I was driving on the interstate at approximately 70 mph when I attempted to accelerate to pass another vehicle. There was no response. This vehicle has had numerous problems since I leased it. To summarize, at times the vehicle randomly shifts out of the ECO mode, usually into the SNOW mode. The engine has also raced, hesitated, bucked, rattled and vibrated when trying to change gears. Sometimes, these actions are subtle and sometimes they are borderline violent. This happens more frequently when the engine is cold. Addition
... ally, the AEB system has activated a number of times while in stop and go traffic, even though the vehicle in front was moving forward. On at least 3 of these occasions, i barely avoided being rear-ended by the vehicle following me. The final issue that I have noticed is a problem with the odometer. It has appeared to work normally most of the time, but I have noticed times where I didn't think the odometer accurately reflected the distance I had traveled. I was able to synch the odometer with the trip odometer on a local road that I was quite familiar with. The trip odometer registered 1.8 miles while the odometer only registered 1 mile traveled. All of these issues happened multiple times and are intermittent. I have had the vehicle to Hyundai dealers on 5 occasions, and they claim to have not been able to duplicate any of the problems that I have been consistently logging in to a "diary" that represents a list of when I have noticed these issues. I have been in constant contact with Hyundai, but they promised to send out a factory rep, then reneged, instead relying on the local service techs. Said techs had the vehicle for nearly 2 weeks, but only drove it for 16 miles (until I called AFTER a determination was made and they hurriedly drove it an additional 22 miles to give the appearance of a more complete exam), using less than a quarter tank of gas. To me, this wasn't a thorough exam and appeared to be a cover-up to avoid having to buy back the vehicle. Note: these problems have occurred on multiple occasions, therefore the date below is the lease date.Read more

Continually get a power loss error while driving and accelerating. Took to BMW dealership and not able to clear error after having the car for a over month.
Incident date: Feb 25, 2025
Safety issue: The vehicle experiences intermittent loss of acceleration and transmission “slip/rev” events, especially when cold, from a stop, or at low speed. At times, the vehicle revs as if in neutral, then re-engages. The driver sometimes must press the accelerator fully to gain speed. In one instance, the vehicle was driven slowly for several miles, pulled over, shut off, restarted, and t
... he function temporarily improved. When it started: Symptoms began immediately after a routine service visit at a Toyota dealership ([XXX]). First noted deceleration going uphill with recovery. Next day, multiple warning indicators/messages appeared and vehicle was towed to the same dealership. Dealer response: Dealership advised transmission replacement based on diagnostic codes and quoted approximately $10,000, stating fluid check was not necessary. Second opinion: Independent shop found transmission fluid level extremely low (reported approx. 1/4 full). They refilled fluid; initially vehicle drove normally, but intermittent slipping/revving continued after pickup (worse when cold/low speed). No external leaks observed by second shop at that time. Current status: Vehicle continues to show intermittent loss of acceleration/slipping behavior, creating a safety risk in traffic. No current check engine light, but symptoms persist. Codes/records available: Codes: P07457F, P28167F, P088A7F, P27137F, C137BA2, C1241, C1280, U1117, B2227. Location: New York. I have taken this vehicle into the Honda dealership on five separate occasions to report a power transfer problem. I first visited the Honda dealership to report this problem on 9/2024. It hasn't been until my most recent visit on 12/18/25, that a customer service representative told me that my vehicle needed a new torque converter. It is available for inspection upon request. The first occas
... ion when the vehicle caused a safety issue, we were preparing to make a left hand turn and the car jerked, the dash lights flashed, and the vehicle completely turned off. This was in the summer of 2024. We have had many smaller incidents leading up to this first occurrence but they haven't resulted in the vehicle completely shutting off. Most of the power transfer problems occur when we first back out of the driveway, put the car into drive, press down on the gas pedal, and there is no propulsion forward. This has occurred on a regular basis since the purchase of the vehicle. After two to three seconds the car jerks forward and then drives. This will also occasionally occur when we are stopped at a stop sign or at a traffic light waiting to propel forward or make a turn. Many of the safety issues occur when trying to make a turn with traffic, and there is no power to propel the vehicle forward. We have been fortunate to have the other vehicles slow to allow the Pilot to pause before moving forward. We now have to ensure there is a large distance before making a turn. We have had the dealership, Continental Honda of Anchorage, reproduce the lag problem, July 2025. Recently, we had a Transmission System Problem warning light come on the vehicle. We have also had a flashing P, R, D indicator light when the vehicle was in those gears. The Transmission System Problem light appeared on December 9th, then it reappeared on the 17th.Read more

