Rear differential failed around 97,000 miles. We were three months past our certified pre-owned warranty. Total cost to repair was around $4400 after a discount on parts and labor through our Costco membership. Contacted Mazda to see if they’d be willing to provide any assistance to cover the repair, they declined. Service advisor told us that it was a catastrophic failure that we did not cause.
... We purchased this car in 2020 and have always had it serviced at the dealership and at no point did they bring it up as it being an issue. The rear differential should not fail.Read more

Good evening, I wanted to bring to your attention an ongoing issue I've experienced with two 2019 Transit Connect vehicles that I purchased together. Both vehicles have encountered a transmission problem when shifting into reverse; initially, they would not move, but after shifting to park and then back to reverse, they would engage. I visited the dealership where I bought both vehicles, and the
... y inspected them, indicating that they found no problems. However, both vehicles began showing transmission issues around the 50,000 to 58,000-mile mark. I was able to replace the transmissions under the manufacturer’s warranty. Unfortunately, after approximately 30,000 miles—about a year and a half later—I encountered the same issue again. Once more, the vehicles struggled to engage in reverse. When I returned to the dealership to report this recurring issue, I learned that my warranty had expired, and I would need to replace the transmission again. I would like to formally express my concerns regarding this situation. It seems that there may be a common issue with the transmission in the 2019 model, and I believe it's important for this to be addressed. Thank you for your attention to this matter.Read more

I lose power steering randomly. When this occurs I get a service rear diff message on my dash, the fan motor cuts out, and gauges like the speedometer and tachometer go to zero. I took it to a dealer because I saw there was a recall for the exact problem I'm having with power steering, but I guess it's VIN specific and my car is not included in the recall. They also told me they couldn't get it to
... reproduce the symptoms I was talking about but after I left 10 minutes later it did it in a Walmart parking lot. I got it on video. This has caused one almost serious accident and multiple minor ones, and I can't afford the expensive dealer diagnostic fee and repair. I don't feel safe driving with it. I'm confused why I have the same make model and year of the recall but mine's not included. I called GM and the lady I talked to just kept repeating how my van wasn't included in the VIN specific group, and offered no help other than to go to a dealer and pay them to fix the same problem that is covered by other cars of the same year, make, and model. I asked her if it was possible that maybe their VIN groups should be expanded, especially because it's an 8 year old car and the recall just happened like 6 months ago, so maybe they didnt have all the data or info when they made the group of effected vehicles, and she hung up on me. Please help.Read more

After Ford Recall 24E13 (Powertrain Control Module reprogramming) was completed on my 2019 Ford Expedition Max, the vehicle immediately developed transmission problems. It began hesitating to go into reverse and then produced DTC P0736 (reverse gear ratio incorrect). Within days, it failed to shift properly and was returned by the dealership. The issue occurred only after the recall repair, indic
... ating a possible calibration or programming fault that affected the powertrain. Ford dealership staff acknowledged metal in the fluid but gave inconsistent explanations and then claimed the transmission had to be replaced. The vehicle also displayed a “Powertrain System Fault” warning. No such problems existed before the recall. This raises a safety concern because transmission malfunction after a recall could cause loss of control or failure to move the vehicle. The vehicle was returned to us on October 7, 2025; it remains unsafe to drive. Ford Corporate has been notified (Case CXH-05773791-T4G0R8), but no resolution has been provided. I am requesting NHTSA review this as a potential post-recall powertrain safety failure related to Recall 24E13.Read more

I purchased a new 2024 Audi Q4 e-tron. The first malfunction occurred in January 2025 when a dashboard warning appeared stating the vehicle was “unable to read traffic or road signs.” The dealer confirmed a front-camera sensor failure and replaced the camera under Repair Order #193312 (1/17/25 – 1/29/25) at approximately 397 miles. In February 2025, the vehicle displayed repeated “drive s
... ystem malfunction” warnings and would not start reliably. The dealer determined the issue was linked to the 12-volt battery and replaced it under RO #194457 (2/24/25 – 3/14/25, 866 miles). Soon after, persistent drive-system and electrical faults continued (RO #195096 and #195241 from 3/17 – 3/27 at 866–1,081 miles) but no repair was completed. On 4/10/25, an “AWD malfunction” occurred, and the vehicle was towed. Under RO #195846 (4/10/25 – 4/25/25, 1,594 miles), the front electric motor/drive unit was replaced. The vehicle remained out of service for extended periods. I was provided an EV loaner during the initial repair in January and gasoline loaner vehicles thereafter. In June 2025 (RO #198283 at 3,161 miles), a factory recall for the onboard charger was performed. The warning “Pedestrians cannot hear motor sound” appeared both before and after the motor replacement, suggesting a persistent fault in the Acoustic Vehicle Alerting System (AVAS). These repeated electrical, sensor, and propulsion failures — all within 3,000 miles of use — raise concerns about the reliability and safety of the ADAS camera, drive unit, and pedestrian-alert system. Power loss and a nonfunctional pedestrian-warning feature could increase risk to the driver and others. The manufacturer has been notified, and both dealer and manufacturer-updated repair records — showing mileage inconsistencies between versions — are available for review upon request.Read more

