My 2024 Equinox was returned to the dealership on approximately October 1, 2025, due to a check engine light related to the torque converter. I required a rental vehicle during this time. I picked up my vehicle on October 10, 2025, and all repairs were covered under my extended warranty. I was informed that my rental reimbursement would be issued once I submitted the required paperwork. My safety

Since I've purchased the truck in 2023 I've been receiving in the mail notices about a recall for the rear-axel shaft for my 2022 f150. I am a major proponent of safety and maintenance. Year over year I've attempted to get it fixed at authorized ford dealerships but I've been told a handful of times that the parts are not available and I'm not getting the appropriate urgency required for an issue

The vehicle has an ongoing transmission downshifting malfunction. The powertrain unexpectedly downshifts while driving, causing sudden changes in speed and unstable vehicle behavior. The issue can be inspected at any time, as the defect occurs intermittently and has not been repaired. This malfunction puts my safety and the safety of others at risk because the vehicle can slow down abruptly or sh
... ift aggressively without any input from the driver. This can create dangerous situations in traffic, especially at highway speeds or during acceleration. The problem has been reproduced and observed multiple times by the Mercedes-Benz dealership. I have taken the vehicle to the service center repeatedly for this same issue. Most recently, the vehicle remained at the service center for 10 consecutive days, but the defect was not resolved. The vehicle has been inspected by Mercedes-Benz technicians on several occasions. Despite multiple repair attempts, the issue continues to occur. There were no warning lamps or dashboard messages prior to the malfunction. The issue first appeared months ago and has persisted despite repeated service visits and repair attempts.Read more

I was driving on a very busy road and the vehicle lost power. I consider that my life was in danger because of me the traffic piled up and other drivers were furious with me. No matter what I tried, the car simply would not stop. AAA came and was unable to make the car start. It was towed to a dealership. Prior to this I already had two service visits related to the Start Stop warning light coming
... on in my dashboard. The first time, the service department claimed that they could not figure out what caused it, but they replaced the auxiliary battery that powers the start stop feature. Basically one day later, the start stop warning light came on again. I had to take it back to the dealership and they kept the vehicle for two days and I don't think anything was done. After the vehicle was returned to me, within days the warning light came on and then would go off and come back on sporadically. On the date of the complete power loss, the warning light came on in my dashboard about one hour before the power loss. So far, Jeep has said that they won't even be able to look at my vehicle for a week and it will probably take at least two weeks because they are short-staffed. I think I have a lemon.Read more

Unintended acceleration event. Entered vehicle, put my foot on the brake, cranked it, held in that position while seatbelting. Engine had a VERY loud revving sound. Within seconds vehicle moving backwards on its own. Foot still on the brake, had not touched gas pedal, had not touched gear shifter, and was still sitting straight forward with my foot on the brake and confirmed gear shifter in pa
... rk. The car quickly accelerated backwards faster and brake pedal had no effect on stopping or slowing vehicle. Went approx. 30’ across parking lot and only stopped because we hit truck parked behind us. Quickly turned off engine and removed keys. Shifter still shows Park. Careful driver. No history of accidents. I found there were several thousand reported events of unintended acceleration (Ford vehicles 2002 to 2010). Ford tried to explain many as driver error or floor mat touching the gas pedal. My mat was clipped down, as designed, with no all-weather mat on top of them, and my foot did not leave the brake pedal. AI says revving as described is "classic behavior of failing Electronic Throttle Body on 2010 Fords", "Surging without touching the pedal and known defect are extremely common". Says if "Worn or failing shifter linkage/bushings vibration form high revving could make it slip into Reverse". Ford has a 19S54 recall for AntiLock Brake Systems (not notified), where "A normally closed valve may be stuck open, which may result in extended brake pedal travel". Ford reports on similar vehicles "lunging" was due to deposits in the throttle body. Ford has installed brake override on vehicles after 2010 but that doesn't protect people driving older vehicles. Please look at combo of factors that could cause this. Disappointed to read Ford purged customer acceleration reports prior to hold 5 yrs and an expert they provided at trial was someone sheltered from all research Ford had on this issue so he pushed "driver error" as the case. Clearly not here.Read more