Description of Incident: The vehicle exhibits a severe and unpredictable transmission "shudder," "stutter," and hesitation during gear synchronization, specifically during 1st–3rd and 8th–10th gear transitions. This manifest as a violent "shaking" sensation and a loss of momentary acceleration while merging into high-speed traffic. This issue was first reported to an authorized Ford dealershi
... p on December 17, 2024. At that time, the vehicle exhibited harsh shifting and "shuddering" consistent with Ford Technical Service Bulletin (TSB) 22-2428. Despite being under a 150,000-mile service contract, the dealer misdiagnosed the mechanical transmission defect as an engine misfire, resulting in ineffective repairs. The defect has persisted and worsened. The internal failure has been identified as a defective CDF Clutch Drum (Cylinder) where an internal bushing moves out of place, obstructing fluid flow and causing mechanical "slip." This is a known manufacturing defect documented in multiple Ford TSBs (22-2428, 23-2250, and 24-2101). Safety Risk: The failure causes unpredictable power delivery. On multiple occasions, the vehicle has hesitated or "stuttered" while attempting to merge onto highways or cross intersections, leaving the vehicle vulnerable to side-impact or rear-end collisions. The lack of consistent throttle response and the violent nature of the shifting creates a significant safety hazard for the driver and surrounding motorists. Verification/Inspection: The symptoms were reported to a Ford dealership multiple times. As of December 2025, the dealership has opened a formal case with the manufacturer to verify the internal "cause of failure" by the Shop Foreman. No warning lamps or Diagnostic Trouble Codes (DTCs) appeared on the dashboard, as the failure is a mechanical internal bushing slip that does not always trigger a software sensor until total failure occurs. The vehicle is available for inspection.Read more

My transmission started jumping gears and slipping at 65k miles. I had it checked out then and the mechanic said that these transmissions shifted rough, but they were not really sure why and that they reprogrammed the computer and that should fix it. It went on intermittently for the next 6 months to a year and I took it in again and was told they couldn't get the transmission to shift rough or sk
... ip so I would have to come back once it got worse. The longer I drove it, the worse it got until I would regularly lose all power to the wheels even at high speeds as the transmission failed to find a gear to engage. I finally took it back to the dealer again and this time they found that the front differential had failed and was full of metal shavings, the transmission was shifting really hard and missing gears and the driveshaft seal had failed and begun leaking as well. The vehicle had approximately 90k miles on it, and Ford didn't even recommend a transmission service/fluid change until 100k. I drive almost exclusively highway, rarely use 4WD, and had only towed twice with less than half it's capacity. I pressed the Dealer and Ford on how it's possible that a transmission fails before it's first recommended service. They said it shouldn't have and offered to replace my transmission with a remanufactured one for the low price of over $9,000. I had the transmission rebuilt at a local shop who said they are regularly rebuilding these expedition transmissions and that they fail frequently. The vehicle losing all power to the wheels, with no warning lights and searching for gears with no warning is incredibly dangerous. After doing this several times, it did eventually throw the first loss of power blinking lights across the dashboard, and day transmission fault.Read more

