I own a 2014 Porsche Panamera S E-Hybrid (VIN XXX). I experienced repeated hybrid system warning lights and high-voltage battery fault codes. I brought the vehicle to Porsche Atlanta Perimeter for a safety inspection. The dealership did NOT perform the required PIWIS Hybrid Battery Test Plan, did NOT check individual cell voltages, and did NOT assess the hybrid battery control unit. Instead, they
... provided a basic code scan and immediately recommended a $37,000 battery replacement without verifying the root cause. This is a high-voltage system handling 300+ volts, and an improper or incomplete diagnosis is a safety concern. Porsche headquarters was contacted by the dealership, but the dealership stated they received no technical support or guidance. Porsche appears unable or unwilling to diagnose early-generation hybrid vehicles properly, leaving customers without accurate safety assessment of the high-voltage system. I am filing this complaint because improper hybrid battery diagnosis can lead to thermal events, sudden loss of power, or misinterpretation of critical battery faults. I am requesting that NHTSA review Porsche’s handling of hybrid diagnostics for this model. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

In March 2025, my 2023 Cadillac Escalade ESV experienced a sudden engine failure. The engine began running rough, the check engine light turned on, and the vehicle became unsafe to drive. This matches the defect described in GM Recall N252494000 (connecting rod/crankshaft internal engine def

COMPONENT FAILURE: Body Control Module causing systematic electrical failures: door locks, power windows, window regulators, four-wheel drive transfer case display. Power windows fail affecting communication/egress. 4WD display fault (yellow AUTO vs white) indicates transfer case control iss
... ues affecting traction in mountain winter driving at 10,000ft elevation. Unpredictable electrical failures create safety risk during remote travel. DEALER CONFIRMATION: Approximately 20 dealer visits. Each time: scan codes, test drive, "no fault found." Problem temporarily clears via battery disconnect/reconnect, proving electrical fault not mechanical. Dealer cannot diagnose root cause, hasn't performed systematic electrical testing. MANUFACTURER INSPECTION: None despite repeated failures. WARNING SYMPTOMS: Intermittent: door ajar warnings, power window/lock failures, 4WD indicator yellow, various warning lights. All symptoms simultaneously resolve after battery disconnect/reconnect, indicating BCM or power distribution fault. Pattern repeats with increasing frequency. CRITICAL: Research shows multiple 2023 Colorado owners report identical patterns - intermittent multi-system failures resolving via battery reset. Indicates systematic 2023 model year defect, likely BCM firmware/hardware issue. Problem occurs unpredictably, clears during dealer transport. See attached document for complaint. I am writing to you about the attached letter from BRP (reference letter dated January 22, 2025 re: Engine Output Shaft Can Break- Potential Loss of Drive Power-NHTSA Campaign ID Number 24V728). My Can-Am Spyder is affected by this issue. As stated in the instructions in the letter, I contacted my local authorized BRP Can-Am dealer to have the repair completed
... . I provided my Can-Am Spyder RT to Coleman Powers ports in Falls Church, VA on March 26, 2025. The letter goes on to state that the repair takes approximately 8 hours. After discussion with Coleman Powers ports, they advised this is grossly incorrect, as the engine must be removed and the work scope is orders of magnitude greater than 8 hours. Bottom line: I am writing this letter on October 18, 2025, and I still do not have my Spyder back. I periodically call the shop, and they have been understanding of my frustration and seem to be doing their best, but I understand they have stopped taking in vehicles for this warranty repair due to the extensive work scope and hours required for each unit. I am not satisfied that BRP has done their best to remedy this situation. They should offer alternate repair options, assist the shop, or do whatever it takes it get my Spyder back to me (6+ months and counting is completely unreasonable for a repair that BRP says in the letter should take 8 hours). If there is anything you can do to push BRP to get involved, that would be appreciated. (Note: I've called them twice already, but no meaningful advice was provided other than for me to contact the shop).Read more

