... aking any turns at 90 degrees or greater as the steering feels insecure and sounds indicate possible hazard/parts failure during vehicle operation. Almost all turns feel as though they are pushing the vehicle to the limit and I fear the parts/joints will fail while the vehicle is in operation. U-Turns are nearly out of the question. If a vehicle cannot safely and effectively make a greater degree of turn, then the turning radius should not allow a driver to put the vehicle into that position. However, for this vehicle, it seems almost any turning at all would then be completely impossible. NHTSA’s VIN decoder also returned an “ambiguous model year” notice, suggesting this vehicle may have been built during the transition period where Ford continued using the same defective 10R140 components. Symptoms experienced prior to failure included harsh shifting, delayed engagement, gear slipping, and eventual loss of drive. A transmission shop stated they “won’t touch these transmissions” due to known issues. The failure created a serious safety risk due to sudden loss of power while driving. This appears to be a recall scope error. Early 2021 units like mine are experiencing the same 10R140 internal failures as the recalled 2020 population. I request NHTSA investigate whether Ford under-reported the affected VIN range and whether the recall should be expanded to include early-2021 F-250 trucks. 10R140 transmission failure on a 2021 Ford F-250. ... ended warranty they sold me. To justify selling me a six-year extended warranty through NESNA, HGreg Nissan hid the fact that the manufacturer warranties were still in effect. They also added three additional warranties I explicitly said I did not want. All documents were signed on a tablet, and I was never given physical copies. The USB they provided did not include any warranty information. I repeatedly requested the warranty documents and received no response. NESNA later confirmed that the extended warranty could not be activated because the manufacturer warranty was still active. HGreg Nissan also failed to disclose that the vehicle had been part of a commercial fleet in California, was auctioned, retitled, and then sold to me as if it were a clean personal-use vehicle. Since day one, the vehicle has experienced serious and repeated failures: electrical system malfunctions, powertrain warnings, brake system faults, vacuum pump failures, sudden loss of power, reduced speed mode, and spontaneous shutdowns. The vehicle is unsafe and is currently completely inoperable. HGreg Nissan sold me a defective vehicle, concealed critical history, misrepresented warranty coverage, added unwanted contracts, and refused to provide documentation or support. Over the past nine months, the vehicle has again developed severe electrical faults while driving: sudden loss of power steering assist, all dashboard lights illuminating, windshield wipers activating spontaneously, and the engine failing to shut off using the Start/Stop button. The vehicle has also stalled without warning while driving in traffic or idling at red lights, restarting only after 5–10 minutes and blocking traffic lanes, placing me and surrounding drivers at significant risk. Two major repairs in the last two weeks failed to resolve the defects: (1) repair of the CAN-bus system after the vehicle would not start and acted erratically while driving, and a week later (2) replacement of the brake-booster vacuum pump, which was shorted internally and caused CAN-bus shutdown, complete steering loss, sudden braking, and no engine power. The vehicle is currently at Dependable Chrysler Dodge Jeep Ram, Canoga Park, CA, awaiting further diagnosis despite repeated repair attempts. These symptoms match Chrysler TSBs 08-007-17 and 08-033-21 concerning Body Control Module water intrusion and communication faults. This is a known defect documented by Tesla in Service Bulletin SB-21-39-001, which describes excessive front drive unit half-shaft vibration during acceleration and defines the repair with updated components. The latest revision of this bulletin (R3, dated January 19, 2024) explicitly adds “Customer Pay billing” instructions. Despite this, my local Tesla Service Center in Tempe, AZ (Supervisor: Lewis Holguin) refused to apply the SB-21-39-001 Customer Pay billing and instead charged full retail for the repair. On 11/28/2025, I was billed $1,694.86 out of pocket to replace both front half-shafts again (invoice shows approx. $1,046 in parts and $564 in labor). I have documentation showing that other Tesla customers with the same defect were billed only $350 under SB-21-39-001, which demonstrates that Tesla does apply a discounted bulletin rate in some cases. In my case, they refused to honor it even though this is a repeat failure of the same known defect and the prior warranty repairs did not permanently fix the problem. This is a repeat driveline/suspension defect that affects drivability and safety under acceleration. The shaking and vibration make it difficult to maintain stable vehicle control, especially during highway merging or lane changes. I am requesting that NHTSA investigate recurring front half-shaft failures and vibration/shudder issues on Tesla Model X vehicles and Tesla’s inconsistent handling of this known defect and service bulletin.Read more

The vehicle was driven to the dealer to be diagnosed; however, the vehicle remained at the dealer. The manufacturer was notified of the failure. The contact related the failure to Technical Service Bulletin: 21-009-25. The manufacturer suggested that the vehicle be taken to the dealer for an inspection, and depending on the findings, the manufacturer might assist with the cost of the repair. On several occasions I have a dead pedal situation on acceleration from a brief stop. When you press the accelerator, the truck does not accelerate immediately. Instead there is a second or two pause before truck responds to accelerator input. When crossing traffic, this hesitation has caused several situations in which i have almost been t-boned.

Just bought a 2017 Nissan Altima SR. Took it for a ride. Attempted to merge into traffic and the car had zero acceleration once I merged and then the transmission seemed to completely disengage. The RPMs week to around 3500 so I quickly had to pull off so as not to get rear ended. Brought the car to a stop and attempted several times to push the gas but the car would not pick up any momentum.

