PTU unit on my Jeep Cherokee failed at 70 miles per hour on the highway. The car is located on a Jeep dealer lot and is available for inspection. Traveling at high speeds I suddenly lost and transfer of power from the motor to the wheels and was unable to keep speed in traffic. The problem has been noted on Jeep models of prior production with this exact problem per consumer reports. The car went
... through a full Jeep inspections and found that the PTU was the culprit of the issue. When the PTU failed it displayed a message stating “Service 4WD”. This did not occur until after the incident occurred. Additionally when the issue occurred the car turned itself into snow/ice mode. I was driving in the standard drive mode as there was no snow or ice. I had driven for over an hour prior to this failure in the car. It was mid morning when the failure happened.Read more

While driving through the middle of a busy intersection, my 2013 Audi RS5 suddenly lost all power and was unable to engage any gears. The car came to a stop in active cross-traffic with vehicles approaching from multiple directions. I could not restart or move the car under its own power and had to push it to safety with the help of a passerby. Component that failed: Transmission / Power Train –
... specifically the DL501 S-Tronic Mechatronic Unit (Transmission Control Module). The part is still available for inspection at the dealership. Safety risk: This failure caused a complete loss of propulsion in active traffic, putting both myself and other drivers at risk of a collision. Dealer confirmation: The issue has been inspected and confirmed by an Audi dealership (Audi North Houston). The diagnostic code was P060A00 – “Internal Control Module Processor Performance.” Manufacturer documentation: Audi has previously issued Technical Service Bulletins 35-13-15 and 37-13-17, which identify this same defect as an internal circuit board failure and delamination in the DL501 mechatronic unit. Manufacturer response: Audi of America declined goodwill assistance despite acknowledging a previous case and documentation of this known failure mode. Warning or prior symptoms: A “Gearbox Malfunction – You may continue driving” warning briefly appeared moments before the car lost all power. After stopping, the vehicle could not be restarted or driven, leaving it immobilized in the intersection. Availability for inspection: The failed components and diagnostic report remain available through the Audi dealership that inspected the vehicle.Read more

The vehicle experienced a lifter and valve train failure within the warranty period, as addressed in GM TSM MC-10209953. This failure can cause sudden engine noise, loss of power, and potential engine seizure while driving, creating a serious safety hazard for the driver, passengers, and others on the road. The component failure is reproducible and has been confirmed by a repair shop; it is availa
... ble for inspection upon request. An engine rebuild was completed just after the warranty expired, and the manufacturer has denied warranty coverage and refused to address the issue. Prior to failure, no warning lights or messages indicated a problem, meaning drivers have little to no warning before a potentially catastrophic malfunction. Because this is a documented issue affecting multiple vehicles and the manufacturer is refusing corrective action, this defect presents a clear risk to public safety. I urge NHTSA to investigate this matter, determine whether a safety recall is warranted, and require the manufacturer to address this known defect to prevent accidents and injuries.Read more

Ford Explorer Recall – Ongoing Safety Hazard In July 2025, Ford issued a recall notice for my vehicle. Shortly after, the check engine light came on. Starting October 15, 2025, my vehicle began failing to start after refueling—making noise and taking multiple attempts to start. The same issue occurred repeatedly on October 23 and October 30, worsening each time. On November 3, we brough
... t the car to a Ford dealership. They confirmed there is no available fix for the recall and said there’s “nothing they can do.” I explained this was a safety issue since I drive my son with a disability to therapy daily. They told me I would have to pay out of pocket for repairs even though the problem is linked to the recall. On November 8, while driving in Arizona, my vehicle again failed to start after refueling. Later that day, it lost power on the road, the speed dropped suddenly, and it would not accelerate. The same stalling and jerking occurred again on November 10 while my husband was driving. We contacted Ford Corporate and NHTSA, but both said nothing could be done because the recall label says the car is “still drivable.” This is inaccurate and dangerous. The car stalls and loses acceleration randomly, creating a serious risk of an accident. On November 21, the car again failed to start after refueling. This has happened multiple times each week, and Ford continues to delay a fix. My vehicle is unsafe to drive, and this unresolved recall poses an ongoing safety hazard to my family and others on the road.Read more

