There is a known issue with the transmission in my 2017 Toyota Highlander that causes premature failure. This is noted in a Toyota Customer Support Program Bulletin. The issues symptoms are the same as those my car has: a whining noise from the transmission while driving, harsh shifting, reduced power, and master warning light/check engine light illumination. I have contacted Toyota and explained
... this to them, but they have told me that my car's VIN number does not match the identified vehicles. I am reaching out as I believe there are vehicles with this problem that Toyota has not identified, and it could be a dangerous issue.Read more

The vehicle has experienced repeated transmission-related failures beginning at approximately 5,000 miles. The dashboard warning “Transmission System Error – Check Transmission Oil Immediately” first appeared while driving and was accompanied by sudden loss of power. When this occurs, the vehicle loses acceleration and drivability while in motion, creating a safety risk in traffic. The only
... way to temporarily clear the warning is to pull over, shut the vehicle off, and restart it. Photographs of the warning message were documented at approximately 6,738 miles on [XXX] and again at approximately 12,664 miles on [XXX]. The vehicle was returned to the selling dealer in Massachusetts multiple times for the same issue. During the first visit, the dealer drove the vehicle approximately 19 miles but stated they could not duplicate the problem. During the second visit on or about 3/25/2025, the dealer kept the vehicle for approximately six days while providing a loaner vehicle, but again stated they could not duplicate the condition. The vehicle was later inspected by another Hyundai dealer in New Hampshire on 8/19/2025 for the same dashboard warning and loss of power condition while driving. That dealer found the transmission fluid level below specification but stated they could not duplicate the warning message during inspection. Following that visit, the transmission warning did not reappear until [XXX], when the same “Transmission System Error – Check Transmission Oil Immediately” warning returned again while driving. On [XXX], the situation escalated into a possible rollaway event. While at an auto detailing facility, the vehicle was reportedly placed in Park with the engine running after being repositioned. The operator stated the vehicle indicated it was in Park. Moments later, after the operator exited the vehicle, the vehicle moved from its parked position and rolled into a building. The event was documented in writing by the detailing facility. INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ARead more

Dealership admitted that they don't check fuse boxes and cables if the lights on the dash aren't illuminated. This is a huge failure of safety. All is on a recorded line from their phonelines from the date range of 4/27/26-5/2/26. All is notated in documentation and photographic evidence. Finally settled and they bought the vehicle back on [XXX] and assured me it would be sent back with the correct salvage title, and it is currently out for resale on their lot as of today 5/24/26. They are fully aware of all the issues. The SRS,ABS, HVAC 40 Relays, and the Traction control system have all been disengaged/compromised to bypass the system of any issues ongoing. The middle row seat isn't secured down, and the airbags have been entirely removed. [XXX] Above is the website listing for this vehicle. The carfax is not updated to the correct report. I am filing a safety complaint regarding my 2014 Ford F-150 XLT equipped with the 6R80 transmission. My vehicle previously received Recall 24S37 in 2024 for the known transmission downshift safety defect involving Output Shaft Speed (OSS) sensor signal loss. Despite completion of the recall repair, my vehicle is now exhibiting the same dangerous symptoms associated with the recall condition, incl
... uding: • Sudden erratic transmission behavior• Speedometer intermittently dropping to zero while driving• Wrench warning light activation• Check engine warning light• Airbag warning light• Unsafe drivability conditions and intermittent limp mode operation These symptoms create a serious safety concern because the vehicle unexpectedly changes driving behavior and loses speed signal information while operating on public roads. I presented the vehicle to Dick Smith Ford in Columbia, South Carolina for evaluation and was informed the truck requires a complete transmission replacement estimated at approximately $11,000. After extensive research, I discovered many Ford truck owners nationwide reporting nearly identical symptoms involving the 6R80 transmission, molded lead frame failures, OSS sensor failures, and unintended downshifting conditions. Ford previously acknowledged similar issues in Recall 24S37 and Customer Satisfaction Program 19N01. I am concerned the original recall remedy may not fully address the underlying hardware failure associated with this transmission system. My vehicle is demonstrating the same safety-related symptoms after the recall repair was already completed. Vehicle Information:2014 Ford F-150 XLTVIN: [XXX] I respectfully request NHTSA review this issue for continued safety concerns involving the Ford 6R80 transmission system and post-recall failures associated with Recall 24S37. While driving my 2023 BMW i4, the vehicle suddenly displayed multiple drivetrain and chassis malfunction warnings shortly after charging the vehicle at a ChargePoint station. The vehicle experienced a sudden loss of power/propulsion while driving, creating a serious safety concern. After stopping, the vehicle began rolling backward unexpectedly and several systems appeared to malfunction. The cen
... ter display/screens became partially or fully unresponsive/blank and multiple warning messages appeared simultaneously. Due to the loss of propulsion and safety concerns, CHP assistance was required to safely move the vehicle off the roadway. The vehicle was later towed to BMW service for inspection and diagnosis. I am concerned this may involve a failure related to the electrical system, high-voltage battery system, propulsion system, drivetrain, or vehicle software. The sudden loss of power while operating the vehicle created a dangerous driving condition. The vehicle had approximately 76,000 miles at the time of the incident.Read more

