I had pulled the vehicle into my garage. When I tried to shut the car off the car it would not shut off. In fact many components of the car seem to be completely out of sync. The car would not go into any gear. I could not shut the car off. No matter what I did the car just kept running. I finally more or less gave up and just let it keep running for a couple of hours. When I went back into the ga
... rage to check what was happening the car had finally stopped running. At that point I had call a towing service and arranged to have the vehicle taken to Autonation Jeep Chrysler the next morning where I had purchased the vehicle. This was a newly purchased vehicle and only had a total of 78 miles on it. Surprisingly the next morning I went into the garage to check on what was happening with the vehicle and to my surprise the vehicle had stopped running. I then disconnected The 12 Volt battery and left it disconnected for an hour or two. After this time I reconnected the battery. I again attempted to start the vehicle and surprisingly the vehicle started up and all functions in the car seem to be performing normally. At this point I drove the car about a mile to the Autonation Jeep Chrysler dealership and had conversation with service technician. Surprisingly he told me that Jeep has had this problem with other vehicles and the solution is to disconnect the 12 Volt battery for 20 or 30 minutes and then reconnect the battery. At that point the car seems to reset itself and it will run normally. End This car of mine only had 78 miles on it This is totally unacceptable. Furthermore this seems to be a problem with other Jeep vehicles. It is my opinion that Jeep Needs to correct this problem so I will not be stuck with a car that's running in my garage and I can't shut it off. All in all it seems to me this is a software problem that needs to be reworked and corrected.Read more

The vehicle has a recurring safety defect where it suddenly shuts off / stalls while driving. This has happened multiple times, including immediately after the dealership performed repairs and issued "clean" diagnostic reports stating no issues were found. Key details: PCU codes were present on the first visit but were not properly addressed. Repair orders contain clear inconsistencies, includin
... g claims of test drives and mileage that do not match the actual odometer readings. On the most recent visit, I did not authorize any further repairs, yet the dealership now claims the vehicle is "resolved" and ready for pickup. The vehicle has been at Honda dealerships for a total of 57 days across multiple failed repair attempts. I have refused to take possession of the vehicle due to the ongoing safety risk of it shutting off while driving.Read more

Model: Sierra Denali Ultimate Engine: 6.2L V8 VIN: [XXX] Mileage at Most Recent Incident: Approximately 52,000 miles Safety Complaint Summary I am submitting this complaint to report a recurring loss-of-propulsion and engine power event involving my 2023 GMC Sierra Denali Ultimate equipped with the 6.2L V8 engine. This vehicle has experienced two separate incidents in which it suddenly lost power
... while being operated at highway speeds. During both events, the vehicle unexpectedly lost propulsion without warning and without any apparent driver input or mechanical indication beforehand. No warning lights, check-engine lights, reduced-engine-power messages, service transmission messages, or other warning indicators were displayed prior to the failure. The vehicle simply lost power while traveling in traffic. During both occurrences, I was forced to safely maneuver the vehicle to the shoulder of the roadway while rapidly losing speed among surrounding traffic. Once stopped, the vehicle would not resume normal operation until it was restarted. After restarting, the vehicle appeared to operate normally; however, the fact that this condition has now occurred twice raises significant concerns regarding the reliability and safety of the vehicle. Safety Risk This condition presents a significant and immediate safety risk because sudden loss of propulsion during highway operation can leave the driver unable to maintain speed, safely merge, maneuver, or avoid surrounding traffic. The unexpected reduction in speed creates the potential for rear-end collisions, loss of vehicle control, or other traffic accidents, particularly when operating on high-speed roadways. Because the vehicle provided no warning before losing power, there is little opportunity for a driver to take preventive action. I am writing regarding my BMW X5 xDrive40i, model year 2019. For this G05 model coded cars, repeated transfer case failures have been reported due to a known issue: on the factory they put a wrong oil in it and then published a Technical Bulletin about it as if they are not the ones responsible for that. The problem is, due to that mistake, transfer cases of cars manufactured between 2018-2020
... becomes dead in time. Cars shudders, jerking at low speeds, cannot deliver the power to the ground effectively due to intermittent and very disturbing, powerful shudders. This creates a huge safety concern cause: 1) if you are on an intersection the car might not join safely to the traffic in a perceived timing due to the shudders, intermittent power delivery. 2) If you are on a driveway and moving to the street the car might not move to the street for the perceived timing or once moved cannot move safely to join the traffic. 3) If there is a failure on the transfer case and if there is a sudden stop is needed. On the road with no shoulder access or if this is a bridge where there is no shoulder, it creates a huge risk to safely move the vehicle. An unavoidable sudden stop can create a huge accident risk for everybody. This is a very known and very common problem for BMW cars equipped with B58TU engine especially years 2018-2020. BMW is also definitely aware of that and this safety concern but instead of fixing it they moved away their responsibility as if they are not the ones made the mistake. But, when contacted with BMW NA, they refused to help.Read more

