I am the owner of a 2019 Infiniti QX50, VIN [XXX] , and I am filing this complaint due to repeated, severe mechanical failures that have rendered my vehicle unreliable and unsafe. Since purchasing the vehicle, I have experienced catastrophic failures requiring two complete engine replacements, two turbocharger replacements, and one transmission replacement. My vehicle has spent over one year cumul
... atively at repair shops, leaving me without transportation and in constant fear for my safety. I am unable to drive this vehicle on the highway or for long distances due to the very real risk of sudden engine stall or failure at speed. I am aware that Nissan has issued Recall 25V-437 acknowledging manufacturing defects in the VC-Turbo engine bearings on the 2019–2022 Infiniti QX50, and I believe my vehicle's repeated failures are directly related to this defect. Despite multiple repair attempts by authorized Infiniti dealerships, the underlying issues have not been resolved. I am requesting that NHTSA investigate this vehicle and pressure Nissan/Infiniti to provide an appropriate remedy, including a full vehicle repurchase or buyback. My family's safety is at risk every time we are forced to drive this vehicle. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I am experiencing multiple unresolved defects with my 2026 Chevrolet Equinox affecting drivability, braking behavior, and vehicle structure/fit and finish. The primary issue is a persistent vibration, shimmy, or wobble during moderate acceleration from a stop. This condition is felt through the steering wheel, seat, and floor, and occurs consistently during low‑speed launch and normal accelerati
... on. In addition, the vehicle produces a snapping or grunting noise during hard braking, as well as rough and loud movement noises from underneath the vehicle during normal operation, which raises concerns about braking and drivetrain integrity. There are also body and interior fitment defects. The front passenger door does not sit flush with the rear door panel and appears slightly open when closed. The dealership service manager stated that correcting this condition would require removing and pulling the rear door panel outward so both doors align. The glove box door also does not sit flush when closed, and the service manager advised that replacement might make the condition worse. These concerns were reported to an authorized Chevrolet dealer. The dealer confirmed that General Motors has acknowledged the acceleration vibration condition in a Preliminary Information service bulletin (PIT6466) affecting 2025–2026 Equinox vehicles and advised that GM engineering is investigating the issue and that no repair is currently available. The combination of drivability issues, abnormal noises during braking, underbody movement sensations, and body misalignment affects the use, value, and safety perception of the vehicle. The vehicle has very low mileage, and several of these issues have been present since early ownership. I am submitting this complaint to document these conditions and request that NHTSA monitor potential powertrain, braking, suspension, and vehicle structure concerns if a defect trend exists. All of these defects have been acknowledged by the Dealership Service DeptRead more

The vehicle experienced engine failure due to coolant intrusion into the engine cylinders. The issue presented with engine misfire, including diagnostic trouble code P0302, rough running, and coolant entering the combustion chamber. The condition was confirmed by a dealership, which determined that the engine has internal damage and requires full engine replacement. There were no external coolant
... leaks observed. This failure occurred at approximately 88,000 miles and was not consistent with normal wear and tear. The issue developed without significant prior warning other than engine misfire and rough operation. This condition may create a safety risk, as engine misfire and internal damage can lead to loss of power, reduced vehicle performance, or potential engine stall while driving. Technical Service Bulletins have been issued for similar vehicles equipped with the same engine, including TSB 19-2172, TSB 19-2208, and TSB 22-2229, which describe coolant intrusion into engine cylinders and require engine replacement. This suggests the issue is not isolated and may affect other vehicles with the same engine design.Read more