There were not warning lamps or anything at the time of my transmission quitting. Truck started to run rough and shifting hard this summer but without warning lamps I didn’t take it anywhere. While driving my Honda HR-V under normal conditions around noon on Saturday on NC Highway 54, the vehicle suddenly illuminated multiple dashboard lights and began to decelerate despite pressing the accelerator. It had been serviced at Autopark Honda Service Center on September 19, 2025, including a transmission service, and nothing abnormal (such as debris or metal) was noted or communicated to me. After the incident and I had my car towed to Autopark Honda, I was informed that the transmission had failed. I later learned of Honda Service Bulletin #21-047 (April 27, 2021) concerning premature CVT belt deterioration in 2016–2020 HR-V models. Despite this known issue, Honda has refused responsibility because my vehicle is about 10 months beyond the 7-year warranty extension. This sudden failure caused complete loss of power, steering, braking, and hazard functions, representing a serious safety risk that warrants investigation.Read more

On August 30, 2025, while driving on the highway, my 2019 Nissan Altima displayed a “CVT (AT) Hot / Power Reduced” warning. Power was limited, but the engine did not shut off. I immediately scheduled service at Paul Miller Nissan in Fairfield, CT. At the September 2, 2025 visit, the repair order stated: “Customer states CVT (AT) Hot Power Reduced display in dash. All past codes stored; clear
... ed all past codes stored.” No complete diagnostic test, CVT pressure test, or fluid inspection was performed. The dealership did not preserve the freeze-frame data from the TCM. I was told that no current fault codes were present and that the vehicle was safe to operate. Relying on the dealership’s professional assurance, I continued normal use of the vehicle. About a couple of weeks later, the vehicle began to jerk and lose smooth acceleration intermittently, worsening by the sixth week. I promptly returned for service. On October 21–22, 2025, the same dealership confirmed transmission codes P0776 and P17F0, diagnosing a CVT failure and recommending full replacement. When questioned about prior diagnostic data, the service manager stated that all data from the September visit had been cleared and that the freeze-frame information was deleted and no longer available. This handling contradicts Nissan’s standard CVT diagnostic protocol, which requires documenting stored codes, inspecting CVT fluid condition, and recording internal temperatures before clearing data. Nissan Consumer Affairs later stated their review relied solely on the dealership’s findings and that missing diagnostic data would not be retrieved. Since the confirmed failure, I have been forced to rent a vehicle at my own expense, exceeding $1000 to date. The vehicle’s reduced power and transmission failure created a serious safety risk, especially as I transport my children in this vehicle. The dealership’s erasure of diagnostic evidence prevented accurate evaluation and timely repair. The issue remains unresolved.Read more

Infiniti has acknowledged that certain vehicles equipped with this engine may experience bearing failures caused by a manufacturing defect affecting the main, A-, C-, and L-link bearings. These defects can result in engine damage or complete engine failure, as outlined in Infiniti’s recall notice. In September 2025, I took my vehicle to an authorized Infiniti service center after receiving notic

However, on October 16, 2025, the Service Engine Light illuminated unexpectedly. I immediately brought the vehicle to the nearest certified repair facility the following day (October 17, 2025). Diagnostic testing determined that the engine had suffered internal bearing failure, consistent with the known recall condition. The vehicle required a complete engine replacement, which I had to proceed with at significant personal expense, covering both parts and labor. At no time prior to this incident did my vehicle display any warning indicators or abnormal performance. The sudden nature of the engine failure created a serious safety concern and considerable financial hardship. I am therefore formally requesting reimbursement or assistance with the full cost of the engine replacement and associated labor. I have attached supporting documentation, including repair invoices, diagnostic reports, and proof of payment, for your review. Given that the failure directly relates to the known defect identified in the recall, I believe reimbursement is both appropriate and warranted.Read more

The slave cylinder on this car is made out of plastic and is located inside of the transmission, when the slave cylinder fails, it leaks brake fluid all over causing the car to lock into gear and not be able to shif
... t which is a major safety risk when there is no signs of failure. My car locked up in the middle of a busy highway and had to be pushed off the side of the road. Not only is this a safety risk, but since the part is located inside of the transmission, which is not normal, it leaks brake fluid all over the clutch and flywheel, which will all need replacement. The dealership quotes between 4-5 thousand dollars to repair, and is not covered under warranty. Toyota knows that this part is a defective part because the new models received an updated slave cylinder made out of metal, rather than plastic, but no cars have been recalled. The car was towed to my local dealership and it is currently being worked on. The incident was reported to Toyota Corporate and a case was opened to investigate this issue. Every single model made between those years are effected and has caused thousands of people to be left with an expensive repair that is unavoidable.Read more

