For powertrain: failure of right front CV axle before 100k miles on a certified pre-owned vehicle; associated failure of shocks and struts in right front axle. The problem was inspected by an ASE certified VW mechanic, attested to be true and shown to us, and repaired. There were no warning lamps/lights, and the finding was made on a full vehicle inspection when brought in for a brake pad change.
... The issue appeared at approx 79000 mi on the odometer, 3 months after purchasing the vehicle. Obviously failure of the CV joints and components provide a risk to drivers and other motorists with the potential for injury due to steering and suspension failure during operation. Engine: the water pump was discovered to have begun to fail at approx 89000mi during a routine oil change service in May 2026. The mechanic states the issue is not noticeable without removal of the brush guard from under the engine compartment, and therefore may be missed by owners. A warning for “low coolant” is only shown once the level has dropped sufficiently. There is not currently a recall open for this part, confirmed with VW service center at a dealership May 27, 2026. This is harmful physically and financially for owners, passengers, and motorists as it can lead to catastrophic engine failure.Read more

The dealer determined that the failure was related to GM Special Coverage Number: N242454441 (Momentary Rear Wheel Lock-Up); however, the VIN was not included. The vehicle was not repaired. The manufacturer was notified of the failure and referred the contact to the NHTSA Hotline for assistance in adding the VIN to the Special Coverage. Gear shift makes noise when moving it, as in you can actually hear the gears/electrical equipment moving and making noise when going from park to drive. The keyless entry when the fob is near, just doesn't work well from the drivers side... I exited the highway and restarted the vehicle before it temporarily resumed normal operation. A second major incident occurred on May 7, 2026, while driving on Interstate 5 near Flag City, California at approximately 65 mph. The vehicle suddenly decelerated uncontrollably to approximately 20 mph. The transmission again shifted directly into 2nd gear, and the accelerator pedal became completely unresponsive. I was forced to make an emergency stop on the shoulder of the freeway with hazard lights activated due to the risk of being rear-ended by high-speed traffic and trucks. A third incident occurred on May 13, 2026, while accelerating onto Highway 99 in Modesto, California. During a shift from 4th to 5th gear, the transmission jerked violently and the vehicle suddenly lost power while merging into highway traffic. The vehicle has been taken to Mercedes-Benz of Stockton multiple times for diagnosis and repair attempts. The dealership has performed software updates and extensive testing, and dealership personnel have personally verified the transmission defect during road testing. Mercedes-Benz engineering has also become involved in the diagnosis process. However, the defect remains unresolved. This defect creates a serious highway safety hazard due to sudden loss of propulsion, uncontrolled deceleration, and loss of accelerator response while driving at freeway speeds.Read more

The defect affects multiple critical safety systems including AWD/4WD, traction/stability control, hill assist, and collision avoidance systems. I have contacted Ford multiple times. Ford only opens customer service cases but has not provided a permanent repair. Ford is aware this is a common issue with 2020 Explorers specifically, and later model years appear to have been corrected. Numerous owners report the exact same failures online. Ford has issued service information involving software reprogramming, module communication faults, and wiring harness issues, but only Ford dealerships can perform the required programming. This appears to be a widespread manufacturing or engineering defect and creates a serious safety concern while driving. ... recall that involves flashing/re-calibrating the Engine Control Unit (FI-ECU) software was done. Just a few weeks after new fuel pump, the vehicle displayed sudden stalling symptoms. The engine suffered a terminal, catastrophic internal collapse. The vehicle was towed to Grubbs Acura where Corporate Honda directed a forced out-of-pocket internal engine teardown. The physical teardown explicitly confirmed a catastrophic connecting rod bearing failure (spun bearing) identical to the structural manufacturing flaws currently under active federal investigation by the NHTSA under Preliminary Evaluation PE25008 / RQ24-025. Corporate Honda subsequently issued a verbal agreement to fully reimburse my out-of-pocket teardown fees. Because a manufacturer will never fund diagnostic labor if evidence of owner fault is present, this corporate reimbursement serves as formal validation that no owner maintenance neglect occurred, and that the engine died exclusively from an inherent component defect. Gunn Acura (San Antonio, TX) confirmed with my vin, that Engine #3 was a warranty/dealer-installed replacement unit at 22,000 miles. It failed after less than 40,000 miles of total operation. A professionally manufactured and dealer-installed engine is reasonably expected to last well beyond 40,000 miles. The manufacturer network cannot utilize arbitrary part-ownership rules or uncooperative, fragmented dealer communications. Has the problem been reproduced or confirmed by a dealer or independent service center? - Yes. I took the vehicle to Mark Grainger's Quality Service Center, Rincon Transmission, and the J.C. Lewis Ford dealership, which all reproduced and confirmed the issue with coresponding error codes P0751 and P0752. Has the vehicle or component been inspected by the manufacturer, police, insurance representatives or others? - Yes. Mark Grainger's Quality Service Center told us the transmission work required was beyond their scope. They sent us to Rincon Transmission. Rincon Transmission told us they'd never serviced that engine before and said they'd prefer the work to be done by a Ford dealership. J.C. Lewis Ford told us a new transmission was required, but wouldn't produce results of any testing to verify that verdict. The dealer had diagnosed the vehicle with a defective PTU(Power Transfer Unit) and it was later discovered that a recall had been placed on the vehicle for NHTSA Campaign Number: 26V290000(Power Train). Although a recall was placed on the vehicle; the part for the repair was unavailable. The contact stated that the manufacturer had exceeded a reasonable amount of time for the recall repair. The manufacturer was made aware of the issue and the contact was informed that the repair would not be covered under warranty. The strong vibration makes the driver feel fatigued, light-headed and dizzy after 30 minutes of highway driving at any speed over 60 mph, impacting driving the vehicle safely, increasing the risk of road accidents and potentially resulting in death or serious bodily injury. The vehicle does not feel safe to drive at highway speed or for longer distances. Driving at 60 miles per hour or more becomes extremely uncomfortable, as a strong vibration is felt in the seat shaking the back, in the steering wheel, shaking hands and arms and in the floor shaking the legs. These repeated and spontaneous engine failures, including when driving at highway speeds, are an extreme risk to my life and the life of others and are unacceptable for a vehicle with 45,000 miles.Read more

