I am writing to formally document serious engine defects I have experienced with my 2021 Chevrolet Tahoe equipped with the 5.3L L84 V8 engine. 1.Crankshaft Failure: My Tahoe suffered a catastrophic crankshaft failure that required major repair/replacement. I am aware that GM has recalled certain vehicles equipped with the 6.2L L87 engine for crankshaft and rod-bearing defects. Based on my experien
... ce, the same or a very similar defect has also occurred in my 5.3L engine, which is currently not included in that recall. 2.Valve Lifter Failure: In addition, my vehicle has suffered from valve lifter collapse—a well-documented problem in 2021–2022 GM trucks and SUVs with both the 5.3L and 6.2L engines. This caused severe misfiring, drivability issues, and expensive repair needs. Why this is a concern: •Both failures occurred well before what would be expected of a modern engine. •The crankshaft failure raises a safety risk: sudden loss of propulsion while driving could lead to accidents. •The lifter issue is widely reported by other owners and has been the subject of GM service bulletins. •Taken together, these problems suggest systemic defects in the 2021 Tahoe’s 5.3L engine that have not been adequately addressed by GM. Requested Action: •I ask GM to extend recall and warranty coverage to include the 5.3L L84 engine in addition to the 6.2L L87. •I ask NHTSA to consider investigating whether the scope of the current 6.2L recall should be broadened to include 5.3L engines, given failures like mine. •At minimum, I request assistance in covering repair/replacement costs for both the crankshaft and lifter defects, which are clearly not due to normal wear and tear. 6 months and 20,000 miles after purchasing the 2021 Chevy Silverado custom 4x4 with the 8 automatic transmission and the 2.7L turbo charged engine with 32,000 original miles on it from Volume Chevrolet in Forsyth GA I noticed the transmission was jerking when the truck was first crunk and warming up after about 10 minutes and once it got to 8 mph after a while when the temperature gauge was half w
... ay up it would stop and when you slowed down from 55 mph to about 5 MPH to make a turn it would suddenly jerk again, I reported the issue to the dealership service mechanic shop reps during every oil change and they assured me that it was normal and that it would stop happening when I let the truck warm up for about 30 minutes prior to driving, once the factory power train warranty ran out, they informed me that it needed a transmission service and they would have to keep it a few days and it would cost around $800 or so to get it done, the purge pump on the emissions system has a factory defect and I received a recall letter about it 6 months after purchase and they are refusing to replace the purge pump until they perform a full diagnostic first even though they have done a scan diagnostic and it shows the purge pump failed and I have just been struggling with all the cost of living and increased prices I can't afford any kind of costly repairs right now and my truck now has 120,350 miles on it and the issue is getting worse to the point that I am sure that the transmission valve body and some other sensors and control modules are failing, this is not a single issue and other dealerships have said that the transmission was starting to have problems in other trucks at around 55,000 miles and some earlier than 55,000 miles, I believe that this is an ongoing transmission issue with the 2.7L turbo charged engine in both the 4x4 and 2x4 models and with the 8 speed automatic transmission In general, I have talked with several other owners and they have had similar issues .Read more

The car suffered a critical electrical failure which resulted in the car abruptly losing power when driving at a speed of about 30 mph. The speed at which the car became unresponsive was so rapid that the car could not even be maneuvered from the middle lane into the right most lane for safety. The cars digital display system indicated nothing except that the hazards were on (after I had attempted
... to turn them on) and a red box with two cog wheels inside of it. In reality, the hazard lights were NOT on externally as was evidenced by a number of vehicles stopping to tell me to turn on my hazards. Additionally the car would not respond to any commands whatsoever. The car would not go in to park or drive and could not be powered on/off. The brake pedal was able to be pushed to the floor. The doors which have an electronic assist system were difficult to open because the whole electrical system was in a failure state. The car had to be towed to a Porsche dealership. Upon arrival to the dealership, the car indicated an engine control error on the digital dash and showed a turtle icon. This was not the case during the original failure (prior to tow) and only showed that state after a multi-hour tow trip. There were absolutely no indications of this problem prior to it happening. The car was at around 40% state-of-charge. Porsche performed a software update on the car and drove it 120 miles but could not recreate it. The very same day I took the car back from Porsche, the same issue occurred but in a less critical way and in a parking lot. This time, the car suffered an engine control error but after walking away from the car for 10 minutes or so, the error was gone and the car was drive-able. In both cases, one thing i noticed was that the rear spoiler was stuck in an extended state after the failure had occurred.Read more

My 2024 Honda CR-V EX is experiencing a premature head gasket failure at approximately one year of ownership. A head gasket is a long-life component designed to maintain proper combustion sealing and prevent coolant/oil cross-contamination. This type of failure should not occur on a new vehicle and suggests a potential manufacturing or engineering defect in the powertrain. Before diagnosis, the ve
... hicle displayed symptoms consistent with gasket integrity loss, including rough running, shaking during acceleration, reduced power output, inconsistent engine temperature, and abnormal vibration. These issues indicate possible combustion leakage, coolant intrusion into the cylinders, or compromised cooling system performance. Given the low age and mileage of the vehicle, the failure does not align with normal wear. Possible causes include improper factory installation, uneven torque application, defective gasket materials, engine block or cylinder-head surface irregularities, or thermal management issues that could cause localized overheating or warpage. Any of these would point to a systemic defect rather than owner misuse. A failing head gasket can lead to engine overheating, stalling, power loss, oil/coolant contamination, and eventual engine failure. These conditions present clear safety risks if they occur while the vehicle is in motion. Because this is a major powertrain failure occurring far earlier than expected, I believe this issue may affect other 2024 CR-V models and warrants investigation. I am submitting this complaint so NHTSA can review whether a potential defect exists that could require further action to protect consumers.Read more

