While driving my 2013 F150 Eco boost is started shifting erratically. While at 60 MPH the truck would down shift on it's own to 1st gear and my truck would skid. Luckily I did not crash but it's dangerous and it's my only transportation. I took it to Ford and they said I needed a $8K transmission replacement. I ended up taking to a transmission shop for a rebuild but shortly after if did it again
... . The transmission shop replaced the valve but said Ford needed to reflash the TCM. When I asked for them to follow recall 19S07 they said I would need to bring it in and pay them another $400 for a diagnostic and they couldn't just reflash it. Now I'm in a position where I have to drive what feels like a timebomb and I have no way out of it.Read more

The transmission in my 2019 Honda Odyssey failed prematurely at approximately 102,800 miles, requiring a full transmission repair/replacement. Prior to failure, the vehicle exhibited known warning symptoms including harsh and erratic shifting, hesitation, delayed engagement, and loss of smooth acceleration. These symptoms progressively worsened and ultimately resulted in unsafe drivability. This
... failure is consistent with a well-documented pattern of transmission defects affecting 2018–2019 Honda Odyssey models. Numerous owners have reported similar issues through NHTSA complaints and Honda service channels. A transmission failure at this mileage is not consistent with reasonable consumer expectations for a modern vehicle and presents a safety concern due to unpredictable shifting behavior, hesitation during acceleration, and loss of drivability. These issues increase the risk of collision, particularly when merging, accelerating from stops, or driving in traffic. Honda has declined assistance solely on the basis that the vehicle is outside the powertrain warranty, despite the failure aligning with known defect patterns rather than normal wear and tear. I am submitting this complaint to document a recurring defect and request further investigation into premature transmission failures in this model year. After receiving a replacement transmission in my 2019 Honda Odyssey, the vehicle continues to experience the same failure symptoms: harsh shifting, delayed engagement, and intermittent slipping. These ongoing issues indicate that the replacement transmission did not fully correct the underlying defect, which is consistent with other complaints submitted to NHTSA regarding 2018–2019 Odyssey models. I am requesting that NHTSA investigate this recurring defect and track the persistent issues associated with replacement transmissions in this model year.Read more

Vehicle: 2016 Chevrolet Tahoe Mileage at failure: 96,700 miles Component: Powertrain – Transmission While driving on the highway returning home from Columbus, OH, my 2016 Chevrolet Tahoe experienced a sudden transmission failure without any warning. We were traveling in the third lane at approximately 70–74 mph when the vehicle abruptly lost power and rapidly slowed down to approximately 15–
... 25 mph. This occurred in active traffic and created an immediate and severe safety hazard. The sudden loss of speed nearly caused a collision with surrounding vehicles traveling at highway speeds. There were five occupants in the vehicle, including three children. Due to traffic conditions, other drivers were braking, honking, and swerving. I was only able to avoid a crash by immediately maneuvering across lanes to reach the shoulder. The vehicle was disabled and had to be towed. The transmission required full replacement. Total repair cost exceeded $6,000, and the vehicle was out of service for approximately two weeks. I am submitting the repair receipt from the transmission shop with this complaint. After the failure, I learned that sudden transmission failure is a commonly reported issue with Chevrolet vehicles, including the Tahoe. This failure occurred without warning and represents a serious safety defect that put my family and other motorists at risk.Read more