My 2012 Subaru Legacy experiences involuntary engine stalling when coming to a quick stop, such as in stop-and-go traffic or during sudden braking at intersections. When the engine shuts off without warning, power steering and braking assist are lost, creating a serious risk of collision. This is a known issue documented by Subaru of America in Technical Service Bulletin 16-90-13R, applicable to 2
... 010–2012 Legacy and Outback vehicles with CVT transmissions. Subaru attributes the condition to excessive thrust washer wear inside the torque converter, which prevents proper release of lock-up clutch pressure. Subaru describes the condition as similar to stopping a manual transmission vehicle without depressing the clutch, which inherently results in engine stalling. Subaru later redesigned the torque converter as a countermeasure beginning in October 2013, indicating prior knowledge of a defect. However, Subaru never issued a recall or notified owners and addressed the issue only through a non-customer-facing service bulletin. I contacted Subaru Customer Advocacy to report this safety concern and request assistance with the repair (quoted over $4,000). Subaru acknowledged my concern but declined responsibility, citing the vehicle being out of warranty and stating that TSBs are not customer facing. Subaru did not confirm whether the vehicle is safe to drive with this condition. This issue represents a safety defect, not a warranty dispute. Vehicles that can stall unexpectedly during normal braking events pose a serious safety risk, and many affected vehicles remain on the road today.Read more

The Situation: My vehicle has an open safety recall, NHTSA #15V-574 (Honda Recall JU2). Because this recall was never performed on my vehicle, the drive pulley shaft has now failed exactly as described, rendering the transmission useless. L
... egal Basis for Total Replacement: 1. Consequential Damage: The software update (the original recall remedy) was designed to prevent physical breakage. Since that remedy was not provided, the physical hardware has now failed. A software update cannot fix a broken shaft; therefore, a total replacement of the transmission is the only viable remedy for this safety defect. 2. Federal Law: Under the National Traffic and Motor Vehicle Safety Act, manufacturers must provide a remedy for safety defects free of charge. Because the unaddressed defect led to the total failure of the transmission, Honda is obligated to cover the cost of the replacement.Read more

I purchased my Hyundai Ioniq 5 on [XXX]. Since then, I have experienced repeated and dangerous failures of the air-conditioning system, which Hyundai and the selling dealership have failed to properly diagnose or repair. On [XXX], while driving, I heard a very loud banging noise coming from the front passenger side of the vehicle. Immediately afterward, the air-conditioning system completely shut
... down and stopped producing cold air. Before the extreme Arizona heat began, I repeatedly asked Hyundai on Bell Road in Glendale to properly diagnose the A/C system due to ongoing concerns. Despite the vehicle being under warranty, the technician told me I would have to pay a diagnostic fee before they would evaluate the A/C. As a result, the underlying problem was not properly diagnosed or repaired. On [XXX], the exact same failure occurred again the same loud banging noise followed by a complete loss of air conditioning. The vehicle was taken to Hyundai on Bell Road in Glendale a total of four (4) times, spending 33 days out of service for the same issue. Despite this, the dealership failed to repair the vehicle. On several visits, the car was simply parked, and the dealer later claimed it had been “fixed” one time, even though the problem continued. This defect created a serious safety hazard. During one incident, while driving the vehicle in extreme heat to the dealership for repair, the interior temperature became dangerously high. My phone overheated and shut down, and I nearly passed out while driving. Hyundai Corporate has been unresponsive and dismissive. Rather than addressing the repeated failures and safety risks, I was referred to Lemon Law, where my claim was denied, despite the extensive repair history and time out of service. They said they have sent me emails with the pictures which I have yet to recieve. I went to the dealer and asked them to jack up my van so I can see the damage personally and they don’t have a lift available at the time, so I told them I would be back in town at a certain time from work and to call me so I could see it and they have yet to show me the scuffed IDCM module or the dented muffler. We haven’t run over anything to cause this kind of damage to a module like this because all the hybrid batteries are under the van. If the Chrysler Pacifica Hybrid is so cheaply made for a $62,000 vehicle that something can “scuff” a module housing and shut a vehicle down, this is potentially very dangerous if someone were driving on the highway if it were to shutdown and someone (including myself or any of my family) were driving and lose all propulsion and steering (because it’s electric steering) or worse braking! It could be catastrophic with people dying. This needs to be investigated to see how fragile this component is and if they need to “toughen” it up or re-locate it. This is clarification and additional information to a previous complaint filed: 11706675. The vehicle was experiencing grinding during hard turns shortly after i purchased it off the dealership lot in 2017. I didn’t know why, I thought I should practiced not making hard turns and thats what I did. Years later, around 2021 or 2022, I needed the first service on my vehicle, outside of regular mai
... ntenance (coolant hose piece replacement) and the mechanic mentioned the grinding during test drive while turning. I told him it always did that and that was the extent of that. He said nothing else and I didn’t know any better. Tjat shop has since closed down and I’ve been going to another mechanic since then. Most recently, December 2025, the vehicle started shutting off while driving. Im going to get my regular mechanic to inspect the PTU.Read more

While in 6th gear driving on the highway at 70 mph the truck downshifted to 3rd and drastically decelerated. When this happened the instrument display went from having all 6 gear numbers viable to only 1 - 3. It had to be put into Manual Mode and shifted into 6th manually. This happens twice in the same trip.

