My 2021 Ford F-150 with the 10-speed transmission has started shifting very hard and making grinding noises while accelerating. The truck will sometimes hesitate between gears and then slam into gear unexpectedly. I have also experienced sudden hard downshifts while driving at normal speeds. The issue seems to be getting worse over time. The grinding noise and harsh shifting are most noticeable d
... uring acceleration, but the transmission also feels rough during normal driving. At times it feels like the transmission is slipping before catching again. This creates a safety concern because the truck can unexpectedly jerk, hesitate, or downshift hard in traffic and at highway speeds. I have seen many similar complaints online involving the Ford 10R80 transmission, including issues related to harsh shifting and internal transmission problems. Ford has also issued technical service bulletins related to this transmission. No warning lights consistently appeared before the issue started. The problem has happened multiple times and can be reproduced during regular driving. The vehicle has not yet been fully repaired or diagnosed at the time of this report. The transmission and vehicle are available for inspection upon request.Read more

Subject: Safety Complaint – 2022 Cadillac Escalade ESV Transmission Failure After Recall Repair To Whom It May Concern, I am submitting a formal safety complaint regarding my 2022 Cadillac Escalade ESV. VIN: [XXX] My vehicle experienced a serious transmission malfunction where the vehicle would not properly engage in reverse. The “R” gear indicator blinked multiple times, and the vehicl
... e became unsafe and unreliable to operate. Cadillac dealership diagnostics later confirmed issues related to the transmission valve body and/or transmission control components. After researching this issue, I discovered that GM and NHTSA issued Recall No. 26V085000 regarding transmission control valve body wear in certain 2022 Cadillac Escalade vehicles equipped with the 10-speed transmission. The major concern is that my vehicle had already received the recall/software update before this failure occurred. Despite completing the recall remedy, the exact transmission problem still happened. I never received any warning before the failure. I was forced to spend significant time and money dealing with towing, dealership visits, diagnostics, and transportation disruption for my family. This raises serious concerns that the recall software update does not actually resolve the underlying mechanical defect within the transmission valve body. For a luxury SUV of this value and age, this type of major transmission failure is unacceptable and creates a serious safety concern. I request a full investigation into why the recall remedy failed, confirmation of all recall work completed on my VIN, and extended warranty coverage for the transmission and related drivetrain components. About 6 months ago, while driving my kids to school, the transmission aggressively downshifted causing the truck to skid momentarily. This occurred on the highway so the transmission was most likely in 6th gear. The event was so abrupt, it felt like it either switched into Park or 1st gear and felt like we hit something. A wrench icon appeared and the fan was abnormally loud so I pulled over and t
... urned the truck off to see if there was any damage which there wasn't any. The truck wouldn't start so we were stuck on the side of a busy highway during morning rush hour traffic. When it finally started about 2 1/5 hours later, I hooked up a code reader which indicated codes P0706 and P0707. Ever since, this condition occurs after the truck's been driving for about 5-10 mins without exception. The truck seems to stay in the current gear that was engaged at the time that the wrench icon appears. So if the truck is a stop and the issue occurs, it'll be stuck in 1st and run at high rpm. If it happens when the gear is in 5th or 6th and you then come to a stop, it makes pulling out in traffic dangerous due to the lag while getting the truck up to speed. I've been out of work and couldn't spare the $1200 repair cost. I've had to avoid highways and speeds above 30 mph in fear of crashing or damaging the transmission. Even at low speeds, transmissions shifts are hard and abrupt so I'm concerned that there's transmission damage. There's a burning belt smell in the cabin so I'm also concerned that's there's advanced belt wear damage due to the high fan condition caused by this. I have to leave the truck running if I have to go anywhere since it won't restart for approx 2 1/2 hours after I turn the truck off. Other symptoms noticed while driving: the gear indicator below the speedometer glitches. It'll change to other gears on the indicator while not actually changing physical gears. The gear indicators will disappear then reappear randomly.Read more