On January 7, 2026, shortly after delivery of a used 2015 Hyundai Tucson (VIN [XXX] ), the vehicle experienced immediate mechanical failure while being driven, resulting in sudden loss of power and drivability. Multiple warning lights illuminated on the dashboard at the time of failure. The vehicle became disabled and left the occupants stranded roadside in Fairhope, Alabama. The exact component
... that failed is UNKNOWN at this time. The failure appears related to the engine, powertrain, fuel/propulsion, and/or electrical systems. The vehicle was not operable after the incident. On January 8, 2026, the selling dealership (Sandy Sansing Nissan, Pensacola, Florida) towed the vehicle back to their dealership. The vehicle has remained in the dealership’s possession since that date and is available for inspection upon request. This sudden failure created a significant safety risk due to loss of propulsion while in traffic and exposure to roadside hazards. The problem has not been independently repaired or evaluated by the consumer. No manufacturer inspection has occurred to the consumer’s knowledge. The dealer has not provided a diagnostic report or repair findings. No warning lights or symptoms were disclosed prior to delivery. The failure occurred almost immediately after delivery. The vehicle was sold and delivered by a licensed dealership. The sale is currently in dispute because the vehicle became disabled almost immediately after delivery due to this safety-related failure. 10L1000 transmission, began having problems with delays and harsh gear engagement. Flare shifting and shuddering. Finally starting having transmission overheating issues while towing. I had to make several unsafe stops with a trailer to wait for the transmission to cool down again before continuing to get it home. Saw trans temps up to 240°F on flat land with 40° outside temps. I could smell the
... hot transmission fluid inside the truck. When I went to check the fluid level of the transmission, the fluid was full but had a large amount of glitter like metallic flakes. The fluid was very dark brown in color and smelled terribly burnt. The last recall I took in to the dealer for was about the transmissions possibility of failing, locking up, along with the rear wheels at any speed. I have young children that ride in this truck, that in it self was unsettling and unsafe. But then to have issues with absolutely no warning light was concerning. Ive since ordered an aftermarket built transmission to avoid repeating the same issues with a factory GM replacement. My factory warranty had also ended a few months prior to having these problems with it. Leaving me to completely foot the bill for it to be replaced. I had to pull money from my retirement early to cover costs, in fact costing me much more than just the sticker price.Read more

I am reporting multiple safety-related failures with my 2025 Honda Prologue. The vehicle has an unresolved drivetrain/axle defect that affects safe operation, and the dealer has stated that axle components are on backorder with no repair timeline. In addition, the main vehicle control screen completely blacked out and will not turn back on. This screen controls critical vehicle and charging funct
... ions. Without it, I cannot control charging or access essential vehicle systems, rendering the vehicle unsafe and effectively unusable. Further, the charging port sensor failed to alert me that the charging port was open. Because no warning was provided, the port remained open and subsequently broke. This represents a failure of a safety-related sensor system that directly impacts the vehicle’s ability to charge and operate properly. The dealer has not provided a loaner vehicle or alternative transportation despite the vehicle being unsafe to operate. These combined defects substantially impair the safety, use, and value of the vehicle.Read more

Around 55,000 miles, my 2021 Chevrolet Blazer experienced a throttle body failure. The check engine light illuminated and the vehicle entered a reduced power/limp mode condition. Throttle response was significantly limited, and the vehicle could not accelerate normally. While the vehicle remained drivable, it took substantially longer to reach speed, which could pose a safety risk when merging int
... o traffic or crossing intersections. Diagnostic trouble code P2101 (Throttle Actuator Position Performance) was present. A Chevrolet dealer confirmed an internal throttle body failure and replaced the throttle body assembly. This component is a critical engine control system, and failure could affect acceleration and vehicle control, posing a potential safety risk. The failure occurred well before expected service life and required a costly repair. The component has been replaced and is available for inspection upon request. I believe this may represent a premature defect and should be investigated for similar failures in other vehicles.?Read more

At approximately 5 years and 2 months of ownership, with approximately 53,000 miles, the clutch slave cylinder failed in such a way that contaminated both the clutch and flywheel with brake fluid. Repair of this issue requires replacement of the slave cylinder, clutch, and flywheel, as described by Toyota of Pullman in Pullman, WA who performed the inspection and quoted the repair. The symptoms of
... this failure included an inability to shift the manual transmission between gears, most notably out of 1st gear to get to 2nd. This inability to change gears resulted in limited speed in traffic, and created an obstruction. In less fortunate traffic conditions, this speed restriction could have created a collision. Other owners of the same model vehicle online have reported active failures in traffic, some of which have resulted in damage to the transmission as well as the clutch and flywheel. Toyota appears to have redesigned the slave cylinder at least twice, implying that they know the parts for these vehicles are subpar, and there are no flywheels available for the repair, which also implies this may be an all too common repair.Read more

