My 2020 Lincoln Aviator Grand Touring Plug‑In Hybrid has experienced multiple high‑voltage powertrain failures that create an immediate safety hazard. The vehicle has twice lost propulsion while driving, displaying “Powertrain Malfunction / Reduced Power” and “Service Engine Soon” before shutting down completely. During the first failure, the vehicle stopped in active traffic and would
... not restart; a police officer stopped to assist and called a tow truck because the disabled vehicle posed a roadway hazard. In addition, the digital instrument cluster has repeatedly gone completely black while the vehicle is in motion, eliminating all visibility of speed, gear position, warning indicators, and hybrid system status. During the most recent blackout, an [XXX] child was in the vehicle with me, significantly increasing the danger. These failures are accompanied by hybrid‑system fault code P0A1E (Starter/Generator Control Module), a pink charging‑port indicator signaling a charging fault, and irregular hybrid‑battery behavior (range not updating, battery not depleting). The vehicle has been inspected four times by authorized Lincoln/Ford dealers and has been out of service more than 34 cumulative days. Although the P0A1E code and symptoms were documented, the dealers repeatedly stated they were “unable to verify” the concern and returned the vehicle unrepaired. Required high‑voltage diagnostics were not completed. The problem is ongoing, intermittent, and has not been corrected. Sudden propulsion loss, dashboard blackout, and high‑voltage control failures put occupants and other motorists at significant risk. I am requesting NHTSA investigate this as a potential safety defect. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

While driving on the highway at normal speed, my vehicle was operating completely normally with no warning lights, messages, or prior issues. Without any warning, the vehicle suddenly lost the ability to accelerate and felt as if it had been shifted into neutral. I immediately had to pull over to the side of the highway, as this created a dangerous situation with surrounding traffic. The vehicle h
... ad to be towed to a mechanic. Upon inspection, I was informed that a coolant line connected to the transmission had completely blew off, causing a rapid loss of transmission fluid and resulting in total transmission failure. I was told this was not due to a rusted or deteriorated clamp, but rather a sudden component failure. This vehicle has been meticulously maintained and I have owned it for less than one year. There were no prior symptoms, leaks, or warning signs leading up to this incident. This appears to be a sudden and unexpected mechanical failure, not caused by neglect or misuse. The loss of power while driving at highway speed posed a serious safety risk to myself and others on the road.Read more

I managed to start the engine once, accelerating before it shut down again, and coasting into a parking lot. The vehicle would not start again. The dashboard and lights would only flicker and click (even while the vehicle was totally off). We called the roadside emergency assistance that was sold with the vehicle, only to learn that we had been lied to and that Coachmen Forest River is not covered. We had to cover all expenses. The vehicle was towed to Burt Ogden Dodge Ram service center in Harlingen, Texas. They attempted to repair it 3 times before finally telling me it could not be repaired and it was the upfitter's fault. The service case was closed and we were told the vehicle was sent to Forest River (RV upfitter). Nobody has contacted us about it and we do not know the status of the vehicle. It has now been 7 months since we purchased this new RV. We don't know where it is, the repair status, what happened, or what to do next.Read more

UNINTENDED VEHICLE MOVEMENT AT STARTUP - POTENTIAL CLUTCH DEFECT On January 17, 2026, at approximately 1:59 PM, I started my 2019 Genesis G70 (manual transmission) while parked in my garage. The vehicle was in Neutral with the parking brake engaged. I depressed the clutch pedal fully before pressing the start button, with no foot on the accelerator. Immediately upon starting, the vehicle lurch

VEHICLE INFORMATION Year: 2018 Make: Honda Model: HR-V VIN: [XXX] Mileage at Time of Incident: 96,993 Incident Date: 03/10/2026 I am filing this complaint regarding the premature and complete failure of the CVT (Continuously Variable Transmission) on my 2018 Honda HR-V with approximately 96,993 miles. DESCRIPTION OF FAILURE: The transmission on my 2018 Honda HR-V failed without prior warning.
... Symptoms included sudden loss of acceleration, vehicle unable to move under its own power, etc. The vehicle was taken to Page Honda of Bloomfield where it was diagnosed with a complete CVT transmission failure. The repair cost totaled $8,873. SAFETY CONCERN: This failure poses a serious safety risk. I received the first recall notice in August 2025 and the second (25V512) in March 2026. My dealer, Scenic Ford in Mount Airy, NC, told me they do not have the parts and cannot get them. On September 17, 2025, I contacted Ford directly and was told the recall would be elevated to an emergency order and parts would be shipped. As of today 03/24/2026, the dealer still has no parts, and Ford has not provided a repair date. I contacted Ford Consumer Care and was told that they do not have parts. The safety recall states that the parts are now available. The rear‑end noise is getting worse, indicating the defect is progressing. I am concerned the vehicle may become unsafe to drive. I have owned Fords since 1967 and have never experienced this level of delay on a safety recall. I am writing to document that Ford cannot give me a date to fix my F-150 and has told me to continue to drive my 2024 F-150 and will not give me a replacement vehicle to use until mine can be repaired. The vehicle was brought in for service on February 20, 2026, and has remained at the dealership since that time. During this period, no clear resolution timeline has been provided, and communication from both the dealership and manufacturer has been limited to general status updates with no substantive progress. The lack of heating functionality raises safety concerns, especially given the vehicle’s reliance on electronic systems for climate control. A loaner vehicle has been provided; however, the extended delay without repair or replacement raises concerns about potential defects in the HVAC or related electrical systems. The inability to resolve the issue within a reasonable timeframe suggests a possible systemic defect that is affecting other vehicles of the same make and model. I am reporting a transmission failure in my 2022 Honda HR-V at approximately 129,000 miles. While driving on the interstate highway at normal speed, the vehicle suddenly began revving excessively without accelerating. The engine RPM increased, but the vehicle failed to maintain proper speed or respond as expected. This occurred without any prior warning signs or indicators. Shortly after, the ve
... hicle continued to have issues where it would shift into gear but not properly move or engage. All dashboard warning lights became illuminated following the incident. I took the vehicle to a dealership for diagnosis, and they confirmed that the transmission is failing. They reported finding metal shavings inside the transmission, indicating internal mechanical damage and premature wear. This sudden loss of power and inability to accelerate properly while driving at highway speeds created a serious safety hazard. The vehicle could have been rear-ended or caused an accident due to the unexpected loss of performance. A major component like the transmission should not fail in this manner, especially without warning, as it poses a significant safety risk to the driver and others on the road.Read more

Driving my truck and I suddenly got a warning on my dashboard that said, "Low Oil Pressure. Turn Engine Off." I pulled over immediately, shut off the the engine and called for a tow. Dealership called to let me know the engine was seized and would cost $22,270 to repair. Looking online I found that Chevy had released a Customer Service Program (CSP) detailing that the oil coolant lines in 2019 Sil
... verado 1500 could have a weak / defective crimp holding the oil coolant line onto the connector and could separate causing catastrophic oil loss. It is also stated in the CSP that cold weather makes the issue more likely. I have tried getting GM to cover the repair since they knew about the issue and never contacted me to make the fix. GM is maintaining that they did nothing wrong and that my VIN is not listed as one of the vehicles in need of repair. This, despite the acknowledgement of the CSP and that the CSP "is for the dealerships." Which I pointed out the correction listed was for me to have the oil lines replaced and that never happened. I also pointed out as they said this wouldn't have been listined on my VIN because it was for dealerships and yet no one: not GM, not the dealership I bought the truck from, nor the dealership that regularly serviced my truck mentioned the oil line issue to me.Read more

I could have hit someone or even got hurt my self.