My 2018 Mazda 3 has an intermittent electrical failure on the high-speed CAN network that causes multiple safety-critical systems to fail while driving. When the fault occurs, the dash lights up with warnings for powertrain, ABS/traction, lane departure, collision-avoidance, TPMS, immobilizer, and the check-engine light. The car also hesitates or “lurches” between 15–30 mph during these even
... ts. At one point in 2024, the vehicle went into limp mode on the freeway, creating a safety hazard. These failures are intermittent but repeatable. Diagnostic scans from 2023–2025 show multiple U-codes for lost communication between control modules; two later became permanent. The issue appears unrelated to any one component and affects many systems at once. The vehicle has been to every Mazda dealership in the North Texas region (Mesquite, Freeman, Hiley, Norm Reeves, Town North). All performed only basic scans; none completed a full CAN-bus or wiring-harness diagnostic. Town North kept the vehicle for six weeks (Sept–Oct 2025), replaced a coil and spark plugs, performed a camera calibration and a TCM update, but the network failures returned within 38 miles. We sought independent inspection only after dealerships repeatedly refused diagnosis whenever the warning lights were not on at the moment of arrival. In November 2025, a technician found significant internal corrosion inside the negative battery cable; touching the cable reproduced the full failure immediately. The corrosion extends deep into the wire sheathing. This appears to be an electrical defect affecting CAN communication and power/ground integrity. When the fault occurs, multiple safety systems disable at once and the vehicle behaves unpredictably. Mazda Corporate has declined further assistance, and the defect remains unresolved. I am filing this report because this is a recurring safety issue that may affect other vehicles of the same model and year.Read more

My car has been serviced multiple times and this should have been checked or errors/updates reviewed. This auto/stop start seems to be a key in the safety issues as well since that light comes on every time there is an issue. We attempted to drive towards our destination and approximately 30 minutes later the same thing happened again. We drove the vehicle 15-20 mins at a time and then exited the highway over and over giving the vehicle time turned off. After returning home with no additional events the engine warning light went off the next warning. This was on a Sunday so we were unable to get it to a dealer. The next day we dropped off the vehicle at Nappletom Chrysler dealership. The called us the next day and told us they could not cause the event to happen again. ... issue that should've been addressed by the manufacturer. there were no warning lamps messages or symptoms of the problem prior to the failure. this car is a safety hazard and it is negligence to allow the issue to persist. the problem was reproduced by the shop, fletcher car care - Fletcher Car Care is an auto repair shop located at 38 Heritage Park Dr, Fletcher, NC 28732 - the issues i believe began when the cable bushing began changing the linkage between the shifter and transmission around march. this was when i started having issues with the rear camera - the rear camera at times is full of artifacts and interference because i believe it is in a intermediate (out of full engagement position)Read more

Detailed Description of the Defect: Friday Incident: While picking up my son from school, the vehicle abruptly disabled multiple safety and driver-assist features. The car rolled backward on a slight incline, failed to display any surrounding vehicles on the FSD visualization, and retained accelerator input even after I removed my foot from the pedal. The vehicle continued to speed up slightly in
... Drive and Reverse without my input. Soft resets did not resolve the issue. Monday Incident (Collision Caused by Malfunction): On Monday morning, the same malfunction occurred. As I reversed into my garage with the door opening, the car again held the accelerator input after I lifted my foot, causing the vehicle to continue moving backward unexpectedly. The vehicle struck my garage door, causing property damage and a dent to the trunk. This was not driver error. It was unexpected propulsion after pedal release — an extremely dangerous condition.Read more

Transmission failure at 19,481 miles in my 2021Jeep Grand Cherokee Limited 4x4 V6. The vehicle has not been abused, and the mileage is low for its age. My 2022 Kia EV6 developed a serious electrical/MCU malfunction on 10/19/2025 at approximately 64,300 miles. While driving, the vehicle began to stutter during acceleration, all driver-assistance and safety systems disabled (LKAS, collision avoidance, lane keeping), and the vehicle could not accelerate normally (topping out around ~60 mph when fully pressing the pedal). The Kia Connect app displaye
... d fault codes MCU P0A4100 and LKAS C164286. The malfunction recurred immediately after a dealer visit on 10/20/2025. The vehicle has since been inspected by Huffines Kia in Corinth, Texas, but no confirmed diagnosis has been provided, nor have they notified me of dropping/reconnecting the HV battery. On 11/18/2025, a new fault code appeared: MCU P0C1700. This failure affects the high-voltage electronics/MCU and causes sudden loss of acceleration capability and complete disabling of active safety systems while the vehicle is in motion, placing me and other drivers at risk. The issue is reproducible, continues to occur, and the vehicle remains at the dealership. I am requesting NHTSA review of this electrical/MCU malfunction as a safety-related defect.Read more