Tuesday 11/11 my 2019 Jeep Cherokee was involved in an accident due to the PTU not working properly. In PA we had very poor road conditions due to snow throughout the night into the morning. ... me it could have been worse if it weren’t so early in the morning and little traffic on the roads. I had my vehicle in the Scranton Dodge and Jeep dealership about a month prior to inspect the issue and they were unable to fix it due to the part being on back order and my cars VIN not being specified on the recall for this issue. I was also told that my vehicle was safe to continue driving in the meantime which is not the case. I have had the vehicle inspected by a dealership and my insurance company was involved for the damages produced by the accident. The warning message has been on but no one has the part to do anything about it and like previously stated was told the vehicle was fine to drive. I had this issue inspected in the beginning of october. I have also contacted Jeep and they told me there is nothing they can do for me since my VIN is not specified. I bought this car to get safely to and from places during the winter months in PA and that clearly is not the case now. It is very frustrating being a full time student and paying on a car that does not do the job that it’s supposed to do when I bought it and now I am thousands into a vehicle for parts that is more than the actual value of the car itself.Read more

I was told that they had seen several customers with the same issue and wouldn’t be surprised if GM eventually issues a recall. They also said they couldn’t be sure the replacement part wouldn’t have the same issue. I did some research and found that GM has had this issue with 2019 - 2024 vehicles. What component or system failed or malfunctioned, and is it available for inspection upon request? Answer: The transmission is slipping or malfunctioning when driving the car, specifically when accelerating. Yes, it is available for inspection upon request. How was your safety or the safety of others put at risk? Answer: When the transmission slips or malfunctions, the acceleration of the car doe
... sn't act as it should. For example, when waiting to join a flow of vehicle traffic, or when waiting to cross a busy intersection, the delay in acceleration or the slipping of the transmission is increasing the risk of being in the wrong place at the wrong time, or causing my vehicle to block other vehicles. Has the problem been reproduced or confirmed by a dealer or independent service center? Answer: Yes. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? Answer: No. Were there any warning lamps, messages or other symptoms of the problem prior to the failure, and when did they first appear? Answer: No. Additional information: I spoke with Honda in 2023 to see if there was a recall related to my CVT transmission. They took my information, but advised there was no recall. I had a similar conversation with them in 2024 because it seemed to be getting worse, but there still was no recall. Now in 2025, it still feels like it's getting worse and making it feel unsafe to drive the car.Read more

My car was under warranty when this started happen

When the shuddering returned, I returned to the freedom Chevrolet where I bought the vehicle (actual mileage 53768) and explained the shuddering returning. I was advised to leave the car which now has been at the dealership for months and they refuse to fix the problem stating it is out of warranty. I provided all the bulletins I found on related issue which GM has stated they fix the problem by swapping the transmission on new cars but nothing has been done on the cars that have the existing problem because “there is no recall” as the service advisor stated. This problem could’ve been resolved when I took the car the first time but I wasn’t aware of the severity of the problem or that I would have so much trouble with the dealership now. This report covers 2 events for the same problem. The first occurred in the spring of 2025 and I did not document the date or time. The second one occurred on November 10, 2025 @ about 10:15 a.m. (1) Last spring my wife was driving the Venza up the street to our residence going a slow rate of speed. As she approached the house, she took her foot off on the gas and coasted toward the left curb
... so she could back into our driveway. At that point, car suddenly surged forward and accelerated toward the curb. My wife rapidly applied the brake and averted any problem. (2) The second event occurred on November 10, 2025 @ 10:15 a.m. My wife was starting to park at a parking lot in the Tanasbourne Town Center. She was going to back into a parking spot so she drove forward going slightly left. Then, with the car still in drive, the car surged forward. She quickly stepped on the brake and the car stopped abruptly. (She was angling the car and was about ready to stop and put the car in reverse when this happened) Again, with her quick response braking, she was able to avert hitting person or thing. As to which component or system failed, there appears to be a random malfunction of sensors or perhaps the CPU...something that is calling for full throttle when the gas petal is not pushed. The safety risk is possible harm to the driver, outside property, or the worse situation would be harm to pedestrians that might be in the way of the car. We have not been able to reproduce the problem. The same maneuvers have been repeated 100's of times without incident. As of this writing, the vehicle is being taken to the Toyota dealer to have the oil changed and at that time they will be asked to do a thorough inspection of the systems that might cause this, check for any recorded malfunctions, etc. To the best of our knowledge, there were no warning lamps or messages that appeared.Read more

