I received a safety recall for the PTU unit on my 2019 jeep cherokee, the service 4x4 system light was on. I took it to the dealership where they said they cleared the recall but the light was still on and the 4x4 system still did not work. They told me they did what they were supposed to…period. Now when i look up for recalls nothing comes up, but they light is still on and the system doesn’t

While traveling on vacation April 15, my 2025 F150 truck suffered a total loss of forward momentum on the highway and in city of Los Angeles —a severe safety failure. The repairing Ford dealership, diagnosed a factory-born "OPEN" in the main wiring harness (#14A005) behind the dash (DTCs P2128/P2138). To rectify this assembly-line defect, the dealership had to completely remove the dashboard (I

As an experienced truck owner, I know that an invasive repair of this magnitude on a brand-new vehicle significantly increases the risk of long-term electrical issues and permanent dashboard rattles. ICCU failed despite all prior recall updates being performed. Incident began when home circuit breaker tripped; vehicle subsequently refused Level 1 charging with "Check Electrical System" warning. Per dealer suggestion, tested public chargers. Level 3 (DC) worked, but upon connecting to Level 2 (AC/J1772), a loud "pop" occurred, and the public charging station immediately suffered a mechanical fa
... ilure/shutdown. Vehicle then displayed "Stop vehicle and check power supply," and was unable to drive above 40mph (with a turtle icon visible on the dash) once I pulled away from the charging station. This created a significant safety hazard while navigating traffic. Within two hours, the 12V battery was completely depleted; vehicle was unresponsive and required a jump-start for towing. Dealer confirmed ICCU failure requiring replacement. The fact that the vehicle's internal failure caused the immediate malfunction of public infrastructure is deeply concerning, as was the loss of motive power on the road.Read more

I am filing a safety complaint regarding a 2022 vehicle with approximately 47,000 miles that experienced a sudden and dangerous mechanical failure while in operation. While driving at approximately 37 mph, a dashboard warning message appeared indicating that the vehicle required attention but could continue to be driven for a short period. Shortly after the warning, the vehicle suddenly lost the
... ability to maintain proper gear engagement. At one point, no gear was engaged at all, creating a hazardous driving situation. I pulled over and turned the vehicle off. After restarting it, I was able to regain limited operation; however, the vehicle would not shift beyond second gear, significantly limiting its drivability and creating an unsafe condition in traffic. This vehicle has been consistently maintained at an authorized dealership according to all recommended service intervals, with no missed or delayed maintenance. The dealership later informed me that the failure was related to the mechatronic system and SOS system. I was also told that the repair would not be covered under warranty. This incident raises serious safety concerns due to the sudden loss of power and gear function while the vehicle was in motion. I am requesting that this issue be reviewed for potential defects that could affect vehicle safety and reliability. I am submitting this complaint so that the issue can be investigated and to help prevent similar incidents from affecting other drivers.Read more

I am the owner of a 2019 Ford F-150 Lariat equipped with a 10-sp transmission. In late 2021, early 2022 - whiled the vehicle still had warranty - I reported to a Ford dealership that the transmission was experiencing harsh shifting and jerking. At that time, I was informed that no issue could be identified. I complained of this each time I took the vehicle into the dealer for an oil change. After
... the warranty expired on this vehicle in 2023, the condition worsened. By 2024, the transmission began intermittently shifting into neutral while the vehicle was in motion. This occurred both when accelerating through lower gears (notably between 3rd, 4th, and 8th,9th) and while driving at highway speeds. These incidents created serious safety risks. On multiple occasions, the vehicle unexpectedly shifted into neutral while changing lanes on the interstate highway. The vehicle RPMs spiked and I was unable to regain power and had to maneuver to the shoulder while a semi-truck traveling at high speed approached from behind. This placed me in a dangerous and potentially life-threatening situation. On another occassion, the vehicle unexpectedly shifted into neutral while I was proceeding through intersections after a green light, leaving me without power, and unable to shift back into drive in active traffic. There were no service, or warning lights on prior to these failures. The issues occurred suddenly without notice, creating unpredictable and hazardous conditions. Warning indicators only appeared after the condition progressed where the vehicle became un-driveable. Two dealers have since inspected the vehicle and confirmed that the internal transmission components have failed. I was advised that a full transmission replacement is necessary. The issue has occurred multiple times and has progressively worsened since it was first reported under warranty. There was failure to identify and address safety risks sooner and this is extremely concerning. This is a known issue!Read more

