The All Wheel Drive (AWD) system of the vehicle became partially inoperable after hearing a loud bang under normal vehicle acceleration some time in early 2025. The vehicle's ability to distribute power evenly to all 4 wheels was hindered and operating as front wheel drive only after the loud noise was observed. Additionally, transmission gear oil began leaking underneath the vehicle around the transmission casing. The issue has been reproduced, confirmed, and validated by technicians of a local Acura dealership. The primary failure is a total loss of engine power and electrical systems while the vehicle is in motion. The dealership (Ray Skillman Southside Mitsubishi) has diagnosed this as a failed engine/body control computer system. The vehicle is currently located at the dealership's service center and is available for inspection. The vehicle has stalled/shut off completely while driving on four separat
... e occasions. This resulted in an immediate loss of power steering and motive power, nearly causing collisions in traffic. A vehicle that loses all power without warning is a severe safety hazard to the driver, passengers, and other motorists on the road. Yes. The problem has been confirmed by the Ray Skillman service department. They have attempted to repair the issue 3 times without success and currently state that the vehicle is inoperable while they search for a used replacement computer component. The vehicle has been inspected multiple times by the Ray Skillman dealership technicians. To date, no representative from the manufacturer (General Motors) or insurance has performed an independent inspection, though a formal complaint has been initiated with the dealership's corporate management. There were no consistent warning lamps or symptoms prior to the failures. The vehicle would unexpectedly lose all power and shut down. These symptoms first appeared within the first 100 miles of ownership (purchased at 11,000 miles; current mileage approximately 11,800 The dealership has refused to provide a loaner vehicle and is currently attempting to resolve a recurring safety failure using a used/salvage electronic component rather than a certified new part, which I believe further compromises the safety of the vehicle.Read more

My 2022 Kia K5 GT-Line has had a recurring check engine light fault (P0471 — exhaust pressure sensor circuit) since approximately 9,874 miles in September 2022. The vehicle has been brought to the servicing Kia dealer a minimum of five times for this same fault across nearly 65,000 miles. It has never been permanently resolved. Most critically: dealer repair records from May 2025 (56,501 miles)
... confirm a technician identified fault code P047100, noted it as a history code, cleared it, test drove the vehicle, and returned it without performing any repair and without informing me. Ten months later, the same dealer presented this identical fault as a $1,121.07 customer-pay repair on a vehicle under active 10-year/100,000-mile powertrain warranty. The repair was only approved after I contacted Kia Consumer Affairs directly — the dealer had not contacted them despite claiming otherwise. The February 2026 repair failed within 15 minutes of pickup. I made five documented phone calls to the dealer over 33 days reporting the failure. No action was taken. The April 2026 repair — sensor and hose replacement — failed within one hour of pickup on April 1, 2026. Of additional concern: Recall SC288/SC288A (Fuel Tank Inspection and ECM Re-Update) was performed on this vehicle twice — July 2024 and August 2025. The ECM governs the exhaust pressure sensor circuit. The P0471 fault first appeared in dealer records between these two recall dates and has persisted through both completions. CARFAX confirms the April 2026 repair involved the engine/powertrain computer module. I am requesting NHTSA investigate whether SC288/SC288A is connected to recurring P0471 faults in 2022 Kia K5 vehicles. Kia Consumer Affairs Case escalated April 2, 2026.Read more

In July 2023, at 55,496 miles on the odometer, the original CVT transmission failed and was replaced at Port City Nissan (120 Spaulding Tpke N, Portsmouth, NH 03801), an authorized Nissan dealership, with a Nissan-supplied rebuilt CVT transmission at a cost of approximately $7,000. The rebuilt transmission was warranted for one year. Approximately 2.5 years and only 26,500 miles after installatio
... n, the rebuilt CVT transmission has failed again at approximately 82,000 miles. CVT transmission failure while driving presents a serious safety hazard, as the vehicle may lose power unexpectedly, particularly dangerous at highway speeds, during merges, or in intersections. Pattern of Defect: Nissan’s CVT transmissions have been the subject of widespread consumer complaints and multiple class action lawsuits across various model years. Significantly, Nissan recently updated its warranty on rebuilt CVT transmissions from one year to three years with unlimited mileage for replacements performed after July 2024. This warranty extension strongly suggests Nissan is aware of systemic premature failures in its rebuilt CVT units. The failure of a rebuilt transmission at only 26,500 miles indicates a fundamental quality control or design deficiency in the rebuilt units Nissan is supplying to dealerships. Safety Concern: Sudden CVT failure causes immediate loss of propulsion and can result in loss of vehicle control. This is particularly hazardous on highways, in heavy traffic, and during adverse weather conditions. Given the pattern of premature failure in Nissan’s rebuilt CVT units, I believe this constitutes a safety defect affecting a population of vehicles and warrants investigation. I request that NHTSA investigate Nissan’s rebuilt CVT transmission program and the adequacy of Nissan’s response to known premature failures.Read more

