The mechanics diagnosis (uploaded in attachment) is that the torque converter is in a constant locked state and that something was "bad" in the transmission and likely a torque converter failure. The shop estimated $9,138.33 to repair. Owner declined the excessive repair cost and limped the car home and into owner's garage where it currently sits awaiting some resolution. As this transmission has been the subject of prior recalls, owner believes the recall should be expanded to include this vehicle. Also noted, this transmission has been and may still be the subject of active litigation. Note: an extended warranty the owner had purchased with the car when new, expired just 4 months prior. No warning lights or messages appeared prior to the incident. I had not received the recall notification letter at the time I purchased the vehicle. The first occurrence happened suddenly on 12/10/2025, when the vehicle began slowing down and jerking in traffic. I was forced to navigate from the far-left lane across multiple lanes of traffic to reach the exit while the vehicle was effectively crippled, moving at speeds under 10 mph amidst highway-speed traffic. The hybrid system effectively shut down and the vehicle could not maintain normal road speed. I brought the vehicle to a Toyota dealership. The dealership performed only ECM and hybrid ECU software updates and cleared codes. No mechanical inspection, oil pressure analysis, particulate inspection, turbo system evaluation, hybrid motor synchronization testing, or engineering-level diagnostics were performed. Toyota Corporate subsequently stated that the software update was the “final” resolution and declined further diagnostics. I was advised to return to the dealership if the issue reoccurs. This failure mode matches the documented loss-of-propulsion behavior described in Toyota’s V35A engine machining-defect recall (sudden power loss, limp mode, cascading system failures). Hybrid models are currently excluded despite using the same engine platform and exhibiting identical symptoms. Sudden loss of propulsion at road speed presents a serious crash risk. This appears to be a safety-related defect that is not being adequately investigated or remedied for hybrid models.Read more

... g an immediate risk of collision with surrounding traffic. My family was in the vehicle during these incidents, significantly increasing the risk of serious injury or death. The problem has been reproduced multiple times, including after the vehicle was taken to an authorized Acura dealership. During the most recent visit, diagnostic trouble codes were cleared, but the recalled fuel pump was not repaired or replaced. After being released by the dealership, the vehicle stalled again under similar driving conditions. The vehicle was inspected by the dealership, but no corrective repair was performed beyond clearing codes. The manufacturer has not yet properly remedied the recurring safety defect. Prior to stalling, the vehicle exhibited intermittent hesitation, loss of power, and engine shutdown. Warning indicators and fault codes were present at times and were cleared by the dealership without resolving the underlying issue. These symptoms have continued intermittently since the recall repair. This appears to be a failure to properly diagnose and correct a known safety defect, resulting in repeated dangerous highway stalls and placing occupants at serious risk.Read more

The contact stated that a month later while her husband was driving on the freeway at an undisclosed speed, the engine seized and the rear-end locked and threw the driver into the steering wheel even though the seat belt was securely buckled. The contact stated that the transmission was stuck in drive(D), and the vehicle was unable to move forward. The driver shifted into reverse(R) and was able to pull off the freeway. The contact stated that the vehicle was then driven to an empty parking lot and left overnight. The vehicle was then picked up the following day and was driven to the dealer; however, no cause for the failure was found. The contact stated that the failure persisted. The manufacturer was notified of the failure. The contact hired a GMC engineer to inspect the vehicle and informed the engineer that the vehicle needed to be driven a minimum of three days and 300 miles before experiencing the failure. The contact stated that the GMC engineer and one of the employees at the dealer suggested replacing the PCM module because there was no fault code present during the failure. When at a stop, pushing the gas pedal to go, the truck hesitates. Almost like the truck dies then all of the sudden takes off. I have had multiple instances were i was almostnT-boned. Dealer said that is normal for a vehicle with turbos. We don't experience this with my wife's Kia. No warning lights appear. Just to be clear the truck hesitates at almost every stop when trying to accelerate.

The dealer was contacted. However, parts to do the recall repairs were not yet available. The manufacturer was contacted; however, the phone call was dropped five times. The contact had not experienced a failure.

I am filing an official safety complaint regarding my 2015 Ford Explorer Sport 3.5L due to a dangerous and sudden power-steering failure that occurred while I was driving with my children in the vehicle. ... eering wheel locked up and became extremely difficult to turn, nearly causing multiple accidents. I had children in the car at the time, and this situation created an immediate and severe safety hazard for everyone in my vehicle and on the road. After this happened, I contacted (by phone) Ford directly. Without any inspection, hesitation, or further explanation, I was immediately told that the vehicle “needs a new steering rack and pinion.” The quoted repair cost exceeds $3,000, on a vehicle I am still paying $500 a month for. I have since discovered that there are thousands of other Ford Explorer owners reporting the exact same issue, with the same model years, same sudden failure, and the same required repair. This is clearly not an isolated incident. This pattern strongly indicates a widespread manufacturing or component defect that poses a significant risk of injury or death. I am deeply concerned that Ford has not issued a formal recall for a problem that can cause an immediate loss of steering control at highway speeds and in everyday driving conditions. The number of publicly documented complaints online demonstrates that this is a systemic and dangerous failure that should not be ignored. I am requesting that NHTSA fully investigate this issue and require Ford Motor Company to issue a recall for the defective steering rack/power steering system in the 2015 Ford Explorer Sport 3.5L ecoboost w/o park assist, as well as any related model years or trims using the same components.