The vehicle experienced a sudden and total loss of transmission engagement and motive power while driving at highway speeds due to a Transmission Control Module (TCM) failure. While driving at a speed just under 45 mph, my Mazda CX-70 suddenly displayed and chimed “Hybrid System Malfunction. Have Vehicle Inspected.“. This warning startled me because I am familiar with issues since I had briefly owned a CX-90 PHEV the first year this model came out and it had the same exact problem. I discovered that this issue carries serious safety concerns and ended up convincing
... the dealer to swap the vehicle for a CX-90 inline 6 gas only model after hours of pleading. I ended up signing up for another lease a couple of years later with this current CX-70 PHEV, as I was told that the “bugs” have pretty much been worked out so I would give it another try. This warning continued to stay on as I drove for a few minutes so I immediately took the car straight to the Mazda dealership service department where it was kept 24 hours. The dealer said that the issue resolved itself - something to do with not driving the vehicle interchangeably enough between gas and EV mode. The photo was taken once I slowed down to stop at an intersection since it took me 15 minutes to drive to the dealer.Read more

2019 Ford Escape with a documented history of powertrain failures. White smoke on cold start and engine stalling began around 48,000 miles; the dealer noted this engine is known for coolant intrusion. At about 62,000 miles the engine short block was replaced under Ford Field Service Action 21N12 / TSB 22-2322. Smoke, shaking, and stalling continued, and the engine intermittently cuts out when shif
... ting into reverse and must be restarted. At about 87,000 miles the transmission failed and the full transmission assembly was replaced (work order Jan 2026, vehicle returned end of March 2026). The owner repeatedly told the Ford dealer the vehicle felt unsafe. On [XXX] the owner reported renewed exhaust smoke, an illuminated engine light, and a clicking sound at startup and asked to bring it in; instead of inspecting it, the dealer advised taking the vehicle straight to CarMax. About 9 days later, on [XXX], while driving in Los Angeles about 60 miles from home with young children aboard, the transmission would not stay in gear and the engine warning lights began blinking. The owner called the Ford dealership for guidance but no one could advise and the service desk would not answer. After consulting a mechanic, the family agreed it was unsafe to keep driving given the noises and pulled over in Anaheim, about 30 miles from home, at a mechanic shop to read the codes. A paid tow (~$600) was required. Loss of gear engagement and stalling in traffic with children aboard is a serious crash and stranding hazard. The scan returned codes P0012 and P0014 (camshaft timing, stored as permanent), P0300 (random/multiple misfire), and P0301, P0302, P0304 (cylinder misfires); the dash showed "Auto Start/Stop not ready - Stall Alert - Restart Engine." The defect has been reproduced and confirmed repeatedly by the Ford dealer, which performed the engine and transmission replacements. A repurchase request was denied. This is my son's vehicle, purchased by me in January 2025. He attends a college 110 miles from home in PA. As the warmer/hot weather came along in 2025, he was experiencing random scary situations of the car just going into "limp mode" while driving 70 - 75 mph on [XXX]. No warning indicators, no dash lights. No acceleration, couldn't maintain speed, bogged down to 15 mph until he pulled over and