Beginning in approximately May 2025, the vehicle began displaying diagnostic trouble codes identified by independent auto parts retailers and service providers. These codes indicated potential issues within the engine, electrical, and powertrain systems. The vehicle was subsequently brought to a dealership for diagnostic evaluation and remained there from approximately May 15 through May 19, 2025
... . During this time, the dealership reported that no diagnostic codes were present and did not resolve the reported issue. However, the vehicle had recently exhibited repeatable symptoms, including entering “limp mode” and triggering diagnostic trouble codes, prior to being delivered to the dealership. Similar codes and issues were again identified after the dealership visit. The vehicle has experienced multiple incidents of entering “limp mode,” resulting in sudden loss of power and reduced drivability while in operation. This condition occurred without reliable warning and created unsafe driving situations, including the inability to maintain normal traffic flow. Due to safety concerns, the vehicle has not been driven under normal conditions since approximately May 2025 and has been limited to short, local movements only. The issue poses a safety risk to the driver, passengers, and surrounding traffic due to unexpected loss of power and inconsistent system diagnostics. The problem appears to be ongoing and unresolved. This is related to the incident report number XXX I filed previously. Before, between, and after that incident and this one reported today. The vehicle performs mostly normal, the severe issues are completely random and unpredictable. On 4/28/26 I started my truck and left to take my daughter to her [XXX] pediatrician appointment. I left my neighborhood and turned left into traffic. I didn’t
... speed up much, as the red light on the street stopped me about 100 yards down the road. Then, when green light hit I attempted to accelerate. The vehicle appeared to be stuck in 6th gear and would only rev up the RPMs to about 4000 and barely move forward. I managed to drive about 50ft and get the vehicle off onto the grass shoulder infront of someone’s house. I shifted the vehicle from park to drive and turned it on and off multiple times. This did not resolve the issue and I eventually put the truck in 4x4 in an attempt to make more power and drive forward. This entire time I’m on the shoulder of morning 55mph traffic, hazard lights on, with my infant in the back seat. I managed to get the vehicle to shift into 2nd gear and drive up the road. Once it came up to operating temperature it would shift into higher gears and we made it to the pediatrician. The truck stalled out similarly at two other stopped intersections and on the parking deck before we arrived at our destination. I called my wife and had her drive our daughter home after the appointment. I drove the truck home, on the same route, and it acted as if nothing was wrong. It shifted hard between 1-3rd but otherwise performed well enough. I have scheduled a transmission rebuild with a non-Ford service center for 5/4/26. It is extremely concerning that these issues are happening to myself and others with the 10R80 10-speed and Ford isn’t being forced to recall these vehicles. My truck is a 2019 with 86000 miles now. **Incident / Defect Description for NHTSA Safety Complaint** **Vehicle Information** - Make: Tesla - Model: Model S - Year: 2014 - VIN: [XXX] - Approximate Mileage: 148,129 miles - Date of Discovery / Failure: 04/23/2026 **Description of the Defect** This vehicle is equipped with Tesla’s Large Drive Unit (LDU / rear drive unit), which was used in the majority of 2012–early 2020 Mod
... el S vehicles (RWD and Performance models). The LDU design routes coolant through the hollow rotor shaft. Over time, the rotor seal fails and allows ethylene glycol coolant to leak internally into the motor and inverter. Evidence of this failure is easily visible by removing the rear speed sensor, where blue coolant, corrosion, and debris are present. This is a well-documented design flaw affecting thousands of vehicles. **Consequences of the Defect** Coolant intrusion causes: • Corrosion and rust in the stator windings • Washing out of bearing grease • High-voltage isolation faults • Inverter damage • Complete drive unit failure **Safety Risks** This defect presents an unreasonable risk to motor vehicle safety because: • Drive unit failure can occur suddenly and without warning while driving, resulting in total loss of propulsion. This is especially dangerous on highways, in traffic, or at high speeds. • Coolant inside high-voltage electrical components increases the risk of electrical shorts, arcing, or fire. • Reduced power or complete shutdown can lead to loss of vehicle control and potential crashes. I have experienced complete drive unit failure. Many other owners report identical issues, often between 50,000–120,000 miles. Tesla has not issued a recall despite this being a widespread, predictable design defect. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

