While driving my 2019 Jeep Cherokee, the vehicle's start/stop system malfunctioned and caused the engine to completely shut off at a traffic light. A catastrophic failure occurred in the continuously variable transmission (CVT) of a 2022 model year vehicle at approximately 81,000 miles. While driving on the highway, the vehicle suddenly lost motive power; the engine revved rapidly without transferring any power to the wheels. The failure was diagnosed by an authorized dealership service center (documented on Repair Order #349133). The diagno
... sis confirmed that the transmission failed due to an internal component breakdown, stating: "FOUND SEVERAL METAL DEBRIS FOUND WHEN DRAINING AND STRAINING FLUID." The manufacturer (American Honda) has since confirmed in writing that the root cause was a manufacturing defect, effectively acknowledging that the failure was not due to wear, tear, or maintenance neglect. The problem has been reproduced and confirmed by the dealer. The component is available for inspection. This premature, total failure of a major powertrain component on a relatively new vehicle poses a safety risk due to the sudden loss of power while operating at highway speeds.Read more

Transmission CVT failure of the 2019 Toyota CHR at 107,000 miles, was not inspected until there was an undeniable sound coming from the transmission and specific request was made for the source of the sound. There were no dashboard lights or warnings prior or even now that I’ve been told the transmission needs replaced and is in failure. Transmission failure has been inspected and confirmed by
... Toyota Dealership. As a consumer I was not forewarned transmission will require specialty maintenance in order to keep the vehicle in service especially at just 100k miles, no formal requirements were ever made concerning the transmission to maintain the functionality of this particular vehicle. The sound began emitting from the transmission about 2 to 3 weeks prior and due to the government shutdown as a federal employee I took my car to get serviced for the issue when it was feasible to do so. There is a Toyota CHR forum and multiple media outlets discussing the faulty CVT transmissions in the Toyota CHR costing upwards of $9000 -$14000 in replacement with no warnings but solely based on driver intellect that something is functionally wrong with the vehicle. In my observations and experience the transmission issue simply equates to replacement rather than a repair issue.Read more

XXX XXX XXX XXX XXX 11/13/2025 National Highway Traffic Safety Administration Office of Defects Investigation 1200 New Jersey Avenue SE Washington, DC 20590 Toyota Motor Sales, U.S.A., Inc. 19001 South Western Avenue Torrance, CA 90501 Subject: Complaint Regarding Torque Converter Shudder – 2018 Toyota RAV4 AWD To Whom It May Concern, I am submitting this complaint regarding
... a serious transmission defect in my 2018 Toyota RAV4 AWD, specifically a torque converter shudder that has been documented by Toyota technicians. The vehicle exhibits pronounced shuddering between 25–50 mph under light throttle, consistent with the torque converter failures previously acknowledged by Toyota in earlier RAV4 models (2013–2015). Toyota has extended warranty coverage for torque converter replacement in those earlier models, yet my 2018 RAV4—despite showing identical symptoms—has been excluded. The dealership has quoted me $9,000 for a full transmission replacement, even though the issue is well-documented and appears to be systemic across multiple model years. This situation raises serious concerns: - Toyota is aware of the defect but has limited coverage to select VIN ranges. - The cost of repair is excessive and disproportionate to the defect. - Consumers are left without recourse despite clear precedent for warranty extension. I respectfully request: 1. That NHTSA investigate whether the torque converter shudder in 2016–2018 RAV4 models constitutes a broader safety defect. 2. That Toyota extend its warranty program to include affected 2018 RAV4 vehicles. 3. That Toyota provide reimbursement or coverage for necessary repairs to restore safe vehicle operation. I am prepared to provide service records, dealership documentation, and correspondence confirming Toyota’s acknowledgment of the defect. Please advise on next steps and ensure this issue receives the attention it deserve INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

The transmission has issues when temperatures drop below 5 Fahrenheit. Specifically if the outside temperature drops below 5 the transmission does not engage gears correctly and results in sluggish acceleration, erratic RPMs and poor fuel economy. To be clear this is NOT with cold starting the vehicle. This is documented instances of the vehicle after sufficient driving time to warm up. I have do
... cumented multiple instances of the vehicle having the erratic RPMs and sluggish acceleration when the temperature drops to below 5. Each documented instance is after driving for a period of time (I note this to again document the vehicle is warm and not a cold start). During these drives the outside temperature drops and goes below 5 degrees during the drive at which point the vehicle begins having the sluggish acceleration and erratic RPMs. To reiterate and ensure it is clear. The vehicle is warmed up. It is not a cold start concern. I keep reiterating this because each time I raise the concern to Nissan and the dealer they simply say it is cold, this issue occurs after sufficient time for the vehicle to warm up. This is being raised to NHTSA because when it is happening the sluggish acceleration leads to general drivability issues, along with the erratic RPMs result in substantial loss of MPG. When it occurs gas mileage drops to generally about 8mpgs at best far below the rated MPG of the vehicle and normal driving characteristics.Read more

Subject: Formal Complaint – 2020 Ford Explorer Transmission Failures and Unfair Coverage Practices Dear Ford Customer Service, I am writing to formally express my concern and disappointment regarding repeated transmission failures in my 2020 Ford Explorer. The vehicle required its first transmission replacement at just 4,000 miles, and again recently at approximately 100,000 miles. It has com

It is unreasonable for a $45,000 vehicle, only a few years old, to require two complete transmission replacements. This is not an isolated case — thousands of 2020–2022 Ford Explorer owners have reported similar failures with the same transmission model, often shortly after warranty expiration. I am requesting that Ford review this case for goodwill repair coverage or reimbursement, and to consider expanding the recall parameters to include vehicles like mine that were evidently affected by the same defective components and production issues. I have purchased over $100,000 on Ford Cars, and have only purchased Fords. I am going to purchase a brand new car in 2 years, and it most likely will not be with Ford due to their shady recall practices, and lack of care for the quality of their product and customer service.Read more