Took my 2020 BMW 7 Series into BMW San Diego because there was fluid leaking. Upon diagnosis at the dealership, I was notified I needed to replace the front differential as it was coming apart. The leaking fluid was identified as differential fluid. The vehicle has 54,308 miles on it. My research indicates front differentials should not need replacement until nearly 150,000 miles. A broken front
... differential is considered to be very bad and makes a car unsafe to drive as it causes severe handling issues, loss of control and potential damage to the transmission. A broken differential can cause the vehicle to lock up or lose steering control. Me and my family have been at risk as it is unclear how long this issue has been happening. The problem has been confirmed by BMW San Diego. The cost to replace was quoted as about $10,251. There are no warning lamps, and there are no messages indicating there is a problem with the vehicle. New leased 2025 Nissan Murano vehicle 7,386 miles with recent software upgrade by Nissan dealer, Keyport, New Jersey. As I accelerated from a complete stop, the vehicle experienced an unintended increased excessive speed acceleration and when I applied the brakes to slow the vehicle, the acceleration wouldn't stop knocking me back into my headrest hurting my neck and right ankle. I had to shut off

A message displayed to update the vehicle software which I did immediately and was unable to restart during vehicle requested update. I put on my hazard lights remaining in the vehicle as snow banks covered each side of the road and it wasn't safe to exit my vehicle with traffic and ice conditions. I was put into a situation where I could not exit the roadway and vehicles were approaching my rear as I was in a single lane roadway with traffic had going around me when safe to pass. After the upgrade I attempted to restart the vehicle and it would not until I opened the door and reapplied the brake pedal realizing that moment I injured my ankle pressing on the brakes so hard to stop my speeding vehicle. It did finally start after the 5th attempt afyer the software upgrade. After this event upon starting the vehicle I drove it off the roadway and the seats began to heat up hotter than tolerable and I could smell what seemed like burning plastic with the steering wheel heating up too hot to touch. While driving, a warning message displayed on the dashboard stating that there was a fault in the electrical system and that I should pull over and stop the car. As I tried to find a safe place to pull over the car lost power and I pulled onto the shoulder. The shoulder was only as wide as the car. I was stopped on the side of a two lane road with a 55 mph speed limit. In order to exit the car I
... had to step into the travel lane. The electrical fault resulted in no power available to the warning flasher lights or headlights. This occurred at night with no street lighting to show oncoming drivers that there was a stopped vehicle immediately next to the traffic lane. This put me and other drivers at risk. The vehicle was towed to a dealer capable of working on this type of failure. The dealer determined that the failed component is known as the ICCU. This is a critical part and had been known to fail in this model. My vehicle had already received every software update in an attempt by the manufacturer to avoid the ICCU failure. The dealer replaced the failed ICCU with a new working ICCU. It is my understanding that the replacement ICCU is identical to the failed ICCU and a risk remains that the new one could fail as well with a loss of power while driving at speed.Read more

SEE FULL COMPLAINT NOTES IN ATTACHED PDF; SUMMARY HERE (you do not provide sufficient space to provide all info you request) I own a 2019 Ram 1500 4WD Limited (not eTorque). On multiple occasions, the vehicle has unexpectedly shifted out of Drive into Neutral while driving. When this occurs, a warning chime dings and a message on display directs me to “Shift to Park”. The vehicle immediately l
... oses forward power (because it has shifted itself to Neutral), until I come to a complete stop, shift to Park, and then shift back to Drive to move forward. Usually the engine stays on, but on at least one occasion the engine turned off and I had to restart. This has happened at least five times total, including three times within the past month. The events have occurred while decelerating to a stop, braking but not stopping, and accelerating from a slow down. Each time, I experienced a sudden and unexpected loss of propulsion, requiring me to stop the vehicle in traffic, shift fully into Park, and then reselect Drive before the vehicle would move again. These incidents have occurred in active traffic situations and have placed me and others at risk of a collision due to the sudden loss of power (SEE DETAILS IN PDF). I previously brought the vehicle to a dealer for this issue after it had just occurred (~1 year ago, before the recent increase in frequency); however, the dealer stated that the vehicle did not throw a code, or no code was stored. They could not replicate the problem and no repair was made. There are at least 55 NHTSA complaints filed that appear the same as my issue; none appear to have been resolved by dealers or RAM. This appears to be a systemic safety defect involving the powertrain and electronic shifting system. The unexpected loss of propulsion in traffic presents a serious risk of injury or death. I am submitting this complaint due to the ongoing and unresolved safety concern. I am scared to drive my family in my vehicle until it is known to be fixed.Read more

