The vehicle just downshifted from 6th gear to 1st gear on the freeway nearly causing a accident and it caused a blown transmission.
Incident date: Mar 30, 2026
The vehicle experienced a sudden loss of drive power and entered a 'fail-safe' mode with check engine light, warning chimes and service vehicle warning message. A diagnostic scan revealed fault codes P1C2D03 (High Pressure Electric Oil Pump Motor Position Sensor), P193B79 (High Pressure Electric Oil Pump Performance), and P086800 (Transmission Fluid Pressure Low). These three specific codes are ci
... ted in Kia Recall SC250 (NHTSA 22V760) and associated technical bulletins as evidence of an internal electric oil pump failure requiring full transmission replacement. The safety of occupants was compromised by the sudden loss of motive power on the interstate. I presented these codes to Value Kia 6915 Essington Ave, Philadelphia, PA 19153 and dropped my car off on 4/7/2026. The dealer stated the safety recall was 'closed' and that the vehicle was 'out of warranty' due to its 95,000 miles and second-owner status. They would only agree to perform a diagnostic at my personal cost. The vehicle has been at the dealership for two days without an inspection report being provided to me. I have contacted Kia Corporate Consumer Assistance (Awaiting Response) and am filing this report because the vehicle is exhibiting the exact safety defect and 'triple-threat' fault codes of SC250 but is being denied a free safety remedy by the authorized dealer due to mileage and second ownership. As of 4/9/2026, the vehicle has been at the dealership for 2 days. Despite signing an authorization for a $190 diagnostic fee, the dealership has been completely silent and has not provided a status update or the diagnostic report. I am being left without transportation while the dealer fails to address a documented safety defect.Read more

While driving on a highway at approximately 50 mph, the vehicle suddenly lost acceleration without warning and was unable to exceed approximately 10–15 mph. This created a dangerous situation requiring immediate maneuvering across multiple lanes of traffic to reach safety. At the time of the incident, the check engine light was illuminated. Diagnostic trouble codes P0191, P2177, and P2179 were
... later identified, indicating fuel pressure irregularities and air/fuel imbalance. The vehicle was inspected by a dealership, which diagnosed a purge valve vacuum leak and faulty high-pressure fuel system components. The repair required replacement of the purge valve and high-pressure fuel system components. The failure involved components within the fuel/propulsion system and engine systems, which are controlled in part by the electronic control module (ECM). This resulted in a sudden and unexpected loss of propulsion while driving at highway speed. Prior to the incident, there were issues with refueling, including difficulty filling the fuel tank when near empty, indicating a potential developing issue within the fuel system. This condition created a significant safety risk due to the sudden and unexpected loss of vehicle speed in active highway traffic, increasing the risk of collision.Read more

During a manufacturer-mandated software module update, North Central Ford (Plano, TX) bypassed continuous power supply safety protocols. This negligence caused a voltage drop, interrupted the software flash, and completely corrupted the Accessory Protocol Interface Module (APIM). The dealership returned the vehicle to me, stating the update was complete. The corrupted module failed to power down,
... causing a severe parasitic drain that completely killed the battery overnight, leaving the vehicle immobilized and immune to jump-starting. The APIM is completely corrupted. The disabled vehicle was towed back to North Central Ford and remains available for immediate inspection. Safety Risks: The corrupted APIM permanently disabled the federally mandated backup camera (violating FMVSS 111) and touch-screen climate/defroster controls, creating severe rear and forward visibility hazards. The module's failure to sleep causes a continuous parasitic battery drain that has already stranded me and introduces a high risk of sudden electrical failure in traffic. Dealer Confirmation: North Central Ford's paperwork confirms they caused the failure. Their Day 1 intake Multi-Point Inspection officially documented my battery in "GOOD CONDITION" prior to service, and their initial invoice explicitly states the update was "UNSUCCESSFUL." Furthermore, after I supplied a new replacement battery for a second diagnostic, they confirmed a U2100 code (Initial Configuration Not Complete), proving the module was electronically wiped during their botched procedure. Prior Symptoms / Inspections: None. The electrical system and screen were fully functional prior to the software update. Inspected solely by North Central Ford. Status: The General Manager dismisses their own diagnostic codes, refuses to repair these technician-caused safety defects under consequential damage guidelines, and is attempting to charge me out-of-pocket, The vehicle remains at the dealership due to active safety hazards.Read more