I own a 2019 BMW X5 xDrive40i with the B58 3.0L turbocharged inline-six engine. While driving in Minnesota, the vehicle suddenly displayed an urgent warning stating “oil pressure too low” and instructed me to stop the engine. This occurred while the vehicle was being operated on the road, requiring me to stop driving immediately. The vehicle was later inspected by an authorized BMW dealer. Th
... e dealer diagnosed the failure as a defective oil pump and stated that the oil pump must be replaced. The estimated repair cost was approximately $7,500 or more. This is a serious safety concern because a sudden loss of engine oil pressure can lead to engine seizure, loss of motive power, and loss of vehicle control risk depending on traffic conditions. The oil pump is a critical lubrication component and should not fail as a normal wear item. I have found numerous similar owner reports involving BMW B58 oil pump failures, including reports suggesting that cold-weather operation may contribute to or expose the failure. My vehicle is operated in Minnesota, a cold-weather state. The vehicle was inspected by Park Place BMW in Rochester, MN. I am requesting that NHTSA investigate whether this is a safety-related defect affecting 2019 BMW X5 xDrive40i vehicles and other BMW models equipped with the B58 engine.Read more

A major drivetrain service and repair was completed by an authorized manufacturer dealership. After picking up the vehicle, an unusual front-end/steering noise was immediately reported to the staff. Over just 390 miles of driving, the symptom worsened. A subsequent formal inspection by a separate, independent authorized service center confirmed the root cause of the safety failure: the primary axl
... e nut had been left completely unstaked during the initial drivetrain assembly, rendering the vehicle entirely unsafe to operate. The vehicle was grounded to prevent an accident. Because the mandatory safety protocol of staking the axle nut was omitted, the critical fastening hardware was allowed to back off under normal driving conditions. This component failure directly compromised the structural integrity of the front drivetrain, steering knuckle, and wheel hub assembly. Had the vehicle continued to be driven, the omission presented an imminent risk of complete mechanical separation, which could cause a wheel assembly to detach or lock up while in motion at high speeds, putting the safety of the occupants and other motorists at extreme risk. The vehicle remains grounded and is available for inspection. The safety-critical assembly failure has been fully documented by the inspecting service center via technical video (available upon request) and photographic evidence. The manufacturer has been notified of the dealer workmanship defect but has failed to authorize corrective action or safe transport of the vehicle to a certified facility for remediation.Read more

Vehicle experienced sudden severe loss of motive power consistent with Ford Safety Recall 23S64 (NHTSA Campaign 23V905000). Prior to repair, the vehicle entered limp mode and could not accelerate beyond approximately 30 to 35 MPH. Low oil pressure warning indicators and check engine light illuminated repeatedly. Engine produced abnormal grinding and strained startup noises. Vehicle also experience
... d hesitation, shaking, and difficulty maintaining safe roadway speeds. While I was trying stop at a red light the vehicle struggles to brake although the brakes are applied, after coming to a full stop the vehicle shutdowns and would have restart. During operation, vehicle struggled to accelerate in traffic and braking performance became concerning due to severe engine power loss and unstable operation. On multiple occasions, the engine nearly stalled or shut down completely after stopping at intersections. Vehicle was delivered to Ford dealership for Recall 23S64 repair. Dealer completed the recall repair and stated oil pressure had been restored; however, the vehicle continued exhibiting the same severe loss of power and limp mode symptoms after completion of the recall procedure. Dealer later performed additional diagnostics and identified DTC P0087 (Fuel Rail/System Pressure Too Low) and a broken intake camshaft requiring major engine repairs estimated at approximately $9,000. Dealer stated the engine damage was not covered or related under the recall despite the vehicle originally presenting with the same low oil pressure, loss of motive power, and drivability symptoms described in Recall 23S64. Vehicle remains unsafe to operate due to unresolved loss of power and internal engine damage. Concern exists that the recall related oil pressure failure may have caused consequential internal engine damage that was not fully corrected by the recall repair procedure. Problem has been confirmed by authorized Ford dealership inspection and diagnostic testing.Read more

