I was driving and the engine abruptly stalled. The truck self-shifted to Neutral. Once stopped, I shifted to Park to attempt a restart. It would not crank. I was on a 2-lane road so I did not have a shoulder. Once I realized it would not restart I tried to shift to Neutral so I could push it to a subdivision road about 50 yards away only to find out my transmission has no ability to shift to Neut
... ral (the dealer confirmed this later). So I was stuck on the road for hours just over the crest of a hill with people coming up on me and my family at 50mph. It was a windy 10 degrees so we could not wait on the side of the road where we would have been safer. This is how people get killed; being stopped where people can't see you and do not expect you to be stopped. Why can I not get the truck off the road? If this happened on an 8-lane highway how would I get to a shoulder, even after traffic had stopped behind me? On top of all that, the problem with the truck ended up being an engine bearing failure (per the dealership service dept) that is a known problem with this engine and truck build date. General Motors needs to proactively diagnose and get these engines replaced before someone gets hurt. There is a TSB issued for the bearing issue but when I asked my dealer about it a month ago, before the failure, they acted like they knew nothing about it and told me to bring it in if I hear noises. If they had offered to check the crankshaft endplay when I asked about it ( as the TSB suggests), I may not have gotten stranded and my family's safety put at risk. There were no warning lamps, and no strange noises before the engine seized. My fuel economy had decreased by about 20% in the last month but that may be from cold weather operating conditions. I have a separate electrical issue that I've been chasing down and I assumed this stall condition was related to that. But the dealer confirmed this was a bearing failure.Read more

Transmission will randomly downshift while driving or stay in a certain gear without shifting down or up resulting in unexpected power loss. Is sporadic in occurrences sometimes several times in a short drive other times a week without incident
Incident date: Oct 24, 2025
Uncommanded downshift at highway speed causing sudden deceleration / loss of vehicle stability. Has happened twice, once in 2020 and again in 2025. Typically in slower than highway speeds when traffic is moderate to high and traffic starts to clear. Accelerator is pressed to return to highway speeds and the vehicle downshifts suddenly. Nearly caused collision from the rear vehicles both times.

I am writing to report an additional safety‑related incident involving my 2024 Jeep Wrangler Rubicon 4xe. This vehicle previously experienced propulsion issues that I reported under NHTSA Case Number 11692887. On the morning of February 4, 2026, while driving to work on a busy interstate (approximately 12 miles from home), the vehicle again began exhibiting propulsion instability. The vehicle e
... xperienced repeated surging and thrusting movements that became progressively more severe during the drive. During these events, the check‑engine light intermittently illuminated and then turned off. This incident occurred exactly seven days after my vehicle was evaluated by Sam Leman Dodge on January 27, 2026, for three open safety recalls. At that visit, I was provided paperwork indicating that the issues had been addressed. However, as of today, the official Jeep/Mopar recall website still shows multiple serious and active safety recalls for my VIN, including recalls involving fire risk and sudden loss of motive power. After the February 4 incident, I contacted the dealership, and their service department advised me not to drive the vehicle and recommended that it be towed to the dealership for evaluation. I followed this guidance due to the severity of the propulsion behavior and the safety concerns involved. Given the recurrence of propulsion instability, the worsening severity of the surging behavior, the dealer’s recommendation not to drive the vehicle, and the presence of multiple unresolved safety recalls, I am requesting NHTSA’s assistance in ensuring that the manufacturer addresses these issues and restores the vehicle to a safe operating condition. Thank you for your attention to this matter. Recognize that this is the second time I have emailed the NHTSA regarding propulsion issues with this vehicle (Earlier Case Number 11692887). Please help me build a relationship with Jeep to ensure this vehicle is restored to a safe condition.Read more

