My 2018 Ford Escape, which has experienced catastrophic engine failure due to the well-documented coolant intrusion defect in the 1.5L EcoBoost engine. In early March 2026, without any prewarning or engine light, my vehicle displayed an “Engine Fault Service Now” warning and stored diagnostic trouble code P2601 (Coolant Pump A Control Circuit Performance/Range). I immediately took the vehicle
... to Bidwell Automotive in [XXX] , where on March 10–11, 2026, technician Gannon performed a thorough inspection. The findings were as follows: the coolant level was critically low; the water pump showed signs of leaking; the top of the piston in the affected cylinder appeared washed clean (a hallmark of coolant entering the combustion chamber); and the technician detected an odor consistent with combustion gases in the cooling system. The diagnosis was a suspected failed head gasket consistent with internal coolant intrusion. I then brought the vehicle to Future Ford Lincoln of Roseville, your authorized dealership, on March 12, 2026 (RO #XXX, Service Consultant [XXX] , Certified Technician [XXX] ). Future Ford verified the coolant intrusion into Cylinder 2 by removing the spark plugs and performing a borescope inspection. The dealership’s own technician referenced TSB 22-2322 and recommended either a short block replacement ($9,396.45) or a long block replacement ($13,331.01). The dealership confirmed that my vehicle has no remaining warranty coverage and offered no assistance beyond the paid repair estimate. Ford’s Documented Knowledge of This Defect Ford Motor Company has long been aware that the 1.5L EcoBoost engine in 2017–2019 Escape vehicles suffers from a structural design defect in the engine block’s cooling passages that causes coolant to intrude into the cylinders. Ford’s own actions confirm this knowledge. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Incident Summary: On 2/2/2026, my vehicle was serviced under Safety Recall 25V-274 at an authorized GMC dealership. The service technician performed the mandated diagnostic protocol (including the PicoScope test) and informed me that the engine had "passed" inspection. No internal components were replaced; the remedy was limited to an oil change to a higher-viscosity oil (0W-40) and a new oil fill
... er cap. The Failure: Approximately 48 hours after this "successful" recall remedy, the vehicle suffered a catastrophic engine failure while in operation. The engine failed entirely. The vehicle lost all propulsion while in motion, creating an immediate safety hazard for myself and other motorists. Diagnostic and Risk: The failure has been confirmed by a GMC dealer, who subsequently performed a full engine and radiator replacement. In the 48-hour prior to failure, there were no warning lamps or symptoms; the vehicle had just been cleared by the manufacturer's own diagnostic tools as safe and free of defects. Systemic Concern: This incident demonstrates that the current diagnostic protocol for Recall 25V-274 is insufficient to identify terminal manufacturing defects in the connecting rods and crankshaft of the L87 6.2L V8 engine. The "remedy" of higher-viscosity oil failed to prevent the exact catastrophic loss of propulsion the recall was intended to address. The replaced engine is available for inspection upon request by the NHTSA Office of Defects Investigation.Read more

Beginning in January 2026, the vehicle exhibited progressive symptoms including repeated lurching and difficulty shifting gears. These symptoms are consistent with the open and incomplete recall for the transmission shifter cable bushing and the powertrain control module reprogram. On or around March 25–26, 2026, while operating the vehicle at approximately 35 mph on a surface street, the foll
... owing failure sequence occurred: the vehicle began producing a repetitive thudding sensation (thud-thud-thud), followed by violent shaking of the steering wheel, followed by a complete loss of steering response. The vehicle drifted uncontrollably into an adjacent lane of active traffic under clear, dry weather conditions — a loss of control consistent with brake hose failure causing uneven braking force, compounded by the known transmission and PCM defects. Collision with another vehicle was narrowly avoided. The vehicle currently has five open, incomplete recalls: (1) front brake flexible hose leak, (2) rear brake flexible hose leak, (3) transmission shifter cable bushing, (4) rear-view camera inspection, (5) powertrain control module reprogram. None have been completed. The described failure is consistent with and likely caused by these unresolved manufacturing defects. Over about seven years and roughly 70,000 miles of ownership, this 2012 BMW X3 generated repeated “Drivetrain malfunction” warnings and loss of drivability despite extensive repairs by multiple authorized BMW dealers. In December 2025, an authorized BMW dealer diagnosed a drivetrain malfunction as failure of the fuel pump control module, allegedly caused by a small water spill on the rear seat, charged me $1,690.71 to replace the module, and returned the vehicle as repaired. Shortly afterward, while driving normally, the same “Drivetrain malfunction” warning reappeared, and drivability issues returned, indicating the defect was not corrected and raising serious concerns about the safety and reliability of the drivetrain and related electronic controls. The dealer’s attribution of the failure to minor interior water exposure also caused the service‑contract administrator to void coverage for the fuel system, leaving me with this large repair bill for an unresolved safety‑related condition that ultimately forced me to dispose of the vehicle at a significant loss.Read more

