Transmission downshifts from 6th to 3rd gear . It’s has recently done it it about 4 times on the road.
Incident date: Apr 23, 2026
The contact owns a 2016 Dodge Dart. The contact received notification of NHTSA Campaign Number: 25V674000 (Power Train); however, the part to do the recall repair was not yet available. The contact stated that while attempting to shift to park(P), the gear shifter was stuck and failed to move to the intended gear. The contact engaged the parking brake. The failure recurred while attempting to shif
... t to drive(D) or reverse(R), rendering the vehicle undrivable. There was no warning light illuminated. The contact stated that the failure was intermittent, and the gear shifter would respond as intended after several attempts. The dealer was contacted and informed the contact that the vehicle could not be repaired because the part for the recall repair was not yet available. The vehicle was not diagnosed or repaired. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was not made aware of the failure. The failure mileage was unknown.Read more

My 2025 Ford Ranger Lariat FX4 has experienced sudden and complete loss of accelerator pedal response while driving at low speed on at least 4 to 6 separate occasions. The engine continues to run normally, but pressing the accelerator produces no power and no vehicle response. The condition has consistently occurred while pulling into my subdivision, which has an inclined entrance. The truck also
... feels noticeably sluggish nearly every time I make this turn, even when full dropout does not occur. No warning lights, check engine light, or messages in the instrument cluster illuminate during these events. No diagnostic trouble codes have been stored that I am aware of. The vehicle is available for inspection upon request. The symptoms closely match two active Ford recalls — 26S10 (EGR valve failure causing unexpected loss of drive power, stated by Ford to be most likely at speeds under 13 mph) and 25V-239 (PCM reset causing temporary loss of acceleration). My vehicle is not currently listed as included in either recall per Ford’s VIN lookup. Numerous owner reports on public Ford Ranger forums describe identical symptoms on 2024 and 2025 Ranger trucks, many of which are also not covered by the existing recalls and for which dealers have been unable to find stored codes. A sudden, unannounced loss of drive power at low speed on an incline or while turning into traffic represents a clear crash risk. I am requesting that NHTSA investigate whether the affected vehicle population should be expanded.Read more

Started car, placed transmission in 'reverse' and backed up into a public thoroughfare zone thinking enough time was alloted to place vehicle in 'drive' and merge into traffic. Car would not go into 'drive' nor would it go back into 'reverse' let alone 'neutral. So after realizing the car was reporting a transmission fault and placing itself back into 'park' each time, due to a transmission sensor
... detection issue. I was stranded in traffic. After calling for towing service, we learned that only a few could drag the car up onto a transport bed using wheel skids, since all four tires were locked up. This model does not have a manual neutral override feature. Poor design, placing my wife, who is handicapped, into a perilous situation.Read more

While driving at a low speed (approx. 5–10 mph), the front passenger-side lower ball joint suffered a catastrophic structural shear without warning. This caused the wheel assembly to collapse outward and the CV axle to pull out of the transmission. The failure caused a complete loss of steering control, veering the vehicle into a curb. Physical evidence (outward axle pull and high-side rim scrat
... ches) indicates the mechanical failure preceded the impact. This occurred at only 38,155 miles on a 2022 Chevrolet Bolt EV. Outward Dislocation: Photos show the CV axle pulled OUT of the transmission and the wheel leaning OUT (positive camber). A curb impact would push these components IN. This proves the wheel fell away from the car first. Pre-Impact Grinding: The interior barrel of the rim and the brake dust shield show heavy rotational grinding and metal-on-metal scoring. This confirms the suspension collapsed while the wheel was still spinning at speed, prior to the car coming to a stop. Structural Shear: The lower ball joint stud suffered a complete transverse shear. High-resolution photos of the broken stud face show distinct "beach marks" (concentric rings), which are the physical fingerprint of a slow-growing metal fatigue crack rather than a sudden impact fracture. Rim Damage Profile: Deep gouges are located on the flat face of the wheel spokes rather than the outer lip. This confirms the wheel was already at an extreme tilt (collapsed) when it made contact with the curb. CONCLUSION: The vehicle was maintained and driven under normal conditions. This was a spontaneous, catastrophic structural failure of a safety-critical component. I am requesting that NHTSA investigate this as a potential manufacturing defect for the 2022 Chevrolet Bolt EV.Read more

Transmission Gear Failure VEHICLE: 2019 Ford F-150 MILEAGE: 67,000 Description of Incident: I am reporting a catastrophic internal mechanical failure of the 10R80 transmission. While driving, the vehicle emitted a strong, pungent odor of burnt transmission fluid followed by a total loss of gear engagement. The failure was sudden and occurred without a "Check Engine" light or warning, leaving th
... e vehicle stranded in a dangerous position in active traffic. Technical Assessment of Cause: Upon professional teardown, it was determined that the internal gear sets were severely damaged and required full replacement. The transmission fluid was heavily oxidized (burnt), and the planetary gear assemblies showed signs of extreme thermal distress and mechanical chipping. This indicates a failure of the internal lubrication system or a structural defect in the gear hardening process, leading to metal-on-metal contact and total component failure at only 67,000 miles. Safety Concern: The suddenness of this gear failure creates a significant risk of a "stalled vehicle" scenario in high-speed environments. Furthermore, a mechanical gear failure carries the risk of internal components fracturing and potentially locking the rear wheels, which could lead to a complete loss of vehicle control. This is a premature failure of a major powertrain component that is advertised to last the life of the vehicle.Read more

