 catalyst-efficiency faults, and repeated catalytic-converter-related repairs. At approximately 75,000 miles, the vehicle shut down on the freeway. At approximately 121,000 miles, the first catalytic converter failed. Chevrolet of Wesley Chapel documented P
... 0420 and catalyst failure in November 2025, and GM service history reflects catalytic-converter replacements at 131,468 miles and approximately 141,082 miles. The vehicle also underwent major injector-related repair in early 2026. At 141,620 miles, Chevrolet of Wesley Chapel started an oil consumption test and stated in writing that oil consumption can lead to repeat failed converters and false oxygen sensor readings. Despite substantial repairs, the problem pattern continued. I am reporting this as a possible safety-related defect affecting engine operation, emissions-system failure, and drivability, and I am asking NHTSA to review whether this indicates a broader safety-related defect.Read more

The vehicle has 2 permanent codes. One is a communication blackout, the other code means the hardware is melting. GM is doing a formal inspection in April. The Ucode meant that the engine stopped talking to the transmission or visa versa. The vehicle “ran my daughter and I” off the highway into the dirt, on 2 different occasions. They are described as “catastrophic engine failures”. The la
... st one occurred on MLK Jr Day 2026, before the vehicle was permanently grounded. It could have been fatal. GM is doing a formal investigation to see what liability the have for it because the vehicle is now considered a “lemon”. There were no external (dashboard) warnings before the events, but, there may have been internal ones that require a professional grade scanner to detect. The permanent Ucode, which causes a communication blackout, never appeared in the vehicles memory, on a scanner, until AFTER the local GM franchise conducted “repairs” (unsuccessfully) to fix the vehicle. (It wouldnt start, which is often a symptom of a Permanent UCode. The GM investigation will determine if the dealership knew the safety liability, (the permanent Ucode) and they tried bypassing or ignoring it, or, if they CAUSED the safety defect. Also, the vehicle is now showing a Permanent PCode, which means it is now a potential fire hazard.Read more

I am filing this complaint regarding a manual transmission failure on my 2017 Mazda MX-5 Miata (VIN: [XXX] ) at 55,000 miles. Mazda issued Technical Service Bulletin 05 [XXX] c addressing this exact defect — defective synchros in 2nd and 3rd gear causing grinding, inability to engage g
... ears, and transmission failure. My VIN falls within the TSB coverage range. The cutoff is sequential number XXX and my sequential number is XXX, placing me within the affected range. Despite this documented defect and my vehicle being under Mazda's Powertrain Limited Warranty at time of failure (under 60,000 miles), Mazda refused full coverage. The dealer offered only 50% goodwill. I escalated to the Mazda regional representative, who also declined full coverage and redirected me back to the dealer — evasive given the documented nature of this defect. This is not isolated. Multiple 2016–2017 MX-5 owners have reported identical failures, and a Mazda engineer publicly acknowledged on video that weak transmission tolerances cause gear teeth to shear under load. Active petitions are calling for a formal recall. While I was able to drive to the dealership, this failure occurred without warning. A similar failure at highway speeds or in a less favorable situation could result in complete loss of vehicle control, posing significant danger to the driver and surrounding traffic. I request that NHTSA: (1) investigate this defect across all 2016–2017 MX-5 Miata manual transmission vehicles, (2) determine whether a formal recall is warranted, and (3) require Mazda to honor full warranty coverage for VINs within the TSB range. I am filing a formal safety complaint regarding the execution of NHTSA Recall 24-002 (Connecting Rod Bearing Replacement) by an authorized Acura dealership, and a fundamental safety flaw in Acura’s official Service Bulletin for this recall. My vehicle underwent "Repair #1" under this recall, which involved opening the engine block and replacing the rod bearings due to a factory defect that caus
... es metal-on-metal wear. However, upon picking up my vehicle, I discovered the dealership left the old, dirty oil filter attached to the engine. I reviewed Acura’s Service Bulletin 24-002 and realized the "Required Materials" list explicitly omits a new oil filter for Repair #1. Because dealerships strictly follow this bulletin for warranty reimbursement, they are systematically leaving old filters on engines that just received new bearings. This means old, metal-contaminated oil trapped in the filter is immediately recirculated through the brand-new precision bearings. This procedural flaw directly jeopardizes the integrity of the engine, completely undermining the purpose of the safety recall and putting drivers at risk of sudden engine failure at highway speeds. I have time-stamped video and photographic evidence of my vehicle’s mileage (61,325) and the untouched, original oil filter immediately after returning home from the dealership. Acura Corporate was contacted regarding this procedural negligence and failed to provide a resolution. I am requesting the NHTSA investigate Acura's repair procedures for Recall 24-002 to prevent further unsafe vehicle modifications.Read more

