I own a 2019 Honda HR-V that has been consistently maintained at Honda dealerships, including proactive transmission fluid changes every 30,000–40,000 miles. At approximately 104,700 miles, during a routine maintenance visit, the dealer diagnosed a failing CVT transmission. Technicians identified metal debris in the transmission, indicating internal deterioration. This condition is consistent w
... ith Honda Technical Service Bulletin 21-047, which acknowledges CVT-related issues in 2016–2019 HR-V models and provides extended coverage for affected vehicles. However, my vehicle was deemed ineligible for coverage because the failure was identified 27 days after the 7-year in-service deadline. Given that transmission deterioration is progressive, it is likely the issue began within the coverage period. Despite this, Honda declined to provide any goodwill assistance. I was quoted approximately $11,000 for the repair. This issue also raises safety concerns. A failing transmission can result in loss of power, hesitation, or inability to accelerate, particularly when merging into traffic or crossing intersections. These conditions increase the risk of an accident, especially if failure occurs unexpectedly while driving. This failure occurred despite proper and proactive maintenance and at a mileage where a transmission failure would not typically be expected.Read more

How was your safety or the safety of others put at risk?* Complete loss of reverse gear created an immediate safety hazard the vehicle cannot be moved out of parking spaces, driveways, or roadway positions where reversing is required to avoid a collision or escape danger. The failure occurred without warning during normal vehicle operation. An inability to select a transmission gear while in traffic or a parking situation creates collision risk for the driver, passengers, and surrounding vehicles and pedestrians. 3. Has the problem been reproduced or confirmed by a dealer or independent service center? Yes. Ford authorized dealer Koons Woodbridge Ford confirmed the failure on May 4, 2026, under RO# XXX. Diagnosing technician: David Ford (Tech ID XXX). Official finding documented on the dealer repair estimate: "VERIFIED CONCERN. THE TRANSMISSION HAS INTERNAL DAMAGE." As of today, there are class action cases for this. Ford has correct the know deficiency in the latest (2025+) versions for the transmission, so there should be a recall to correct the problems with previous versions. Ford Motor Company has known about the defect with the lack of lubrication on the CDF drum, as well as other issues, and sold the defective parts anyway. This incident did not result in an accident, but only because traffic was light enough that other motorists were able to swerve around me. I, [XXX] was driving home from work on Friday, May 1st on [XXX] in [XXX] during heavy traffic when my vehicle suddenly shut off while traveling approximately 65–70 mph. I initially thought I may not have been pressing the accelerator hard enough, but the vehicle would not respond at all. I then looked at the dashboard and saw a message that said “push brake to start.” At that moment I re
... alized the vehicle had shut down completely while I was in active traffic. I had to coast across three lanes of traffic without any propulsion to reach the shoulder. During this time, I was nearly struck twice from behind because I could not maintain speed with surrounding traffic. Once I reached the shoulder, I was able to restart the vehicle. I was extremely shaken and frightened by the incident. I do not feel safe driving this vehicle again. I was very aware that I could have been seriously injured or killed, and I am thankful I was not struck. I am especially concerned about what could have happened if my grandchild had been in the vehicle with me. This incident occurred after a recall repair had already been performed in September 2025 related to powertrain/hybrid system issues. The failure appears to be the same or similar to the condition the recall was intended to correct. After restarting the vehicle, I drove cautiously to my husband’s workplace and left the vehicle parked. Since the incident, I have experienced ongoing anxiety and distress reliving what happened. This vehicle was intended to be a long-term purchase, and it is extremely concerning that it now appears unsafe to operate. This event represents a serious safety hazard. A sudden loss of propulsion at highway speeds without warning creates a high risk of crash, injury, or death. I am requesting that this issue be investigated, as the prior recall repair does not appear to have corrected the problem. I do not feel this vehicle is safe to operate. At 90,181 miles, the 10R80 transmission in my 2017 Ford F-150 suffered a catastrophic internal failure. The vehicle exhibited violent, hard shifts between gears 2 through 4 with no check engine light or warning indicator, giving zero warning the drivetrain was failing. Sheehy Ford in [XXX] reproduced the condition. Their diagnosis on RO XXX: "HARSH 4-6 SHIFT-INTERNAL FAULT." Teardown revealed a
