Incident date: Jan 9, 2026
INCIDENT SUMMARY: On January 9, 2026, at approximately 1:30 PM, while traveling at 50 MPH in North Carolina, the vehicle experienced a sudden, catastrophic engine seizure without any prior "Limp Mode" warning or Check Engine Light. This resulted in an immediate, total loss of motive power in active traffic. I was forced to navigate a dead multi-ton vehicle across lanes without power steering or va
... cuum-assisted braking, narrowly avoiding a high-speed collision. TIMELINE & NEGLIGENCE: This vehicle was purchased from an independent retail dealer on December 20, 2025—less than 21 days prior to this life-threatening failure. The vehicle was sold with an "Open" status for Safety Recall 23V-707. The failure occurred exactly as described in the recall’s warning: a catastrophic stall at speed. TECHNICAL EVIDENCE: A non-invasive borescope inspection of the cylinders has confirmed the presence of metallic "silver chunks" (sheared bolt heads) and "half-moon" impact marks on the piston crowns, indicating a total timing failure and internal mechanical collision (piston-to-valve contact). REQUIRED REMEDY: This incident is a direct manifestation of the safety risk documented in Recall 23V-707. Per BMW Service Bulletin SIB 11 09 23, the presence of sheared bolt heads and seized internals constitutes "Catastrophic Consequential Damage." I am filing this report to ensure federal oversight of BMW North America’s obligation to provide a complete engine replacement, as the safety defect was present at the time of sale and resulted in a near-fatal incident within 300 miles of ownership.Read more

Unexpected rear wheel lock-up at 29000 miles. While engine braking from highway speed onto an off-ramp at approximately 50 mph, the rear wheels unexpectedly locked up. Due to prior rear brake actuator issues, I suspected the emergency brake had activated without my input. It was only momentary, and no fault codes or warnings were displayed. It happened again under the same circumstances two months
... later, with no fault codes. On May 9, 2025, the check engine light came on, and I scheduled service. The dealer addressed a recall regarding the transmission by installing software. In August 2025, I received a letter from GM regarding recall N242454440 for excessive wear in the transmission control valve, which was causing rear-wheel lockup. On October 31, 2025, while towing a small enclosed trailer at 70 mph on the highway with cruise control engaged, the truck suddenly decelerated, and I was nearly rear-ended by a semi-truck (no brake lights to warn them). The truck would not go over 50 mph unless I was going downhill. The transmission would not shift into higher gears. I returned it to the dealer, who checked the computer and found no record of the incident, so they said there was no problem. They informed me that the new software was designed to limit the truck's speed if a potential lockup was detected. This recall did not fix the transmission problem or the safety issue. It was simply a way to avoid costly repairs. There is a problem with the physical transmission valve that, for me, resulted in an extremely dangerous situation. The NHTSA needs to take this issue seriously and require GM to replace the prematurely worn valves in the affected vehicles.Read more

While driving during colder weather the car will show HEV (hybrid system) warning and tell you to shut it off. I was in a construction zone with no where to pull over when the car died and lost all propulsion and brakes. This seems to be a common problem on KIA Optima hybrids from 2012-2015.

While traveling on a highway at approximately 70 mph, the vehicle experienced a sudden and severe loss of motive power. At the same time, the dashboard illuminated multiple warning lights simultaneously and the vehicle entered limp mode without warning. The vehicle rapidly decelerated and was unable to maintain speed with surrounding traffic. This created an immediate and serious safety hazard and
... nearly resulted in a collision, as vehicles behind were approaching at highway speed. The driver was forced to make an emergency maneuver to reach the shoulder and stop to avoid a crash. After this incident, the vehicle was parked and not driven. Several days later, the vehicle would start and shift normally, but would not move in Drive, Reverse, or any gear. The gear selector moves and the gear indicator changes, but the vehicle does not engage or move at all. The engine revs normally with no propulsion. This condition is consistent and reproducible. The failure appears to involve the continuously variable transmission (CVT) or an internal drivetrain component. The vehicle is available for inspection upon request. This defect posed a significant risk to the safety of the driver and other motorists due to the sudden loss of propulsion at highway speed and the inability to safely maintain speed or clear traffic. The later total loss of drive further increases the risk of becoming stranded in unsafe locations such as roadways or intersections. A Honda dealership was contacted but did not provide a resolution. No repairs have been performed. No collision occurred. No police report filed.Read more

