MY 2020 5.3 L GMC AT4 IS EXPERIENCING LOSS OF POWER WITH BUCKING & STALLING. CYLINDERS ARE NOT ACTIVATING WHEN CALLED CHECK ENGINE LIGHT IS FLASHING, IT STARTS ROUGH AND MAKES NUMEROUS NOISES AT VARIOUS SPEEDS, AS WELL AS AT IDLE. HE WARNED ME THAT FURTHER USE OF MY MOTOR (WITH ONLY 42K MILES ON IT) WILL CAUSE A CATASTROPHIC FAILURE. MY RESEARCH SHOWS THAT THESE ISSUES ARE BECOMING COMMON OF 5.3 GMC MOTORS. WE SEARCHED THE COUNTRY FOR A REBUILT MOTOR AND FOUND THAT NONE ARE AVAILABLE FOR THE NEXT SEVERAL MONTHS. HOWEVER, I WAS ABLE TO FIND A NEW MOTOR AT MUCH GREATER COST. GMC HAS DECLINED ASSISTANCE AT THIS TIME. MY VEHICLE HAS NO VALUE AS IS, SO I ORDERED THE NEW GM ENGINE. WITHOUT THE DOD SYSTEM, THESE ENGINES WERE DESIGNED TO GO WELL OVER 100,000 MILES. I AM A FAITHFUL GMC OWNER AND NONE OF MY PREVIOUS GMC TRUCKS (2003, 2013, 2015, 2019) HAD SUCH A MAJOR PROBLEM. NOW THE 2020'S FUEL SAVING SYSTEM HAS FAILED AND REQUIRES REPAIRS THAT AMOUNT TO HALF THE VALUE OF MY TRUCK. WHEN A FULL SIZE TRUCK STALLS ON THE HIGHWAY IT'S CERTAINLY LIFE THREATENING.Read more

While driving my 2022 Ford Ranger, the transmission failed to engage Drive. I started my morning as usual, letting the vehicle warm up and the RPMs settle before leaving my neighborhood. As I pulled onto the main road, the truck began acting strangely—I noticed it wasn’t accelerating normally and felt constant gear shifting without gaining speed. To avoid blocking traffic, I pulled over at a l

I turned the vehicle off for about a minute and restarted it, but the same issue occurred. I then called my local dealer, who suggested using Sport mode and manually selecting gear 2. This worked, and I was able to drive home safely since I was only about a minute away. After that, I contacted the warranty company and arranged for a tow. Component/System: Transmission Safety Risk: Loss of acceleration on a main road created a risk of collision with other vehicles. My 2013 Chevrolet Equinox (~132,000 miles) has a serious and unsafe oil consumption problem. I had a full oil change, and only two weeks later the dipstick was completely dry. The vehicle provided no warning lights, no low-oil indication, and the GM Oil Life Monitoring System gave an inaccurate reading, failing to alert me to the critically low oil level. On November 25, 2025, while stopped with
... my foot firmly on the brake, the vehicle began constantly jumping and jerking forward as if it was trying to move on its own. During driving, I also began hearing a loud crunching, metal-on-metal noise whenever I accelerated or drove under 50 mph. Because these symptoms started suddenly and were severe, I took the vehicle to my mechanic the same day. On November 26, 2025, the mechanic confirmed that the engine had suffered major internal damage due to running with almost no oil. The timing chain and VVT components needed to be replaced. The mechanic confirmed that oil starvation caused this failure despite a recent oil change, and the inaccurate oil life monitoring system contributed to the hazard by failing to alert me to the critically low oil. This defect presents a significant safety hazard, as the vehicle can jerk unexpectedly, stall, lose power, or suffer sudden engine failure while driving, all without warning. I have since learned that excessive oil consumption and inaccurate oil life readings are widespread in GM 2.4L Ecotec engines. I am requesting that NHTSA investigate this safety-related defect.Read more

I am reporting a serious safety and reliability concern with my 2025 Hyundai Tucson. At only about 2,000 miles, I was informed by the Hyundai dealership that the transmission has already failed and needs to be replaced. This vehicle is essentially brand new and should not be experiencing a major powertrain failure so early in its lifespan. The transmission issue has caused significant concern reg
... arding the safety of the vehicle. Sudden failure of a major drivetrain component creates an increased risk of loss of power, unpredictable drivability, and unsafe operation in traffic. This is especially concerning because this vehicle was already given to me as a replacement for a previous Hyundai Tucson that qualified under my state’s lemon law. A transmission failure at such a low mileage indicates a possible manufacturing defect or a systemic quality issue that could affect other consumers. I am requesting that NHTSA investigate this matter to determine if this is part of a broader problem in the 2025 Hyundai Tucson model line. This situation is unacceptable for a vehicle this new, and it raises serious concerns about long term safety and reliability. I am seeking further review and action so that other consumers are not placed in similar unsafe situations.Read more

