The vehicle was then towed to an independent mechanic, where it was diagnosed and determined that the transmission needed to be rebuilt, and that the ABS system was defective and needed to be replaced. The transmission was replaced but the ABS system was not replaced because the part was discontinued by the manufacturer. The manufacturer was notified of the failure. The transmission is an automatic and stopped going in the drive gear on the highway. Diagnostic scans from both the dealership and an independent mechanic repeatedly show: P1578 — Brake System Malfunction P265B — Rocker Arm Actuator Position Sensor Circuit Range/Performance (Bank 1) Both codes reappear even after being cleared. The vehicle was with Loving Toyota Lufkin, TX multiple times over seven months. They received over $7,100 in insurance payments for repairs, $463 towing cost (due to the negligence of Loving Toyota) and almost $1,500 in rental cars (Paid by State Farm) but were unable to diagnose or fix the underlying failure. Loving Toyota also provided a courtesy rental for 7+ weeks while they did no repairs to the car. The safety warnings continue to occur. Toyota Corporate is now involved but the vehicle remains unsafe to drive. The same code P265B is present that was present back in April 2025. This car is not safe! ... in GM Special Coverage Bulletin N192271110 (pulley damage in 2014–2015 Spark CVT). Safety risk: Complete and unexpected loss of propulsion created an immediate safety hazard. The vehicle could not accelerate from a stop or maintain speed, placing my 15-year-old son and other motorists at risk of rear-end collision or stranding in active traffic lanes.Dealer confirmation: RK Chevrolet in Virginia Beach, VA has inspected the vehicle and confirmed the failure matches N192271110 – full transmission replacement required (quote $6,637.19). Vehicle remains at the dealership and is available for inspection.Warning lamps/symptoms: Check-engine light and reduced-power message appeared at time of failure. No prior warnings before the sudden loss of power event. My Jeep will accelerate forward when breaking, stall at a red light and roll back, massive humming and grinding on the peddle, start/stop failure and failure to start. I've been able to avoid two crashes so far and used my own scanner to detect the problem. Nothing comes up. I bought this shady vehicle oct/3/25. dates of incidents include oct 3rd- November 10th.

Car intermittently will studder and not go into first gear or go into second gear dangerously having car bog down and almost stall and then it violently downshifts having car RMP Hague go very high then slip and have no power to wheels and then a few seconds later go into second gear violently taking off again. The ICCU (Integrated Charge Control Unit) failed. The car was in a Hyundai dealer repair shop on November 18, 2025, for an unrelated regular maintenance procedure when (i) the technician noticed a logged ICCU error code generated sometime prior to the shop visit and (ii) after the technician cleared that prior error a second new ICCU error code was generated. The shop replaced the defective ICCU.

This car had been serviced on prior occasions to install updated ICCU software, most recently as part of NHTSA's recall 24V868000 of November 18, 2024 (also known as Hyundai Safety Recall 272), intended to eliminate such ICCU failures. Installation of that software as the remedy for recall 24V868000 clearly did not prevent the very ICCU failure it was intended to guard against. Please note also that it is common knowledge (as disseminated on various Ioniq 5 forums and Reddit groups) that large numbers of later Ioniq 5 vehicles with newer VINs not included in recall 24V868000, i.e., from model year 2025 onward, have also suffered from these failures despite being initially placed into service with the updated ICCU software, as well as many other pre-2025 remedied vehicles. The Hyundai ICCU software currently in operation on both remedied cars from recall 24V868000 as well as newer cars not subject to that recall is clearly entirely ineffective as a solution to this very serious safety issue. I strongly urge NHTSA to (i) declare the existing ICCU software to be an ineffective remedy for recall 24V868000, (ii) include all Hyundai Ioniq 5 cars for all model years, whether running current ICCU software or not, to be part of the group of unremedied and unsafe vehicles, and (iii) extend recall 24V868000 until such time as Hyundai can actually demonstrate that an effective remedy exists and is being made available to all Ioniq 5 vehicles.Read more

Borescope inspection of the engine shows multiple signs of internal mechanical failure involving oil control, piston ring integrity, coolant intrusion, and cylinder wall damage. Excessive oil buildup is visible above the oil control ring, far beyond what is expected at 59k miles. Cylinder walls show heavy polishing where cross-hatching should be visible, indicating loss of lubrication and abnormal
... wear. Carbon deposits are extremely thick; in several areas carbon has flaked off and been pulled downward into the cylinder, creating scoring. Straight vertical scoring marks cut through the normal diagonal honing pattern, confirming blow-by and damage to the cylinder sleeves. Evidence of coolant intrusion is present, including orange discoloration consistent with rust/coolant contamination, excessive moisture, and steam-cleaned areas of the combustion chamber. The intake valve and combustion surfaces appear abnormally clean, consistent with coolant steam-washing. This suggests head-gasket failure, a block or head casting defect, or ring damage (possible cracked scraper ring). Coolant in the cylinders can wash away oil from the bore, leading to rusted pistons and accelerated scoring. Multiple pictures show severe scoring and loss of lubrication. This compromises the ability of new pistons or rings to seal, meaning the cylinder walls would continue to allow blow-by, oil burning, coolant intrusion, and friction. Even attempting to polish or recondition these bores would enlarge them, causing piston flutter, noise, and long-term failure. Based on the level of damage, the engine is no longer safe or reliable. A seized piston could cause sudden loss of power at highway speeds, creating a safety risk. The observed defects indicate a potential manufacturing or casting issue that may affect other engines. The dealership and KIA Consumer Affairs have been notified but there has been no replacement or resolution after 3 months of ongoing issues, and 8 weeks of holding my vehicle.Read more

... overheat and/or illuminated malfunction indicator (MIL). This may be due to a crack in the exhaust heat exchanger coolant spigots on the top of the exhaust heat exchanger. The exhaust heat exchanger coolant spigots are now released and should be serviced in pairs. It is no longer necessary to replace the muffler inlet pipe assembly for this type of concern. Workshop Manual, Section 309-00D has been updated to include a new service procedure. Stellantis is only offering reimbursement for a rental, which I cannot access because I cannot afford to pay for a rental vehicle upfront. Reimbursement for what could be an unlimited or months-long period of rental fees is not a reasonable or accessible remedy. This leaves me without any safe or practical transportation option. I have now been without transportation for over 48 hours, and the manufacturer is not providing a reasonable remedy or safe transportation while the recall repair remains unavailable. I am filing this complaint so NHTSA can document the delay, the lack of an accessible remedy, and the manufacturer’s failure to provide a safe, timely solution for an undrivable recall-affected vehicle.
