My 2019 Toyota RAV4 Adventure suffered a sudden catastrophic transmission failure while driving on the highway on April 28, 2026, resulting in loss of forward gears and a dangerous situation. Their diagnosis (copy of repair order attached) revealed severe internal damage including: Torque converter clutch actuator stuck off Multiple pressure control solenoids stuck Transmission slipping and complete loss of all forward gears Fluid full but contaminated with metal debris and clutch material Whining noise The dealer quoted approximately $15,000 for a full transmission + torque converter replacement and denied goodwill assistance. This appears to be part of a known issue with the 8-speed automatic transmission (UB80) in 2019+ Toyota RAV4 models. I am concerned this is a safety defect that can cause sudden loss of power while driving, increasing the risk of a crash. Back passenger door wont shut it just keeps popping back open 12. Doors dont lock when you walk away 13. Passenger doors will unlock but driver side doesn't 14. The passenger door got stuck and we had to crawl through and push it open 15. Transmission gets stuck and wont go over 30 and the rpms go up but the speed doesn't move, it shifts hard and makes a thud sound, and the engine is crazy loud 16. Has a crazy vibration coming from it 17. Will brake on its own with nothing around it 18. Was switching to sport mode on its own in the middle of driving 19. The new steering column was getting stuck and acting up and it was trying to merge in to the other lane but lane assist off 0. We had an exhaust leak 21. Pieces of trim with vents broken inside 22. The selling Chevrolet dealer replaced the K212 pushbutton circuit board and reprogrammed the HPCM2 module. Second failure occurred at approximately 54,969 miles in March 2026, only about 9,300 miles after the first repair. Same diagnostic code family, including the identical U18D2 code, plus U0076, U18C6, and U240E. The same dealer replaced the same K212 pushbutton circuit board a second time. The repair was processed under manufacturer warranty. Safety risk: when the failure occurs, communication with the transmission range selector is lost intermittently and without warning, leaving the vehicle unable to be reliably shifted into Drive, Reverse, or Park. This creates a clear hazard in traffic, parking lots, and on inclines. The failure mode is sudden and the warning lamps appear only after the malfunction has begun. The component is located beneath the center cup holder area. There is no visible waterproofing or moisture isolation around the circuit board, despite its location directly under a known liquid exposure zone. There are complexities as to wh this lawsuit is on behalf of the turbo engines, and not the non turbo engines, however the same problems are well documented and widely acknowledged throughout hundreds if not more instances of people online and by Mazda's acknowledgment of the problem through the TSB's. This put me at great safety risk. I was driving north bound right beside semi trucks and busy traffic at speed limits of 70 mph when my engine started smoking. I was able to pull over and get it to a nearby mechanic. They officially diagnosed the vehicle w the cracked cylinder head matching the aforementioned TSBs. Mazda denied my goodwill case, even though it was made without complete information on my situation and car, despite my best attempts to get to the one representing my case with the dealership(I tried to reach him for 2 days with no success). Getting a repair from Mazda dealerships was inconvenient for my location and schedule, but I've always had my vehicle properly serviced. Mazda said this repair would cost near $7500. The lights turning on all at once on the dashboard became more frequent and this time, the check engine light illuminated, indicating a misfiring issue. I took the vehicle to a mechanic shop for a thorough inspection, and they made a few minor adjustments. However, two months later, the same problem recurred. All the lights came on again, and the vehicle misfired once more, accompanied by the check engine light. Upon further investigation, it was discovered that the issue was a head gasket leak. The "shift tower" typically refers to the external mechanism on top of the transmission where the shifter cables connect. Problems with this part, which is a broken internal pin, result in a shifter that moves freely without engaging any gears. The dealer has confirmed catastrophic engine failure and submitted an engine replacement request to Hyundai Motor America. Hyundai Motor America declined the engine replacement request, citing incomplete status on Service Campaign 966 (Knock Sensor Detection System update for connecting rod bearing wear). The owner purchased the vehicle used and was never notified by Hyundai of Service Campaign 966. The owner also was never notified of NHTSA Recall 20V543000 (ABS module fire risk), which remains open and unrepaired on this vehicle, demonstrating a pattern of failed manufacturer notification to the current registered owner. This vehicle is a Class Vehicle in the Hyundai/Kia Theta II engine class action settlement (In re: Hyundai and Kia Engine Litigation), which extended powertrain coverage to a lifetime warranty for connecting rod bearing failure on 2018 Tucson vehicles equipped with the Theta II 2.0L engine. The failure mode experienced is precisely the defect this settlement and Campaign 966 were designed to address.
