My 2017 Ford Expedition is currently at an authorized dealership (Arlington Heights Ford, IL) for a failed transmission lead frame (DTCs P0706, P0707, P1702), which causes intermittent loss of gear engagement and sudden shift issues. This specific part is subject to NHTSA Engineering Analysis EA26001. Ford is refusing to honor the 10-year/150000-mile extended coverage (Program 19N01 Supplement 4)
... and a separate safety recall (25S37 for Brake master Cylinder) because the Vehicle was originally manufactured for Canadian market. Despite the Vehicle being in a U.S. facility, Ford claims a 'system limitation' prevents them from bridging the VIN for these safety repairs. Incident date: Apr 1, 2026
System Failure: The Power Train (6-speed Automatic Transmission) suffered a major malfunction, documented by diagnostic codes P0731 (Gear 1 Incorrect Ratio) and P0780 (Shift Error). The vehicle is available for inspection. Safety Risk: The failure caused a sudden loss of propulsion while attempting to accelerate. This created an immediate risk of a rear-end collision and left the vehicle unrespon
... sive in the path of oncoming traffic. The transmission's unpredictable shifting makes the vehicle unsafe to operate. Confirmation: The failure and the P0731 code were officially confirmed by Castle Mazda on March 2, 2026. The manufacturer (Mazda) has been notified but has denied a remedy, despite TSB MC-10160555-0001 addressing this specific internal defect. Warning Symptoms: There was no Check Engine Light or dashboard warning prior to or during the failure. The only warning was an extreme, abnormal mechanical grinding/clunking noise from the transmission. The lack of a malfunction indicator light (MIL) meant I had no electronic warning of a looming mechanical failure while in motion.Read more

STATEMENT OF FACTS: I presented my Kia K5 for two safety recalls (SC356 and SC263) with an active Check Engine Light for code P047100 (Exhaust Pressure Sensor). I informed the service advisor, Aliyah, that this code is a symptomatic secondary fault of the pressure irregularities addressed in Recall SC356, which explicitly lists an "Illuminated Check Engine Light" (MIL) as a primary warning sign. T
... he dealership dismissed my technical assessment—which was backed by advanced AI data analysis. After the software update was performed, the advisor claimed the code remained "active," thereby "proving" it was unrelated to the recall. Based on this, I was charged a $248 diagnostic fee and told I needed a $534.02 out-of-pocket hardware repair for a "physically failing" differential pressure sensor that was allegedly reading 2 HPA against a spec of 23.09 HPA. EVIDENCE OF ERROR: Upon taking delivery of the vehicle, the Check Engine Light was completely extinguished. If the light were truly "still active" after the update, as I was told to justify the fee, the fault would have been present at pickup. Since then, I have driven over 120 miles (including multiple cold-start drive cycles between [XXX] and my place of work) with a 100% clear dashboard. My XXX App confirms the XXX fault is resolved, while the recalls remain "Open" administratively. REGULATORY VIOLATION: Under 49 U.S.C. Chapter 301, a manufacturer must remedy a safety defect without charge. A software "Logic Update" cannot physically repair a broken pressure sensor. The fact that the fault disappeared immediately following the update proves the sensor was functional and the fault was caused by the recall defect. By misdiagnosing a recall symptom as a non-covered hardware failure, the dealer effectively charged me for a federal safety repair. I request an investigation into these "diagnostic gatekeeping" practices and a full refund of the $248.00 fee. While driving my 2021 Cadillac Escalade on an interstate highway at approximately 70 mph with my family, including two minor children, the vehicle suddenly and without warning lost propulsion, shut off, and shifted into neutral. This occurred in active traffic and created an immediate safety hazard. A warning message appeared instructing me to restart the vehicle. After several minutes navigating
... traffic, I was able to restart the vehicle briefly and exit the highway. Shortly thereafter, additional dashboard alerts appeared indicating critical engine and transmission failure. After exiting the highway, the vehicle shut down completely at a traffic signal and failed to restart. The vehicle was immobilized in an active roadway until police assistance arrived to prevent a rear‑end collision and a flatbed tow was arranged. The sudden loss of propulsion at highway speed placed vehicle occupants and surrounding motorists at serious risk. The vehicle is available for inspection and was diagnosed as having a seized engine on March 9, 2026, by Diamond Cadillac in [XXX] . This failure did not occur without warning. Prior to this incident, I repeatedly reported excessive oil consumption to the selling dealer, Otto Cadillac, from whom I purchased the vehicle in April 2025 as a Certified Pre-Owned vehicle. On two separate service visits, I reported oil consumption exceeding two quarts between recommended service intervals and requested that this be documented. I was advised that the consumption was “within tolerance” and was later informed—incorrectly—that this tolerance applied to my engine configuration. Otto Cadillac performed a recall-related inspection and implemented the manufacturer-directed remedy consisting of an oil cap replacement and a change in oil viscosity to SAE 40. No mechanical repair or deeper diagnostic action was taken. The subsequent engine seizure confirms that these mitigation measures did not resolve the underlying defect. The dealer has performed two separate transmission fluid flushes, which temporarily

