My 2023 Ram 1500 TRX has recurring safety-related failures of the infotainment system affecting both Uconnect 5 (12” screen) and earlier Uconnect 4 units. These issues are widespread in 2021–2024 Ram TRX and Ram 1500 models and began or worsened after forced over-the-air updates. The primary defect is black-screen failures while driving. When the screen goes black or freezes, the truck loses
... all functions controlled through the display, including HVAC/defrost, backup camera, vehicle settings, seat controls, and performance gauges. This creates an immediate safety hazard: I cannot adjust climate/defrost, cannot see the required rear-view camera when shifting to reverse, and cannot access key vehicle information. Owners across multiple years consistently report: • Black screens, freezes, and reboots • Loss of HVAC control and dual-zone functions • Backup camera not displaying • Vehicle/Apps tabs failing to open • Uconnect error messages • Smaller, harder-to-read gauges after updates • Wireless CarPlay/Android Auto failures • No option to decline or roll back OTA updates These failures distract the driver, remove access to federally required equipment (rear camera), and eliminate visibility and climate controls that directly affect safe operation. The system becomes unpredictable and unstable despite the vehicle being in motion. My vehicle experiences the same issues, including the inability to properly use the various vehicle drive-modes as driving conditions dictate. Because this affects essential safety-related controls and visibility, I believe this is a safety defect in Ram/Stellantis Uconnect 4 and Uconnect 5 systems. I request NHTSA open a defect investigation and require Ram to correct these failures.Read more

On April 17, 2025, while driving a 2024 Lexus GX 550 Luxury Plus with a minor passenger in the vehicle, the vehicle locked up without warning as I was exiting the highway. The sudden lockup caused skid marks on the tires. The failure occurred at low speed only because I had just turned off the highway — had it occurred moments earlier at highway speed, the consequences could have been fatal. The
... failed component appears to be the drivetrain or braking/locking system. The vehicle is currently in the possession of the Lexus dealership and is available for inspection. A Lexus field inspector has been scheduled for early May 2025. Safety was put at serious risk. A child was in the vehicle. The lockup was sudden and without warning, with no ability to anticipate or respond. This incident was preceded by two prior issues with the same vehicle: (1) approximately one month prior, a transmission concern was reported to the dealer, who attributed it to abnormal driving habits and took no corrective action; (2) on April 7, 2025, brake rotors were replaced under a recall for rotors that squeak when exposed to moisture. The problem has been confirmed by the dealership, which has retained the vehicle since April 17, 2025. I am aware of at least 8 to 10 other reported incidents involving the same model vehicle with the same lockup failure. No warning lamps or messages appeared prior to the failure.Read more

The Power Conversion System (PCS) in the Tesla Cybertruck is experiencing a high rate of hardware-level failure (specifically the PCS2 module) that poses three distinct safety risks beyond the loss of AC charging. Loss of Steer-by-Wire Integrity: The Cybertruck utilizes a full steer-by-wire system and a 48V low-voltage architecture. The PCS is responsible for managing the 48V bus and charging the
... LV battery. A hardware failure in the PCS (common alerts: PCS2_a094, PCS2_a137) threatens the stability of the 48V system. Any sudden collapse of the 48V bus while in motion could result in a total loss of steering control and vehicle maneuverability, creating an immediate risk of a high-speed collision. Loss of Propulsion and Stranding: PCS failure frequently leads to "limp mode" or a "no-start" condition where the vehicle cannot engage Drive or Reverse. Experiencing a loss of propulsion while in active traffic or being stranded on a highway shoulder without the ability to move the vehicle creates a severe life-safety hazard for occupants and other motorists. Failure of Visibility Systems: Critical safety components, including the primary windshield wiper and exterior lighting, are powered via the 48V system managed by the PCS. Hardware instability has led to reports of these systems failing during inclement weather, severely compromising driver visibility. Tesla has implemented software bypasses (OTA updates) to allow DC Supercharging when the AC hardware fails, but this is a "band-aid" that does not address the underlying hardware defect—likely MOSFET degradation. With replacement parts on backorder for 8+ weeks, owners are forced to operate vehicles with compromised electrical architectures. I am reporting this as a systemic hardware defect that requires an official safety recall.Read more

Our 2019 traverse transmission around 130,000 miles started shuddering when slowly accelerating or driving. Not sure what causes this but I do know that it should be doing this as well as getting the Shift to Park message. I looked online and found that it's supposed to be the torque converter. I then contacted a transmission repair specialist and he stated that changing the fluid may make it wors
... e or make it possibly fail altogether. Then I found a video about Shudder Fix and I added one of those packets to the transmission and it stopped the shudder but it now seems to be trying to creep back. This is a known issue with most people not being able to afford the $9000+ repair costs. The Shift to park issue is another issue that shouldn't be happening and the dealership will charge an arm and a leg for the repairs on this as well. Pressing the gear shift button about 5 to 10 times after shifting to park is a short term solution because that issue is tied to a spring action underneath the shifter to tell the car I'm in park. I found a bypass piece that is required to resolve the issue but I shouldn't have to do that to get this vehicle to work properly. Chevy needs to do a recall on this or they run the risk of having a bunch of them on the market where we all got rid of them and won't buy anymore Chevrolet vehicles. This is the second newer model vehicle I had from them. The first one was an Impala and the dealership didn't know why the screen would go black and cause the car to start hesitating horribly. So I left it with the dealership, they can have it back.Read more