The vehicle is a 2019 Lincoln Navigator. The component that failed is the transmission. The vehicle experienced a sudden transmission failure resulting in loss of normal drivability. The issue is currently available for inspection at the repair facility upon request. This failure created a safety concern due to loss of power, hesitation, and unreliable shifting, which can increase the risk of col
... lision, particularly in traffic or during acceleration. The problem has been confirmed by the dealership and independent repair facilities. Multiple repair professionals indicated that this failure is consistent with a known issue addressed by a Ford Technical Service Bulletin (TSB), suggesting this is not an isolated incident. The vehicle has been inspected by the repair facility. The manufacturer has not directly inspected the vehicle to my knowledge. There were no significant prior warning indicators that would suggest imminent failure. Any symptoms prior to failure were minimal or not clearly indicative of a major transmission issue. Based on information from repair facilities, this appears to be a known defect pattern rather than normal wear or maintenance-related failure. The use of a TSB instead of a recall raises concerns that a broader safety-related defect may not be adequately addressed. This failure occurred after proper maintenance and outside any initial waiting period for warranty coverage. The issue appears systemic and warrants further investigation.Read more

The 68RFE automatic transmission in this 2021 Ram 2500 6.7L Cummins fails to engage reverse when the transmission reaches operating temperature. When the gear selector is placed in reverse and the accelerator is applied, the engine revs but the truck does not move. The defect is intermittent and temperature-dependent, which means it cannot be predicted or avoided by the driver. The vehicle is used
... to tow an enclosed cargo trailer weighing approximately 7,000 pounds. The safety risk: inability to engage reverse on demand creates a hazard in any situation requiring reverse motion, including backing away from a stopped vehicle, reversing out of a fuel station, backing a loaded trailer at a campground or boat ramp, or executing an evasive reverse maneuver. With a trailer attached, loss of reverse function can leave the truck and trailer stranded in active traffic lanes or at intersections. The defect was first reported to a dealer in August 2023 with diagnostic trouble code P2701 stored (Friction Element B implausible pressure, internal transmission fault). Three months after the recall remedy was performed, the vehicle suffered complete loss of electrical power and woul
... d not move. It was towed to the Kia dealer. The dealer's own diagnostic recorded code P1A9096 — the exact diagnostic code Kia's recall instructs technicians to check for — and replaced the ICCU and high-voltage fuse. This is the recall-defined failure occurring AFTER the recall remedy was applied. Approximately three months later, the vehicle again suffered total loss of power and would not start in cold weather. It was towed in. The dealer replaced only the 12-volt battery and did not address the ICCU, despite the documented prior ICCU failure on this VIN. The 12V battery failure is a known downstream symptom of ICCU failure on this platform. Approximately one month after that, the vehicle returned for charging system failure ("vehicle charges briefly then stops") and a "check electrical/hybrid system" warning that appeared while driving. Diagnosed as charging port failure. Vehicle was out of service approximately 27 consecutive days. Total cumulative days out of service across four visits: approximately 35-40. Two safety-related stranding incidents resulted from these failures. Both occurred in winter conditions in Pennsylvania. In the first, the driver was stranded in a parking lot at night during a snowstorm in sub-zero conditions; three tow trucks were dispatched before a flatbed could recover the vehicle because the EV could not be towed conventionally with no power. 2021 Toyota Highalnder UA80F Transmission Whine / Known Defect - Potential for Sudden Failure I own a 2021 Toyota Highlander Platinum V6 AWD (UA80F 8-speed automatic transmisstion) purchased in July 2021. Current milage is 78k. Starting 3 days ago I noticed a faint whine noise coming from the transmission. The pitch changes with speed and when the throttle is depressed (It's louder under load/ac
... celeration). This matches the exact symptoms described in Toyota TSB T-SB-0008-21 for whine/grind noise caused by the front carrier assembly pinion shaft issues in 2021 Highlanders with UA80 transmissions. On April 27, I went to a Toyota dealer to confirm the source of the noise and diagnose. As per their diagnosis report "Tech took vehicle on road test to duplicate concern, after confirming complaint vehicle was then lifted up and inspected while under a load, tech as able to isolate noise to transmission at this time, fluid level and condition was inspected to ensure there were no signs of shavings or discoloration and after inspection tech found nothing abnormal during fluid level check. Tech then ran a health check to ensure there were no DTC present related to transmission concern and found no DTC present. Recommend replacing transmission and torque converter at this time". They provided a quote of $9,392. This condition represents a safety concern because the internal wear can progress to harsher shifting, loss of power, limp mode, or complete transmission failure while driving, potentially causing a crash or stranding the vehicle in traffic. I have audio/vehicle recordings of the whine and service records showing regular Toyota dealer maintenance. I am requesting the the NHTSA investigate this ongoing UA80 transmission issue as it appears to be a known manufacturing or design defect tat continues beyond the narrow TSB applicability and warranty limits.Read more