The dealer has now recommended a triple transmission flush at a cost of approximately $1,000, which the dealer acknowledged is a suboptimal solution. The dealer stated their hands are tied because no recall has been issued for the 2019 model year. This is the same transmission shudder defect that led to recalls and Technical Service Bulletins for the 2016, 2017, and 2018 Honda Pilot models. The 9-speed ZF automatic transmission (9HP) in the 2019 Pilot is the same or substantially similar unit. Shift to Park. Unable to turn the car off even though the car is in park. This was previously fixed in 10/24 and started up in 10/25 a few days after the 1yr warranty. I reached out to the dealership that repaired it in 2024 and was told that I would have to pay for the service as the part is not covered under the powertrain warranty. I reached out to GM for assistance and that was a lost cause. I
... was told that the repairs will only last 1 to 2 yrs, so this appears that it will be a temporary fix. My car has about 55000 miles on it. To know that I may possibly have to continue to get this fixed, I feel like this should be considered a lemon and they should refund me for the cost of the vehicle or give me another car. GM knows that the Shift to Park is an ongoing issue with this year, make, model and have failed to issue a recall.Read more

I reported problems with my truck to Beck and Masten South GMC. I was put off a number of times with ridiculous excuses. My vehicle sat in the shop for a month to get my OnStar safety equipment repaired. I also had arguments with them regarding the oil light coming on and it burning excessive amounts of oil. I was told this was normal. I know better. Also, I needed to get it back into the shop for
... service and inspection for a squeeling noise under the hood. And to have them check the oul consumption. I was told it idle too often, and this was the reason oil was dissapearing. I was told they would put me on the loaner list so I could have means to get to my oncologist for cancer care. Well, I called numerous times because they never furnished a loaner for these issues I was told a third, then forth time I was next in line for a loaner so I could drop it off for repairs and recall work. After 8 months of these lies I finally went to Monument Chevrolet. Beck and Masten never intended to help me. Total I had tried and was told by newer service advisors, the manager, and GMC this would be remedied over 10 times in over 6 months. I finally gave up and moved on to never return to this dealer I had purchased two brand new GMC 1500 trucks from in a two year time frame. I did get my OnStar gateway repaired by replacement. It had stopped working again, shortly after getting it back. I kept getting put off. They stated it needed another Gateway for OnStar. I kept calling, and was told they would contact me ince it came in. I ended up troubleshooting the entire modem/router assy. I found inconsitant configuration parametrs. So, I reconfigured it all from start to finish after clearing the memory by taking battery cables off and allowing it to power down and clear memory. Then I did perform a reconfiguration and username/password change as well. I repaired it myself. The Onstar has never acted up again. These were my first issues. I had to take it to Monument Chevrolet for serviceRead more

My drive motors keep failing (so far 3 times) and my emergency braking abruptly stops my vehicle when no true danger is present and I’m in complete control of my vehicle. Currently my vehicle has a constant shuddering while braking which doesn’t feel safe because it’s not braking properly. Audi regional and the dealership refuses to take ownership of the issue. Dealer has confirmed issue and
... replaced drive motors three times. Each time my braking system has gotten worse. The part was replaced each time under warranty but Audi has not confirmed that a fix has been found and corrected. This means it can occur again and abruptly which can absolutely be a safety hazard as it was each time it failed on me.Read more

I am reporting a safety issue involving my 2019 Honda Odyssey transmission. Since early ownership, the vehicle has had ongoing hesitation and delayed response when accelerating, especially during gear shifts. Over time, this condition has worsened and now includes intermittent loss of power when pressing the accelerator, followed by sudden jerking or lurching forward. The most serious incident o
... ccurred recently when the vehicle unexpectedly shifted into park while moving in a parking lot, creating a dangerous and unsafe condition. Additionally, there have been multiple instances where the vehicle hesitated in intersections, creating a risk of collision due to lack of response when attempting to accelerate. Honda dealership technicians have acknowledged that these vehicles are known to have rough or problematic shifting, indicating this may be a broader issue. The final issue has resulted in the vehicle being towed to the dealership for a full evaluation.Read more