Dear NHTSA, I am submitting this complaint because my 2014 Jeep Grand Cherokee Limited is experiencing multiple safety-related failures that I believe are part of a wider defect pattern affecting this model year. Several of these issues involve systems that are critical for emergency safety, braking, and vehicle control. ⸻ 1. Telematics Control Unit (TCU) Failure Causing Loss of 9-1-1/SOS Emergency Function My Jeep is displaying repeated “Service Emergency Vehicle Phone” errors and the SOS/911 emergency button no longer functions. This is a major safety concern, especially in the event of an accident or breakdown. The TCU failure also causes: •Uconnect 8.4 system not booting •HVAC controls becoming non-functional or unsafe (blowing incorrect temperatures, failing to defrost, turning off unexpectedly) •Random system resets •Loss of ability to control climate during driving (creating visibility issues in cold/wet weather) These issues align with widespread failures of the Uconnect 8.4 / TCU hardware on 2013–2015 Grand Cherokees, which were not included in a recall despite many vehicles exhibiting the same dangerous symptoms. ⸻ 2. TCU-Related CANBUS Interference Causing Transmission and Drivability Safety Issues When the TCU fails, the vehicle experiences: •Transmission communication errors •Vehicle entering limp mode unexpectedly •“Service Transmission” and “Press Brake to Shift” warnings •Delayed or unpredictable shiRead more

My 2019 Toyota RAV4 Hybrid (VIN: [XXX] ) suffered sudden engine failure due to coolant intrusion. On Nov 20, 2025, Johnstons Toyota in Harriman, NY (RO#XXX) diagnosed the issue and recorded DTCs P26CA31 and P26CB, which indicate failure of the coolant flow control/bypass valve. The dealer confirmed heavy white smoke from the exhaust with a strong coolant smell and advised the engine had failed, re
... commending a replacement. This vehicle is part of the early 2019 Japan-built batch widely known for premature electric water pump and coolant-flow system failures. Internal impeller swelling, seal leakage, and bearing seizure can cause coolant pressure spikes and ultimately head-gasket failure with coolant entering the cylinders. This defect has been reported by thousands of RAV4 Hybrid owners (2019–2021). Toyota has addressed it quietly through internal TSBs such as XXX and XXX but no public recall has been issued. The safety risk is severe: the engine began emitting thick white smoke and ran roughly, risking sudden power loss, overheating, or engine seizure while driving. A stall at highway speed could easily cause a collision. Toyota USA opened Case #XXX but declined assistance despite the defect being well documented. This failure was sudden, with no warning lights prior to the incident. Given the widespread nature of this defect and its clear safety implications, I request that NHTSA open a formal investigation into the electric water pump/coolant system failures in 2019–2021 Toyota RAV4 Hybrids and consider requiring a safety recall. My 2019 Infiniti QX60 (VIN [XXX] ) experienced a sudden transmission failure at 99,995 miles. The vehicle would no longer shift into gear and became undriveable. This appears to be part of a known pattern of CVT failures affecting Infiniti/Nissan veh

This failure occurred without warning and poses a major safety hazard, as the vehicle could not accelerate or be driven. My wife was pulling out of the parking area after picking up my [XXX] and as soon as she pulled out and shifted it to drive it would not. Ended up with a vehicle almost hitting them, which was a close call. After shifting it back-and-forth it finally went into gear and took it straight to the service center. 23S64-Engine Oil Pump RECALL 8/19/2025 driving from Hillsboro Beach, Florida to St. Petersburg, Florida. We had a full engine seizure, we lost oil pressure, and also lost use of the transmission. The Florida Hi

They did the replacement of the 23S64 recall for the oil pump belt and tensioner. The car was there for 24 days. We drove down on 9/12/25 and picked up the car. When they pulled the car out, the car was still covered in oil in the back. We found out within days that they neglected to fix the radiator. Once we got into the car and started driving, we had yet another very loud noise, forcing us to bring it to AutoNation, St. Pete, Florida. At AutoNation, we had a multitude of problems with it. We had severe vibration with the car when we brought it there 9/18/25. This was only a few days later. They changed out the radiator and replacing the full radiator because they said there was a full leak that was not fixed by DeSoto. They fixed the car and gave it back to us 9/30/25. On 10/22/25, I had the exact same experience with the car that we originally had on the highway in Arcadia. The engine fully seized in the intersection of 3rd Street South and 6th Ave south in St Petersburg just as it had done before. The car overheated and had full loss of power/motive power. I was able to restart it briefly to get out of the cross section. The car stalled two more times before I was able to bring it to a safe place. We ended up having it towed from 15th Ave S at 3rd Street S and brought to AutoNation. Vehicle broke down again 11/7 and has been in shop ever since. Much more details available.Read more

The pinion and carrier bearings failed on a less than 3 year old truck with 66k miles. This is way to early for this type of repair and was just out of warranty. This could have caused the truck to lock up while driving possibly injuring myself and other drivers. The noise caused from this was heard months before but was believed to be tire related.
Incident date: Oct 31, 2024
At approximately 66k and even thousands of miles before this the rear axle bearings failed and needed to be replaced. This is much sooner than a component like this should fail and oddly just outside of the warranty period. Although no safety incident happened, it very well could have locked up the rear axle putting myself and others in danger.