While driving my 2020 Ford F-150 on the highway, the transmission suddenly failed, causing loss of normal drivability and creating an immediate safety concern due to reduced vehicle control at highway speeds. The failure involved the transmission system, which I later learned may be related to Ford’s recall notice stating that the installed remanufactured transmission may be missing an internal
... bearing. Prior to the complete failure, the vehicle exhibited symptoms consistent with the recall, including harsh shifting, shuddering, and vibration. These symptoms escalated until the transmission malfunctioned to the point the vehicle was no longer safely operable, requiring me to tow the truck to Rocklin Transmission. The transmission was replaced out of pocket by that independent service center, and the failed component should be available for inspection upon request through that facility. The malfunction created a serious safety risk because the transmission could become stuck in certain gears, illuminate the check engine light, or potentially fail to properly engage Park, all of which increase the risk of a crash or injury. The issue was confirmed by the independent repair facility when diagnosing and replacing the transmission. The vehicle has not been inspected by Ford or other representatives prior to the repair because the failure required immediate corrective action to restore safe vehicle operation.Read more

This model Civic Si was build by Honda with a defective clutch. Mine began to fail at less than 60K miles. It started in the higher RPM 5K to 6K in the taller overdrive gears. This clutch design was unable to support the torque of the motor, which led to its premature failure. It's well documented in Honda forums that the clutch is weak and prone to early failure. It usually starts to fail in
... the upper gears under moderate acceleration. Once it starts to fail it does become a problem. Honda appears to be aware of the problem as they did have a TSB out that allowed for replacement for 5 years of 60K miles. However, reports on the internet seem to state Honda charges $2K just to tear down the car to look at the clutch. My car was low mileage and still hasn't hit 60K miles but outside the 5 year warranty. However, even if Honda had agreed to replace the clutch they reportedly leave the original dual mass flywheel, which would contribute to a shorter life span for the new clutch they just installed. The OEM manufacturer Exedy won't sell you the same clutch that Honda put into this car, and will only sell you the clutch from the type R. Thus it does appear clear that both Exedy and Honda were fully aware of the manufacturer defective clutch. A clutch like this should last over 100K miles. However, Honda rather then recalling them appears to have just thrown it to the wind and is willing to just let whatever happens pan out and see what's left . The failure of this clutch once it starts to slip does appear to be a relatively short process before total failure. Although it appears there has been no deaths so far there is a good chance someone will delay replacing that clutch because of cost, which could lead to a severe auto-accident or death. Even if Honda doesn't want to do a recall or reimburse their car owners they should at least issue an alert to owners about their defective clutch with free inspections. Respectfully submitted.Read more

Incident date: Nov 1, 2025
I am reporting two safety-related problems in my 2022 Lexus GX 460, both present since shortly after purchase and the vehicle is available for inspection upon request. 1. Power Train / Speed Control (delayed acceleration / no throttle response): When I transition from the brake pedal to the accelerator pedal quickly (normal driving), the vehicle intermittently displays a warning message indicatin
... g the accelerator is being pressed while the brake is also pressed (or similar wording). When this happens, the vehicle does not respond to the accelerator for several seconds even if I press the accelerator to the floor. This has put my safety and the safety of others at risk because the vehicle fails to accelerate when needed (e.g., when merging, turning, or avoiding traffic), creating multiple dangerous near-miss situations. The warning message appears at the time of the event; I did not notice a warning lamp or message consistently prior to the event. This issue has not yet been reproduced or confirmed by a dealer or independent service center. A dealer service visit is scheduled for next week. The vehicle has not yet been inspected by the manufacturer, police, or insurance representatives for this problem. 2. Seats (driver seat causes leg numbness/spasms impairing pedal control): The driver seat bottom is unusually firm, and after approximately 30 minutes of driving my legs become numb from the lower thighs down to my feet and I experience spasms to the point that I cannot safely operate the brake and accelerator pedals. This is a safety risk because it can prevent proper pedal control while driving. This condition has been consistent since shortly after purchase. It has not yet been confirmed by a dealer or independent service center, and the vehicle/seat has not yet been inspected by the manufacturer, police, or insurance representatives for this issue.Read more

