I am submitting a vehicle defect complaint pursuant to 49 U.S.C. Chapter 301 regarding a potential safety related defect involving engine, electrical, and powertrain systems. The vehicle experienced early engine and electrical malfunction at relatively low mileage, requiring replacement of the engine wiring harness and removal and reinstallation of the engine and transaxle assembly. These are majo
... r powertrain and electrical repairs not reasonably expected on a nearly new vehicle and raise concerns regarding vehicle reliability and safe operation. The vehicle displayed a Malfunction Indicator Light warning indicating an engine and emissions system malfunction, with warnings that continued operation could cause additional damage. Such warnings and failures create a risk of reduced power, drivability issues, or further system failure while operating the vehicle. Insurance repair documentation reflects the selection of a recycled LKQ drivetrain related component, including a recycled engine, transaxle, or CVT related component, which raises additional concerns about durability and safety when installed in a nearly new vehicle. Early powertrain and electrical failures involving engine management and emissions systems may implicate compliance with federal motor vehicle safety standards and warrant review under NHTSA defect monitoring authority. I am requesting that NHTSA log, track, and evaluate this complaint for defect trend analysis, potential noncompliance, or further investigation under 49 U.S.C. Chapter 301 to determine whether similar failures are occurring in other vehicles of the same make and model.Read more

I was leaving a light and turning left onto incoming traffic Columbia and John young parkway. Vehicle turned off with no option of turning back on . I rolled onto the right curb but was sticking my vehicle out. Me and my wife and teenage son. Feared for our life due to the car not turning on and incoming traffic . This is a serious issue and hazard. The car asked me to put the car in park. After I

The car would not turn off at all. The Push Start Button would not turn the vehicle off or on. The Round buttons for D/N would not turn at all. The Tow truck driver use a red pull handle and put the transmission in neutral. It’s a release that was on the driver side pillar of the vehicle. After we arrived to Napleton Jeep the car immediately turned on. Our biggest safety concern is that this would’ve happened on a highway and my wife would not know how to react resulting in an accident or fatality. This is an issue that never in our 30 Years of experience we had with any of our vehicle brands. I wish the National Highway Safety Administration take measures to Jeep and investigate this matter. ... ill didn’t include transmission fluid on the materials list. I called the service advisor and asked if it had been changed. The service advisor said he couldn’t prove it as it was not listed so he asked that I bring it back in and he will make sure it is changed. I brought the car back in.. The service team never mentioned any concerns with transmission. Whining started a couple weeks after. I took my car to transmission shop, who inspected a found black burnt transmission fluid with metal pieces. I informed them that I brought my car in for service which was supposed to include a transmission fluid change. They said it didn’t look like it was changed, and if it was, the the service tech would have noticed the black fluid and should have advised of transmission concerns. I was advised that the transmission is damaged beyond being rebuild and needs to be replaced. This failure is a known and acknowledged issue with Toyota UA80F/E transmissions. I am reporting a safety-related failure involving the clutch system on my 2022 BMW M3. At approximately 26,000 miles when I purchased, I began experiencing clutch-related issues, including slipping and abnormal engagement. I brought the vehicle to the BMW dealership on two separate occasions to report these concerns, but the dealership dismissed the issue and did not document or inspect the clutc
... h. At approximately 33,000 miles, while driving on the freeway, the clutch failed to properly engage and would not catch gears, creating an unsafe driving condition and placing myself and other motorists at risk. This loss of engagement occurred during normal driving and was not related to abuse, racing, or track use. After this incident, the dealership inspected the vehicle and discovered excessive clutch debris through the bellhousing inspection port, indicating accelerated and premature wear. The clutch and flywheel now require replacement. This issue was reported earlier but not documented, delaying diagnosis and allowing the clutch to degrade to a point where it created a safety hazard. I am concerned this represents a premature failure of a critical drivetrain component under normal driving conditions. I am submitting this complaint so the issue is formally documented as a potential safety defect.Read more