The DCT transmission causes significant hesitation at slow speeds or starting from a stop. This often leaves the driver and it’s occupants in a vulnerable position if the car hesitates when trying to navigate through traffic. My 2018 Honda experienced a catastrophic engine failure (blown head gasket) at only 30,012 miles. A head gasket is not a wear item and should not fail at such low mileage. Repeatedly there has been a cylinder 1 misfire causing the check engine light to activate. The vehicle starts to shake and idling is rough. It has happened repeatedly over the past 3-4 years. ... to the faulty VCM system. The problem has repeatedly been identified by multiple mechanics. I use my vehicle primarily to transport my children to and from school. We have all been in the car during these misfires endangering their safety. At many of the times when the misfires have happened my children have been in the car and any engine failure while operating the vehicle puts them in danger. My children are ADHD/autistic and during the times the vehicle has broken down while driving it has caused them extreme distress. The vehicle is available for inspection at any time. I have reached out to Honda repeatedly to have the issue resolved however despite repeated promises the vehicle would be fixed due to recalls and extended warranties Honda officials have repeatedly made erroneous claims and insist my vehicle's vin needs to be approved for any repairs. This is a well documented known issue for this make and model due to the faulty VCM system causing oil to get into the cylinders leading to the misfires and piston ring wear and corroded spark plugs. All I am requesting is for Honda to make the necessary repairs so my vehicle is safe to transport my children. I have kept up with the maintenance of the vehicle. The problems we have experienced are directly caused by the known VCM/MIL failures. Also we have consistently had codes come up for the ABS as well. My 2025 Toyota Camry XSE (Hybrid) is subject to an open safety recall issued December 16, 2025. The recall involves a serious safety risk, and I was advised by the dealership not to drive the vehicle. At the time of the recall, the vehicle was approximately two months old with about 1,500 miles and in like-new condition. Because there is currently no available remedy, I was required to leave the
... vehicle stored at the dealership. I have been without use of my own vehicle since January 6, 2026 and am using a loaner vehicle solely due to the safety recall, not by choice. Toyota has stated a fix may not be available until at least Q1 2026, with no defined repair timeline, and customers are being told to wait without interim mitigation. I am concerned about the inability to safely operate my vehicle and the long-term impact of extended storage on a nearly new hybrid vehicle, including hybrid battery health, fuel and fluid systems, seals, tires, and interior and exterior materials. Prior to the recall, the vehicle was routinely garaged and properly maintained, and extended outdoor storage due to the recall introduces risk of premature wear unrelated to normal ownership use. While the dealership has indicated the vehicle may be started periodically, this does not replace normal driving and ownership use. In addition to loss of use, I am required to maintain a $300 monthly loaner vehicle deposit while my vehicle remains unusable due to the recall. Toyota Customer Service has stated they cannot escalate or provide assistance and are instructed to close cases without resolution, creating ongoing financial burden through no fault of my own. Toyota has acknowledged similar recalls on other models lasting extended periods, yet no interim remedy, mitigation plan, or customer relief is offered here. I believe this raises concerns regarding recall remedy timeliness, consumer safety, and loss of use.Read more

10R80 ten speed automatic transmission first exhibited indications of failure 9/16/2020, Ford's response being reprogram PCM. Transmission has progressively declined in reliability complete with harsh shifts, jerking, erratic up and downshifts, suddenly reverting into neutral at highway speeds. Safety of self, wife and other family members imperiled due to the vehicle suddenly lurching, jerking,
... and slamming forward in an unsafe manner at highway speeds. Ford dealerships have reprogrammed the transmission PCM four times over several years with progressively worsening transmission functionality. Dealership service specialist has confirmed that the transmission failure is due to internal components with burnt fluid as a component. But, Ford will not allow them to open the transmission up, denied repeatedly by the manufacturer as the issue with this 10R80 transmission is widespread with thousands of complaints filed, sone with NHTSA and others online and with Ford Customer Service to no avail. Ford is denying this manufacturer's defective transmission to avoid financial liability likely in the multi million dollar range over a wide array of Ford products with this faulty transmission installed in them. During the four times that Ford dealerships have reprogrammed the transmission's computer at the behest and mandate of Ford Motor Company, this vehicle's transmission repair was covered by both the manufacturers warranty as well as a privately purchased Ford Extended Service Plan (Extra Care). Ford chose deny coverage under either warranty though it is warrantied under both plans explicitly. This failure can result in vehicular accidents because of transmission's erratic and flawed functionality. Stranded on an interstate highway due to this failure especially is dangerous and may leave the vehicle exposed to collision.Read more