I am submitting this complaint to report a significant safety defect in my Lexus vehicle. Without any prior warning or history of mechanical problems, the vehicle experienced a sudden and severe loss of engine power while I was merging onto a busy highway. The vehicle rapidly decelerated despite continued acceleration input, forcing me to navigate to the shoulder while surrounding traffic was trav
... eling at high speed. This unexpected loss of power created an immediate and substantial risk of a collision and posed a serious danger to myself and other motorists. Following this incident, I took the vehicle to a local auto shop, which identified diagnostic code P030002, indicating a misfire in Cylinder 2. I then promptly brought the vehicle to an authorized Lexus dealership. Their diagnostic confirmed the same code and attributed the failure to a “Cylinder 2 direct fuel injector leaking.” Lexus has already acknowledged this defect in TSB L-SB-0007-21, demonstrating that the manufacturer is aware of this recurring issue. My vehicle is five years old with fewer than 100,000 miles, yet this defect resulted in a dangerous loss of engine power under highway conditions. The dealership quoted over $3,000 to replace a single injector, and the remaining injectors may fail at any time, further compounding the safety risk. Despite its knowledge of this defect, Lexus has not issued a recall, implemented preventive measures, or offered assistance, citing that the vehicle is slightly outside the 70,000-mile warranty. I have therefore been left with a vehicle that is unsafe for continued highway operation. Given the severity of the incident, the known recurrence of this defect, and the manufacturer’s lack of corrective action, I respectfully request that NHTSA investigate this matter, as it presents a substantial and ongoing hazard to consumers and the driving public.Read more

THIS IS A RECALL REMEDY FAILURE REPORT UNDER 49 U.S.C. §30120. My 2020 Chevrolet Silverado 1500 (VIN: [XXX] ) received Recall 24V797000 on June 9, 2025. The ECM update is intended to detect incipient clutch-pressure or gear-ratio deviations—early signs of a developing slip—and automatically force limp mode to prevent rear-wheel lockup. On August 23, 2025, after the recall was performed, the
... truck experienced rear-wheel lockup at highway speed. Limp mode did not activate, which shows the recall remedy failed to operate as required. Mangino Chevrolet documented symptoms matching the recall defect, including abnormal ratio behavior. No metal debris was found in the pan, indicating the failure was not caused by wear. GM TAC also noted behavior consistent with a developing slip. GM first recommended full transmission replacement at my expense before acknowledging the recall. GM later authorized a valve-body replacement, which also failed. The dealership owner provided a written statement confirming that: (1) the recall remedy did not correct the defect, and (2) the valve-body repair did not correct it. On December 3, 2025, GM Customer Assistance attempted to close my case as “not recall-related,” despite the post-remedy lockup, limp-mode failure, dealer findings, and the written admission that the recall remedy was ineffective. The safety defect persists after recall completion, and GM has not provided a remedy that corrects it, as required under federal law. I am reporting this as a failed recall remedy and an ongoing safety defect. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

I am reporting a recurring safety defect involving hydraulic lifter failure in Ram trucks equipped with the Cummins diesel engine (model years 2019–2024). This issue is widely acknowledged by Stellantis and multiple dealers. It typically begins with a knocking sound and has been documented in many cases to progress to complete engine failure. My vehicle has fewer than 20,000 miles. The lifters
... were already replaced once, which required the cab to be removed. Within less than 400 miles of that repair, the knocking sound has returned. I brought the truck to a second dealer, who confirmed the problem and informed me that Stellantis has an active STAR case acknowledging the defect. They stated there is currently no known permanent fix. This defect presents a serious safety concern. Sudden engine failure while driving, especially under load or at highway speeds, could lead to loss of power, loss of steering/braking assist, and potential accidents. In my situation, I frequently travel with family members who have medical conditions, and being stranded in hazardous or remote conditions poses significant risk. At this point, my vehicle is not reliable or safe to operate. I am requesting NHTSA investigate the hydraulic lifter defect in 2019–2024 Ram Cummins trucks due to the safety implications and the lack of an available permanent repair.Read more