Since shortly after purchase, the vehicle has experienced intermittent drivability and shifting issues along with repeated EPC and check engine warnings. Diagnostic trouble code P0111 (intake air temperature plausibility) has been present along with other electrical and engine management faults. A Volkswagen dealership diagnosis indicated that the intake air temperature sensor and an additional e
... ngine insulation “hood blanket” component (not originally installed on the vehicle) are required to address the issue. These components were presented as necessary to correct the fault and resolve intermittent shifting behavior. The shifting issue is intermittent but occurs during normal driving and raises safety concerns due to unpredictable drivability and transmission behavior. The vehicle continues to exhibit warning lights and unresolved performance issues despite multiple repair attempts. The dealership has not identified a clear root cause, and prior repairs including sensor and EVAP-related components have not resolved the condition. Given the involvement of transmission-related drivability concerns and repeated unresolved electronic faults affecting engine performance, I am filing this complaint as a potential safety-related defect affecting normal vehicle operation.Read more

SUBJ: Vehicle Safety Complaint – 2023 Cadillac Escalade Sudden Loss of Power I am reporting a safety issue involving a 2023 Cadillac Escalade ESV (VIN: [XXX] ). On April 19, 2026 (late evening into early April 20), while traveling approximately 70 mph on [XXX] , the vehicle suddenly and completely lost engine power without warning. This created an immediate safety hazard, nearly resulting in
... a collision. I was able to exit the highway, but the vehicle lost all acceleration and became disabled in the middle of a dark off-ramp with oncoming traffic approaching from behind. OnStar was contacted and Virginia State Police responded due to the unsafe location. A trooper remained on scene with emergency lights to redirect traffic and prevent a collision. Due to the hazardous conditions, the trooper arranged an emergency tow. I incurred $389.61 in out-of-pocket towing costs. The vehicle was taken to a tow yard, and my AAA Premier coverage then towed it 189 miles to Miller Brothers Cadillac in Ellicott City, MD. The dealership diagnosed catastrophic engine failure requiring full engine replacement at approximately 53,000 miles. There were no meaningful warning indicators prior to the sudden loss of power. This vehicle was purchased from Sons Chevrolet / Cadillac in Columbus, GA in December 2025 as a Certified Pre-Owned Cadillac and had already required multiple repairs shortly after purchase. The vehicle is currently at Miller Brothers Cadillac awaiting complete engine replacement and is available for inspection. This sudden loss of power at highway speed presents a serious safety risk and could have resulted in a severe accident or loss of life. Given the absence of warning and catastrophic failure at relatively low mileage, I am concerned this may indicate a broader safety defect rather than an isolated incident. While driving on interstate at highway speeds (70mph), the transmission experienced sudden downshift, which increased the engine RPMs to redline and rapidly decelerated the vehicle. Transmission would not select proper gear which made it difficult to proceed to a safe pull-off p