Just after merging from I-65 North to 264 East at 65 mph in Louisville KY on November 14th, the display indicated to "press start" and the engine stalled. I was only able to get the vehicle about 3 inches off of the left side of the yellow line before running out of inertia. I was stuck there for 2 hours while waiting for a tow truck. The first truck could not lift my vehicle because with the e
... ngine off, the electric shifter would not move out of park. A second tow truck was called with dolly's and then a 3rd tow truck was required to block traffic. During this 2 hour period several cars swerved left of the yellow line and I was in fear for my life. The traffic was so heavy that I was trapped between the concrete median wall and the left lane. I called 911 to ask for a safety vehicle to shield me from traffic, but they never arrived as promised.Read more

I have a 2018 Toyota CHR with of course a CVT transmission. I am around 105k miles with normal maintenance and have had small issues but nothing crazy until now. I have been noticing a strange noise coming from my car and after replacing pretty much every moving part on the car (besides the transmission), the issue is a bearing coming from within the transmission. After doing further research thro
... ugh the NHTSA and other websites it seems like this is an inevitable problem with all models from this make within the year 2017-2022. This is extremely disappointing coming from a lifelong Toyota customer. There seems to be a recall for these units in other parts of North America but not the United States. I would not recommend this car or this company to anyone. Failures in this part exceed the value of the car as of 2026 and it is completely ridiculous that Toyota has not done anything. As a loyal Toyota customer you have lost my business and every time I see someone in this model CHR I can only feel bad for them knowing their car is most likely not going to make it 5 years or even to 100k miles. This is a great little car on the interior and exterior but is unfortunate that Toyota made the wrong decision mass producing and rushing the import of them into the United States with a Turkish made transmission. Not the Toyota quality that anyone expected.Read more

I was driving 45 mph on a state route when all of a sudden my drive shaft bolt came completely out and split my drive shaft in two causing me to lose control. No damage at all or rust or anything is visible on the drive shaft. Could have caused a serious injury or worse. I just had a Multi Point Inspection done the day before the incident occurred

I purchased a pre-owned Chevrolet Silverado Z71, 4WD, off-road, LTZ pickup truck (Model # C1500 k 1500, VIN [XXX] , NAIC # XXX) from Wally Edgar Chevrolet in Lake Orion, Michigan on 3/28/24. It has the V8 – 5.3 L gasoline engine with an 8-speed automatic transmission. The vehicle was, and still is, in very good condition (no scratches, dents, and a good running engine). However, on 10/29/2025 th
... e vehicle started not being able to shift out of 1st gear. I immediately took it to Cruise Transmission in Rochester Hills, Michigan to have it evaluated. They indicated that the transmission “valve body” needed to be replaced at a cost of about $3,000. Their problem was that they couldn’t locate that part anywhere, and said it might be a month or more before it would be available! After not hearing anything for a couple of months (checking-in occasionally), I began to try to find out why that part wasn’t available. When I did a Google search on “can you sue GM if they can’t provide parts for a [XXX] vehicle”, it came back with the fact that there was a class-action lawsuit in progress for transmission issues on the 2015-2019 Chevy Silverado pickup trucks with 8-speed transmissions. When I inquired with two of the main law firms associated with the class-action lawsuit they indicated that I wasn’t a candidate for the suit because my vehicle was out of the warranty period. I indicated that I understood that, but that I wasn’t able to use my vehicle because the part wasn’t available anywhere. I also called several local dealerships to see if they could get the part. All of which indicated that they couldn’t get the part either, and also expressed their frustration with the issue. On 1/6/2026 and 1/7/2026, I spoke with Chevrolet Customer Service who indicated they didn't know when or if the part would be available. I can't afford to throw away an $18,000 vehicle!!! The transmission stutters at sustained modest speed (25-45 mph). It was taken to the dealer, who determined it was affected by tsb 25-2134. This is a widely known issue and the dealer refused any servicing without payment. This should have been a recall, since hundreds of known issues in and out of warranty have occurred.