I took my vehicle (2017 Honda pilot) to a Honda dealer in Selma, TX to have a pre-road trip checkup. They performed mileage related maintenance and assured me the vehicle was ready for my 2200 mile road trip. 600 miles into the trip, I stopped for gas. As I put the car in park, I received multiple errors on the driver's info panel. "1.Emissions system problem. 2.Transmission system problem. 3.Stop
... driving when safe. Transmission problem. See manual." Without any warning there was complete and sudden failure of the transmission. The car is available for inspection at a Honda dealer in Little Rock, AR where they ran diagnostics and found a list of problems. Apparently the Honda dealer in TX saw at least one of the errors and deleted it without informing me. If the transmission had failed while I was driving on the highway, average speeds of 70 mph, it could have caused a fatal accident. Right now the failed transmission has only been inspected by the Honda service dept in Little Rock, AR. There were no warnings from the vehicle to the driver prior to the complete and sudden failure whatsoever. After the failure the power train was locked in park. Drive, reverse, and neutral could not be engaged. I understand the mileage on the vehicle is high, but it is a grave safety concern when a major component of a vehicle can fail without reason or warning.Read more

The AWD light came on while the vehicle was in normal operation on October 25, 2025. Two days later, a diagnostic service was done at Bay Ridge Honda (Bay Ridge Honda did not previously worked on my vehicle), and it was determined that the rear differential on my vehicle failed. A differential is quite durable and does not fail easily. The mechanic who performed the diagnostic, stated that it look
... ed like someone tried to repair it. In addition, an independent mechanic confirmed that the damage occurred as a result of improper service at a Honda dealership. Because of the failure, I also noticed the rear of the vehicle seemed to be dragging. The failure occurred after the vehicle had been serviced at authorized Honda dealerships, Plaza Honda and Middletown Honda. Those are the only two places where routine and repair services have been performed on my vehicle. This created a safety hazard because a failed differential can lead to loss of vehicle control, putting the safety and lives of others and myself at risk. I'm concerned that improper workmanship caused the failure. Since I don't know which dealership is responsible, I reached out to American Honda Motor Co, Inc. for assistance, but they have not provided a satisfactory resolution and insisted that I resolve the issue with both dealerships. I'm requesting that NHTSA investigate and document if improper servicing contributed to the differential failure.Read more

Transmission failed, Ford diagnosed it as needing the torque converter replaced, however, after this was fixed at a transmission shop, the problem persisted and that mechanic said the transmission needs to be rebuilt. My safety was out at risk when my vehicle stalled at an intersection and would not accelerate. Also, on two occasions, when the vehicle was put into reverse, it jerked forward and
... did not go backwards. Luckily nothing was hit in front of my vehicle, although it could have due to being in a parking spot when theirs happened. Ford confirmed an issue on 11/3/25 and another independent mechanic replaced the converter, but stayed the transmission needs to be rebuilt after that did not correct the problem. He states that the torque converter would not cause the reverse issues. Initially, no warning lamps came on, but now the wrench light and check engine light are on. Ford app gave two errors, one on 11/13/25: Powertrain malfunction/reduced power What Is Happening? The transmission control module has detected a concern when trying to shift to 8th gear. A failsafe logic disables 7th gear for the remainder of this key cycle. What Should I Do? Have your vehicle checked by an authorized dealer as soon as possible And one on 11/21/25 Powertrain Malfunction/Reduced Power: What Is Happening? The powertrain control system has detected one of the transmission clutches did not apply within the expected time as expected. Some transmission gears may not being available resulting in delayed or harsh shifts into another gear. Additional alerts that may provide additional information may accompany this alert. What Should I Do? Have your vehicle checked by an authorized dealer as soon as possible. Vehicle already had Powertrain issues last year when transaxle assembly was replaced and there was an issue with the exhaust, which ended up falling off the engine after Ford worked on it. At approximately 33,188 miles, an “AWD System Problem” warning appeared on the instrument cluster of my 2019 Honda CR‑V AWD. Darrell Waltrip Honda in Nashville, TN scanned the vehicle and retrieved DTC C1851‑1E, which indicates a rear differential pump motor high‑current fault. The dealer misinterpreted this code and advised that the AWD control unit (Part No. 48310‑5TG‑H52) needed r
... eplacement, but stated the vehicle was safe to drive so repairs could be completed at my home dealership in Lexington, KY. At Don Jacobs Honda in Lexington, KY, the same DTC C1851‑1E was correctly interpreted as a failure of the rear differential pump motor, which was then replaced under warranty. No AWD control unit replacement was required. Honda Service Bulletin 23‑031 (“AWD Light ON: DTC C1851‑1E / DTC C1851‑18 Stored”) documents this rear differential pump motor issue for 2020–2021 CR‑V AWD models. My vehicle is a 2019 CR‑V AWD with the same DTC and failure mode, suggesting that this defect is not limited to 2020–2021 model years. I am submitting this complaint so that NHTSA is aware of this similar failure on a 2019 vehicle and can evaluate whether the affected model‑year range should be expanded.Read more