I was leaving the parking lot at XXX downtown [XXX] around 12:30 a.m. with my passenger when the vehicle out of nowhere started throwing multiple errors and warnings. BMW assistance was not able to provide Tow Service due to the time of the hour and availability so I called Boca Raton police department and they provided with a towing service to the dealership at BMW Coconut Creek where the car has been since. The service at the above dealership is currently inspecting the car and I'm looking to find out the result of their investigation. I am aware of the current recall of the brake booster system however I want to emphasize that "safe mode" degraded breaking was not available does not match what happened in my situation. The Integrated Thermal Management System (ITMS) in my Kia K5 GT (2.5T engine) has experienced a recurring internal electronic and mechanical fault. This is the third failure of this component. I own a 2022 Acura MDX SH-AWD with approximately 85,000 miles. The issue began suddenly while driving around 5 mph when two loud bangs were heard from the rear drivetrain/transfer case area. After the initial incident, the vehicle was driven home, and the problem became consistently noticeable during any acceleration from a stop or low speed. The drivetrain would clunk/slip during acceleration and
... exhibited abnormal engagement behavior. Due to concern about causing further drivetrain damage or a potential safety issue, the vehicle was subsequently towed to an Acura dealer for diagnosis. The Acura dealer confirmed that both the transfer case and transmission require replacement due to “worn teeth on splines.” The repair estimate specifically states: “REPLACE TRANSFER CASE AND TRANSMISSION DUE TO WORN TEETH ON SPLINES.” Estimated repair cost is approximately $11,400. I have photographs and repair documentation related to the failure. This failure created a potential safety concern because the drivetrain was no longer operating normally and produced loud clunking/slipping during acceleration from stops and low speeds. A drivetrain or power transfer failure could increase the risk of loss of propulsion or inability to accelerate normally into traffic. The issue was reproduced and confirmed by the Acura dealer. The vehicle has been regularly maintained, including drivetrain fluid services, through Acura/Honda service facilities. There were no warning lights, dashboard messages, or electronic indications before or after the failure event. The only indication was the loud banging noise from the drivetrain/transfer case area and the resulting abnormal drivetrain behavior during acceleration. The service manager also indicated they had seen several similar cases involving this issue.Read more

My 2024 Mazda CX-90 MHEV has experienced multiple life-threatening sudden engine stalls and a complete loss of motive power while driving in active traffic lanes. The first incident occurred on December 31, 2024, when the vehicle abruptly shut down with an "Engine Malfunction" dashboard alert. The vehicle was towed to an authorized Mazda dealership (Palatine, IL), where it was held for a week, bu
... t technicians stated they could not replicate the issue and returned the car to me as safe. On March 30, 2026, the exact same failure recurred. While driving, the engine suddenly shut off completely at a traffic intersection. After turning the vehicle off for 5 minutes, it was restarted and driven to another authorized Mazda dealership (Crystal Lake, IL) for specialized diagnostic scanning. The second dealership successfully retrieved stored diagnostic trouble code (DTC) P061B, which is directly linked to safety recall 24V-815 (Loss of Drive Power from Software Errors). Crucially, the detailed diagnostic scan revealed that this code was historically stored with a timestamp dating back to December 2024. This proves that the safety defect was active and detectable during the first dealer visit 16 months prior, but was completely missed by the first dealership, leaving my family with an ongoing crash hazard. To address the failure, the dealership replaced the low-pressure fuel pump in the fuel tank and a mass airflow (MAF) sensor. However, Mazda Corporate customer experience has formally stated to me in writing that they are "not in a position to guarantee the concern will not reoccur." Because this safety-critical propulsion failure causes the vehicle to suddenly die in traffic and has a documented history of ineffective recall remediation, it poses an unresolved, severe crash risk. The vehicle is currently in my possession and available for inspection.Read more