... fractured CDF drum with metal debris that migrated into the valve body. The dealer removed the transmission and performed a full overhaul, replacing the CDF drum, clutch cylinder, forward clutches, and valve body, and flushed the cooler and converter. Total cost: $6,846.30, entirely out of pocket. This is a serious safety defect. A fractured CDF drum can cause sudden loss of gear engagement, unexpected downshifts, or transmission lockup at highway speed, any of which can result in loss of vehicle control and severe risk of injury or death. I use this vehicle daily to transport my two young children (ages [XXX] and [XXX] ) to and from daycare. This defect endangered their lives, my life, and other motorists, all with no diagnostic warning. This failure is consistent with widespread defects in Ford's 10R80 transmission, currently the subject of class-action litigation. A transmission should not need a $7,000 overhaul before 100,000 miles. Ford is aware of this pattern and has failed to recall. Coolant in high-voltage areas also raises electrical fault/fire risks. The problem is widely reproduced and confirmed: Tesla service centers are familiar with it and replace failed units with updated “-U” revisions that include a coolant bypass. Independent EV shops confirm the exact root cause (rotor seal failure) via encoder inspections showing coolant intrusion and through disassembly. This is a documented pattern failure in all 2012–2020 LDU vehicles. No police or insurance inspections have occurred yet, as no crash or total failure has happened. I recently purchased this vehicle, and shortly after, it began having serious powertrain issues. When I accelerate, the vehicle intermittently loses power and starts shaking violently. It feels like the engine or transmission is slipping or failing. The RPMs surge and drop rapidly, and the vehicle hesitates, jerks, and sometimes briefly slows down before suddenly accelerating again. This creates
... an unsafe and unpredictable driving condition, especially when merging or driving in traffic. I’m concerned about a loss of propulsion while the vehicle is in motion. My mechanic indicated the issue may involve the torque converter clutch not engaging properly (possible “stuck off” or circuit performance issue), which would explain the shuddering and inconsistent power transfer. Despite how severe this is, there are no warning lights or check engine light, and no diagnostic codes, which raises concern that the failure is not being detected by the vehicle’s onboard systems. I am also experiencing low and fluctuating engine oil levels even after topping it off and a recent oil change, which may indicate an additional engine-related defect. The vehicle has a clean title and no accident history. - Failed system: Powertrain (engine/transmission), suspected torque converter clutch malfunction. Vehicle is available for inspection. - Safety risk: Intermittent loss of power, hesitation, and surging creates a high risk of collision, especially in traffic or at highway speeds. - Confirmation: My independent mechanic has identified as likely a torque converter related issue. - Inspection status: Not yet inspected by manufacturer or insurance. - Warnings/symptoms: No warning lights or codes. Issues with Nissan’s Resolution Process: Premature Part Failure: The transmission installed by Nissan in 2023 should have a reasonable life expectancy beyond 31,000 miles. A secondary failure this soon suggests a defective replacement part or improper initial repair. Safety: Luckily, my husband was stopped at a traffic light when the transmission failed. However, had to receive help to push the vehicle out of the busy intersection. There was no warning message displayed on the dashboard prior to this incident. A metallic clinking/rattling noise developed from the engine/transmission area while the vehicle was running. The issue was diagnosed as a failed flexplate. The flexplate is a critical component that connects the engine to the transmission. Continued operation with a failed flexplate can lead to further mechanical failure and could result in loss of propulsion or inability to move the vehicle, cr
... eating a potential safety risk, particularly in traffic or when acceleration is needed. The problem was confirmed by a dealership service center. The vehicle was still operable at the time, but the noise was persistent and indicated a failure condition. No warning lights or messages were observed prior to the issue. The noise appeared suddenly. The component was replaced during repair. It is unknown if the failed part is available for inspection. There are reports of similar flexplate failures in comparable vehicles, raising concern this may not be an isolated issue.Read more