The vehicle's auto idle stop system is malfunctioning, resulting in the engine failing to restart from a complete stop. This total failure has occurred on five separate occasions, including while stopped in active traffic intersections. Prior to these total restart failures, the vehicle showed the following symptoms: 1) Initially, the vehicle experienced rough engine idling, abnormal vibrations,
... and a raw gasoline odor after driving. 2) This progressed to a noticeable delay between releasing the brake pedal and the engine attempting to ignite, during which the starter sometimes sounded strained. 3) This has culminated in the current defect where the engine shuts off normally when stopped at an intersection, but completely fails to immediately restart when the brake is released. This defect completely immobilizes the vehicle in active traffic lanes, exposing us to an immediate and severe risk of a rear-end collision. When the engine fails to restart, the vehicle locks the transmission out of drive and triggers a warning on the dashboard. It is a highly hazardous situation, especially with a lot of distracted drivers on the roadways.Read more

I am reporting a serious safety issue with my 2021 Chevrolet Trailblazer related to transmission failure. The vehicle has approximately 83,000 miles. While driving, the vehicle experiences a sudden loss of power when attempting to accelerate. In some instances, the vehicle has stalled or shut off while I was actively driving, creating a dangerous situation, especially when merging into traffic or
... driving through intersections. The issue is intermittent but has become more frequent over time. The vehicle does not respond properly when the accelerator is pressed, and there is a noticeable delay or complete lack of power. At times, the vehicle will not move as expected, which poses a significant safety risk. Diagnostic codes associated with this issue include P2714 and P0700, which indicate a transmission-related malfunction. I have been advised that the vehicle requires a full transmission replacement. This failure occurred at relatively low mileage for a major powertrain component, and I am concerned this may indicate a broader defect. The stalling and loss of power create a hazardous driving condition that could result in an accident. I am submitting this complaint to report a potential safety defect and to request further investigation into this issue.Read more

Hello. We purchased this 2018 Honda CR-V new in 2018 from the local Honda dealer, Honda of Lake Jackson, TX. In the Spring of 2025 the A/C stopped working. We took it to a local shop and they had to replaced the following: Blower motor replaced, blower motor replaced, A/C evaporator replaced, A/C expansion/orifice valve replaced, Heater core replaced. This was around a $3,000 repair. A month or
... so later in May of 2025 we took the car to the Honda dealer because of steering instability issues. We were advised that the entire Steering Column would need to replaced due to known safety issues (drive shaft can fail completely at highway speeds). The following was repaired at this time: --Rack and pinion replaced, steering gear replaced, valve cover gasket replaced Vehicle was at 61,984 miles at this time. This was a $5,000+ repair. It cost so much that we actually had to take out financing to pay it, which in essence essentially renewed our car payment (car was already paid off at this time). Now for the past 8 months or so we have had a "juddering" issue where you can feel the transmission CTV system jump and jerk as you accelerate to highway speeds. This issue also puts the entire transmission at risk of failure. The Honda dealer here checked the vehicle out and suggested we do a transmission fluid swap. This cost $189 for the "diagnostic fee" and $179 for the swap. So another nearly $400. They did the swap and said that it did not fix the issue and now we need to pay $2800 to have the "Torque convertor" replaced. This is $10,000 plus dollars in repairs, nearly half to cost of the vehicle. This is a ridiculous amount of issues for a vehicle that had only 61,000 miles. We have always had regular maintenance performed also. From what I have read, Honda knows about these issues but has not issued a recall. In addition I've read that "juddering" CTV issue is a known issue in 2015-2018 Honda CR-V's. Needless to say we are not happy with Honda about any of this.Read more

Incident 1: While stopped with my foot on the brake, the engine shut off due to auto start/stop. When I released the brake, the engine restar
... ted but the vehicle did not move forward and instead revved slightly. I immediately pulled the selector back and the vehicle resumed normal forward movement. I did not intentionally shift gears and believe the vehicle had unintentionally shifted into Neutral. Incident 2: While stopped with my foot on the brake, I reached for a drink in the cupholder. During this normal action, the gear selector was likely contacted unintentionally. The vehicle shifted into Reverse without my intent, confirmed by the backup camera activating. The vehicle did not move because I remained on the brake and recognized the issue immediately. Afterward, I tested this interaction in a controlled environment in my driveway. While stationary with my foot on the brake, repeatedly reaching for a cup caused the vehicle to shift into Neutral or Reverse multiple times without intentional input or clear tactile feedback. The selector’s location next to the cupholder and its sensitivity allow incidental contact during normal use. As a left-handed driver reaching across the console (with a wristwatch and jacket), contact may occur without awareness. This creates risk of unintended gear selection, unexpected movement, or failure to move when expected.Read more