Back to the dealer to have checked, dealer found that the head gasket during manufacture/casting has defect causing the head gasket to protrude into the mounting surface for the timing cover gasket/seal causing improper seal and securement. New head gasket being replaced and installed inspection of all mounting surfaces on cylinders valves and pistons for additional damage, and replacing timing cover gasket again. This caused loss of engine oil over time even after dealer fix and replacement the first time, this could cause drivers to not know they are low on engine oil causing potential catastrophic engine damage or failure/seizing during normal operating conditions. This is a common failure in the Jeep 4xe of timing cover seal issues and leaks.Read more

I am reporting a serious defect involving my 2017 Kia Sorento 2.4 Liter GDI engine (VIN [XXX] ) At approximately 84000 miles, while still under the powertrain warranty, I brought the vehicle to the dealership from which I purchased it (Hutchinson Kia in Macon GA) due to engine hesitation and abnormal performance. ... the issue was only a relay/software problem and performed a software update (likely the KSDS knock sensor update) At approximately 94000 miles, the engine suffered a SUDDEN CATASTROPHIC FAILURE while driving on the interstate, The failure was abrupt and created a dangerous situation in traffic. The Vehicle is now inoperable. The engine is equipped with the THETA II GDI engine, which has a documented history of bearing and connecting -rod failures, fires and sudden engine seizure. KIA is refusing to cover the failure, even though the symptoms began during warrantly and the KSDS update was reportedly completed. This appears to be consistent with the widespread THETA II engine safety defect previously investigated by NHTSA. The sudden failure of the engine posed a significant safety risk to myself, passengers and other motorists. 2020 Jeep Cherokee Power Transfer Unit (PTU) Per a discussion the dealer service manager, I realized the vehicle is undrivable because the PTU could fail at anytime. The danger is losing propulsion to the wheels on an interstate or other high speed or high capacity road. The problem was discovered by the dealer when I took my Cherokee in on 11/24/25 to have them fix the "SVC 4WD" warning light.
... Once they explained the issue I decided to leave the vehicle at the dealer to replace the PTU. There is no timeframe for them to receive a new PTU. Jeep had recalled 2017-2019 Cherokees due to issues with the PTU. The 2020 Cherokees should also be recalled so owners don't have to pay for an issue Jeep is aware of with the power train.Read more

Instead, they are requiring me to pay for the repair out-of-pocket, which would not have been necessary if the damage had not been caused by the defective clutch slave cylinder. I believe this is unfair and unsafe, as the failure of a clutch slave cylinder directly impacts the drivability and safety of the vehicle. This is a significant safety issue. A defective clutch slave cylinder can lead to sudden clutch failure, loss of vehicle control, and increased risk of accidents. This is not a wear-and-tear issue but a manufacturing defect that compromises the safety of the vehicle. I respectfully request that NHTSA investigate this matter to determine whether similar failures are occurring in other Toyota Corolla vehicles of this model year. They later stated to a regulator that the vehicle was “trickle charged at 1 amp for 6 hours,” but the vehicle’s range increased by approximately 100 miles, which is not physically possible at that charging current (1 amp at 240V for 6 hours = 1.44 kWh ≈ 4–5 miles). This discrepancy suggests the vehicle may have been charged at a higher rate than claimed and without proper cooling, despite the documented A/C failure that is necessary for battery thermal management. Battery condition is unknown because the manufacturer refused to perform or provide a battery health analysis, isolation resistance values, thermal logs, or charging session data. The safety defect could not be verified as repaired. While this safety issue remained unresolved, the manufacturer rejected a Tesla roadside tow arranged under regulator instruction to second service center and instead arranged an unauthorized tow to remove the vehicle from their facility without my consent. The vehicle was taken to a non-manufacturer storage lot while still in an unsafe state. I am concerned about: • high-voltage isolation faults, • shutdown risk during driving, • unsafe charging performed by the manufacturer, • possible battery damage from low SOC and charging without A/C cooling, and • mishandling of an active safety defect. This resulted in complete oil loss and catastrophic engine damage due to oil starvation. The dealer confirmed the failure and determined that the issue originated from the turbocharger assembly and not from misuse or neglect. The failure occurred suddenly and without any oil-pressure warning to alert the driver. The safety concern is that a turbocharger failure caused total loss of engine oil without triggering any oil-pressure warning or driver alert, creating a risk of sudden engine failure, loss of power, or engine seizure while driving. My 2021 Kia Seltos is experiencing engine problems that match the piston-ring defect involved in Kia Recall SC336. I first took the vehicle to the dealership in August 2025 for excessive oil consumption and engine knocking. The vehicle is now consuming more oil than before, the knocking/ticking noises have become louder and more frequent, and fuel economy has declined. I am adding oil between changes to keep the engine from running dangerously low. There are no external leaks, so the oil is being burned internally, which is consistent with piston-ring failure. These symptoms are identical to other Seltos vehicles already included in the recall, yet my VIN is not currently part of it. The progression of engine noise and oil consumption raises serious safety concerns, including potential engine failure, loss of power, or stalling while driving.