While I am driving in normal drive when my vehicle is in 6th gear cruising there is a shutter in the transmissionthat happens (like running over rumble strips)and only sixth gear. After picking my truck back up they said since I have no codes and they found a flaw in my rim. And that is what is causing the only sixth gear low rpm transmission shutter. And the shutter doesn't happen during higher rpms even at the same speeds. I have read up on this shutter and everyone was saying the same feeling in there f150s and was a transmission shutter. 2018 Kia Sorento 2.4 GDI. In late 2024 a Kia dealer performed an oil-consumption test and confirmed the engine was burning excessive oil. In Feb 2025 the engine long block was replaced. Within about 4,000 miles the vehicle began setting P1326 warnings, misfiring, losing power, and struggling to climb hills. On a highway trip from South Carolina back to Indiana, the vehicle could barely maintain 3
... 0–40 mph on grades with the accelerator floored while traffic was moving much faster. The check-engine light returned repeatedly. The vehicle eventually died on an exit ramp and had to be pushed out of traffic and towed. I felt it was unsafe to drive but was repeatedly told by dealers that it was “OK to drive” or that the issue was “bad gas.” After visits to multiple dealers, the current Kia service center finally diagnosed a severely clogged exhaust. They report the manifold catalytic converter and underbody converter have broken down internally and are heavily coated in carbon/oil deposits, and state this likely came from the engine burning oil before and around the time of the rebuild. Exhaust temperatures have been recorded at very high levels, and the engine will only run briefly with an O2 sensor removed to vent exhaust; with the exhaust sealed it will not stay running. The dealer has warned there may now be internal engine damage from prolonged backpressure. The vehicle has been undriveable for many months and the failed converters are being retained and are available for inspection. My concern is that excessive oil consumption on these GDI engines is causing secondary catalytic-converter failure, extreme power loss, stalling risk, and possible fire risk from overheated exhaust components, even after engine replacement.Read more

The vehicle experiences a severe loss of power, placing it into a reduced-power or limp mode that prevents the vehicle from exceeding approximately 10 mph. The issue has occurred repeatedly despite multiple repair attempts. The battery was replaced initially, after which the vehicle broke down while still at the dealership. The service department removed the new battery & reinstalled the original
... battery, at which point the vehicle temporarily operated normally. On a subsequent visit for the same reduced-power condition, the dealership replaced the throttle body chamber and multiple gaskets. This repair resolved the issue only for a short period of time. During a third service visit, I paid approximately $3,000 for additional repairs; however, the same issue occurred again, & I was unable to safely leave the dealership parking lot. The vehicle is currently available for inspection, testing, & servicing upon request. All prior inspections & repair attempts are documented in dealership service records. My safety & the safety of others were placed at significant risk because the vehicle unexpectedly loses power while being driven. The vehicle abruptly slows & cannot exceed 10 mph. This occurred while driving on the highway during heavy 5:00 p.m. traffic, as well as during normal roadway driving. A sudden and severe reduction in speed in traffic creates a high risk of rear-end collisions or other serious accidents, particularly if surrounding drivers are unable to react in time. Yes. I was able to coast into a parking lot at which point an even larger warning light appeared and the vehicle then lost all electricity completely. I again called Hyundai Roadside Assist and they sent someone from AAA a few hours later who determined that the car was unable to be “jump started” and would have to be towed to the local dealer, Manly Hyundai. The dealer reported back to me that the component that failed is called an “ICCU” and is some kind of controller related to the charging and motors of the electric drivetrain. It is available to be inspected at the dealer currently, Manly Hyundai. My safety was put as risk because the car became essentially inoperable while driving downhill and gave no warning signs ahead of time before the lights lit up on the dash and I lost control. At the repair level, Nissan has offered a fix outside of warranty coverage (a patch) which does not provide the continuing coverage provided by a warranty repair (which requires replacement of the panel). I have spent countless hours taking the car in to have it looked at and on the phone to try to get coverage with no success. 2 - The structural metal on the inside of the drivers door is cracking. We took the car in for repair because the window was not working but when they opened it up, they found that the the structural metal of the door itself where the window mechanism is attached has cracked. We have video indicating this may be a design defect in addition to poor materials. The Nissan dealer is not helpful, saying only that they don't have a body shop. Body shops say the entire door needs to be replaced but do not want to get involved with seeking help from Nissan. 3 - The CVT transmission failed just after the manufacturers warranty expired which is way too early for this problem. The vehicle remains unavailable for use due solely to the lack of this GM-supplied component. A warning indicator and drivability symptoms were present prior to removal from service. The component is currently retained by the servicing dealer and is available for inspection upon request. General Motors has advised that this component is subject to a widespread national backorder affecting numerous vehicles, resulting in extended periods where affected vehicles cannot be repaired or safely returned to service.Read more