The vehicle was subject to an active manufacturer safety recall involving the oil pressure control system and diagnostic trouble code P06DA, which requires inspection and replacement of the Engine Room Sub-Harness if the condition is present. The defect was confirmed by an authorized dealer, DTC P06DA was verified, and the Engine Room Sub-Harness was replaced in accordance with the recall remedy
... instructions. However, the manufacturer failed to fully remedy the recall, as the consumer was required to pay a significant portion of the repair cost out-of-pocket. While the manufacturer provided partial reimbursement, the remaining balance was charged to the vehicle owner despite the repair being performed solely to address a safety recall condition. Requiring consumer payment to obtain a mandated safety recall repair does not constitute a complete or timely remedy under recall obligations. This complaint is submitted to document the manufacturer’s failure to fully comply with safety recall requirements and to seek appropriate oversight and corrective action.Read more

Driving on the highway at highway speeds, heard clanking then a loud bang, then vehicle would not longer respond to the accelerator so we coasted onto the median. We had the vehicle towed and found out from a Ford dealer that the rear axle had broken due to Ford's failure to repair under Manufacturer Recall Number23S65 NHTSA Recall Number23V896 and that had caused damage to the rear differential.
... We were fortunate not to have been in or caused a significant accident. I had the vehicle at a Ford dealer in Illinois in 2024, but the recall was not performed due to lack of parts. This incident happened thereafter, in November of 2025, and the vehicle has been off of the road at a Ford dealer in Indiana since. Same story, no parts. Ford offers no timeline for repair, advising it could be 3 months, could be a year - it is indefinite. I recently was advised that there are 70,000 orders for the parts for this repair, and although I am on the "emergency off road list" that is still 15,000 orders long. Additionally, I had the vehicle in early in my ownership window for lags when the transmission shifted at certain times, it was checked and given the all clear, but - intermittently arose - given Ford representations I ignored it but never felt the delays were safe as it slowed vehicle responsiveness, albeit it slightly. I now see active litigation around these transmission issues in the F-150.Read more

I am writing to formally request that the National Highway Traffic Safety Administration review and consider an investigation into a potential safety-related defect affecting 2017 Dodge Grand Caravan vehicles. My vehicle recently experienced a failure of the engine oil filter housing, which cracked and leaked oil and required an expensive repair. According to the repair facility, Pro Auto Service
... , this failure is not isolated. I was advised that the shop has repaired a significant number of identical failures over the past year, to the extent that replacement parts have at times been difficult to obtain. The repair facility also indicated that many similar failures have occurred in recent weeks, coinciding with extreme cold weather conditions. The oil filter housing is a critical engine component responsible for maintaining proper oil circulation and pressure. Failure of this component can lead to oil loss, reduced lubrication, engine damage, or potential engine shutdown if not immediately addressed. Because this defect directly impacts engine operation, it presents a powertrain-related safety concern. Based on the frequency of reported failures, the consistency across the same model year, and the apparent correlation with cold weather conditions, this issue appears to be the result of a design or material defect rather than normal wear and tear. I respectfully request that NHTSA review this matter for possible investigation and recall consideration to protect vehicle owners from unexpected engine failure and costly repairs. Thank you for your attention to this concern and for your continued commitment to vehicle safety.Read more