A persistent humming/whining noise occurs at highway speeds (around 60–70 mph). The noise has been present for more than two years and has continued through multiple major repairs. The sound appears to come from the drivetrain area and raises concerns about a potential driveline or mechanical failure at speed. The issue has been reproduced and confirmed by multiple authorized dealerships. The v
... ehicle has undergone two rear differential replacements, a transfer‑case replacement, chassis ear testing, STAR engineering cases, and testing with OEM wheels and tires. The noise remains after all repairs. The manufacturer’s Area Representative test‑drove the vehicle and verified the noise. Despite this, the manufacturer has stated that no further repairs will be approved, and the vehicle was returned with the defect still present. The vehicle has been out of service for more than 100 days for attempts to diagnose and repair this issue. The unresolved noise at highway speeds creates a safety concern because it may indicate an underlying drivetrain problem that could worsen or fail while driving. There were no warning lights or messages. The only symptom is the persistent noise at highway speeds.Read more

The vehicle experienced multiple electrical and safety-related malfunctions while driving. On several occasions the instrument cluster and vehicle screens suddenly went black while the vehicle was in motion, causing loss of critical driving information such as speed and warnings. In another incident the vehicle displayed a “High Voltage Malfunction” warning while driving and the vehicle shut
... down shortly after. This resulted in a sudden loss of propulsion and created a dangerous situation on the road. The vehicle has also displayed a “Coolant Too Hot – Stop Vehicle” warning which required replacement of the coolant pump at the dealership. In a separate event the vehicle lost battery charge unexpectedly after indicating a full charge and had to be towed to a charging station. The vehicle is also subject to Safety Recall 26V073 involving the high-voltage battery. According to the recall notice, the battery may fail due to an internal short circuit which could lead to a vehicle fire while parked or driving. The manufacturer currently does not have a repair available and advises owners to limit the battery charge to 80% and park the vehicle outside when not in use due to the potential fire risk. These repeated failures combined with the unresolved safety recall raise serious concerns about the safety and reliability of the vehicle. The issues began around September 2024 and have continued through multiple repair visits into March 2026.Read more

Transmission won't go into reverse at times, auto shifting back to park before check engine light comes on. Vehicle initially started with hard shifts at low speeds about 25 mph. Issue progressed to the point where vehicle has to be put into reverse. Otherwise vehicle would not shift at the same point about 25 mph and would be stuck in the low gear until vehicle is shut off and restarted then put
... into reverse before proceeding. The issue has continued to grow where shifting into reverse causes the vehicle to noticeably lurch back with a very heavy thunk before putting it into drive. Reverse takes time engaging. However the issue will persist and it will not go into reverse before Auto shifting to park and the check engine light comes on. Vehicle would drive for a little while before the transmission would stop shifting and go into limp mode where I wouldn't be able to drive over 30 miles per hour without very high RPMs. During some of those times the vehicle would take a long time before the transmission would engage and shift which would cause the vehicle to jerk abruptly forward. And at times back tires have lost traction. Recently the vehicles transmission has disengaged entirely while driving. At times it'll be 40 mph where it will disengage or in one case about 15 mph as I was making a turn and in the intersection it disengaged. First time the vehicle went to a dealership. They were unable to replicate the problem and that was approximately 30,000 mi on the odometer. The second time it went in with the issue it was diagnosed as being a service bulletin which was then updated and issue remained. The third time it was brought in there was a code that showed that the clutch gear ratio was incorrect. Mechanics have said the vehicle needs a new transmission and this has been a known issue with the 8-speed 8 HP75 by ZF transmissions. Transmission has also surged and lunged while braking at various points. While completely stopped, vehicle has lurched forward.Read more