Recall was completed on 11/25/14 for a shudder when hitting 35 to 50. The recall was a software issue and they show that a torque converter may have been needed at a later date to further correct the issue. Looking for Toyota to follow up with a complete recall vs just the software.

This hasn't caused an accident or stranded me yet, but Ford won't do anything to help me avoid danger in the faulty vehicle they sold. There have been several recalls on my 2022 Ford Escape PHEV over the past year or so, including safety recalls. The number of recalls, requiring me to take the time to deal with scheduling, drop off/pickup the car, and being unable to use the car when it is in the
... shop for the recall rectifications seems excessive. Worse yet, they have safety related recalls from 2025 that still do not have a fix. I do not feel safe in my car. I went through the company's lemon recall process and they refused to repurchase the vehicle. People should expect a basic level of quality and definitely safety when buying a car. I will never buy another Ford and would never recommend that anyone else do so. This company no longer represents quality and they don't even care to try.Read more

2022 Ioniq 5 SE. ICCU failed April 2026 with around 29325 miles. Original owner. Purchased June 2022. I had the original Recall 272 software update performed back in February 2025, but the hardware still failed a year later. **Timeline of Failure:** 1. **Phase 1 (The Trigger):** Level 2 charging stopped unexpectedly before reaching the limit. It tripped my house breaker and unfortunately kille
... d my Level 2 EVSE (Grizzl-E) with a stuck relay. Didn't think too much of it because I had to be somewhere. Car started up normally and I was able to drive throughout the day with no problems. 2. **Phase 2 (The Warning):** The next day, I attempted Level 1 charging. It immediately tripped the breaker. I tried another outlet on a different breaker; the breaker didn't trip, but the car failed to initiate a charge after three attempts. 3. **Phase 3 (Dashboard Error):** After third failed attempt with the L1, I took a peek inside and saw the dashboard had a yellow triangle. With the charger still plugged in, I sat in the driver's seat and turned the car on. The **"Check EV System"** error appeared. I turned off the car and went inside to call the dealer. I called the dealer and they said I can have it towed there. They also scheduled an appointment for the next morning. I went back to car to attempt to position the car to make it easier for the tow truck, but the check ev error was not there when I turned on the car. It said ready and I was able to drive around the block a few times so I figured I'll just try to drive to the dealer. I drove the car to the dealer the next morning. It took **one week** to get my car back. **Diagnostics & Parts Replaced:** The dealer confirmed the following codes and performed a full hardware replacement: * **DTC P1E1300:** AC Power Module Failure * **DTC P1A9096:** DC/DC Converter Fault * **ICCU Part:** `36400-1XAA0-AQQH` * **HV Fuse:** `375F2-GI040-QQH` * **Coolant:** Install of Pink Antifreeze (`00232-19098`)Read more

I purchased this 2024 Kia Carnival on 2/7/2026 with less than 10K miles. The vehicle has died while I was driving 4 different times. Twice each day that it happened. The first time was on 4/7/26. My car died on the way home from getting my kids from school after fixing a flat tire. The check engine light came on after getting a jump and when I took it to Auto Zone the battery was fully charged and
... the codes were for the Power Train, which didn't make sense for a brand new car. Miracle Mart told me that Kia needed to be utilized for any issues because I purchased the vehicle as is. Their solution was to tighten the "loose cables". On 4/25/26 the car died again two more times. Something is terribly wrong with this car.Read more