On the latest visit (4th since 73B recall) it's been at the dealership for 31 days and counting. The dealership is waiting on Chrysler to figure out a fix for the van. The vehicle has over 50 stored messages. This recall was supposed to monitor the hybrid battery for early indications for a fire, but according to the original recall all batteries are assumed to have the manufacturing defect that leads to spontaneous fires. It can not be driven now. The lights that came on were the Rear axle system off, forward collision system, service traction control, engine light flashing, and service ESC light. Not to forget that it would not go into reverse. Also the suspension system seemed to malfunction and not lift or lower the car.It seemed to clunk and try, but nothing happened. NHTSA Vehicle Safety Complaint I am reporting a serious transmission defect in my 2024 Chevrolet Silverado 3500HD LT that creates a high risk of sudden power loss, erratic shifting, or loss of control while driving. Vehicle Details: - Make/Model/Year: Chevrolet Silverado 3500HD LT, 2024 - VIN: 2GC4YTEYXR1263768 - Engine: 6.6L Duramax diesel - Transmission: 10-speed Allison (10L1000) - Mileage at
... diagnosis: ~46,000 miles - Title: Rebuilt (warranty denied due to branding) Defect Description: The transmission shifts normally when cold (fluid below ~138°F). Once warm (during normal driving), it fails to upshift properly in gears 5-8: holds lower gears, causes high RPMs (3,000+), delays shifts excessively, or refuses to shift. When shifts occur, they are hard and jerky. Automatic (Drive) mode is affected; manual/L mode works normally as a workaround. No warning lights appeared initially, but the issue has worsened and is now frequent at operating temperature. Diagnosis: On January 28, 2026, Seminole GM (1405 N Milt Phillips, Seminole, OK 74868; 405-382-6130) diagnosed a defective valve body requiring replacement. Quote: - Valve Body Control Module (Part #24065357): $687.48 - Transmission Fluid (12 qt @ $10.92): $131.04 - Labor (diagnosis + pan/filter/relearn): $805.50 - Supplies/taxes: ~$140 - Total: $1,763.94 Dealer confirmed this is a common issue; photos of quote available for upload. Safety Risks: At highway speeds, delayed/no upshifts can cause: - Sudden inability to accelerate (merging, passing, hills) - Harsh engagement leading to wheel slip or loss of control - Potential limp mode entry, stranding in low gears This intermittent warm-only failure is unpredictable and dangerous. Context: This matches widespread complaints on 2023-2025 GM HD trucks with the 10L1000 (forums: DuramaxForum, Reddit r/Duramax/r/Silverado, GM-Trucks.com), involving valve body wear/pressure loss. While driving my 2025 Nissan Armada Platinum Reserve during morning rush hour traffic and moving with the speed of traffic, the accelerator (gas) pedal assembly physically broke off, causing an immediate loss of throttle control. The failure occurred in active traffic at a busy intersection located at the top of a hill, leaving the vehicle stranded partially in a turn lane and straight-through la
... ne. I was unable to safely move the vehicle out of traffic due to the pedal detaching from its mounting point. There were no warning lights, no prior looseness, and no unusual resistance prior to the failure. This appears to be a mechanical/structural failure of the accelerator pedal assembly, not an electronic throttle or sensor malfunction. Due to the dangerous location and inability to move the vehicle, law enforcement (a sheriff) responded approximately 30 minutes later to assist with traffic safety. The vehicle remained immobilized in traffic for an extended period, and it took nearly two hours for a tow truck to arrive. This incident created a serious and prolonged safety hazard, exposing me and other motorists to a high risk of collision. The vehicle had approximately 9,700 miles and has been driven under normal conditions. The vehicle is currently not being driven due to safety concerns. The failed accelerator pedal assembly is available for inspection. I am reporting this as a potential manufacturing or material defect and believe it warrants investigation to prevent similar failures in other vehicles.Read more

Had this occurred at highway speeds or in active traffic, it would likely have resulted in a dangerous situation with a high risk of collision, especially given the severe winter weather, low visibility, and slippery road conditions. The vehicle has approximately 21,000 miles. This is the second failure of the Integrated Charging Control Unit (ICCU) in this vehicle. The vehicle was inspected by an authorized Kia dealer, who has officially diagnosed the failure as a failed ICCU and associated fuse. There were no warning lights or messages prior to the failure. The first indication of a problem was the popping sound, immediately followed by limp mode and then a complete shutdown without further warning. The vehicle is available for inspection upon request. The repeat nature of this failure, lack of advance warning, and sudden loss of propulsion represent a serious safety defect.Read more

2020 Ford F250 with 85,000 miles. We purchased new and all maintenance has been done on time and by Ford Service. Transmission began jerking when going from park to drive (or reverse). Now the transmissio
... n is not shifting gears smoothly when the truck is in motion. This is a known problem with trucks manufactured during this time period with 10R80 transmissions. Ford has been less than helpful in helping me come to a resolution. They tell me there was a TSB sent out. They attempted to reprogram software (at a cost of $424 to me). I will upload the invoice from Ford. This only made the problem worse. They quoted me a "special price" of $8,500 to replace the transmission, however their service department is SIX WEEKS out. For this reason we have chosen to have the transmission replaced at a local transmission shop. Their price is $9,932.41. I have attached that invoice. Ford needs to take accountability for this safety problem and attempt to make these vehicles safe. They should absorb at least some of the cost to fix this design problem. We paid full price to purchase a new, safe vehicle. That is not what we received. Rather than a "Technical Service Bulletin", Ford should be forced to do a recall and make this right for customers who paid top dollar for these vehicles.Read more