Subject: Safety Complaint – Loss of Drive Power Prior to Recall Resulting in Crash (2025 Honda Accord Hybrid) I am submitting a safety complaint regarding a 2025 Honda Accord Sport Hybrid (VIN: [XXX]) involved in a crash on [XXX], which we believe is related to a recall issued later in January 2026. At the time of the incident, the vehicle was being driven under normal conditions, below the sp
... eed limit, on an empty road. The driver suddenly experienced a loss of control, the vehicle became unresponsive, veered out of its lane, and did not respond to steering correction. The vehicle ultimately collided with an electric pole and was declared a total loss. At the time of the crash, no recall or warning had been issued. We reported the incident to Honda on October 23, 2025 (Case #[XXX]), describing the loss of control and unresponsiveness. However, no mention was made of any potential defect or safety concern. In January 2026, we received a recall notice stating that if the ICM CPU resets while the vehicle is in operation, it may cause loss of drive power and increase the risk of a crash. This description closely matches what occurred during our accident. We also informed the dealership, Scott Honda (Vineland, NJ), but our concerns were not investigated. Instead, the dealership attempted to sell us another vehicle without addressing the safety issue. We believe the defect existed prior to the recall and may have directly contributed to the crash. We are also concerned that the manufacturer did not warn us despite our earlier complaint describing similar symptoms. This incident resulted in the total loss of our vehicle and ongoing financial hardship, as we have been without a replacement vehicle for over six months. We respectfully request that NHTSA investigate the timing of the recall, the manufacturer’s awareness of the defect, and how pre-recall complaints like ours were handled. We are willing to provide all supporting documents. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5Read more

Each time I return the vehicle for service, I am informed that it is a “different issue,” even though the symptoms remain unchanged. This strongly indicates a defective powertrain and/or electrical system that has not been properly repaired. The most serious concern occurred while I was driving on public streets, when the vehicle lost acceleration, electrical power shut off, and the vehicle completely shut down, nearly causing me to be hit by other vehicles. This incident created an extreme safety hazard for myself, my passengers, and other motorists. For several months I have been having problems off and on with my vehicle not shifting into drive, neutral, or reverse. It runs fine but becomes undrivable. Until this last time, I was always able to keep trying, let it sit overnight, etc... and it would eventually shift into gear. Currently it does not, I had to have it towed to a shop to be repaired. This is a problem, the vehicle has left m
... e stranded on several occasions. My daughter was recently home from collage and I could not let her use my car for fear she would get stranded somewhere. It was towed to a local shop, who was able to confirm that the transmission has an internal short. This is a very common occurrence with this vehicle. The transmission can not be repaired and it needs a new one. Each time the transmission would fail a dash light would come on with a P in a circle with an explanation mark next to it and an engine light. This happened several times, in the past I was able to clear the light and drive the car, but this time it would not clear. The transmission is shoot and needs to be replaced. I purchased the car new in February of 2017, and have always properly maintained the vehicle. It has approximately 79,800 miles on it. I have researched this issue and it is very common, this is a known problem with the 2017 Chrysler Pacifica. I have attached a Warranty Bulletin that seems like it should cover my vehicle, but I called the Chrysler Customer Hotline and they are telling me my vehicle is not covered. I have taken care of this vehicle, I drive it very gentle, there is no way this transmission should have gone bad. I am going to get this repaired at the shop, they have ordered me a new transmission.