... turned off the car. It scared him of course, but sadly this became his summertime travel ritual when traveling to and from college. Every trip in 87°< temperatures, a 2-3 time limp mode was in store. I realized in June of 2025 that this problem doesn't throw any DTCs when it happens and because sometimes he didn't wait long enough to clear the limp mode, I had him disconnecting the negative battery cable then reattaching it in order to clear the resident memory to get him going right away. I went a step further and installed gate switch on the negative battery post so he could just lift the hood, break the circuit with the gate arm and put it back down. I write this today because the car's 1st episodes for 2026 was on a similar mileage trip home, however it was only 82°. The CVT fluid has been changed, the level is correct and his driving habits appear to be in line with GPS ETAs vs. his actual arrival times. I went down the rabbit hole regarding the 2013 Kizashi TCM recall and I truly believe the defective TCMs they found have been around for some time before the 2013 Model year. He loves his car, but the car is putting him at risk as well as the driving public around him EVERY TIME THIS HAPPENS. My 2023 Lexus RX350 with 67,000 miles displayed a check engine light along with a Drive-Start Control Malfunction warning. Toyota has issued Technical Service Bulletin T-SB-0084-23 covering this exact fault code on the same 2.4L turbocharged engine used in the 2023 Lexus RX350. The bulletin states the repair is covered under the Federal Emission Warranty for 96 months or 80,000 miles. However, the Lexus dealership has declined warranty coverage citing the absence of a Lexus-specific bulletin, despite the identical engine and fault code. The vehicle has experienced repeated failures associated with diagnostic trouble code P06DD. The issue is intermittent but has occurred repeatedly. The vehicle has been presented to authorized Dodge/Stellantis dealerships on five separate occasions fo
... r diagnosis and repair. The oil pump was previously replaced for this issue at approximately 5,000 miles, yet the condition continues to recur. Most recently, the dealership advised that the issue was resolved and returned the vehicle to me. Within approximately 30 minutes of leaving the dealership and driving on the freeway, the check engine light returned and the same stalling and loss-of-power condition immediately reappeared. The defect creates a significant safety concern because the vehicle loses the ability to accelerate normally while operating in traffic and on freeways. When the condition occurs, the vehicle cannot exceed approximately 3,500 RPM stalls while driving, creating a risk of collision. The vehicle currently has approximately 21,000 miles and remains under manufacturer warranty. Despite multiple repair attempts, the defect has not been permanently corrected.Read more

My 2025 Cadillac Escalade ESV lost power and shut down while I was driving on the highway. The gas pedal stopped responding, and the vehicle would not restart afterward. Pepe Cadillac confirmed that this is the second replacement engine, which means the third engine counting the original engine. The dealer also confirmed that the engine locked up due to internal bearing material failure and needs to be replaced again. The repair order number is 140134. I am concerned this is a serious safety-related engine defect. A brand-new GM replacement engine should not lock up after only about 22,000 miles, especially while driving on the highway. My 2024 Hyundai Santa Fe (VIN: [XXX] ) was manufactured on March 14, 2024, in [XXX] . This creates a severe lag in acceleration, leaving the vehicle dangerously exposed to oncoming traffic. This compo