Reoccurring problems include rapid deceleration and high engine revs when transmission suddenly shifts at high speeds, or stalling & lack of acceleration when transmission shifts improperly at low speeds (which makes moving the vehicle safely from the roadway difficult). Researching the problem, it appears to be an issue with the leadframe internal to the 6R80 transmission. On 3/15/2026 I was on my way home from Massachusetts to Pennsylvania, and about 35 minutes into my drive I got an alert stating "High Engine Temperature Stop Safely", so I pulled over immediately and turned the car off. After allowing the engine to cool down, I opened the hood and started the car to see if I could identify any issues, the "Engine Fault Service Now" alert came up. This was the firs
... t time this ever happened, this put me, my two dogs who were in the car with me, and possibly others on the road at risk, had the conditions escalated. (i.e. under hood fire). The vehicle was towed to an Authorized Ford Dealership, Mathieu Ford Sales, Inc. who diagnosed the issue on 3/17/2026. The dealership removed the spark plugs to inspect cylinders and found coolant intrusion into cylinders significantly on cylinder 2. Dealership referenced TSB 22-2322, is recommending short block assembly and related parts to remedy concern. Dealership advised it was safe and okay to drive the vehicle slowly to a nearby residence (to store the vehicle/retain possession), noting to pull over if an alert pops up and to have it towed if I received another message like before. The dealership advised it was not safe to drive the vehicle back to Pennsylvania. There weren't any issues driving the vehicle back on 3/17/2026 following the dealership's guidance; the car hasn't been driven since. The car needed to be jumped to start the vehicle on 4/17/2026 for a tow truck to load the vehicle onto its bed. The vehicle is currently back in possession of the dealership who diagnosed the issue. Upon doing research, this appears to be a widespread manufacturing design defect that Ford has failed to properly address/recall. The vehicle is experiencing a severe and dangerous transmission defect that manifests as intense shuddering and vibrations followed by instances of random, unintended acceleration. Specifically, while operating the vehicle at various speeds, the transmission fails to engage properly, causing a violent vibration/shudder. This is frequently followed by the vehicle surging or accelerating unexpectedl
... y without increased pressure on the gas pedal. This condition creates a significant safety hazard as it compromises the driver's ability to maintain a constant speed and control the vehicle in traffic or during low-speed maneuvers. The random acceleration is unpredictable and poses an immediate risk of rear-end collisions or loss of vehicle control. General Motors (GM) has issued multiple Technical Service Bulletins (TSBs) regarding this specific 8-speed transmission defect (shudder/vibration), yet they have failed to issue a safety recall. My vehicle began exhibiting these life-safety symptoms approximately two weeks after the 100,000-mile powertrain warranty expired in August 2025. A licensed transmission specialist has diagnosed the issue as a torque converter failure and transmission fluid contamination, consistent with the known defects identified in GM's own TSBs. Chevrolet has refused to remedy this safety defect at their expense, citing warranty expiration, despite their documented prior knowledge of the systemic nature of this failure. I am reporting this to NHTSA because the random acceleration component of this defect is a clear and present danger to myself and other motorists on the road.Read more

With foot on brake between 5 to 10 seconds later I can feel the brake disenaging with my foot fully and physically on the brake simultaneously my vehicle screens will light up indicators erractically,(ghost touch, touch screen digitizer failure), while LUNGING FORWARD 5-7 ft on it OWNS,simultaneously screen blackout,& vehicle screen system reset followed by vehicle putting itself in emergency brake. This puts everyone around at RISKS AS MY VEHICLE OPERATES ON ITS OWN, which caused an accident at a red light where my truck plowed into the vehicle in front of me.Ive waited for an independent service contractor to evalute on 4 different occasions which they failed to appear or make contact on THEIR desired scheduled dates. The dealership has failed to reproduce due to unsatisfactory failed service repair attempts, discrepancies in verbal & documention.Absolute lack of concern for my safety or interest in issue, as at least 2 comeback services were met with breach of contract by dealer. The DEALERSHIP has been the only ones servicing my vehicle & according to their top tier diagnosis components successfully checked out in contrast to my many reoccurring service request for same issue.Prior to failure no warning lamps or messages appeared.Read more

Accelerated and heard a loud noise and felt like I had driven through wood and one got stocked to the whelle. There was a lot of traffic so I was driving slow. The noice I heard was like something was stuck inside the wheel, like it was grinding. Dealer diagnosed a bad transfer case and transmission and they replaced both under warranty.