Component is transmission. Stutter vibrate shift while driving. There is a recall on the 2021 Chevy 1500 Silverado which is exactly the issue we are having from our 2021 Silverado. The VIN for our truck is falling under the current recall. Chevy wants to charge to even diagnose the problem which has to be done with a dealer and not a private place. That is absurd and puts our safety at risk s
... hould the vehicle stop functioning while driving it. The dealer ship has inspected the vehicle for the yearly inspections which is interesting since this has happened and been complained about since purchase.Read more

We noticed the "battery" indicator light was lit, so we changed the battery. That addressed the light, but soon it lit up again, followed by an overheated engine warning so we stopped and called for a tow to Puente Hills Jeep. We were informed it was a pulley so it was replaced along with the serpentine belt. Soon after receiving the vehicle, it started smoking, and it appeared it was catching fir
... e so we had it towed to the dealer for the second time in one week. We've been informed it's the air conditioner compressor and it will be replaced. The concern is that we're dealing with this so soon after the basic warranty period has expired.Read more

I was starting to accelerate to a higher level of speed onto the highway; 55 mph going up a hill. I was unable to accelerate much as I pushed on the gas, however the RPM's would get higher. while I was at 45 mpg the RPM's were at 3.5. All of sudden my service engine light came on. I knew something was wrong. Turned around took my car back home very slowly, had my husband come look at my car, wh
... o has mechanical experience. Transmission fluid is dark and gritty; when its suppose to be a red in color and smooth. I am now needing to replace a transmission in a vehicle that is 5 years old. How is this possible? I have owned the car for 2 years. Why has these not been recalled yet? Why has these not been looked into further? We are paying a lot of money for these vehicles to turn around and put a new transmission, which isn't cheap into a vehicle that is 5 years old. Any assistance to look into these complaints would be very helpful. I am just glad no one was hurt and I was able to get my vehicle home safely. I keep up with all of the routine maintenance of this vehicle on a regular basis.Read more

This is an update to NHTSA Complaint #XXX with new safety information. My 2017 Tesla Model S (VIN [XXX] ) has a confirmed high-voltage isolation fault. Tesla replaced the rear drive unit under warranty, but Tesla’s own technician notes now state: “There is a risk of isolation alerts reoccurring during regular driving, use of HVAC system, or DC charging.” “Vehicle may power off due to iso
... lation faults when the air conditioning is used.” This is Tesla acknowledging in writing that the vehicle may shut off during normal driving, even after repair. This presents a serious crash risk. The original failure occurred during Supercharging, with warnings including: • “Electrical system power reduced — vehicle may shut down unexpectedly” • “Vehicle may not restart” • “Air conditioning reduced — DC charging reduced” Tesla reproduced the isolation fault during diagnosis. Even after replacing the rear drive unit, Tesla documented that the isolation defect still exists and prevents a battery health check or DC fast-charge validation. Tesla has attempted to return the vehicle to me in this unsafe condition, while admitting the defect persists. They also stated they may charge storage fees or tow the vehicle, despite the unresolved safety issue. The defect affects the HV bus and can cause loss of propulsion, shutdown during driving, and charging failure. Tesla has not provided numeric test results (megohm isolation readings, insulation tests, or validation data) proving the vehicle is safe. This is an ongoing, manufacturer-confirmed HV isolation defect that can lead to sudden power loss. I request NHTSA review Tesla’s handling of this issue and link this filing to Case #XXX. Highlighting a materials defect with the rear crankshaft reluctor ring on the 2.8 LWN Duramax, installed in the Army Infantry Squad Vehicle and the 2015-2023 Chevrolet Colorado, from which it is based. When the ring delaminates, due to age, the vehicle engine shuts off and no start, rendered dead lined, as the ECU can no longer "see" the crank position. Requiring the transmission to be removed to
... rectify the issue. I was pulling a trailer up a hill on the freeway when my truck down shifted, in an effort to slow down for a bus that was merging unexpectedly and the truck died at speed. The increase in RPM initiated an unplanned, instant disassembly of the ring due to cracking on the material and increased centrifugal force, shutting off the engine without warning, at speed. Again, just dead stick to the shoulder, towing a trailer, on the freeway. That could have been bad... Had truck towed to GM dealer, for a month-long repair.Read more

All of a sudden, when I would shift the gear to neutral or reverse the car would only drive forward. While there are other recalls related to this issue for other models, there isn’t a recall for this 2022 model. The ford dealership said it needs a new transmission and that would cost around $10,000.
