Vehicle suddenly stops and is erratic when driving. Coming out of a stop risks the vehicle shuddering; going through an intersection or turning cross traffic is extremely dangerous as the vehicle could stop momentarily and cause a collision. Vehicle is unable to maintain flow of traffic. The problem appears to be widespread - there are various forums we found where others have this same issue. The vehicle was taken to the dealer, where it was diagnosed, and determined that the PCV vacuum hose had failed. The vehicle was repaired; however, the failure persisted. The contact's brother inspected the vehicle and noticed that the PCV vacuum hose was not connected to the upper intake manifold and that the hose had a two-inch split at one of the ends of the hose. The contact's brother reconnected the hose. The contact stated that the dealer was supposed to have replaced the hose. The engine cooling system on my 2021 Dodge Durango GT failed due to ruptured coolant lines, including the rear coolant tubes supplying the rear air conditioning system and both transmission heater hoses. The vehicle and components are available for inspection and are currently at the dealership. My safety and the safety of others were put at risk because the vehicle overheated while I was driving at normal speed in traffic. A major overheating warning appeared while driving. Overheating in traffic creates a risk of sudden loss of power or the need to stop unexpectedly, which could result in a collision. Although I was able to drive slowly due to proximity to home, this condition would be hazardous on longer trips. The issue was confirmed by an authorized Dodge dealership, which verified failure of the coolant lines and provided an estimated repair cost of approximately $3,600. The vehicle has only been inspected by the dealership. A third-party warranty provider (Warranty Solutions) denied coverage, and I am disputing the decision. The vehicle remains at the dealership because I do not feel it is safe to drive. No prior warning signs were present. The overheating warning appeared suddenly at approximately 62,000 miles. This submission supplements my existing NHTSA complaint regarding Recall 17-010 and the 9-speed automatic transmission ATF warmer defect. My vehicle received Recall / Product Update 17-010 service on March 10, 2017, at which time the transmission was replaced. The final redesigned ATF warmer did not exist at that time. ... n. The vehicle returned for a second Recall 17-010 service visit on December 9, 2017. The service record from that visit explicitly states that the ATF warmer was not replaced, noting that it had been replaced during the prior visit. As a result, the recall was initiated but not completed with the final redesigned ATF warmer. Acura later issued TSB B17-026, extending warranty coverage to 10 years from original in-service date with unlimited mileage, because damage from ATF/coolant contamination may occur years after initial exposure, even if fluids later appear normal. In 2025, the vehicle developed transmission noise. An independent transmission specialist advised that the vehicle exhibits bearing failure, with the extent of damage unknown until teardown. Bearing failure is a known delayed consequence of ATF/coolant contamination associated with the ATF warmer defect. The vehicle was delivered to an authorized Acura dealership for evaluation under the warranty extension. However, the dealership has refused to perform the warranty inspection unless payment is made, and has stated that the vehicle is outside the warranty period, despite the warranty extension remaining in effect through January 2026. This supplement is submitted to document that: •Recall 17-010 was started but not completed with the final redesigned ATF warmer; •The final corrective part was never installed on this vehicle; and •Corrective action and inspection related to this incomplete recall and warranty extension are currently being denied.Read more

Issue related to the rear-differential fluid, TSB 21-NA-028. Its a known issue where the rear differential make moan/chatter noises on turning (including at low speeds). Dealers are instructed to service/replace the rear differential fluid and perform procedures to address clutch pack chatter. It is such an issue, the fluid to remedy this is on back order!

... ssue diagnosed and is still not repaired and TSB states for dealerships not to do more work to fix as this is a computer program issue and not a component issue, nonetheless the camera is still not functioning. The second area of failure is the transmission shift module. This module determines when the vehicle has been shifted out of park into R, N or D. The module has been replaced 2 times in 55,000 miles. When the module goes bad the vehicle gives a warning that the ability to shift will be lost. Finally the last safety issue that has been largely reported is the failure of the Telematics Onstar Module which runs the onboard communications for Onstar, Maps, GPS, and other systems that use the GPS signal. This means that any hands free technology tied to these systems no longer work leading to legal issues in most states that have hands free requirements. GM is charging around $1,000 to replace the Onstar module and it is taking 6 months or longer to even access the part if at all. Vehicle has also had running light/headlight, indicator light go out with no way to change a bulb, This requires an entire LED assembly, this issue is intermittent and may reset after the vehicle has been fully powered down, but creates a safety issue while driving with lights and blinker being in operable.Read more

Inspection revealed antifreeze intrusion into the under-hood engine fuse box, causing widespread fuse degradation and electrical failure. This occurred following recall-related service. Vehicle lost power and is unsafe to operate due to electrical contamination and fire risk. Manufacturer and dealer have been unresponsive.