The power liftgate system appears to be malfunctioning. The vehicle intermittently displays a warning that the liftgate is open when it is fully closed and secured. When this occurs, the liftgate will not open using the exterior handle, interior button, or key fob. Additionally, the false “liftgate open” alert triggers the vehicle’s alarm system to activate unexpectedly without any attempt t

This malfunction creates a safety risk because the liftgate cannot be opened when needed, which could prevent access to emergency supplies, child safety items, or the spare tire in an urgent situation. The false alert also causes the vehicle alarm to activate randomly, which can startle the driver and others nearby, potentially causing distraction while driving or in traffic areas. Additionally, the system incorrectly indicates that the liftgate is open, creating uncertainty about whether the vehicle is secure and safe to operate. My Honda Fit with an automatic transmission experienced a driveshaft failure due to corrosion in August 2018. At that time, Honda had announced a recall for similar driveshaft corrosion failures affecting manual‑transmission Honda Fits, but no recall existed for automatic‑transmission vehicles. Because there was no recall for my vehicle configuration, and no notice from Honda, I had the vehic
... le repaired at an independent repair shop at my own expense. Honda later expanded the recall to include automatic‑transmission Honda Fits in June 2019. However, I never received a mailed recall notification while I owned the vehicle. I sold the vehicle in February 2020, before Honda mailed recall notices. I only became aware that my vehicle was included in the automatic‑transmission recall later, while researching issues related to a different Honda Fit I currently own. At that point, I learned that my prior driveshaft failure matched the recalled defect. Honda is now denying reimbursement for the recall‑related repair because they say they must inspect the vehicle to confirm whether it was equipped with a factory dynamic damper. Honda has also stated, however, that it does not have its own records confirming whether my VIN included this component in the build at the factory. The failure occurred before the recall existed for automatic‑transmission vehicles, and Honda failed to notify me of the recall while I owned the car. The vehicle is no longer available for inspection due to this lack of notice. Denying reimbursement under these circumstances shifts Honda’s own delayed recall, delayed notification, and incomplete factory records onto the consumer. I am concerned that Honda’s recall reimbursement process unfairly denies reimbursement for pre‑recall failures when the manufacturer lacks build records and notification was delayedRead more

I am reporting a dangerous and recurring transmission defect in my 2018 Honda Odyssey equipped with the ZF 9-speed automatic transmission. The vehicle experiences sudden and unpredictable delayed acceleration, particularly at low speeds. When attempting to accelerate from a complete stop or at speeds below 25 mph, the transmission fails to engage properly, leaving the vehicle stranded in the middl

The condition is intermittent and unpredictable, making it impossible to anticipate when it will occur. It happens most frequently when accelerating from intersections, merging into traffic, or navigating through slow-moving areas. Honda has acknowledged this defect through TSB 19-124 (issued December 2019), which addresses harsh and jerky upshifts in 2018–2019 Odysseys with the 9-speed transmission. However, the TSB does not fully resolve the issue. A class action lawsuit [XXX] et al. v. American Honda Motor Co., Case No. 5:23-cv-05011-BLF, N.D. I have a 2015 ford transit 150 and I was driving home on the highway and out of nowhere my van downshift into I believe 1st or 2nd gear causing the engine to redline and the rear tire to lock up and cause me to lose control of the vehicle! For about 3-7 seconds in that time the cars behind me had to slam on there breaks almost rear ending me due to the abrupt deceleration from my vans rear tire lo
... cking up and as that happened the back of my van began to sway out from behind me and right at the time I believed I would have rolled over possibly killing me and injuring other drivers the van randomly went back into the proper gear allowing me to gain control and pull over. After I had time to process what just happened the check engine light came on and with my scan tool I read the code and it said that the speed sensor for the transmission had malfunctioned and after some research I found that similar year ford f150s with that same transmission and that same part number had recalled that part for the same issue! And I ask that if you would please save life’s by recalling this part it is not a debate it is a necessity please please recall this partRead more