The component that malfunctioned is the Integrated Charge Control Unit (ICCU) and related high-voltage charging system in my 2022 Hyundai Ioniq 5. The ICCU controls AC (Level 2) charging, DC fast charging functions, and power delivery. The ICCU assembly and charging components were serviced and replaced by an authorized Hyundai dealer and are available for inspection through service records. This
... defect created a safety risk because the vehicle experienced complete charging failure and reduced-power operation. Loss of charging capability can leave drivers stranded, and reduced-power “turtle mode” conditions can limit acceleration and vehicle performance, creating hazards in traffic. The charge port has also not locked during charging, creating a potential risk of electrical arcing during DC fast charging. The failure was reproduced and confirmed by an authorized Hyundai dealer. In approximately January 2025 the vehicle would not charge and remained at the dealership for an extended period (about January through June 2025). After diagnosis and consultation with Hyundai technical support, the dealer replaced the ICCU assembly and high-voltage fuse. The vehicle and charging system have been inspected multiple times by Hyundai service centers. Hyundai issued recall campaigns in 2023 and 2024 involving ICCU software updates and fuse-related work, but these did not prevent the later failure requiring full ICCU replacement. Prior symptoms included abnormal charging behavior and eventual total loss of AC charging capability during the 2025 failure, making home charging impossible. After the ICCU replacement, the vehicle exhibited unusually loud cooling fan operation during Level 2 charging, documented by the dealer. There were also charging-port safety concerns. After one service visit, an important DC pin cover/cap was not properly installed until corrected on a return visit, leaving charging components exposed.Read more

I’ve been having persistent issues with my transmission for some time now. It occasionally would have a hard shift or a lagged shift where it would rev high and not go into the next gear for a few second. This has been accompanied by an odd “clown honk” type sound whenever shifting from 3rd gear into 4th gear after the truck is warm. Sometimes it would get stuck in one gear or another or una
... ble to shift to certain gears until it was shut off and reset. These problems are sometimes accompanied with each other and sometimes on their own. Not until recently did my transmission downshift on its own when I was going around 50-60MPHs to first gear, throwing my back end out and almost causing me to crash. Luckily I was on open and empty road. This occurred sometime in December and it finally threw a code which it had not for any incident previously. This issue has not happened again since. Not only was the incident itself a case of deja vu but when I was able to have the code read, It was almost the exact same issue/code that I had with my previous truck, a 2013 Ford F-150. My 2013 ford f-150 did the same exact thing, almost causing an massive accident as I was merging onto I-85 at 4:30 PM. This issue was recalled and fixed by Ford in the 2013 F-150 but has no correction for my current 2016 F-150. I believe the cause of this is a faulty lead frame and/or Output Speed Shaft Sensor on the 6R80 transmission from Ford. I’ve now had two of these power trains (5.0/6R80) and both have had problems occasionally up until around 120K miles then the problems become consistent and the downshifting issue started which throws the code/is the real dangerous issue. I have reported the Issue to Ford through a dealer and called the customer support line but since there is no open recall on my specific VIN for this issue they are unable to offer assistance. The truck is open for inspection and has the code stored. This can be a very dangerous issue and needs to be addressed.Read more

When driving recently, the car suddenly began showing a warning message “check electrical system” and would not go beyond 25mph. I was on the way home but still far, and had to turn on my hazard lights to alert other drivers. Eventually the car escalated the warning to “pull the vehicle over” and implied that the main battery was having an issue. I had to have th car towed to a dealer
... (closest is 10 miles) who eventually diagnosed the issue as a blown ICCU. My understanding is this is a critical component to the charging infrastructure and must be fully operational for the car to safely drive. Hyundai repaired the part and I had the car back in a few days, but I could get no assurances on what happened or whether or not it would occur again. The car has 14,000 Miles in the Washington DC area. It seems incredibly unsafe that this component could blow at any time. I also understand there are many other owners with this exact issue.Read more

