The contact owns a 2023 Ram 1500. The contact stated that while driving approximately 6 MPH, the vehicle lost motive power, and the parking brake unexpectedly engaged. The contact manually disengaged the parking brake. The contact was able to restart the vehicle; however, a message to “Shift to Park” was displayed, and the low-battery and low-voltage warning lights were illuminated. The contac
... t stated that the failure persisted after restarting the vehicle. The vehicle was taken to an independent mechanic, but was not diagnosed. The mechanic informed the contact that the battery had not failed but that the vehicle was experiencing voltage errors. The contact was informed that the battery did not need to be replaced, and no repairs were performed. The vehicle was then taken to a dealer, but was not diagnosed; however, the dealer retrieved DTC: P032F. The dealer informed the contact that the failure could not be duplicated, but advised the contact that the battery was failing. The exact cause of the battery failure could not be determined. The dealer replaced the battery. The vehicle was later taken to another dealer, but was not diagnosed or repaired. The contact was informed that the failure could not be duplicated and that the cause of the battery failure could not be identified. The manufacturer was not made aware of the failure. The approximate failure mileage was 7,000.Read more

Clutch Slave Cylinder at 51,000 miles failed causing vehicle clutch to not return and loss of fluid. Brake fluid and clutch fluid are used in the same reservoir and could cause issues causing vehicle to not be able to stop. Vehicle has been inspected by shop and has been replicated. The part exploded and leaks fluid inside of transmission housing. This also causes the clutch to fail as-well. Vehic

After the Chrysler dealer in Glendale CA completed Recall S44 – Transmission Oil Pump on my 2016 Chrysler Town & Country in October 3 2025, the transmission began to malfunction and eventually stopped shifting completely. The dealer drained the transmission fluid, removed both front axles, removed all transmission components, and replaced the transmission oil pump, seals, bolts, and torque conve
... rter as required by the recall. The transmission is available for inspection upon request. Safety Risk: The vehicle became unsafe to drive when the transmission began shifting erratically and then failed to shift at all, causing sudden loss of acceleration in traffic and posing a risk to myself and others. Diagnostic Codes: Before the recall, the vehicle had code P06DE (Engine Oil Pressure Control). Chrysler has a documented TSB for P06DE requiring a PCM software update, but the dealer did not perform this update during the recall. After the recall, the vehicle developed new code P0760 (Shift Solenoid C Malfunction), which was not present prior to the recall and appeared only after the transmission was reassembled. Inspection and Confirmation: The dealer confirmed P0760 but refused further inspection or repair, stating it was “not related,” despite the failure occurring immediately after major recall-related transmission work. The vehicle has not been inspected by the manufacturer. The failure is reproducible: the transmission will not shift. Reason for Complaint: This failure happened shortly after the S44 recall, which involved complete transmission disassembly. The combination of new post-recall transmission codes, the absence of a required PCM software update, and the sudden unsafe failure raises concern that the recall work may not have been performed correctly. I request NHTSA review whether Recall S44 repairs are causing secondary failures or being improperly completedRead more

I purchased a 2019 Ford Edge in 2021. About a year ago my service engine light came on. I took it to my dealer and was told there was a torque converter issue, but it would not leave me on the side of the road. I have tolerated this stuttering issues more than a year now every time I drive this vehicle. The maintenance on this car has been well maintained. I took my car in for routine maintenance
... this week and asked about the torque converter issue and was told it would be $6,000 to replace/repair and there could be more issues related to the same that could come up later. It is time for new tires and brakes but doing all of this is more than the value of the car. It appears the torque converter issue is a known problem with the Ford Edge as well as other makers. However, no maker wants to make this a recall issue. How can something that is known and impacting multiple buyers not a recall issue? Because it won't leave me on the side of the road, does that mean it isn't a safety issue? If I have $8000 to repair my car I would just go buy something else. It is also my understanding there is a class action lawsuit regarding this issue. The issue has not caused an accident yet, but the vehicle does not perform as expected and I don't know when the "stutter" will turn to a "stop" and put my family and me in harms way.Read more

My 2013 Honda CRV completely broke down in late August 2025, as I was backing out of my driveway. Our lives were at risk. This could have happened in traffic, causing a terrible collision. My local Honda dealership found the chain guides broken and the timing off, and recommended replacement of several parts (timing chain, guides, tensioner, exhaust cam, rockers, and ... VTC actuator). Initially,
... they thought the cause was a lack of proper maintenance, specifically, infrequent or improper oil changes. I knew we had taken proper care of our car, so I began investigating. The check engine light was on for at least 2 years (as well as the rattling at start-up), and I had not gotten clarity on it until recently, after consulting with multiple professionals. Finally, a Jiffy Lube tech said it was due to the VTC actuator. I then brought my car to the Honda dealership in July 2025 for an oil change and to discuss the check engine light and VTC actuator issue. At that time, they did not alert me to the VTC actuator defect or the potential for complete engine failure. As I continued my investigation, I found a Honda Service Bulletin starting in 2016 (when my car was still under its three-year warranty) and updated in 2017 (when my car was still under its five-year Power Train warranty) describing the VTC actuator as defective. Had American Honda issued a recall at that time, instead of an SB, and had notified me of this defective part, my warranties in those years would have covered the replacement costs of my VTC actuator and related engine parts. And also been significantly less expensive. Flash forward to this fall 2025: as I continued my investigation, I discovered multiple class-action lawsuits against American Honda for this exact issue—a known defective VTC actuator. It turns out they conducted an internal study in 2011 that found the VTC actuator was defective. Now the dealership believes my car's failure was caused by a defective VTC actuator.Read more