Vehicle: 2021 Jeep Grand Cherokee L Issue Type: Electrical System / Loss of Power / Braking System While operating my 2021 Jeep Grand Cherokee L on Interstate 40 at approximately 70 miles per hour, with two minor children in the vehicle, the vehicle suddenly and without any prior warning began to rapidly decelerate, despite continued pressure on the accelerator pedal. At the time the decelerati
... on began, there were no warning lights, alerts, or indications of any malfunction. The vehicle exhibited a significant loss of power, and it felt as though it was being mechanically restricted or “dragging,” severely limiting its ability to maintain highway speed. Only after the vehicle had already begun to decelerate did a brake system warning indicator appear on the dashboard. This sequence of events indicates that the warning system did not provide advance notice of the failure. This sudden and uncommanded deceleration created an immediate and serious safety hazard, particularly given the vehicle’s speed and surrounding interstate traffic. The inability of the vehicle to respond appropriately to acceleration input placed occupants at risk of a rear-end collision or other serious traffic incident. Although the warning indicator later turned off, the vehicle continued to operate abnormally, with ongoing reduced acceleration and diminished responsiveness. This incident occurred without prior warning, and the delayed activation of the brake warning indicator raises serious concerns regarding a potential defect involving the vehicle’s electrical, braking, and/or powertrain control systems, including possible failure of system monitoring or alert mechanisms. Given the circumstances including highway speed, sudden loss of propulsion, delayed warning notification, and the presence of child passengers. I respectfully request that this issue be formally investigated as a safety-related defect.Read more

Vehicle has experienced repeated electrical system failures affecting engine operation, safety systems, and interior electronics from April 2025 through April 2026. Details: April 2025 (under warranty): Vehicle lost power and died. Dealership replaced batteries. December 2025: While driving, multiple warning lights (check engine, blind spot, lane departure, forward collision, accident avoidance)
... flashed repeatedly. Battery died and was replaced by AAA. Vehicle taken to dealership due to safety concerns; dealership found no issues. March 2026: Same warnings occurred again while driving. Vehicle lost all power while stopped at a red light. Hazard lights did not work. Gear selector knob rotated freely and could not be put in park. After opening the hood, vehicle restarted but windows, powered seats, and infotainment stopped working. Dealership replaced fuse array. April 2026 (~3 weeks later): Vehicle would not start. After accessing battery under passenger seat, vehicle turned on with alarm and lights flashing. Same warning lights reappeared while driving. Interior electronics (windows, seats, infotainment) remained non-functional. Safety Risk: Complete loss of power in traffic, inability to use hazard lights, failure of driver-assistance safety systems, and inability to shift into park create significant risk of crash or injury. Inspection/Repair: Vehicle inspected multiple times by dealership and AAA. Batteries and fuse array replaced. Issue persists and root cause is UNKNOWN. Warning Signs: Repeated warning lights prior to failures, including check engine and multiple safety system alerts.Read more

Vehicle: 2015 Subaru Forester Mileage at Failure: 85,000 miles Component: Powertrain – CVT Transmission My 2015 Subaru Forester is experiencing a premature safety‑related CVT transmission failure at approximately 85,000 miles. The vehicle developed a pronounced whining noise that increases with vehicle speed, consistent with internal CVT bearing or pulley failure. This is a known defect patter
... n in Subaru Forester CVTs and is not related to maintenance or wear. The noise is continually progressing and the risk of sudden catastrophic failure makes the vehicle unsafe to operate. An authorized Subaru dealer and the District Parts & Service Manager (DPSM) both confirmed that the CVT has failed and requires full replacement. This failure occurred well within Subaru’s extended CVT warranty mileage limit of 100,000 miles, and matches the defect pattern Subaru acknowledged when it extended CVT coverage nationwide for this model. However, Subaru is refusing to replace the CVT due to the 10 calendar year extension expiration. Subaru’s maintenance schedule for this vehicle does not require any CVT fluid replacement interval, so there is no missed maintenance that contributed to the failure. A failing CVT creates a safety hazard due to to the risk of sudden loss of power, hesitation, or inability to accelerate. I am filing this complaint so NHTSA is aware of this premature CVT failure and the associated safety concerns of SOA's refusal to replace the CVT under their extended warranty.Read more