This creates a severe safety hazard by instantly dropping vehicle velocity on active interstates and high-speed roadways, leaving the vehicle unable to accelerate out of harms way or merge safely, placing myself, my passengers, and surrounding motorists at immediate risk of a high-speed rear-end collision. The malfunction has been repeatedly confirmed and documented by authorized GM service agent Rountree Moore Chevrolet (Lake City, FL) across three separate service visits. The dealer confirmed the active trouble codes and attempted various repairs (including Transmission Control Module reprogramming on 04/24/2026, engine harness chafing repairs on 05/19/2026, and auxiliary fluid/battery adjustments on 05/21/2026). The persistence of the failure confirms the dealer has been unable to successfully remedy the defect. To date, the vehicle has only been inspected and worked on by the authorized dealership's factory-trained technicians. It has not yet been involved in a collision requiring police or insurance representative inspections, nor has a corporate field engineer from General Motors inspected it outside of the local dealership level. The failures are consistently preceded by an illumination of the dashboard Check EngineRead more

My 2022 MDX purchased new in 2021 experienced an unexpected loss of AWD functionality at 5 years old with 77000 miles when the splines of the transfer case sheared with the transmission. The failure reverted the car to FWD and became very easy to lose traction in anything but ideal road conditions. There were no signs of this failure. No warning lights, no indication on the AWD power distributi
... on monitor, and no sounds or other indications. I rigorously followed all maintenance schedules and performed all regularly scheduled maintenance at the Acura dealership. There have been numerous times when I am attempting to accelerate from a stop while turning onto a busy road where the unexpected loss in traction resulted in me nearly causing an accident. I took in my vehicle for service on 5/22/26 and they confirmed the issue is the transfer case spline failure as noted by many other Acura SH-AWD vehicles with the active NHTSA investigation. I would like to add my complaint to that filing and highlight the insidious danger that comes with the failure when the vehicle suddenly does not perform as expected. Given that my vehicle was barely behind the powertrain warranty period when the failure occured, I am now expected to cover an enormous bill that covers the replacement of the transmission and transfer case.Read more

What component or system failed or malfunctioned, and is it available for inspection upon request? How was your safety or the safety of others put at risk? The ignition coil in my 2017 Hyundai Elantra Limited has failed multiple times over the past several years. This recurring issue has caused the vehicle to malfunction unexpectedly, creating safety concerns due to reduced engine performance, mi
... sfiring, and the potential for sudden loss of power while driving, which puts both myself and others on the road at risk. The failed component is available for inspection upon request. Has the problem been reproduced or confirmed by a dealer or independent service center? This issue has occurred multiple times with my vehicle. After researching online consumer reports and owner complaints, I found that ignition coil failures appear to be a common and recurring issue reported by other 2017 Hyundai Elantra Limited owners. Based on these repeated failures and similar reports, this appears to be a known issue affecting this model. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives, or others? I have reported this issue to Hyundai multiple times. Rather than addressing the root cause or issuing a recall for this recurring defect, Hyundai has repeatedly provided credits to assist with repair costs when the ignition coil fails. Despite the repeated failures and safety concerns, the underlying issue has not been permanently resolved. Were there any warning lamps, messages, or other symptoms of the problem prior to the failure, and when did they first appear? Prior to failure, the vehicle would experience rough starts, engine misfires, and poor performance. After these symptoms began, the check engine light would eventually illuminate. Once the warning light appeared, the vehicle became increasingly unreliable and would stall or shut off unexpectedly. On one occasion, the vehicle shut off completely while I was driving.Read more

***SAFETY/DANGER*** On April 12th at around 5:30 p.m. my family of five, to include three minor children were returning home from visiting family which live about an hour and a half away and as we're traveling on the freeway at highway speeds, in traffic, our new Tahoe unexpectedly and without warning went into reduced power mode and cut speed down to approximately 35 MPH in active traffic, which
... out my family and every other car around me in danger and in jeopardy of causing a collision and even worse, a multiple car pile-up as I dodged full speed traffic in front and behind me while traveling at approximately half their speed. I managed to exit the freeway and heard a loud knocking noise from the engine and saw the reduced power mode indicator, all of which had not happened before but again, the Tahoe is relatively new and was just over 5 months and at 8,889 miles at failure. I proceeded to contact GM roadside assistance. They sent a tow truck and I had to make arrangements for my family to return home, an hour away. I made it to the dealership with the tow truck driver after 9 p.m. ***CATASTROPHIC DAMAGE**" After the dealership inspected close to a week later they did determine that the entire engine and turbocharger had to be replaced, which they did in a little over a month, the dealership did their part but confidence is lost. Now I am hesitant to drive the Tahoe as it's the family vehicle but really put my kids, wife, myself and everyone around us on the road in danger as it, out of nowhere, cut speed drastically in active highway traffic.Read more