I was pulling out of driveway, it is a steep driveway, I have kept vehicle in 4WD, I then backed up to turn around, the truck “lurched” and “jerked” forward abruptly. I thought I had hit the home in front of me or ran over something. It was not the AEB. The vehicle had changed from 4WD auto to 2WD on its own, creating a large “jump” in the gears. I took the vehicle to service and they
... could not replicate. This has now happened 3 other times and “jerked” me forward at lower speeds. I have slow had the vehicle “jerk” or roughly change gears while driving from 20mph to 40mpg or when lowering gears and different speeds. I have taken it to the dealership to present and was advised to take it to another dealership and test drive with myself to attempt to replicate. I have had the main center display console turn black, both on CarPlay, and using regularly. The front parking impact sensors not display on camera which could be a safety hazard of you had something or someone that was close to proximity to the vehicle and the camera not display. I have taken the vehicle in for this and was told they could not replicate.Read more

While accelerating on a green light, vehicle shifted down to a crawl. Truck had an abrupt reduction in speed and was difficult to get vehicle to a safe place to stop. Traffic was heavy and dangerous, trying to get truck off roadway was very difficult. I had to park in a no parking zone to allow traffic to continue. The wrench warning lamp symbol did come on, and warning hill climbing is not po
... ssible. The roadway was flat, straight with no incline at all, and clear for easy traffic movement. It was sunny afternoon. I shut truck off and checked underneath vehicle. After 10 minutes, I restarted vehicle and warning lamp was off and truck operated properly.Read more

The vehicle is a 2020 Toyota Sienna. While driving, the engine suddenly shut off without warning, creating a dangerous situation and loss of power. This occurred while transporting children, increasing the safety risk. A mechanic inspected the vehicle and diagnosed a failure of the low-pressure fuel pump. The problem appears intermittently — sometimes the vehicle runs normally, and other time
... s it stalls or loses power. The check engine light has turned on and off. This issue matches the known defect related to the Denso fuel pump recall affecting many Toyota vehicles from similar model years. However, my VIN is not included in the recall. This is a serious safety concern because the engine can shut off unexpectedly while driving, which could lead to an accident. I believe this vehicle may have the same defective component as those included in the recall, even though it is not currently listed. The issue has been confirmed by an independent mechanic and the vehicle is available for inspection if needed.Read more

On [XXX] about [XXX] I was driving North on the [XXX] about a quarter mile north of the [XXX] exit and [XXX] 4 of a mile south of the [XXX] exit. The transmission in my 2019 Toyota Camry started to slip and failed to transfer power to the wheels. The engine reved to about 2800 RPM. Thankfully I was in the second to slowest lane to the right and was able to coast and change to the slowest
... lane and continued to coast and stopped on the shoulder, not blocking the flow of traffic. This was while the traffic was extremely heavy and it had begun to drizzle. It was extremely dangerous and other drivers patiently allowed me to get to the shoulder of the road where b y that time the vehicle had ZERO power to move at all. AAA was called and they towed mehome. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

Was changing the oil on my car and noticed grease running out from between the transfer case and transmission and also fluid build up on the transfer case higher up a little below the driveshaft. The car has 41k miles on it, there should be no leaks. My concern is the transfer case/transmission spline failure issue.

The engine randomly shut off while driving down the interstate and wouldn’t start back up. The car had the recall “remedy” done to it. Inspection and thicker oil. The only warning I got was to restart the engine after it shut off. My safety was compromised by breaking down on a main interstate coming through Memphis with no emergency lane to pull over in.

I am creating this complaint because my vehicle has not been included in the NHTSA recall # 25V-767 on the V35A 3.4L twin-turbo V6 in my 2023 Sequioa. My vehicle was produced on the same manufacturing line and likely had the same engine debris. Toyota gives the reason for not including the hybrid because it is not a safety issue as the hybrid motor can provide power if the engine fails. However ow
... ners who have encountered this issue in a hybrid tundra or Sequioa has said that power-steering and control systems failed creating a safety issue. Additionally we use this vehicle to do long road trips that often put us long distances from any service center or roadside assistance. This creates an additional safety hazard for our family being stranded far from potential help due to this issue. Since this is a known issue covered under a recall for other models with the same issue I am asking that my vehicle be included in the recall so that we can have confidence to use this vehicle as intended. We bought a Toyota and a lifetime Toyota owners due to the reputation of reliability that we could depend on the vehicle to get us across the country in safety. Not including our vehicle in this recall is breaking the trust that we have in the brand to do the right thing to cover their mistakes. I am requesting that the recall be consistent to cover all the engines impacted by manufacturing debris including the hybrid variants of the motor.Read more
