Vehicle has ongoing and recurring electrical and engine-related defects that affect safe operation. The primary systems involved are the electrical system, engine ignition system, and powertrain. The vehicle is available for inspection upon request. Prior service records show issues beginning in 2020 involving repeated electronic system failures requiring multiple repairs and component replacemen
... ts. Since purchasing the vehicle in April 2023, the vehicle has developed more serious failures, including: – September 2024: Check engine light with loss of power and inability to exceed safe driving speeds – October 2024: Engine misfiring due to ignition coil failure requiring replacement of ignition components – September 2025: Additional ignition coil electrical fault (code P0353) The problem has been reproduced and confirmed through dealership service inspections and diagnostic reports. Warning indicators, including the check engine light, appeared prior to failure events. These defects create a safety risk due to potential loss of power, engine failure, and unreliable vehicle operation while driving, which could increase the risk of an accident. The issues have required multiple repairs without permanent resolution and appear to be ongoing and worsening over time.Read more

The contact owns a 2026 Nissan Armada. The contact stated that while driving at an undisclosed speed, the infotainment center independently blinked on and off. The rear-view camera displayed an error message of the rear wheels. The accelerator pedal was depressed; however, the vehicle failed to accelerate as intended. The contact stated that the failure had occurred while driving forward or while
... reversing. Additionally, the transmission was sluggish. The vehicle was taken to the dealer, where it was diagnosed with a failure of the radio. The contact was informed that an unknown recall was performed prior to the contact purchasing the vehicle. The contact was informed that the recall repair was improperly performed and had caused the failure of the AWD system. The contact was informed that a software update was needed, and the AWD system needed to be reset. The vehicle was repaired; however, the failure reoccurred. The failure reoccurred while out of state on a work assignment. The vehicle was not diagnosed or repaired by an independent mechanic or dealer. The manufacturer was made aware of the failure, and a case was filed. The contact was advised to contact the NHTSA Hotline to report the failure. The failure mileage was approximately 3,500.Read more

My vehicle was subject to Recall FSA 23S65, which addresses a defect involving the rear axle hub bolt. I brought the vehicle to a Ford-authorized dealership for the recall repair; however, the repair was not properly completed. The rear axle hub bolt subsequently failed, confirming that the recall defect was not adequately remedied. After the bolt failure, I returned the vehicle to the dealership,
... where technicians stated that the differential is now damaged. They claim that an axle bearing failed independently and dispersed metal contaminants throughout the differential, characterizing this as a coincidence unrelated to the recall issue. However, upon further review by a mechanic, it appears far more likely that the axle bolt failure directly caused or contributed to the bearing failure and resulting differential damage. The vehicle now produces a humming noise while driving, and I have been strongly advised that it is unsafe to operate. Despite multiple attempts to resolve this matter with both the dealership and Ford Motor Company, I have been unable to obtain a satisfactory response or corrective action. This situation raises serious concerns regarding the adequacy of the recall repair and the resulting safety risk.Read more

The issue appears to involve the engine and turbocharger system. The vehicle has shown symptoms consistent with a turbo underboost condition, including loss of power and reduced acceleration. A mechanic indicated that the symptoms may be consistent with diagnostic code P0299 (turbo underboost). The vehicle is available for inspection upon request. This issue affects safe operation because the veh

The loss of power is inconsistent and can occur without warning, increasing the potential safety concern. The problem has been ongoing. The check engine light is currently on and has remained on. The warning light first appeared around January 2026, and the issue has continued since then. Similar turbocharger-related issues have been reported in prior model years, including 2022 Chevrolet Malibu vehicles, where coverage or corrective actions were provided, raising concern that the same or similar issue may be affecting 2023 models.Read more

As I began a drive cycle, I heard a pop sound and received the “check electric vehicle system” warning associated with ICCU failure. With ICCU failure, if I had continued to drive the 12V battery would have run to zero and the car would have died completely. Vehicle goes to dealer tomorrow for confirmation.
Incident date: Jan 14, 2026
The vehicle intermittently fails to reverse properly, hesitates, jerks, and at times locks despite no obstruction present. This occurs during normal use and creates a serious safety concern, particularly when backing out of parking spaces or into traffic. In addition, the vehicle exhibits ongoing electrical issues, including intermittent warning lights, rearview camera distortion or failure, inco
... nsistent system operation, intermittent inability to unlock using the key fob or door handle, and the vehicle fan or air system continuing to run for extended periods after the vehicle has been turned off. These issues occur without consistent warning and have not been resolved despite multiple service visits. The vehicle is also subject to an active safety recall (NHTSA Recall 26V218000, Manufacturer Recall 298) involving the front seatbelt anchor. The recall remains unrepaired, with no remedy currently available. I have personally experienced the seatbelt becoming stuck and not releasing properly, which presents an additional safety risk. The vehicle also experienced a battery failure. I was initially informed that the battery could not be replaced without cost; however, it was later replaced. Despite repeated concerns, the hybrid battery system was not tested or evaluated, and this remains an unresolved issue affecting vehicle reliability and safety. Additionally, the vehicle has experienced tire tread separation and peeling requiring replacement. This is not consistent with normal wear and may indicate alignment, suspension, or other underlying vehicle-related defects. The vehicle has been inspected by the dealership multiple times; however, the issues were not reproduced and no corrective repairs were performed. These problems are intermittent and occur during regular day-to-day operation. Collectively, these unresolved issues present an ongoing safety risk to the driver and others.Read more