Upon normal highway driving a couple of months ago, began to notice shifting that was not smooth and rpm "surges" on hills despite constant throttle. I was driving and the van suddenly lost all propulsion. Brakes and steering and dash lights functioned, but there was a red turtle icon and error message about a system failure. The accelerator pedal did nothing at all. I was able to coast off the side of the road to a place to pull over, but that could have been a terrible accident if I lost power like that on I-95 or any other major highway.
... My husband came and we switch cars (kids, car seats, etc) and he was able to get the van to start after half an hour. It had check engine lights on for things, but the dealer said to bring it in because it was drivable again. Despite brining it in as told, with check engine lights on that they saw, they said they could not reproduce it and couldn't find codes for the check engine light (even though I pulled them from my own OBD2 diagnostic plug tool and took photos of the codes) and eventually said I have to take the car back, despite no cause or resolution. These cars have known faulty battery issues and a large number have been replaced already, but Chrysler isn't authorizing dealers to do that unless they can reliably reproduce the failure. Very frustrating to see Chrysler not go the route of other manufacturers who sourced their batteries from the same plant and ended up proactively replacing them for consumers. While Chrysler's many recalls for the hybrid battery system software have probably made it detect faults earlier to try to prevent fires, they are still known to happen, but now in addition to a fire risk, we also have a car that has a much higher chance of just losing power while driving due to over sensitive battery monitoring, or a plain old faulty battery that just isn't faulty 'enough' for Chrysler to authorize a replacement. So frustrating and dangerous.Read more

I am filing a complaint with the NHTSA regarding persistent transmission slipping in my 2020 Toyota Highlander LE equipped with the UA80-E 8-speed automatic transmission. Since reaching approximately 55,000 miles, I have experienced noticeable slipping, characterized by delayed acceleration, erratic shifting, and occasional lurching, which align with documented defects in the UA80 platform as outl
... ined in over 430 NHTSA complaints and Toyota’s Technical Service Bulletins (e.g., T-SB-0160-18 and T-SB-0008-21). These issues stem from a known manufacturing flaw involving a washer tab failing to secure the counter drive gear’s retaining nut, leading to gear movement, internal wear, and symptoms like mine. This defect, prompting Toyota’s Customer Support Program ZJC for 2017–2018 models and reported in later years like 2020, compromises vehicle safety and reliability, as slipping can cause unpredictable handling. I urge the NHTSA to investigate further and hold Toyota accountable for addressing this widespread issue across affected models.Read more

Summary of Issue: I am the owner of a Toyota Mirai that suffered a sudden loss of power due to contaminated hydrogen fuel. The incident rendered the vehicle undrivable and raised serious safety concerns, as loss of propulsion in a fuel-cell vehicle can occur without warning while in motion. The vehicle was taken to Toyota of [XXX] for diagnosis and repair, where Toyota confirmed the presence of c
... ontaminated fuel. Toyota’s Stance: According to Toyota, the contamination issue was addressed by flushing the contaminated hydrogen from the system and refueling the vehicle. Toyota has stated that this process has restored power and that the company does not accept responsibility for any damage caused by the contamination. They have not disclosed any data on the type or level of contamination found, nor have they provided a written service report detailing inspection results or long-term corrective actions. Owner Concern: Independent research and fuel-cell engineering references indicate that contaminants such as sulfur compounds, carbon monoxide, or heavy metals can permanently degrade or “poison” the fuel-cell stack. Flushing alone cannot reverse such chemical or structural damage. As a result, there is a risk of future performance loss or total stack failure. Without transparency from Toyota regarding contaminant analysis or verification of system integrity, I cannot consider this vehicle safe or reliable to drive. Request for Resolution: I am requesting that Toyota: Provide complete written documentation of the contaminant analysis and testing performed on my vehicle. Disclose the long-term plan for ensuring the fuel-cell stack and related components have not been compromised. Offer either a full buyback/repurchase under applicable consumer protection laws or a complete fuel-cell stack replacement verified by engineering inspection. Until Toyota provides a documented, technically valid repair or an official buyback offer, this issue remains unresolved INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACTRead more