I heard a "pop" and then immediately saw "check electric vehicle system". The code I'm receiving in the app is P1A9096. The car was limited in driving speed, and I managed to get the car to the dealer before it stopped driving completely. I have about 8000 miles on this <1 year old car, and it seems like the ICCU has fried. Hyundai says this is under 1% of all cars, but from the internet it seems
... like this happens way more frequently. Reporting to the NHTSA so that there's record of this happening, given the frequency it seems to happen on the internet and the paucity of solutions (beyond putting in another faulty part) from Hyundai.Read more

Dear Toyota Customer Experience Team, I am writing to report a serious and potentially widespread safety issue with my 2018 Toyota Camry XSE. The automatic transmission has failed. The vehicle exhibits slipping, hard or delayed shifting, intermittent loss of drive, and most critically, the car completely shut off while I was driving on the highway, creating an immediate risk of a serious accident
... . The transmission is available for inspection upon request. These failures pose a direct safety hazard. The unexpected shutdown on the highway, combined with gear engagement problems, could easily lead to loss of control, collisions, or injury to myself, passengers, and other road users. The transmission failure has been confirmed by a certified independent transmission specialist, who stated that the transmission requires either a complete rebuild or replacement. The vehicle has been maintained according to Toyota’s recommended schedule. The vehicle has not been inspected by Toyota representatives. It has been inspected by the independent specialist for diagnostic purposes. Hard shifting and gear slipping began approximately 11/08/2025. The transmission warning light hasn't even come on. Sudden hesitation during acceleration or gear changes became increasingly frequent. The most severe symptom occurred when the car completely shut off on the highway which is super dangerous. Based on my observations and the independent specialist’s evaluation, I believe this failure may be related to known issues affecting 2018 Toyota Camry XSE transmissions, potentially involving internal clutches or valve body components. I respectfully request that Toyota consider offering a goodwill repair or replacement of the transmission for my vehicle. Provide guidance to owners of similar vehicles to prevent accidents or injuries.Read more

The transmission would hang up in 7th gear when stopping at red light, caution light or stop signs without warning. It would roll like normal when letting of brakes however when out in the middle of traffic stop signs or caution lights the truck would not move and would not shift as it was hung in 7th gear. The only way to get the truck back in 1st gear was to place the truck in park and turn the
... ignition off then restart the vehicle. This has happened 3 different times post bringing the truck to silsbee ford for a ecm and tcm reprogram. Originally called dealer back to reports extreme rough shifting and was told they did not mess with anything in the transmission and it was not due to anything they did, never received a call back from manager. No warning lights messages ever appeared to show the transmission had failed to down shift into first gear at the stop lights. All three of these events happened while my 7 year old child was in the truck and two of the times it was a near miss and narrowly avoided accidents. Prior to this since around 50,000 miles truck would shift rough with rough engagement into 3,4th,5th,6th, gears with frequent flare shifts and was told by dealers this was normal characteristic of the transmission.Read more

Incident Report – Gear Shift Failure Date of Incident: 11/10/2025 Vehicle: 2016 Ram 2500 Pickup Truck VIN [XXX] Mileage: [Insert Mileage] Location: [Insert Location of Incident] Description of Incident While attempting to shift the vehicle from Park (P) into Drive (D), the shift lever suddenly went limp and lost all resistance. Upon applying slight additional pressure to engage the transmiss
... ion, the shift lever snapped off completely. The gear shift bracket—the component securing the shift rod—fractured in half at the point where the shift rod inserts into the bracket. This mechanical failure rendered the vehicle inoperable and unable to be shifted out of Park. No warning lights or prior symptoms were observed before the failure. The broken part was visually inspected and confirmed to be the shifting lever bracket, consistent with Mopar part number 68470756AA. Immediate Actions Taken The vehicle was safely secured to prevent unintended movement. Engine was turned off and keys removed. Visual inspection confirmed that the shift lever bracket had broken at the rod insertion point. Tow service was arranged for transport to a repair facility. Photographs of the damaged bracket and shifter assembly were taken for documentation. Assessment This incident represents a critical component failure of the gear shift mechanism, resulting in sudden loss of control over transmission operation. The break occurred under normal shifting force, indicating a possible material or design defect in the bracket assembly. Recommended Actions File a warranty or recall inquiry with Ram/Stellantis regarding potential defects in part 68470756AA. Replace the damaged bracket with a verified OEM replacement. Inspect surrounding linkage and cable components for stress damage or misalignment. Retain the failed part for potential recall or manufacturer investigation.