The vehicle has 57000 miles and i am reporting a commonly known issue with the transmission. Now, 10 months post extended warranty, Ford is telling me it needs a full transmission replacement. Thoroughout, the vehicle shifted eradically up and down, including on the interstate, quickly dro
... pping speed. It was particularly bad a lower speeds when the engine was cool. Sometimes, at low speed or at a stoplight, the vehicle would disengage the drive train and roll in neutral while the RPMs revved when the throttle was pressed. The vehicle was not operating safely, would roughly bounce around gears, when reported to Ford, told that there was nothing wrong and now being told post warranty that the transmission is a total fail that will require replacement. The latest service appointment one month ago, where I was again told that an oil topoff was all that was needed. The transmission (powertrain) developed a delay when shifting into reverse, followed by harsh engagement. The condition worsened over time and resulted in a complete loss of reverse. The vehicle will not move when shifted into reverse. This creates a safety risk when parking, backing out of spaces, or maneuvering in traffic, as the driver cannot safely control vehicle movement. The issue occurred
... at approximately 89,000 miles under normal driving conditions. No warning lights were present prior to failure. The problem was reported to a dealership and diagnosed as a transmission failure. The condition is reproducible and ongoing. I was also informed that multiple vehicles of the same model were experiencing similar transmission issues at the same time. The failure limits safe operation of the vehicle and increases risk in situations where reverse movement is required.Read more

Unjustified Denial of Engine Warranty Coverage - 2015 Kia Sportage "I am filing this complaint because Kia is denying warranty coverage for a known engine defect (rod bearing failure in the Theta II GDI engine) on my 2015 Kia Sportage. The manufacturer’s denial is based on the claim that I did not perform the KSDS (Knock Sensor Detection System) software update in previous years. However, I have
... officially performed this update at an authorized Kia dealership (Hendrick Kia of Cary) on April 17, 2026. Despite my vehicle now fully complying with the recall requirements, Kia continues to deny the warranty claim by retroactively applying a 'missed update' penalty. This is a clear attempt to evade their obligations under the class-action settlement regarding this engine defect. The manufacturer is holding the current owner responsible for maintenance history they cannot control, even though the vehicle is now technically compliant. I request that the NHTSA investigate this practice, as it prevents consumers from accessing the safety-related warranty protections they are entitled to." Tips for your submission:Read more

The vehicle had to be physically pushed by hand to move it. This created a dangerous situation for myself and others on the road. An independent mechanic diagnosed and replaced the clutch actuator assembly and fuel pump. After repairs the problem persists unchanged. The mechanic has noted the vehicle requires a full transmission replacement. This is a known defect with the Ford PowerShift DCT transmission affecting multiple model years. SUPPLEMENTAL TO NHTSA COMPLAINT #11728718 (RECALL 26V248000) On March 2, 2026, my 2023 Land Rover Defender was at a dealership for approximately 3.5 hours for key fob programming. At that time, Jaguar Land Rover had an active service message (SSM76173) related to DC-DC converter failures. Within 24 hours of leaving the dealership, the DC-DC converter failed. The vehicle displayed a “Charging S
... ystem Fault” warning and became inoperable, consistent with the failure described in my original complaint #11728718. I contacted the dealership immediately with information about the issue. They told me that what I experienced aligned with the DC Converter failures that have been reported. I asked if I could get my Defender picked up or if they could at least provide a loaner car if I could somehow get my Defender to them. The dealership declined towing assistance and did not provide a loaner vehicle, citing that I was beyond the mileage limit of the warranty. I was also told that loaner vehicles were reserved for customers who purchased their vehicle at that location. Because I live approximately 4 hours from the dealership and the vehicle was not safely operable, I arranged my own repair. Due to a national backorder, the vehicle was unusable for an extended period (about a month). I purchased a genuine replacement DC-DC converter and paid for expedited shipping. Total out-of-pocket cost exceeded $970. On April 23, 2026, Jaguar Land Rover issued Recall 26V248000 for this defect. Communication with JLR about this incident started on April 7, 2026. On April 23, 2026 at 8:05 am EST (following the announcement of the recall), I was offered a final settlement off of a $458 retailer credit. However, I was instructed that I needed to return to the dealership (4 hours away) for inspection of a vehicle that has already been repaired using an OEM part. I am submitting this update to document the failure, the repair, and the manufacturer’s response following the recall.Read more

The failure involves the automatic transmission system (10-speed 10R60) in my Ford Explorer 2020. The issue appears to be internal, affecting clutch packs and/or internal components (spring-related failure was initially noted by the dealership).Yes, the vehicle and transmission are available for inspection upon request. The transmission malfunction caused delayed engagement and loss of proper powe
... r delivery, which created unsafe driving conditions. There were moments where the vehicle hesitated or failed to respond properly when accelerating, increasing the risk of being struck in traffic or being unable to move out of the way of oncoming vehicles. This presents a clear safety concern, especially in traffic or intersections. Yes, the issue was inspected and confirmed by an authorized Ford dealership. The dealership initially diagnosed the problem as an internal transmission failure involving clutch-related components. However, those diagnostic notes were later removed or altered in the service record. I have retained screenshots of the original diagnosis as evidence. The vehicle has been inspected by an authorized dealership representing Ford Motor Company. At this time, it has not been inspected by police or insurance representatives. The manufacturer has not physically inspected the vehicle but has reviewed the case and offered partial financial assistance. Yes. Prior to the failure, the vehicle experienced symptoms including harsh shifting, delayed engagement when shifting into drive or reverse, and abnormal transmission behavior. These symptoms developed over time before the failure occurred. In some cases, there were no warning lights present before the issue became severe, which made the failure more sudden and unexpected. I was informed the original technician is no longer employed and there is no service manager currently overseeing the department. This lack of oversight makes it difficult to verify the original diagnosis or changes made to service records.Read more