After researching the issue, I discovered multiple similar complaints involving 2021 Toyota Highlanders equipped with the UA80 transmission, including Toyota Technical Service Bulletin T-SB-0008-21 regarding “Whine or Grind Noise From the Transmission.” The symptoms described in the bulletin closely match the symptoms my vehicle is experiencing. Toyota appears aware of this transmission-related issue, yet many owners outside the warranty period are being denied coverage despite similar failures and complaints. I am concerned this may represent a broader transmission defect that could lead to premature transmission failure, unsafe driving conditions, hesitation, loss of power, or expensive repairs for consumers. I believe this issue should be investigated further by NHTSA due to the growing number of similar complaints involving whining transmission noises and transmission failures in Toyota Highlander vehicles. At this time, the vehicle has not yet experienced a complete transmission failure, but the condition is worsening and raises significant concerns regarding reliability and safety.Read more

My 2021 Toyota Highlander was making a high pitched whine sound upon acceleration with the sound level holding steady at a consistent speed. We took the vehicle to a Toyota dealership to have it looked at and were told the transmission needs to be replaced. No other warnings were received. We are concerned about our safety if the transmission goes out while on the road.

For no apparent reason the truck goes into “low power mode” and gives off code p2138 often happens around 30-35 mph. Have had it happen to me also around 45 mph also. I have replaced sensors and parts to which a month or so goes by and it happens again.

Power train (PTU) failed on the highway huge crash sound and then no power to wheels, transfer case visably broke due to catastrophic failure shockwave cracked housing of my rear difrential too. ... there is loss of metal but im sure it was violent enough to blow some fragments of metal around. Problem cant be reproduced since its visably broken due to sudden blowing up of the PTU. No official inspection but I'll do a teardown soon. My own diagnoses as I'm good with cars is sudden PTU failure. Reasoning is that my engine still starts and my transmition works since it still audibly shifts between park and neutral. havnt tested drive or reverse yet since everything is still connected but will do that soon and write another report as an update to this one. No warning lights at all ever and nothing that i or my wife noticed before, just a sudden bang and no power doing 55 miles an hour. Then it automatically put the handbrake on while im trying to pull to the side of the highway. Im requesting my vehicle to be added to the list of recalls under recall 01C / 40D. Clutch pedal kept sticking to floor. Eventually the pedal did not return at all. Concentric slave cylinder failed and brake fluid leaking in bell housing. Dealer stated: found slave cylinder inside the bell housing failed which caused brake fluid to contaminate bell housing components. recommend replace slave cylinder assembly, clutch kit with flywheel, and throw out bearing assembly.

I started to pull forward from a pharmacy drive-through at an intended low speed (approximately10/15 mph) for a right turn onto a driveway. The car was on auto-hold and in drive gear. When I turned right, the vehicle went into a full throttle acceleration and continued until impact. Brakes were totally ineffective. ... MISSION MALFUNCTION". There was no automatic braking or forward collision function. I made a hard left turn in about 40 feet to avoid a shopping center parking lot. The vehicle continued until colliding with a concrete filled bollard and an automotive electric charging station. The vehicle charging at this station was also struck. My vehicle fell back, landing on its right side. The driver's airbag and both side curtains deployed. I was left suspended in the seat belt until the Fire Department, Emergency Services and the Highway Patrol arrived. A Jaws of Life was used to open the sunroof and extract me. I was transported by ambulance to an emergency treatment center. I was examined there and imaging done. I sustained a ruptured tendon in the middle right finger (permanent damage) and sagittal band ruptures in two fingers on that hand. There were numerous scrapes and bruises on the left thigh, both forearms and hands as well as minor cuts on my forehead. Also, post -accident, I experienced neck pain and overall soreness for several days due to whiplash. The Event Data Recorder was unattainable as insurance paid the claim for the totaled car within one week and assumed ownership. According to the salvage yard, it was sold shortly thereafter. The truck has a studder that comes from the transmission when at highway speeds. the studder can be felt at random intervals (seconds) from all passengers. The studder is also present on starting up a slope or accelerating from a stop at a slow pace. when accelerating slowly the jerk is more prominent. We have taken it to the GM Dealership, they confirmed the issue was present. The dealership
... said the GM guidence was to 1st try a re-program and then replace the torque converter. The program flash was unsuccessful, then we paid $4000 to change the torque converter. The Tech stated that this was the GM suggested fix, but noted that we are changing the fluid also which could mask the issue if it is the valve body causing an issue. The replacement torque converter and fluid did somewhat reduce the issue for a short period of about 2 months before it has returned as it did before. The next option from the dealership is in excess of $8,000 for a remanufactured transmission which is outside our reasonable spend, when we dont expect the remanufactured transmission will have any better valve body components. The recent recall did not cover my gas 6.2L sierra 1500, although it clearly is insufficient stating that it just a warning that there is a safety risk present. The valve body of the transmission appears to be an issue across all models and the recall addresses that it is present. We continue to drive this as the primary family vehicle with the hesitation and jerking present as GM dealer offered no support other than $8,000 remanufactured transmission on top of the $4000 we already spent. I believe the recall was insufficent in covering the Gas models with the exact same transmission and known issue. I also believe the offered solution continues to put those driving at risk.Read more