I am writing to formally report a dangerous safety failure on my Ford F-150 that persists after having the recommended recall service (19S07) performed. While traveling at highway speeds, my vehicle experienced an uncommanded downshift from 6th gear to 1st gear. This caused an immediate and violent deceleration, nearly resulting in a high-speed rear-end collision. This exact failure is the core sa
... fety issue addressed by Ford's powertrain control module (PCM) recall, yet the "fix" provided has failed to protect the vehicle from this life-threatening defect. I took the truck to Bolivar Ford in Tennessee for resolution. However, the dealership refused to perform any further repairs under the recall. They stated that because of my current mileage, the recall coverage has "expired." Furthermore, they diagnosed the issue as a faulty lead frame sensor—the very component linked to these downshift events—and quoted me $2,500 just to begin the repair process. A safety recall is intended to permanently rectify a defect that poses a risk to life and limb. The fact that the vehicle is still performing this dangerous maneuver after the recall was "accomplished" suggests the initial remedy was insufficient or the underlying hardware remains defective. I am requesting that Ford Motor Company intervene to ensure this vehicle is repaired at no cost to me. Given the severity of the safety risk, I do not believe mileage should disqualify a customer from having a documented, lethal defect corrected—especially when the first repair attempt failed. I look forward to your immediate response regarding how Ford intends to resolve this safety matter.Read more

I am writing to open a formal complaint regarding a serious transmission defect in my 2019 Honda Odyssey (VIN: [XXX] ). ... d a sudden loss of power. I am aware of Case No.XXX (XXX v. American Honda Motor Co.) and Case No. XXX (XXX v. American Honda Motor Co.), which allege that the 9-speed ZF 9HP transmission in my model year suffers from a design defect. These transmission failures create a significant safety hazard. Recently my van report code as transmission system problem the second time. We got mechanic checked and confirmed it is transmission issue. The first time when it happened, we drove the van on highway and suddenly it lose power and slow down by itself. there was no response even we applied gas. We went down highway and drove to a close-by repair shop. they replaced some part for us but it didn't resolve the issue. This time since the error comes, it won't speed up in lower speed except the speed goes beyond 40-50 miles plus. when we back the car, it responses slow or none. it comes with big noise also now. it definitely gets worst. I am reporting a structural failure of the rear subframe (axle carrier) on my 2016 Mercedes-Benz C300 (W205). Safety Impact: The vehicle is currently exhibiting unstable handling and a 'skittish' 'swerving' sensation causing the car to 'fishtail' under braking duri

This poses an immediate risk of a 'loss of directional control' should the suspension components fully detach from the corroded subframe. The Structural Integrity, Loss of Directional Control, Sudden Suspension Failure, and causes extremely dangerous Safety Hazard for my family and other on the road. The corrosion has compromised the mounting points of the rear suspension, creating a risk that the axle will detach while the vehicle is in motion, leading to a catastrophic loss of control. Context: Mercedes-Benz has acknowledged this defect in previous years (2008-2015) via a 20-year extended warranty and in Service Information S135.00-P-0009A. However, they have failed to include the 2016 W205 in the safety remedy despite identical failure modes and documented safety hazards. Currently, as of April 2026, a 2024/2025 federal court ruling [XXX] v. I own a 2021 Hyundai Palisade and have experienced a complete transmission failure that I believe Hyundai had prior knowledge of and failed to address. Before my warranty expired, I contacted the dealership several times to report transmission symptoms I was experiencing. I was told that because no diagnostic code had been produced at that time, there was nothing they could do. Eventually, my che
... ck engine light came on with a P0741 code — Torque Converter Clutch Circuit Performance. When I brought the vehicle in, the dealer flushed the transmission fluid, which came out dark and contaminated with metal particles. After running the vehicle, the fluid was still dark, indicating active internal damage already in progress. The dealership has now told me I need a full transmission replacement at my own expense. I am the second owner of this vehicle with approximately 105,000 miles. I have since learned that Hyundai issued Technical Service Bulletin #20-AT-005H-1 specifically addressing the P0741 code on the 2021 Palisade, instructing dealers to replace the solenoid and valve body harness. This repair procedure was never applied to my vehicle despite my early report of symptoms. I believe this is a known defect that Hyundai has failed to escalate to a recall, leaving consumers like me with thousands of dollars in repair costs for a problem the manufacturer was already aware of. I am requesting that NHTSA investigate this issue and consider whether a recall is warranted for the 2021 Hyundai Palisade.Read more

When approaching a stop, the vehicle vibrates heavily as if it is trying to fight the brakes to continue moving and avoid downshifting. There is no noted recall for bad torque converter according to Ford, but the service manager informed me that this is the most common issue with the Edge platform and it is expensive (between $5-10k). I contacted customer relations and ford says there is no issue with the platform and all vehicle repairs are the responsibility of the owner. I have independently reviewed documents regarding customer complaints and discussed the issue with Ford technicians and they confirm that Ford knows there is a problem and doesn't want to address it as a recall. Independent repair shops have refused to repair this transmission issue on the premise that it is a known issue that there is no resolution for and they do not want to have any responsibility over repairing a flawed powertrain and risk famage to their reputations, causing a disparate impact to the consumer. This needs to be addressed and remedied by Ford, especially since this is not an isolated issue, but platform wide.Read more

I experienced repeated stalling, loss of drive power while on the highway, limp mode, and all dashboard warning lights over several years. I have since received Honda’s official recall notice confirming that a faulty fuel pump causes the exact symptoms I reported.