I believe the torque converter is failing. While driving approximately 38 mph and also 48 mph up to 51 mph the vehicle have a vibration feeling like driving on a rumble strip. My 2018 Ford F‑150 XLT has the 10R80 automatic transmission with severe issues including harsh shifting, delayed engagement, slipping between gears, and unpredictable shift timing. Symptoms occur during normal driving and create a serious safety hazard. Ford dealer replaced transmission with a remanufactured 10R80, part number JL3Z‑7000‑ERM, and is listed in Ford’s Equipment Safety Recall
... 25SB3 (dated 10/28/2025) for missing internal bearings and unsafe shifting behavior. When the Ford dealer replaced the transmission on 02/26/2025, they recorded mileage as 70,009 miles, 9k miles lower than the actual mileage of 79,009. This discrepancy creates a false service record and misrepresents when the failure occurred. It also affects warranty determinations, recall eligibility, and the accuracy of the vehicle’s service history. The mileage history shows a consistent progression documented by two independent auto shops (see attached files). This mileage discrepancy impacts warranty and goodwill eligibility, recall-related determinations, accuracy of Ford’s internal service history and Carfax/AutoCheck reporting. Despite the replacement, the transmission continues to exhibit the same dangerous symptoms. As a disabled veteran, I rely heavily on safe, predictable vehicle operation. Delayed engagement and slipping created an elevated risk when operating this vehicle. The dealer informed me that Ford has not provided a fix and that they cannot take further action. I have stopped operating the vehicle due to the severity of the transmission defect and the safety risks it creates. I request that Ford correct this mileage entry and ensure the digital service record reflects the accurate mileage. I also request NHTSA investigate the transmission and mileage matters, as the vehicle is unsafe to operate and Ford has not provided a remedy. Thank you in advance.Read more

Rapid loss of oil from the transfer case, Oil was everywhere. Now I have vibration, wooing sound and hard down shifts from a failing transfer case.. The dealership refuse to replace it because they cleaned the area and it did not leak oil again. Probably, because there's no more oil left in it to leak. This is unacceptable.

The CVT transmission failed on my 2018 Toyota C-HR, which I purchased in 2022. The vehicle stalled in the middle of traffic on a day with elevated temperatures while my special needs child was a passenger, creating an immediately life-threatening situation. The car showed no signs of any issue prior to this failure. I attempted to have the vehicle inspected multiple times before the failure occurr
... ed, but this defect is not detectable without a deep diagnostic inspection and by the time that level of inspection is needed, the transmission is already in the process of failing. There are no warning lights or symptoms until complete failure occurs. When I contacted Toyota, the representative acted as though this was an isolated issue and not a known defect. This is false. This is a viral, widespread problem that the vast majority of 2018 Toyota C-HR owners are experiencing and actively reporting across forums, consumer complaint sites, and to NHTSA itself. Toyota is fully aware of this defect. Upon seeking an independent inspection, the mechanic showed me the same transmission from another vehicle of the same make and compared it to mine, explaining that the C-HR uses a larger CVT transmission, making a used replacement approximately $6,000 alone and advised that given the repair cost relative to the car’s value, purchasing a new vehicle may be the only realistic option. I am now completely without transportation due to what is clearly a premature manufacturing defect on a 7-year-old vehicle, causing severe financial hardship. Notably, Toyota Canada has already issued a recall and extended the warranty to 10 years/100,000 miles for this exact same defect on the same model. U.S. owners are being denied the same protection and quoted $10,000–$14,000 for a known manufacturing defect. I am requesting NHTSA investigate and issue a U.S. recall for all affected 2018 Toyota C-HR vehicles immediately.Read more

I own a 2023 Ford F-150 Lightning, with 48,200 miles and 2.5 years of ownership. Recently, my truck began displaying a “Powertrain / Reduced Power” warning while driving, and the air conditioning stopped producing cold air. The dealer scanned the vehicle and found DTC P0534 – A/C System Charge Loss. Further diagnostics indicated that multiple vehicle modules may require reprogramming and sof
... tware updates. The dealer stated they need to evacuate and recharge the AC system, add dye, and remove front trunk components to locate the leak. This AC system is part of the high-voltage battery and eDrive thermal management system, meaning that failure of the HVAC system affects battery cooling and powertrain performance. Loss of cabin cooling is a symptom of a thermal system malfunction, which can lead to battery overheating, powertrain derating, and reduced driving performance. The combination of unexpected reduced power and loss of cabin cooling creates a safety risk, particularly during highway driving, merging, or situations requiring acceleration. These failures occurred at relatively low mileage and are associated with the high-voltage EV system, which should be functioning properly to ensure safe vehicle operation. I am submitting this complaint to report a potential safety defect with the 2023 Ford F-150 Lightning affecting the battery thermal management, high-voltage AC system, and eDrive components, which can result in sudden power loss and compromised vehicle safety.Read more