Because my manufacturing date falls just outside the official recall window, this vehicle has not received a formal recall remedy, yet it actively exhibits the identical, dangerous powertrain defects, suggesting the recall's production scope may be too narrow. Furthermore, in November 2024, the vehicle suffered a catastrophic safety failure where it completely lost all propulsion power for approximately 30 seconds while moving through the middle of a live intersection. Most recently, on XXX, right after being released from an extended 38-day service stay at an authorized dealership, the electronic infotainment displayed an active warning screen stating the car should be shifted to park while the vehicle shifter was physically and securely locked in Park. Hi [XXX] , Not sure if you are still working at NHTSA….. For what it’s worth, I finally diagnosed my ‘other’ 2015 Ducati with a failed spark plug wire (open circuit). This bike has about 5,000 miles, but has had the symptoms since about 1,000 miles (i.e., almost 10 years). So, Ducati is 2 for 2 in 2015 for bad spark plug wires. On ‘dual spark’ bikes (i.e., this one), having on
... e failed wire is not a catastrophe; but very hard to diagnose. For my other 2015 bike (emails below) with only one wire per cylinder, it can lead to erratic engine response and fire while riding the bike. Not much fun. I’m a bit surprised that NHTSA seems to dismiss this so readily. It is also pretty clear that Ducati knows of the issue, but has not bothered to provide owners with any information / warnings. Sad. If you needed them, I could send you some “crispy” parts that burned (actuator for the exhaust flapper valve) and the bad wire. Best, [XXX] From: [XXX] [XXX] > Sent: Friday, April 18, 2025 2:52 PM To: [XXX] (NHTSA)' [XXX] > Subject: RE: VOQ- 11647603 Hello again, Not sure if you are still interested in this motorcycle fire….. Turns out the culprit in this long ordeal was two spark plug wires slowly going bad. Unusual that a “Garage Queen” bike with 1,500 miles on it would have both plug wires go bad (open circuit) at almost the exact same time. Since the wires are a ‘dumb’ component, there were no ECU trouble codes or other indicators (except for the completely misleading ones) that identify this problem. With a partially ‘bad’ wire, a cylinder won’t spark, passing the gas directly into the hot exhaust….. where it burns, making the exhaust hot enough to ignite whatever might be nearby. “I authorize NHTSA to share my Vehicle Identification Number (VIN) and other personally identifiable information in my complaint record with the manufacturer,” INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more

The power transfer unit (PTU) failed at approximately 53,000 miles, which is a premature failure for a major drivetrain component. The issue was confirmed upon inspection. The “Service 4WD” warning light remains illuminated on the dashboard. Additionally, the vehicle pulls slightly to the left while driving. A failure of the PTU affects the all-wheel-drive system and can lead to improper pow
... er distribution, loss of traction, or driveline binding. The pulling condition and drivetrain issues create a safety concern during normal driving. Similar PTU failures have been reported in other 2019 Jeep Cherokees and have resulted in recalls and technical service bulletins for similar components, although this specific vehicle was not included. This suggests a broader pattern of premature PTU failure.Read more

My vehicle repeatedly loses acceleration/motive power while driving. When I press the accelerator, the pedal may go down but the vehicle does not respond normally and begins to decelerate unexpectedly. Nissan advised that the vehicle was part of a CVT-related issue/settlement or warranty extension, but said coverage has expired. I again have noticed intermittent pulsing at low to medium speeds. At roughly 1k-2k rpm the engine at or about 10-20 mph is pulsating or hesitating with an obvious pulse which you can see on the tachometer as well (bouncing.) It seems like there is either a computer, fuel flow or electric issue. Also, at speeds up to the 40 mph range, this is happening as well. Yesterday while towing a small 7'x12
... ' empty dual axle trailer I noticed it again, I occasionally use my trailer to haul small loads up to 4500lbs. This has happened before but this time I decided to notate this on your site, since reading about ongoing problems with the 2020 6.2L GMC Sierra 1500. I was aware of the other following years, 2021-2024? issues and that is why I purchased this 2020 year model. My truck will be available for inspection when the request is sent to me. Safety was not an issue, I ran my truck most of the day. I have not yet notified my Chevrolet dealer, Roger Dean, Cape Coral, FL. My truck has not yet been inspected for this issue. There were not any lights or warnings on display at time of incidents. Thank you, [XXX] [XXX] [XXX] INFORMATION REDACTED PURSUANT TO THE FREEDOM OF INFORMATION ACT (FOIA), 5 U.S.C. 552(B)(6)Read more