About 2 years ago my shifter started coming on like it was all lit up and I got a code saying to service shifter, So I took it in to have it fixed it was on recall. This is very difficult for me because I do not have the $3500 to fix the PTU I recently became a widow and have 3 kids; but I have always taken care of my Jeep --Oil changes, tires, regular wear and tear I take care of it. This statement is to the best of my knowledge. I was driving my 2015 Nissan Murano and when I was almost back to mine [XXX] house the transmission stopped working. I took it and had it towed over Gilman Nissan in Rosenberg. I have a warranty on it through endurance warranty company. They came and inspect the transmission. They indicated that the transmission fell, but also I know that Nissan reca
... ll these transmission on the CVT which is going out. The warranty company indicated that the CVT inside the transmission fell. Also, the alternator failed on the vehicle. They indicating that the all drip from somewhere on to the alternator was made. The alternator fail but I look on the website and I also noticed that if the alternator go out in these 2015 Nissan Murano it's messed up the transmission. So this need to be looking into and taking care cuz I believe they card not telling the real truth on what's going on with this transmission and the reason I couldn't get my car repaired because the mileage has already passed and I got mine recall letter real late INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

The all wheel drive failed do to transfer case splines being sheered off between the transmission and transfer case. No warning lights are on to warn of the rear wheel tires not receiving power to them. I was not aware of this problem and this could have contributed to and accident, as I though the AWD was working, but it wasn't.

My 2020 Hyundai Kona (VIN: [XXX] ) has a persistent engine stalling safety defect that matches Recall 203 (22-01-042H, NHTSA Campaign 21V301000 – Nu 2.0L Atkinson Engine Inspection and Replacement). The recall addresses improperly heat-treated piston oil rings causing cylinder bore scuffing, accelerated oil consumption, abnormal knocking, and potential engine stall while driving (increasing cras
... h/fire risk).I first brought the vehicle to Corona Hyundai for repeated stalling while driving. They performed the recall vibration inspection and software update (Campaign 974), stated the test passed, and declared the vehicle "fixed", safe and "good-to-go" Immediately after pickup, the stalling returned worse: approximately 60-80 miles later on XX near the XXX exit (four-lane high-speed highway), the engine completely stalled at 65 mph while merging for an exit. This caused vehicles and semi-trucks behind me to swerve dangerously and very nearly collide with me. The vehicle stalled four more times on the way to my destination in [XXX] .I then took it to the nearest dealer, Carlsbad Hyundai for safety reasons. They performed the vibration inspection again, which also "passed," yet the real-world symptoms would continue unchanged: accelerated oil consumption (I monitor the dipstick and refill regularly), sputtering, knocking noises, and complete engine stalls (dozens of incidents, especially on local roads and turns). The vehicle remains undrivable due to imminent stall risk while moving and putting myself, my passengers and other commuters at high risk of severe injury or death. Both dealers and Hyundai Customer Care (Case #XXX) have refused further action, stating the vibration test passed, they cannot duplicate the concern, and no additional claim can be submitted for engine replacement. Hyundai's TSB [XXX] requires only the vibration inspection; engine replacement is authorized if "NO PASS" – no separate oil consumption test or monitored drive INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.Read more

Problem started at 11,000 miles as very minor and has become progressively worse up to the now 32,000. Vehicle has been to dealerships 5 times with no repair performed. THE MAJOR SAFETY ISSUE: Hesitation and gear searching on acceleration. Intermittent acuity. 3 times I needed to accelerate quickly in order to merge with 40 mph traffic from a stop sign. I stepped on the throttle and the vehic
... le lurched into traffic then lost power and gear searched for 3 seconds creating a potentially very serious incident. Once the gear engaged, the vehicle took off at a high rate of acceleration and speed causing yet another unsafe condition and making me look like a maniac. THE OTHER SYMPTOMS: 1) Hard Starting, sometimes vehicle seems to sputter, sometimes it revs hard on startup 2) Vehicle sometimes enters some kind of error state similar to the vehicle not being in park when the engine is shut down. An audible alarm sounds, like when the lights are left on or a key is left in the car, and the vehicle refuses to lock the doors on its own as usual. Visual inspection by the driver reveals nothing appears to be wrong. Lights are off, no key left, vehicle is in park. Powering back on, checking park indicator and turning back off seems to reset this condition. 3) Inconsistent accelerator pedal input. Sometimes the vehicle goes much harder or seems to be held back by something at other times. 4) Erratic torque distribution for AWD. Viewed from dash torque distribution readout. AWD engages 100% at all wheels at times of very light throttle on dry pavement. 5) Engine just sounds terrible at idle and at times of hard acceleration. 6) Rough acceleration. Choppy gear shifts. 7) Vehicle continues to push forward under power when throttle is lifted and brakes are applied causing harder than usual brake pedal pressure to be necessary and inconsistent vehicle slowing. 8) No CEL is on. 9) Constant minor weaving due to inconsistent AWD torque delivery. In January of 2026, when driving under normal conditions, the 2022 Acura MDX made a loud clunking noise and power was temporarily disrupted. The vehicle had to be stopped on the side of the road as the clunking noise persisted. This occurred on a major highway and put the driver and passengers at risk as we had to quickly get to the shoulder. After inspecting the vehicle for any visible signs o
... f damage we continued home without issues. Now, March of 2026, we took the car for routine service and was contacted by the dealership and informed the transfer case and transmission needed to be replaced. The dealership indicating they were in contact with Honda USA to see what they could do for us. A quick search of the Internet revealed NHTSA DP26001, so we now know this issue has occurred on many Acura MDX's of the same year. No warning lamps illuminated. We were unaware that the vehicle was running in 2erWD instead of 4WD. There were no warnings leading up to the event.Read more