2024 Dodge Durango (SXT Plus, GT Plus, RT trims) Update to my prior complaint regarding a dangerous shifter defect. Defect Description: While driving and pressing the accelerator (no brake, no shifter button pressed), the transmission shifts into Neutral without warning. The vehicle loses propulsion. The instrument cluster does not display or warn that it is in Neutral — no chime, no message, no
... clear gear indicator. This matches NHTSA Recall 26V262000 (FCA 39D), where the instrument panel fails to show gear selection in other 2024–2025 Stellantis vehicles. Widespread Issue: I have replicated this multiple times on my SXT Plus, a GT, and RT models. Other owners in Durango groups report the same. This is a systemic defect across Dodge Durango trims and likely the broader Stellantis/Chrysler/Mopar lineup. Related Lawsuit: This also appears connected to the class action [XXX] et al. v. FCA US LLC d/b/a Stellantis (Case XXX) regarding defective Uconnect 5 systems in 2022–2024 Durango's and similar models. My Background: I have worked in transportation most of my career, hold FDOT Advanced MOT certification, and am completing my undergraduate degree in Occupational Safety and Health (OSHA).I request NHTSA investigate this shifter and instrument cluster defect across affected Durango models. This has nearly caused a collision and endangers the public. Available: videos, records, owner statements. Car has been running fine and maintenance is up to date . Got up one day to leave for work and when I went to accelerate I started hearing a grinding clatter type noise ( only when accelerating at about 3k-5k rpm ) and once the car got up to needed speed it goes away . No engine light codes ( but having intermittent connection issues with obd port but never been damaged or anything) but it does it
... sometimes consistently and sometimes it will have a few moments under same acceleration condition or higher rpms but its never consistent. When these occur the car feels like power loss in the acceleration and a slight dragging in the drivability but again not always present. Been to exaust shop and they found nothing from.their initial inspection. Its hard to trigger the affects since it happens randomly/intermittently. Very unsave and there are no reasons or explanations to these eventsRead more

This vehicle has theta 2 engine.. which is part of class lawsuit kia settled. My vehicle had high oil consumption and the defect was due to rod bearing would cause engine to fail and cause hi oil consumption. The dealer diagnosed it with burned valve and denied warranty even though my warranty was extended due to this engine being a defect. The reason they stated is that the rod bearing
... is a separate issue from burned valves which is a complete lie because a faulty rod bearing would cause valves problem. I feel kia is just trying to avoid repair cost. They did want to bill me for a new engine immediately. And they failed to test the rod bearing saying they could not determine if it was defect because of the burned valvesRead more

Vehicle: 2021 Chevrolet Silverado VIN: [XXX] Mileage at time of incident: Approximately 68,000 miles Date of incident: April 24, 2026 Approximate speed: 75 mph Weather/road conditions: Clear, sunny day Was the vehicle towed? No Did the engine restart? Yes My 2021 Chevrolet Silverado experienced a sudden loss of power and stalled while I was driving on the highway at approximately 75 mph. The wea
... ther was clear and sunny, and road conditions were normal. This created a serious safety concern because the vehicle lost power while traveling at highway speed in traffic and could have caused a crash. The vehicle did restart after the incident and was not towed, but it had to be taken in for service after the power loss/stalling event. The dealership later advised that the oil was low on the dipstick and stated they would change the oil and complete a recall/service update. However, I have maintained the vehicle with regular oil changes, and at the time of the incident, the vehicle’s oil life monitor still showed approximately 50% oil life remaining. I do not believe this should be dismissed as a simple lack-of-maintenance issue. I am concerned that the sudden loss of power/stalling may be related to an engine, fuel, oil pressure, electrical, ECM, or recall/service campaign issue. I am also concerned that the vehicle was low on oil despite regular oil maintenance and the oil life monitor still showing about 50%. The vehicle is currently at Tyrell Chevrolet for inspection/service. I have not yet received a clear explanation of the cause of the power loss, whether diagnostic trouble codes were found, whether oil pressure was affected, whether oil consumption was checked, whether any recall or service campaign caused or corrected the issue, or whether the truck is safe to drive. This sudden loss of power while driving at approximately 75 mph presents a serious safety risk and should be investigated.