* This complaint concerns a catastrophic failure of the transmission in my vehicle. This defect was first reported to a Ford dealer while still within the 5-year/60,000-mile powertrain warranty (Jan 3, 2023, at 58,679 miles; Invoice #XXX). * The condition worsened. Between July 2024 - Jan. 2026, I received 7 digital "Service Engine Soon" warnings via the Ford App. My dealer advised that the vehicl
... e was "OK to drive" as long as the light (which never stayed on) was not flashing. I was told diagnostics could not be made unless the light was active at the dealership. Jan 16, 2026, the light came on; I was able to take the truck to the dealer. A fault code was finally captured, but the dealer reported a two-month service backlog. I could not wait that long. *Feb 2026, I established a clean engine baseline. I spent $833.15 to replace O2 sensors (EJ’s Automotive) to rule out engine−related performance issues. During this service (at 103,431 miles), an inspection confirmed metallic clutch material contamination in the transmission fluid only 10,248 miles after a 2nd professional flush. This failure occurred despite a total maintenance investment of $11,533.80 across 21 service visits. * This failure resulted in a dangerous Loss of Motive Power, creating a collision risk. The vehicle is currently grounded and completely inoperable. The dealership verbally designated the vehicle as VOR due to the confirmed diagnosis, only to contradict that status by claiming it was "driveable" after I declined a $15,000 quote for a teardown to diagnose the issue. * Since the truck can no longer tow, 2 third-party transports for a horse and a trailer relocation are necessary. The failure has also resulted in a total parasitic battery drain. The tailgate latch and door locks are inoperable, suggesting the transmission fault is interfering with module communication. This is consistent with the root cause Ford has acknowledged in TSBs 24-2101 and 25-2126 involving the CDF clutch drum sleeve. INFORMATION REDACTED PURSUANT TO THE FREEDOM OFRead more

My 2016 Hyundai Sonata is experiencing severe oil consumption due to a known Hyundai engine defect. The engine burns oil rapidly and loses more than a quart between oil changes. Tesla just sold me a car with defective front half shafts axles. This interferes with safety features like, lane departure assistance and blind spot steering wheel feedback, Confusing th
... e driver and creating a distracting scenario where an accident can happen. Upon bringing this to the attention of the service center in Fort Myers Florida, the service center manager took a drive test with me, and was able to replicate the issue, but immediately started dismissing it as "normal" and within Tesla specs. This is becoming a regular practice at that service center, where they deem warrantable issues as normal and within (their tolerance). I insisted that this issue is not to be taken likely by Tesla, because it is something that involves a safety hazard. Upon further research, I was able to find that this issue is happening to many Teslas model S and X, where the front axles bend due to the weight and high torque of the vehicles. The issue falls completely under warranty and the half shafts should be replaced, but apparently Tesla has instructed their service centers to dismiss and disregard the symptoms, and just tell the customers that no warranty replacement will occur, because it is not necessary, we just have to drive the cars like this, until the car completely fails, or an accident happens because one of the bent axle shafts breaks on a highway during normal drive. I recording this report as evidence that the issue has been reported to the highest vehicle safety authorities, and I expect a corrective action so Tesla can fix my car under the existing unexpired warranty.Read more

Was able to coast to a nearby exit and have the car taken to a dealer for service. Very unnerving at 75MPH and a significant safety hazard to me and other drivers.

While attempting a slow right turn from a complete stop at an intersection, I heard a loud thunk sound followed by an immediate loss of power steering response. The steering wheel spun freely with little resistance while the front tires did not respond proportionally. I was able to safely pull over. Upon restarting the vehicle it produced an abnormal wheezing sound suggesting additional mechanical
... stress. At the time of the failure the vehicle had approximately 40,000 miles, which I believe represents a premature failure for a safety-critical steering component. The vehicle was brought to a Hyundai dealership for inspection. The dealer pulled diagnostic fault codes, found none, performed a 20-minute test drive, and returned the vehicle stating they could not find anything wrong. No physical inspection of the steering components was performed. The failure condition — a slow full-lock turn from a complete stop — was never specifically replicated during the dealer's test drive. The problem has not been confirmed or repaired by the dealer. The vehicle is available for inspection. My safety and the safety of other drivers was put at risk by the sudden and unexpected loss of steering control. No warning lights appeared prior to or during the failure.Read more

The eRAD (Electric Rear Axle Drive) on my 2016 Volvo XC90 T8 PHEV failed completely. The warning light appeared approximately 11/3/2025, within one week of purchasing the vehicle at 88,000 miles. The failure was confirmed by a Volvo dealership. The eRAD is the component that powers the rear electric motor and provides AWD and hybrid function — its failure results in complete loss of rear wheel d
... rive and hybrid system operation. Safety risk includes unexpected loss of AWD traction control in adverse weather conditions. This is a known systemic defect acknowledged in Volvo’s own Technical Journals (TJ33358). I contacted Volvo Cars USA customer service to invoke the SULEV emissions warranty and they were unable to locate my claim or explain the warranty coverage. Volvo has not addressed this through a recall despite this being a widespread known failure mode affecting XC90 T8 models. The vehicle is available for inspection.Read more