Summary: My 2025 Ford Mustang Mach-E GT has repeatedly experienced a complete and unexpected loss of propulsion while driving in active traffic. Each time, the vehicle displayed a red “Stop Safely Now” warning with additional critical alerts, followed by an immediate loss of forward motion with no prior indication. This left the vehicle stalled in traffic and created a significant safety hazar
... d. Details of Each Incident: While driving under normal conditions or coming to a stop, the vehicle triggered the red “Stop Safely Now” alert with powertrain and brake warnings. Propulsion shut down instantly and the vehicle locked in Park, leaving it immobilized in traffic lane. I had to exit the vehicle and wait for it to complete a full power-down cycle before it would restart. After restarting, the vehicle operated with reduced functionality and limited range. Ford Roadside Assistance was contacted for every incident. Safety Concern: A sudden loss of forward motion in moving traffic poses a serious crash risk to me and to other drivers. The repeated nature of this failure suggests a defect in the propulsion system, sensors, electrical components or related modules. Current Status: The vehicle is now at the local Ford dealership for evaluation. I have also notified Ford Corporate Customer Care. The root cause has not yet been identified or resolved. Requested Action: I request that NHTSA document this complaint and consider further review due to the recurring safety-related failure and the potential risk to other owners.Read more

In early Nov 2025 my Tesla Model Y began intermittently showing warnings about the forward collision warning system and multiple sensors being offline. Tesla service replaced a faulty connector on Nov 3. On Nov 15, 2025, about two weeks later, the car had a sudden drivetrain failure while I was parking on a steep hill. I heard a loud bang from the front of the car and the vehicle immediately lost
... drive power and had to be towed. Tesla diagnosed a failed front motor inverter that blew the high-voltage pyrofuse and damaged the 12V battery; they replaced those parts. The vehicle is available for inspection and the repaired components and records are with Tesla. After that repair the car needed another visit for alignment and has continued to show worrying behavior: “VCFRONT offline/unable to contact VCFRONT” in the service menu, repeated “autopilot computer overheating” messages, intermittent loss or reduction of regenerative braking (braking feel changes unpredictably), a rear right seatbelt malfunction warning (later inspected and cleared by Tesla), and what appears to be significantly increased battery drain compared to before the inverter failure. This has now resulted in four service visits within about a month, all at Tesla service centers (no police, insurance, or independent shops involved). Tesla has confirmed the inverter/pyrofuse/12V failure and says the overheating was due to an air pocket in the cooling system, but the VCFRONT messages, inconsistent regen braking, and increased drain have not been fully resolved or clearly explained. I am concerned that a sudden loss of power, loss of regen, or failure of VCFRONT/autopilot systems could recur at speed or in traffic, creating a serious safety risk. Please note that I'm only able to view partially-complete paperwork from the latest service appointment in the Tesla app. I have requested this documentation from Tesla and will be able to provide all the service reports and communication as a follow-up.Read more

Dear NHTSA, I am submitting a safety complaint regarding my 2025 Honda Odyssey, which has been diagnosed with a transmission failure requiring full replacement due to faulty internal seals. The vehicle is less than one year old, and this type of premature failure raises serious safety concerns. I first became aware that something might be wrong during a routine oil change on November 11, when tech
... nicians noticed signs that the transmission might be leaking or malfunctioning. I continued to monitor the vehicle, and on November 28, I was informed that the entire transmission must be replaced due to faulty seals inside the transmission assembly. A critical component like the transmission should not fail this early in the life of the vehicle, and a structural defect such as faulty seals may indicate a manufacturing issue that could affect other vehicles of the same model and year. Information from the dealership: SHEEHY HONDA OF ALEXANDRIA Diagnosed transmission failure caused by faulty seals Recommended repair: Complete transmission replacement Explanation: Premature failure of internal transmission seals leading to fluid loss and unsafe operation Repairs completed so far: Diagnostic inspections confirming failure This vehicle is used to transport children, and a sudden transmission failure presents a significant safety risk. For a new vehicle to require a full transmission replacement within its first year of use is highly concerning. I am requesting that NHTSA review this issue to determine whether other 2025 Honda Odyssey vehicles may also be experiencing similar premature transmission failures. Thank you for your attention to this matter. I can provide repair documentation, inspection notes, photos, or any additional information upon request. The component that failed is the internal microswitch/sensor within the gear shifter assembly. The specific part that requires replacement is often documented in General Motors Technical Service Bulletin (TSB) 19-NA-206 or 23-NA-119. The faulty component is available for inspection if the dealership removes the old part and retains it. My safety and the safety of others are put at risk because th
... e vehicle often fails to register that it is fully in "Park," which prevents the vehicle from shutting off properly, potentially leaving it in accessory mode. This could lead to a dead battery, leaving me stranded. More importantly, the failure to confirm the 'Park' state introduces a risk of the vehicle not being securely immobilized, creating a roll-away hazard if parked on an incline. The problem has been confirmed by local Chevrolet dealership service centers (receipts available). The issue is identical to the defect that prompted official safety recalls for numerous other GM models, including the 2017-2018 GMC Acadia. I started looking online and found that many others have reported the same issue. There’s even a YouTube video from someone who experienced it: [XXX] I plan to take the truck to the dealership to have it checked out, but this definitely feels like a significant safety concern. I’ve made no modifications to the truck; it’s completely stock with no tuners or aftermarket adjustments.