Starting back in June 2025 Loss of engine power multiple times. Many times almost hit from behind as I turned onto a highway. Found out vehicle had a stop sale recall at the time of purchase and I had issues with back right tire. Took truck to a tire shop and learned that the wheel was welded in many places and fracture was visible. Finally, got the dealership to pay attention in October to my
... concerns. They were going to allow me to trade the vehicle out for another one while paying off the bank but that’s not what they did. They attempted the repair and tried to hand it back to me. 1/27/26 Was recently told this was corrected, and the sales person told me when he picked it up from shop he couldn’t go more than 30 mph. Dealership management told him not to tell me. I am submitting this complaint for immediate review regarding Ruiz Ranch Motors (Spring Branch, TX) for an illegal vehicle sale, prolonged withholding of my vehicle, misrepresentation of a “loaner” vehicle, and coercive threats involving law enforcement. This matter involves ongoing safety risks, financial harm, and improper dealer conduct affecting both myself and my minor child. Summary of Key Facts (Concise): • On June 25, 2025, I purchased a 2021 Chevrolet Silverado (VIN 1GCUYHET7MZ155657) from Ruiz Ranch Motors, financed through Ally Financial. • The vehicle was subject to a GM Stop-Sale / No-Sale Safety Recall (NHTSA 24V797000) at or around the time of sale. • I repeatedly raised safety concerns beginning shortly after purchase. These concerns were dismissed for months. • In November, Ruiz Ranch Motors took possession of my 2021 Silverado under the representation that they were “working on trade numbers” and a solution to correct the experience. Misrepresented “Loaner / Trade” Vehicle: • I was provided a 2023 Chevrolet Silverado, represented as a potential trade option, no trade was made (only dealership delay) this vehicle now has a engine light and no cooperation from dealershipRead more

Was traveling to Ft. Worth to babysit my grandchildren and when I was on the freeway in afternoon traffic my car was not accelerating and I kept stepping on the gas and the car started to get slower and then all of a sudden it completely stopped in the middle of traffic(I didn't pull over to the side like Mercedes stated, it stopped in the middle lane of the freeway, the engine light was on) and I
... had to locate the hazard lights so no one would run into the back of my car. When it stopped, I had to put it in park and restart the car. I thank God that my grandchildren weren't with me or that someone didn't hit the back of the car. I ask Siri to find the nearest dealership and it was Mercedes-Benz of Grapevine, proceeded to go there first. They called me the next morning and told me it was the Fuel Pump. Then on Jan. 16, 2026, I took my car in for routine maintenance and they found a leak on the Front Differential and they practically removed the whole engine to fix the leak. Had this fix on Jan 20, 2026 This car just turn 2 years old on Dec. 23. Contacted the corporate office as I was directed by the Service Director, Richey Barnes and was told that before they could do anything it had to break down again or anything major. I told them that I have my young grandchildren with me since I often have to babysit them and he told me that there was nothing he could do.Read more

I am filing this complaint regarding a serious and ongoing safety issue with my GMC Yukon Denali that began with a complete engine failure while driving on the highway with my entire family in the vehicle. While driving at highway speed (about 60–70 mph), the vehicle suddenly lost power and shifted into neutral on its own. While still moving, I tried multiple times to shift back into Drive so I
... could safely get off the road, but it would not engage. I was only able to coast partially onto the shoulder before the vehicle came to a complete stop, still partly exposed to traffic. The engine shut off and would not restart at all. It would not go into Drive or Neutral, and the vehicle was completely dead. And it had to be towed. The dealer later told me the engine had failed and that a new engine was installed. About a week later, I picked the vehicle up. Less than 20 minutes after leaving the dealership and getting back on the highway, smoke started coming from the vehicle and warnings appeared, messages that the engine was overheating and that I needed to stop the engine immediately. The vehicle was not drivable again and had to be towed back. I again had to arrange transportation home. A few days later, I picked the vehicle up again. Since then, I have had serious four-wheel-drive and drivetrain issues. The vehicle became stuck in 4 Low and I could not get it out for over a day. When it finally came out, the drive mode buttons stopped working correctly. Selecting 2 High sometimes puts it into 4 High, selecting Auto puts it into 2 High, and Auto mode no longer appears. Even when it shows 4 High, the steering is restricted cannot steer 100% to the right or left and the vehicle binds, pops, and jumps when turning. I believe this vehicle is unsafe and poses a serious risk.Read more

ICCU failed while at highway speeds (65mph). Car went into 'turtle mode' limiting the car to 10mph. No warnings were provided prior to the incident. I was in the left of three lanes and had to make emergency actions to get to the Right shoulder across traffic. Traffic was obstructed. Was left stranded until a tow truck was available. Dealer confirmed ICCU failure.