The torque converter(automatic transmission) began failing causing the vehicle to stall when at a traffic light in heavy city traffic allowing for the possibility of being struck by other vehicles because of a restart delay. The only way to bypass the issue we learned was to put the vehicle in neutral when stopping. This reduced the stalling incidents but created other issues in heavy stop and go
... traffic. The vehicle was inspected by a transmission shop and they confirmed that the transmission was failing and the torque converter was causing the issue. There are no warning lights for transmission failure in these vehicles. In fact you can’t even check fluid levels on these transmissions. Only technicians can. A transmission failure on a 7 year old vehicle under normal wear and tear (113,000 miles) is unacceptable and we had to spend 5000.00 for a new transmission in order to remedy the obvious safety issues. Before the repair, my wife managed the stalling issue at 50+ years old by using neutral in between braking so the vehicle wouldn’t stall but imagine a teenager or elderly person dealing with that. I do have receipts for the repair. There was one interstate fender bender. We could not avoid because of people looking at accident on the side of the road. My wife was unable to shift the vehicle into neutral to keep it from installing in traffic and we ended up hitting somebody from behind. Very minor damage on the caravan to the front end.( I have a pic) the other vehicle was a large truck and received zero damage and was OK letting it go. State trooper at the accident people were watching said there were no harm done. Nobody at fault here , both parties agreeable , no injuries , so we elected not to turn in a police report.. This is just a credit card receipt for part of the repair total and we paid cash/check for remainder. Could look up if necessary.Read more

Despite the engine repair, the contact stated that an abnormal, rattling sound would emit from the vehicle upon start of the engine. In addition, the contact stated that the vehicle would violently jerk upon depression of the accelerator pedal while at a complete stop. The failure started intermittently; however, the contact stated that overtime the failure became more persistent. The vehicle was taken to the dealer where they claimed that the transmission was four quarts low on transmission fluid; they then informed the contact that they could not guarantee that remedy would repair the failure. The contact stated that failure improved; however, she would still experience a slight hesitation upon depression of the accelerator pedal. The manufacturer was notified of the failure but offered no assistance. The vehicle was not repaired. The failure mileage was approximately 39,000.Read more

The electronic parking brake on my 2025 Toyota Grand Highlander intermittently fails to auto-release when shifting from Park to Reverse or Drive. Curiously, the system does respond to the gear shifter when returning to Park, as the brake will then engage or release as expected once cycled. This indicates a failure specifically in the transition out of the Park position. This intermittent delay or
... refusal to release prevents the vehicle from moving when expected, creating a potential safety hazard in traffic or during time-sensitive maneuvers. No dashboard warnings are present, and the issue occurs even when the driver's seat belt is buckled and the foot brake is fully depressed.Read more

Vehicle primarily failed while parked at owner residence [XXX] and remained inoperable for approximately three weeks prior to tow to Cadillac dealership. The ignition key became trapped and could not be removed. A manual override release beneath the driver-side steering column was required to free the key — confirming the lock was held electronically, not mechanically jammed. Fault confirmed by
... authorized Cadillac dealer: intermittent electrical fault within the shifter assembly lock mechanism. A similar intermittent concern was previously documented on this vehicle in 2011 (logged as "Customer Concern Not Duplicated"), establishing a recurring pattern. GM Technical Service Bulletin 07-07-30-032D (2006–2011 Cadillac CTS, automatic transmission) explicitly documents this exact condition: key sticking, Shift to Park DIC message, shifter binding, and battery drain. OEM replacement part is now discontinued. Manufacturer declined goodwill or recall coverage. Dealer refuses repair unless owner assumes full financial responsibility for a used part with no warranty and no guarantee against recurrence. Safety concern: ignition/shift interlock failure causing vehicle immobilization, key entrapment, and loss of transportation for an elderly owner dependent on the vehicle for medical care. Defect evades shop diagnosis due to intermittency. No safe OEM remedy exists. Prior recall: NHTSA 14V394000 / GM Campaign 14172 addressed same interlock system. Current failure represents recurrence beyond that recall's scope. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more