I was driving and all of a sudden I lost acceleration in the middle of an intersection. My truck had no gears and gave me zero indication there was an issue. I then was told Ford has a newly designed part that fixes the issue and I would have to pay $7500. How does a manufacturer knowingly sell such a dangerous product, and not have a recall for that product after updating a faulty design?

The vehicle is experiencing a malfunction of the Power Transfer Unit (PTU), which is part of the drivetrain / power train system. The vehicle is currently available for inspection upon request. The vehicle loses power during driving and produces grinding noises when making hard turns, which creates a risk of loss of propulsion while in motion, increasing the risk of a crash. The vehicle has stop
... ped, loss of power while driving, at lights, and while waiting in traffic to make turns. The problem has been experienced multiple times during normal driving conditions. Formal dealer confirmation is pending / not yet completed. An appointment was made at Jeep dealership for software update (free) and a charge of $240 per hour for a diagnostic test to see what is causing the vehicle to suddenly stop. Not affordable for me at this time. I also went to my regular mechanic, whom is stating no issues detected, he can’t find an issue with the vehicle. I haven’t mentioned the PTU recall issue with this make and model vehicle to my mechanic but i will be taking it back to them and mentioning the recall for the make and model for 2017 Jeep for his inspection of my vehicle. Symptoms occurred without prior warning lights and included drivetrain-related behavior consistent with PTU failure. The vehicle experienced loss of power and grinding noises during hard turning without prior warning lights or messages. Exact mileage at first occurrence is UNKNOWN. The symptoms first appeared at approximately December 7, 2025, and has continued 3 more times since. Similar PTU failures are documented in multiple recalls affecting the same vehicle model and model year range, though this VIN is not currently included.Read more

The front clicks and pops when turning left and right, both on throttle and off. Does not have to be a tight turn. Simply turning into a side street, no bumps or anything abnormal makes a racket and kind of scary due to how loud it is. Started when the car had less than 5,000 miles and I think it's worse when it's cold outside. Speeds that cause this are anywhere from 0 to 35 miles an hour. D

To the National Highway Traffic Safety Administration, I am submitting this letter as a **formal safety complaint** regarding repeated transmission failure in my **2019 Chevrolet Traverse**, which poses a potential risk to vehicle safety. In **December 2024**, while the vehicle was still covered under General Motors’ factory powertrain warranty, an authorized GM dealership diagnosed a serious
... transmission defect. Due to parts being on national backorder, the repair was delayed. When the repair was eventually performed, only partial components (torque converter and valve body) were replaced rather than the full transmission. Despite this repair, the transmission **failed again less than one year later**. In **November 2025**, a second authorized GM dealership confirmed that the transmission now requires **full replacement** and that the prior repair did not correct the underlying defect. The transmission failure has resulted in **slipping, hesitation, and loss of proper power delivery**, which creates a **potential safety hazard**, particularly during acceleration, merging, and highway driving. A loss of propulsion or delayed response in traffic significantly increases the risk of an accident. This issue was identified while the vehicle was under warranty and was not properly resolved. The repeat failure suggests a **systemic defect** rather than normal wear. General Motors has declined further assistance. I am requesting that NHTSA review this complaint as part of its monitoring of potential **powertrain defects** affecting vehicle safety. While driving down the highway at ~70 mph, the truck's powertrain suddenly shut off. The electronics still worked, but the RPMs dropped to zero. I was forced to pull off to the side of the road, where I tried to turn off and turn back on. The truck kept on turning off and on by itself electronically, but the engine and transmission wouldn't engage for roughly 10 minutes. I was eventually able to s