... y fixed. About mid October while driving at 60 mph it shut down again almost causing a wreck went to 30 miles per hour again. Hurt my wife’s neck when it reduced speed so quickly. Took back to dealership and I told them this car is dangerous this was with about 20 k miles Been in shop was told it is scanning with 22 repair codes we have not had the car for 5 weeks and are trying to work with gm and dealer to get us out of it. But as of this report no progress. On interstate in city traffic this car will hurt or kill many people. I’m refusing to let my family or myself drive it. There is no warning when it will flash lights in dash and reduce speed. Also the front crash sensors went out and are currently not working we have all service records and paperwork that has been done including maintenance records We were given vehicle back with no warnings. I was traveling down i-25 at 75 mph because that is what the speed limit here in Colorado. I was traveling south heading to Colorado Springs to visit family and i was entering near Castle Rock and my vechile would not go at all forward and it seemed as if I lost power to acceleration and I believe it was 2 or 3 lanes to exit off an off ramp as cars pasted me by honking their horns it was very scar
... y as I pulled over to the side of the road letting it glide or coast to a stop very slowly I finally came to a stop.. as I have been reading some recalls due too a transmission issues for these types of vechiles I really hope I'm going about it right and someone reaches out to me about what it is that might be going on because even though my car does not have it listed by a Vin number I still believe that my 2019 Chevy Malibu has been subject to recalls for issues including a faulty rear seat belt retractor and a transmission problem that could lead to a loss of propulsion. The rear seat belt recall involves improperly secured retractors, and the transmission recall is due to a start-stop accumulator endcap with missing bolts, which could cause a leak and a loss of propulsion or a fire risk. And would like to have this addressed and or hits the right people concerning this not only for myself but safety for other people traveling at high speeds like myself that night... I was 5 days past my dealership warranty when I purchased the vechile from them and made it 6 days because I could get ahold of anyone on the 5 day when this incident occurred and then I called GM because I believe it would of fell under a drive train warranty but they said which was only 5 years warranty on this vechile from the day it was purchased its first time my vechile only has 82000 miles on it and I got it from a dealership here in Colorado with 64 or 62000 miles and since then I'm paying on a vechile that is no longer in service parked Sincerely and hopefully this reaches the right person...Read more

Owners of 2017–2022 Toyota Highlanders, 2017–2020 Siennas, and 2018–2020 Camrys are experiencing serious transmission issues—including high-pitched whining, erratic shifting, clunks, hesitation, and premature failure. (This incident report is for my replacement transmission, I have already submitted an incident report for my original transmission which also failed in the exact same manner.) 1. My replacement transmission started bucking and clanging and down shifted extremely hard while I was driving in traffic, I lost all power and the vehicle would not accelerate but the RPM's kept rising. 2. My safety was a
... t risk because I lost power while in traffic and my vehicle became disabled on the road and I required the assistance of good Samaritans to help move my car off the road way, causing their safety to be at risk too. 3. My vehicle has been inspected by Honda. Their recommendation was to replace the original transmission, so I did, and the second transmission (the subject of this incident report) also failed, in the same manner. 4. The vehicle monitoring system did not give any prior warning that something was wrong till after the vehicle bucked, down shifted and lost power, only then did the car warnings go off and say "Transmission system" and then "emissions system"Read more

Almost caused high speed accident merging onto toll road! The vehicle is experiencing a severe transmission slippage, shudder, and grinding noise, consistent with TSB-0008-21 (UA80 transmission failure). This defect causes hesitant acceleration and erratic shifting, creating a safety hazard when merging onto highways or crossing intersections. I am concerned the transmission will fail completely
... while driving, resulting in a sudden loss of motive power and increasing the risk of a crash. Toyota has covered this exact safety defect in 2017–2019 models (CSP ZJC) but has failed to recall the 2021 models despite identical symptoms. First reared it's head as slipping when putting vehicle in reverse, 25k+ ago. Was told by Toyota Certified mechanics it was an operator error. Morphed into larger issue.Read more

Sporadically, when shifting from first to second gear, the engine/transmission will stall, causing a loss of power for several seconds. This is generally when pulling away from a stop sign/light into traffic. I picked a single date below, but this continually happens from time to time.
