While driving my 2021 Toyota Sienna Hybrid, the vehicle suddenly displayed a low oil pressure warning, immediately lost power, and entered limp mode. I was able to pull over safely, but the engine then suffered a catastrophic failure — a connecting rod blew through the engine block, leaving large metal fragments inside the engine compartment. I have performed regular oil changes and routine maintenance according to Toyota’s schedule. Safety Concern: This failure happened suddenly and without warning. If it had occurred at highway speeds, it could have caused a serious crash due to loss of power or debris from the engine. This presents a significant safety hazard for drivers and passengers. Toyota has issued TSB T-SB-0104-21 for the 2021 Sienna Hybrid’s A25A-FXS engine addressing “MIL ON with Low Oil Pressure” conditions and DTCs P05202A/P052477 with milky or discolored oil. This suggests Toyota is aware of oil pressure and lubrication issues on this engine family. My catastrophic failure appears consistent with a loss of oil pressure or lubrication. I am filing this complaint to alert NHTSA of a possible defect or safety issue affecting 2021 Toyota Sienna Hybrid engines that can result in sudden catastrophic engine failure.Read more

Honda has a 2 TSBs for all Pilots on all VINs having a transmission juddering issue. They are making us pay out of pocket for a known issue on all 2017 VINs. I’m reporting a safety issue with my 2019 Toyota RAV4 that has recently started and is getting worse. The problem seems related to the start-stop system or something in the powertrain. Sometimes there’s a hesitation, and other times it lurches forward u
... nexpectedly when I lightly press the gas pedal, like you normally would. It feels like a jolt, almost like whiplash. It’s especially dangerous in roundabouts or tight traffic situations because I can’t predict how the car will respond. I’m worried I could accidentally hit another car, or get rear-ended myself because the car doesn’t move smoothly or reliably when restarting from a stop. Even when I turn off the start-stop system, the issue still happens. It feels like a hesitation followed by a sudden surge. The RPMs don’t change, but the car still lurches. Yesterday, for the first time, I noticed the car slightly accelerated on its own after stopping, which was very concerning. I found out through Toyota’s official website that there is a technical service bulletin about this issue that came out before we owned the vehicle. But because it’s not a full recall, the dealership said they wouldn’t fix it unless we paid the $80 diagnostic fee. We’re now just out of warranty. We’re making $580 monthly payments on a car that I don’t currently feel is safe to drive. There are no warning lights or messages on the dash. I’ll be taking it to Le Mieux Toyota in Ashwaubenon, Wisconsin, in two weeks to have it inspected, but I wanted to report this because I believe it’s a safety issue that should be looked into. I have two small kids and I’m genuinely worried about getting into an accident, especially when merging onto highways or driving in busy intersections. I was told by the dealer that my 2019 Outback had a cam carrier oil leak. At the time, my car had 50,700 miles on it. My drivetrain warrantee was for 5 years or 60,000 miles. ... ot addressed. There are many articles on it. This has been a problem because Subaru uses a Boxter engine and this engine has historically always had problems with oil leaks that cannot be fixed. It was also disclosed that there was a chance that oil leaking onto the manifold or exhaust could cause a fire in the car which caused me alarm. My wife and I are seniors, and we cannot afford to get stuck in the car and perhaps with car on fire. The dealer could not do anything, so I called Subaru directly and spoke to their consumer advocate dept. (Incidentally, I had two previous repairs to the electrical system and computer system fixed by Subaru at no cost even though we were past warrantee. Subaru decided to extend the warrantee on the two previous issues.) After several calls Subaru told me that they would give me a $1,500 credit towards the $4,500.00 for the repair, which I said was not nearly enough and did not solve the problems. I explained that we are apprehensive about the car because the leak could happen again and there is a chance of fire. Subaru told me that the chance of fire is low and at an acceptable level. Any chance of fire is NOT acceptable to us. I was going to sell the car outright, but the oil leak would cause a major devaluation of the car, so I had it fixed and repaired last week. My ongoing concern is that this oil leak is a recurring hazard that can cause a fire. I believe this problem should have been handled through a no cost recall. I had to tow the vehicle to a Kia dealer where it took 17 days to repair with a new power module. On [XXX], (after the vehicle was repaired), I was driving the vehicle when the POWER alert flashed on the dashboard. I did not lose total power yet, but the literature advises that I take the vehicle into the dealership asap to have them look into the Power warning light coming on. I have an appointment on September 18, 2025. I have reported my issue to Kia customer service, and the dealership is aware of what is going on. I believe what is happening, (the abrupt loss of total power while driving) may be a widespread defect affecting the Carnival hybrid system in 2025 and 2026 model years. This is a serious safety issue that need corrected before someone is killed or seriously injured. Total loss of power at 40 mph or more cannot be permitted to happen. There should be some sort of safety bypass whereby the gas engine totally engages to the exclusion of the hybrid system, so total power is NOT lost at High speeds. On April 5, 2023, I purchased a used 2021 Nissan Armada SL with 74,429 miles on it. On August 29, 2025, I took the vehicle in for front & rear differential and transmission fluid services. ... continued. On September 15, 2025, I took the vehicle into Nissan for a further look and was told I needed to replace the entire transmission! After speaking with Nissan's Consumer Affairs, I was told there was nothing that can be done for this issue. The vehicle is less than 5 years old. I have owned the vehicle less than 3 of those years and drove it less than 40,000 miles and now Nissan wants me to pay $11,000 for a new transmission. There has been, no check engine or other warning light indicators alerting this issue or concern. There is no way this can be a normal for a powerhouse company who's reputable as built to last vehicles that their transmission is now only built to last barely past 100,000 miles. This has to be a factory defect in this vehicle. I am presently seeking legal actions concerning this matter. This is a very costly and risky issue that could have left me stranded and possibly caused a serious accident. There needs to be a factory recall for this issue and concern.Read more

The contact stated that the failure persisted after the dealer had serviced the vehicle. The vehicle was later taken to an independent mechanic, where it was diagnosed that the transmission clutch had failed. The vehicle was not repaired. Additionally, the contact stated that while driving and depressing the accelerator pedal, the vehicle failed to shift into the intended gear, and the vehicle was jerking, and the warning light was illuminated. At 99500 the transmission went Kaputt! Repair man says metal shavings obstructed flow of fluid, through the filter, into the transmission, not allowing for running the car forward or backward. the transmission for this truck has on several occasions shifted very hard when accelerating, and on a couple occasions has down shifted without warning and without reason. the down shifting incidents were unexpected and could have caused an accident if I was in traffic.

The check oil light indicated that the oil level had become too low. The Volswagen dealership service technician identiifed the cause to be from the exhaust valve seals wearing out. The cylinder head needs to be replaced per their recommendation. No warning apart from the low oil indicator.

The condition produces clanging noises during normal driving, and an independent licensed mechanic has refused to work on the vehicle because the suspension integrity is unsafe. When I returned the vehicle to the dealer, I observed additional brackets being attached under the rear subframe area, suggesting reinforcement of compromised structural points. This confirms that the corrosion directly affects suspension geometry, alignment, and stability. This corrosion is not cosmetic. It is advanced structural deterioration affecting the mounting points for trailing arms and lateral links. These components are critical to safe turning, stability, and control. The corrosion compromises the ability of the suspension to keep the wheels aligned, creating serious risks of loss of control or wheel separation. What makes this more alarming is that the vehicle passed a New York State safety inspection on 07/13/2025, yet within days a mechanic refused to perform unrelated service because of the undercarriage condition.