On September 1, 2025, at approximately 11:30 PM, I was driving my 2016 Ford Edge (VIN: [XXX] ) Southbound on Red Arrow Highway near [XXX] , returning home from work. Without any warning lights or prior symptoms, the vehicle suddenly lost all ability to provide power, even though the engine continued running. The transmission stopped engaging gears, and the vehicle began making grinding noises whil
... e rocking back and forth violently. The posted speed limit on this section of road is 45 MPH, and there were several vehicles traveling behind me. The sudden loss of propulsion created an immediate traffic hazard, forcing me to pull over onto a very narrow shoulder, putting myself and others at risk. After coming to a stop, the vehicle could not shift into drive or reverse and had to be towed. The failure was diagnosed by Pro Transmission in Benton Harbor, MI, which confirmed a complete transmission failure. The odometer reading at the time was approximately 154,000 miles. Ford Motor Company has acknowledged transmission issues in this model and year through a Customer Satisfaction Program, but coverage is limited to vehicles under 10 years old and below a specific mileage threshold. My vehicle qualifies by age but exceeds the mileage limit, even though the transmission failure occurred suddenly and prematurely. This vehicle has also been subject to multiple recalls, including one for engine coolant leaks and two new ones (for the backup camera and brake line leaks). These repeated failures raise broader safety and reliability concerns. The sudden transmission failure at well driving posed a serious safety hazard and nearly caused an accident. I am requesting that NHTSA investigate Ford’s known transmission defect in the 2016 Ford Edge and consider whether the existing Customer Satisfaction Program should be expanded to include higher-mileage vehicles experiencing the same issue. I purchased a 2018 Chevy Traverse with 6 miles on it. I keep my vehicle in mint condition and maintenance as needed and required. Recently in the past few months my car has been bucking when shifting gear engages in an automatic transmission. I thought it was a lot but it’s my main vehicle and needed it. The van has 92k miles and the transmission should not go out that soon. I’m filing a complaint because I believe the transmission should last longer than it did.Read more

Receiving a persistent “Shift to Park” message, after the car has been placed in Park. This causes an inability to turn off my engine completely and is draining my battery. As a temp fix, I've had to wiggle the shifter many times, turn the car on and off, place the car from park into drive many times to get the car to realize it's in Park. Scheduled with the dealership and referenced the T
... SB that was issued for this specific safety issue (in writing). Brought the car in for the repair and the technician and intake person were both aware of this major problem and have seen other Chevy's with the same issue. Repair is $809.00 for a shifter assembly. I am writing regarding my 2021 Jeep Gladiator Mojave, which recently suffered engine damage due to a faulty head gasket. I understand the vehicle is approximately 5,000 miles over the 60,000-mile warranty; however, I believe this issue should not be my financial responsibility given the circumstances. At around 60,000 miles, I brought my Jeep in for a full service and inspection to ensure it was i
... n top condition. I was informed at that time that everything looked fine and that there were no issues to address. Since then, I’ve continued to maintain the vehicle properly and drive it with care. The sudden head gasket failure, which ultimately led to engine damage, came as a complete surprise. This situation has been extremely difficult — not only because of the unexpected cost, but also because this Jeep was meant to be my long-term vehicle. My family and I have been loyal Mopar customers for years, and I’ve always trusted in the quality and reliability of your vehicles and service network. Given that I sought professional service and maintenance through authorized channels, and that no issues were identified at the time, I kindly request that Mopar consider a goodwill repair or assistance. I don’t believe it’s fair to be held financially responsible for a mechanical failure that likely existed but went undetected during a recent inspection. I’m grateful that nothing happened to me on the road, as I was unaware of the developing problem under the hood. I truly hope Mopar can stand behind its reputation for integrity, safety, and customer care by helping me through this situation. Thank you for your time, understanding, and continued support. I look forward to hearing from you soon.Read more

As I am accelerating once the car reaches 30-60 mph there is a very distant shuddering that occurs that cause the engine to lose some power and turns the check engine light on. This appears to be a common problem as I have found many complaints on forums describing very similar symptoms.

This vehicle has less than 3,000 miles on it and has been in the shop at least three times for a recurring issue with the transmission "lunging" and "clunking hard upon down shift and upshift (automatic). The electrical system and computer are confused and cause the vehicle to literally stop and not engage in gear while driving! Today, the truck was in DRIVE awhile I was sitting at a light in he
... avy traffic. When I hit the gas at light change, the truck revved up but did not go! I was almost hit from behind as I had no brake lights indicated. Additionally, I kept getting a flashing gear selector indicator knob saying to put the TRUCK IN PARK AND SELECT THE DRIVE OPTION. I did but it still wouldn't go. I then got a software update light on the screen and the CHECK ENGINE LIGHT came on. I finally had to literally wave people around me in traffic as I was stalled out. I turned the truck off, restarted and it moved forward BUT at a high RPM. I have parked the truck but the dealer service is full up until late in the week and RAM CARE representatives were foreign and did not speak English well (I could not clearly understand them in my emergency call). Dealer is unwilling to act decisively to a life or death situation (opinion mine). SO we need to ensure we don't get killed or injured (God forbid) and IF there is a serious incident with this vehicle, there is documentation to attest it has been "serviced" several times for the electrical system fluctuating, the transmission dropping, and the engine not engaging the power train while driving. Attaching a PHOTO of the CURRENT Dash Lights (Warnings on the Left) and check engine light being on AFTER I got the vehicle re-running.Read more