On October 7, 2024, my vehicle was serviced at Coastal Chrysler Dodge Jeep Ram for a front-end rubbing/scrubbing noise that increased with speed. The dealer diagnosed both front hub assemblies as failed and replaced both front hub assemblies. I was charged a total of $2184.41 for the repairs, including parts and labor. In less than two years, I again experienced significant front-end noise and sa
... fety concerns. On May 19, 2026, because I urgently needed the vehicle and was unable to obtain a timely appointment with the dealership, I took the vehicle to a well-regarded independent repair facility, Koala Automotive Repair LLC. Their findings were extremely concerning. The mechanic documented that: The front wheel bearing was completely worn out. The axle had ripped due to contact with the strut assembly. Bolts on the hub bearing assembly were “very loose” and reportedly “finger loose” The vehicle owner stated that the hubs had recently been replaced by the dealership The independent mechanic further advised that the components replaced in October 2024 should still have been under dealer warranty, and that the second repair involved the same components and directly related failures. I am deeply concerned that: Components replaced by the dealer failed again prematurely; The loose hub bearing assembly bolts may indicate improper installation or insufficient torque procedures during the prior dealer repair; and The subsequent failure created additional damage to the CV axle and related front-end components. This situation caused: Significant unexpected repair expenses: I had to spend a total of $1,932.71 for the second repair. Loss of use of the vehicle. Safety concerns for myself and my passengers. I bought a 2020 Hyundai Elantra in 2024 with 67k miles on it. After driving 22k miles the check engine light went on and when it was brought in to get it checked the transmission went out. I did not buy a warranty so I understood it would not be covered. The issue I had was it was very hard to get information on the condition of the car that was sold to me. What I found out was that the transmissi
... on fluid was likley not changed ever and definetly not checked before the car was sold since the check engine light was not on and the engine sounded fine when I bought it. Which when I did meet with the service manager was informed of. The service manager noted that the IVT transmission has a long history of going out and is a bad design. He stated that most cars are not being made with it anymore and that this is well known problem. My biggest issue is that they knew this when it was sold and since the car was already over 60k when they got it should have at least checked the fluid or put in new fluid due to the history of transmission problems on the car. It is very likely this was not done to the likelihood the fluid was already bad and the transmission would have gone out so they wanted to pass it on to the customer. I know it is on me to know about a car when it is purchased but did not think of checking specifically on what type of transmission was in the car and now always will. I have had two Hyundai and both went over 200k without problem. To have one go 89k and go out was totally unexpected and there were no warnings except a check engine light. The service manager was nice but told me there was nothing that could be done.Read more

Formal Consumer Complaint Statement — GM / Crivelli Buick I am filing a formal consumer complaint against Crivelli Chevrolet Buick GMC and General Motors/Buick regarding what I believe was discriminatory and unfair treatment in connection with a warranty-covered vehicle repair. The customer is [XXX] , owner of a 2017 Buick Encore. The vehicle was serviced by Crivelli Chevrolet Buick GMC in Mt.
... Pleasant, Pennsylvania, on or about May 20, 2026, for a turbocharger failure covered under GM Special Coverage Bulletin N232395330. The vehicle had recently passed Pennsylvania safety and emissions inspection on April 30, 2026. The turbo-related diagnostic code appeared on or about [XXX]. Crivelli completed the minimum turbocharger warranty repair but then informed the customer that additional intake/manifold-related repairs were needed and would not be covered. The customer disputed this because the additional failure appeared during the same repair sequence and appears directly related to the covered turbocharger failure. A written request was faxed asking Crivelli to review the invoice, compare the repair to GM’s covered parts list, and submit the related intake/manifold issue to GM for collateral coverage, goodwill review, or warranty consideration. Crivelli did not respond to the fax notice and did not return the customer’s calls. When the customer attempted to follow up with the service department, Scott from the service department refused to review the concern, refused to provide further service, and refused to contact GM regarding the related warranty issue. Based on the conduct, tone, refusal to review the documentation, refusal to escalate to GM, and refusal to provide equal service, I believe the customer was denied fair consumer service because of race. The customer is not asking for special treatment. The customer is asking for equal treatment, proper review of GM’s own warranty/special coverage bulletin, and fair handling of a related mechanical INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ARead more