This failure is a direct mechanical match for a known factory assembly error (insufficiently bent locking tab) acknowledged by Toyota in Technical Service Bulletin T-SB-0160-18 and Customer Support Program ZJC. Despite the technician's formal diagnosis that the vehicle is mechanically unsound and requires immediate replacement for safe operation, Toyota Corporate denied my request for assistance (Case #XXX) on April 8, 2026. Toyota cited my vehicle's VIN does not fall into the customer support program, although it's essentially the same transmission, effectively refusing to remedy a documented manufacturing defect. Dear American Honda Motor Co., Inc. ,I am writing to formally document my experience with a premature failure of the Continuously Variable Transmission (CVT) in my Honda HR-V. The vehicle was covered under the extended warranty program Honda implemented specifically for the known CVT issues affecting 2016–2020 model years. In On 3/10/26 the CVT failed at approximately 96,000 miles. The vehicle w
... as towed to an authorized Honda dealership, where technicians confirmed premature belt deterioration and breakage. Prior to the failure, I had the vehicle serviced at the dealership for the Powertrain Control Module (PCM) software update outlined in Honda Service Bulletin 21-047. During that visit, I reported a noticeable burning smell, which I now believe originated from the failing transmission belt. The dealership assured me there were no issues and stated that the software update had adjusted the gear ratio to reduce strain on the transmission. The vehicle was returned to me with no further action recommended. Subsequent communication with American Honda revealed that the company had been aware of widespread CVT belt issues in these model years. Rather than issue a full recall, Honda extended the powertrain warranty to 7 years or 150,000 miles (whichever occurs first). My repeated attempts to escalate the matter with Honda Customer Service, including multiple requests for contact information for a regional manager, were unsuccessful. After several weeks of delays—during which my vehicle remained at the dealership for over 20 +days—Honda Corporate initially offered to cover a remanufactured CVT replacement at a cost of $4,500 to me. The dealership ultimately assisted by reducing my out-of-pocket expense to $2,900.I find it unacceptable that a vehicle with only 96,000 miles required a major transmission replacement costing nearly $3,000, especially after Honda had already acknowledged the defect through a technical service bulletin and an extended warranty. The vehicle would not restart. The vehicle was towed and required a complete engine replacement under warranty. After that replacement, additional issues followed. There was an HVAC/defrost concern. In June 2025, the vehicle again became inoperable without warning while parked at a store. It would not start despite repeated attempts, including by a dealership representative. A diagnostic scan produced over a page of error codes, but the dealership could not identify a cause and stated the vehicle was safe. In Dec. 2025, the vehicle engine was inspected under a recall and I was told there were no issues. In Feb. 2026, the vehicle lost heat and was found to have no coolant despite regular maintenance and recent inspection. In Mar. 2026, at about 37,700 miles, the vehicle developed a loud clunking noise while driving. I stopped driving and returned it to the dealership, where I was told the engine had failed again and required another full replacement. I am submitting a safety complaint regarding a serious incident involving my 2025 Kia EV9. The vehicle was serviced on March 1 at World Kia in Joliet, Illinois. On April 4, only four days after that service, the vehicle experienced a sudden and unexpected ICCU failure while I was driving. During my normal travel, in active traffic and at speed, the vehicle abruptly entered limp mode and displayed
... multiple system warnings. This caused an immediate and significant reduction in power, creating a hazardous situation for myself and surrounding drivers. I was forced to maneuver the vehicle out of traffic while it was rapidly losing propulsion capability. The vehicle is currently at Berwyn Kia in Berwyn, Illinois for diagnosis and repair. I am reporting this incident due to the clear safety implications of a sudden loss of power during highway driving operation. A failure of this nature poses a risk of collision, especially in high‑speed or congested traffic conditions. I am requesting that NHTSA review this issue as part of its ongoing monitoring of Kia/Hyundai/Genesis Integrated Charging Control Unit (ICCU) failures and related safety concerns. Thank you for your attention to this matter.Read more