After noticing a "groaning/moaning" sound from the vehicle when turning sharp at low speeds the vehicle was brought into a dealership to be inspected. After inspection, the mechanic stated that the sound is coming from the rear differential and that it would need to be flushed/cleaned up to three times as a potential correction. ... lace the entire rear differential. When asked what happens if the rear differential fails, we were advised that we could experience total failure and lock up. This is a safety issue in that if we are driving and the wheels lock up the potential for collision or major accidents is high. I also understand that the differential is meant to last the life of the vehicle and had 123k miles, the life of the vehicle is far from over. Another issue we are experiencing is excessive oil burning. The vehicle "add engine oil" light comes on after 2,500 miles when being regularly maintenanced. The vehicle should not require the addition of oil after that many miles, more so should not require oil at all unless you weren't properly maintaining the vehicle (which is not the case). On November 10, 2025, my 2016 Hyundai Sonata Hybrid (VIN [XXX] , approximately 150,000 miles) experienced a sudden loss of power while I was turning right and accelerating into traffic. ... traffic lane. I was forced to limp the vehicle into a nearby parking lot to get out of traffic. The vehicle was not safely drivable afterward and was towed home. A few days later, the vehicle was towed to Murdoch Hyundai in Lindon, Utah, for diagnosis. The vehicle was found to have diagnostic trouble code P1326, which Hyundai identifies as activation of the Knock Sensor Detection System (KSDS) related to connecting-rod bearing wear. This condition is part of Hyundai’s known engine defect and safety recall/warranty extension programs. During inspection, the dealer observed that the valvetrain did not show signs of neglect or sludge, which is inconsistent with oil starvation or severe maintenance neglect. This further supports that the failure is consistent with the known connecting-rod bearing defect addressed by Hyundai’s KSDS safety recall, rather than a maintenance-related issue. This is a safety issue because sudden loss of propulsion or reduced power while entering traffic creates a high risk of collision. Hyundai customer care has been unwilling to open a new case number or escalate review, and the dealer has indicated possible denial of engine replacement coverage based on incomplete oil-change records, despite the vehicle exhibiting the KSDS safety condition. I am requesting NHTSA review of this safety-related defect and the manufacturer’s handling of KSDS/P1326 failures. On December 15, 2025, driving on a highway in low temperatures, my 2024 Subaru Solterra (VIN: [XXX] ) experienced a progressive loss of power. At 9:20 a.m., I departed NYC for Amenia, NY (91 miles). The range showed ~130 miles (no AC). For the first ~70 miles, the range indicator consistently remained higher than miles-to-destination, confirming sufficient charge to arrive safely. Around mile 70

We were informed that the dealership could not replicate the problem. Since then, the issue has become more frequent and more severe. We have now been told that we can drop the vehicle off, but that it may take 3–4 weeks or longer before diagnostics even begin, and that we would be responsible for out-of-pocket rental car costs during this time. We were also told that because the issue could not previously be replicated, this would now be considered customer pay, despite the fact that this is a serious powertrain and safety-related issue. We find this unacceptable. This is not a convenience issue—it is a known concern with Ford’s 10-speed transmissions, and it places our safety at risk every time the vehicle is driven. We have opened a case with Ford in hopes that assistance will be provided, including: Proper diagnosis and repair of the transmission issue Coverage of related repair costs Assistance with a rental vehicle while the SUV is being evaluated, as it is currently unsafe to drive We trust Ford to stand behind its product and take this issue seriously before someone is injured.Read more

I have the parts for the driver’s side in my possession and am happy to furnish them for analysis. These failures are catastrophic in that the vehicle was rendered undrivable with a loss of control without warning. This vehicle is used daily for highway commuting and had this failure occurred on the highway at 65 mph, I believe it would have caused a serious crash of this vehicle and likely caused a multi-vehicle accident given the sudden nature of the failure and the lack of control the vehicle experienced upon failure. Honda Service Bulletin 20-125 appears to address a very similar concern for Honda Accord “CVT models.” The photos and inspection area listed in that bulletin seem to indicate that my car experienced a similar failure (Bulletin attached); however, I was told by the dealership that my VIN was not subject to the recall. I believe the vehicle is subject to a similar defect as described in the Service Bulletin for other Honda Accord models and believe it should be investigated and included in the recall. These failures have cost $2,279.47 in repair fees, approximately $400 in emergency towing and do not seem to be in line with routine maintenance or "wear and tear".Read more

Mazda North America has escalated the issue and informed the dealer that they are “waiting on Japan” for engineering guidance, indicating there is currently no approved repair or resolution. My repair shop has told me that they have had to repair MULTIPLE other cars of this make and model for the same reason/ failed parts across multiple years up to until about 2021, but that the company repaired it on more current models, which clearly indicates that this is a KNOWN issue for prior models. While the modified software is included from the factory in the 2025 model year Ioniq 5, it appears that the 24V-868 recall is not a complete fix. Indeed, ICCU failure continues to be one of the most commonly reported owner issues, both for vehicles which have already had the “fix” from recall 24V-868 and for newer vehicles outside of the recall’s scope. This can be confirmed from the numerous posts and videos online (YouTube, Reddit, Ioniq forums, etc.). Given that ICCUs continue to fail at a significant rate, resulting in reduced vehicle speed and vehicle shutdown within minutes (as soon as the 12 V battery is exhausted), I strongly urge NHTSA to do the following: 1. Consider the current ICCU software update to be an ineffective remedy for recall 24V-868. 2. Broaden the scope of recall 24V-868 or create a new recall for all Hyundai/Kia/Genesis eGMP platform vehicles to address this defect. 3. I had no lights or error codes (and still don’t) on the dash and they did not recommend that I don’t drive it so i continued using the car. Last night it made a horrendous noise started revving on its own and shifting very hard ans if something was coming apart. This morning on my way to the dealership the car completely seized up at an intersection. The car has 47900 miles on it. It’s a 2025 and the transmission seems to have completely blown. Ive read online several complaints about the transmission on these vehicles and think that they may be a major concern. For a new car to have this type of issue locking up while on public roads and smoking is absolutely a hazard. My only saving grace is that it happened at an intersection and not on the highway where an accident would have been inevitable. Please look into the DCT transmission on Kia Sorentos. Particularly X-Line SX prestige models. It is a highly unsafe vehicle. Another issue that the car is having is a clicking noise on the steering column that seems to be getting much worse over time and im afraid will result in loss of steering.
